    • Purpose. The aim of this paper is to describe and analyse the current situation of the physiotherapy component of the Rehabilitation of Afghans with Disability (RAD) programme, in order to identify the needs and challenges for further development. Method. The study was conducted as a field study with an anthropological approach by means of participant observation, unstructured and semi-structured interviews and photography. Results. The therapists in RAD work in isolation with little opportunity for further education or professional development. Their approach is mainly medical, where the work is dictated by the patients' expectations and doctors' recommendations. They use primarily passive methods of treatment, and their work is affected by cultural, religious and situational factors. They demonstrate a low capacity of clinical reasoning in their practical work. Conclusions. There is a need for further development of physiotherapy in Afghanistan. Active and individually adapted treatment methods, clinical reasoning processes and evidence-based practice should be encouraged. There are several challenges in this, based on Afghan culture and traditions, gender issues, religious factors, an authoritative society, a medical approach in treatment, and isolation and limitations in access to information. By means of an Action Research project the physiotherapists could be included in further development and research to promote a sustainable and culturally relevant development.

    • Fibromyalgia is a well-known syndrome interpreted as general muscular pain in the whole body and as pressure soreness in the so-called tender points. The cause of the syndrome is not yet clear and it has therefore been difficult to find proper treatment. The aim of this study was to describe how patients with fibromyalgia conceived their health after having taken part in an intervention programme based on ego-strengthening psychotherapy and the salutogenetic model. The intervention was performed by an occupational therapist and a physiotherapist. In this study, 10 patients were interviewed and data were analysed by the phenomenographic method. Four different catagories of health conceptions became evident as a result of the intervention; receiving confirmation, creating distance to things, gaining insight, and gaining control. The results showed that the patients had increased their sense of meaningfulness, comprehensibility and manageability of the pain syndrome, which led to improved health. Due to the fact that this kind of intervention requires comparatively small resources in terms of staff and financial expenditure it is suitable in small clinics, e.g. health care centres. Further research should be aimed at finding out whether this intervention also suits patients with other diagnoses or whether treatment in larger groups might produce equally favourable results. © 1995 Informa UK Ltd All rights reserved: reproduction in whole or part not permitted.

  • Acute stroke patient mobilisation practices and concerns. A survey of 202 stroke professionals

    • Background Stroke unit care is proven to improve outcome for patients. One component responsible for this may be that patients are mobilised earlier and more intensively, but there is lack of unambiguous scientific evidence. Therefore current practices may be driven by historical precedent and/or clinical opinion, and varying approaches to mobilisation are likely. This study sought to determine different health professionals´ beliefs regarding early mobilisation (EM) in acute stroke. EM was defined as intensive out of bed activities within the first 24 hours after stroke onset. Methods A 9 item anonymous questionnaire on benefits and harms with early mobilisation after stroke was used to interview stroke care professionals during the integrated Stroke Society of Australasia (SSA) and Australasian Nursing and Allied Health Conference in Sydney in August 2008. Data were collected using personal digital assistants. Five point Likert scales sought levels of agreement with harm and benefit of EM. Results The survey was completed by 202 professionals, representing 38% of all conference attendees. 65% were females, 50% under 40 years old, 46% worked in an acute stroke unit and 31% in rehabilitation, 35% were nurses, 26% were medical doctors, 19% were physiotherapists and 12% were occupational therapists. Two thirds had less than 10 years experience in stroke care overall. Sixty percent of professionals had concerns about possible harm of early mobilisation and more so for hemorrhagic (59%) than ischemic (23%) stroke (p<0.001). Cardiovascular stability was the major concern (12% ischemic, 34% hemorrhagic). Conclusion Our results show that most professionals had concerns about early mobilisation of stroke patients and these concerns were stronger for hemorrhagic than for ischemic stroke patients. This is surprising given the lack of evidence to support such a dichotomy and the potential harms of bed-confining hemorrhagic stroke patients.

    • The purpose of this study was to examine the prevalence of injury and the extent of preventive actions in elite Swedish volleyball players. Injuries to players in the elite male and female Swedish division, during the 2002-2003 season, were registered by using a questionnaire. Of the 158 volleyball players (70% response rate), a total of 82 players (52%) reported 121 injuries, during a total exposure time of 24 632 h, representing an overall incidence of 0.77 injuries per player. The majority of the injuries were located in the ankle (23%), followed by the knee (18%) and the back (15%). Most injuries (62%) were classified as being of minor severity. Most injuries occurred during training (47%), and 41% of the injuries had a gradual onset. Fifty-four percent of the injuries that could be related to a specific court situation occurred during blocking, and 30% during spiking. Most players (96%) participated in injury prevention training of some kind, generally performed without supervision (58%). Although most players took part in some kind of preventive action, one out of two players incurred an injury during the season, which indicates that the risk of suffering an injury in elite volleyball is relatively high.
