März 2020 für eine regionale Gesundheitsbehörde (Bayerisches Landesamt für Gesundheit und Lebensmittelsicherheit) zu Entscheidungen im Zusammenhang mit Covid-19. In this paper we describe the process and content of our ad hoc public health ethics consultation for a Bavarian health authority in relation to Covid-19.

- Das intestinale Mikrobiom repräsentiert aktuell eines der Topthemen, die im Fokus der modernen Humanmedizin stehen. Doch erstaunlicherweise gab es bisher im deutschen Sprachraum keine handliche, aktuelle Übersicht zu diesem Thema. Daher werden in diesem Band zunächst Zusammensetzung und Dynamik des humanen intestinalen Mikrobioms erläutert, ehe in weiteren Kapiteln dann die wichtigsten Veränderungen und vor allem die Rolle des Mikrobioms bei zahlreichen Krankheiten wie chronisch entzündlichen Darmerkrankungen, Reizdarmsyndrom, Leber- und Gallenwegs- sowie Autoimmunerkrankungen diskutiert werden. Es folgen der Komplex Ernährung, Adipositas und Diabetes sowie Atherosklerose mit jeweils eigenen Kapiteln. Schließlich gibt es überraschende Beziehungen der Mikrobiota zur Malignomentstehung. Es folgt ein Abschnitt über Antibiotika und deren Rolle bei zum Teil dauerhaften Störungen der intestinalen Bakterien und Pilze. Am Ende des Buches wird am Beispiel von Mikrobiomtransfer ("Stuhltransplantation") und Probiotika auf die therapeutischen Konsequenzen eingegangen. Alle Kapitel sind auch für sich lesbar und verständlich.

- Die Durchführung von Funktionstests in der pädiatrischen und in der Erwachsenen-Endokrinologie erfordert eine ganz besondere Präzision und zum Teil langjährige Erfahrung in der Indikationsstellung, Durchführung und Bewertung. Diese Fibel soll einen pragmatischen Zugang zur endokrinologischen Diagnostik ermöglichen. Die Autoren verfügen über eine mehr als 20-jährige eigene Expertise und führen alle beschriebenen Funktionstests selbst wie hier beschrieben durch. Die Funktionstests sind im Laufe der Jahre auch immer wieder angepasst worden, die Testabläufe und auch die Bewertungen haben sich etwas verändert, zum Teil fehlen für ältere Testbeschreibungen essenzielle Medikamente (z. B. Pentagastrin), andere Substanzen sind aktuell wieder verfügbar (z. B. Metyrapon [Metopiron® HRA Pharma]). Weiterhin konnten Fehler in früheren Publikationen berichtigt werden. Besonderes Augenmerk wurde auf die Praktikabilität gelegt: Indikationstellender Arzt, der Patient, die Arzthelferin oder Krankenschwester und das endokrinologische Spezial-Labor bilden ein gemeinsames Team als Garant für ein verwertbares Ergebnis. Alle endokrinologischen Funktionstests werden in einem einheitlichen Schema dargestellt. Alle Hormonwerte werden in konventionellen und in SI-Einheiten angegeben. Aktuelle Literaturangaben ergänzen die vorliegende Übersicht. Diese Auflage enthält bewusst nicht alle theoretisch möglichen endokrinologischen Funktionstests, wir haben uns auf die wichtigsten Indikationen und auf die Durchführbarkeit in einer ambulanten Praxis beschränkt, heute obsolete Funktionstests (z. B. Fludrocortisontest) werden nicht aufgeführt.

- © 2015 Springer-Verlag Berlin Heidelberg Background: In the data report 2014, the German Maternal Convalescence Movement (Müttergenesungswerk) identified a possible discrimination against mothers with a low level of education in health convalescent treatment allocation. Objective: The question whether this discrimination is supported by data is investigated. Material and methods: Based on a special analysis of the microcensus (Statistisches Bundesamt), the percentage of the education level of all economically active mothers with a child under the age of 10 years is compared with the corresponding percentages of health convalescent treatment participants. Results: No discrimination against mothers with a low level of education in health convalescent treatment allocation was found in this data set.

- Iron deficiency is commonly diagnosed by serum ferritin [sFer] determination, iron deficiency anaemia from reduced mean cellular volume (MCV). [sFer] shows a high variance if inflammation or liver dysfunction are present. Athletes often present with hypoferritinaemia. This study was designed to evaluate the relation of total body haemoglobin mass (tHb) which is commonly elevated in athletes to [sFer]. In the present study 56 trained (TR), 72 moderately trained (MT) and 31 untrained (UT) male individuals were investigated for peak oxygen uptake (VO2max), serum iron, [sFer] soluble transferrin receptor [sTfr], serum erythropoietin [EPO], haemoglobin concentration [Hb] haematocrit (Het), MCV blood volume (BV), and plama volume (PV). TR and UT individuals differed significantly in VO2max, tHb, PV, and BV (all p<0.01). [sFer] correlated negatively with tHb (r=-0.31,p<0.05), BV (r=-0.38,p<0.05) and max (r=-0.54,p<0.01) but not with EPO, [Fe], [sTfr], MCV, [Hb], Het, and PV. Since a negative correlation of [sFer] and tub was found, an iron storage shift from the reticuloendothelial system (RES) to the erythroid system could have occurred. This is only pathological if functional iron deficiency occurs, as suggested by increased [sTfr]. Possible causes of functional iron deficiency are gastrointestinal microhaemorrhage, menstrual blood loss and EPO-doping. True iron deficiency should be treated by dietary means or iron supplementation. Iron misuse instead has severe side-effects and uncritical addition of iron to the athlete's nutrition should be avoided.

- Because of its unique anatomy, specific diseases and lesions arise in the upper cervical spine, which differ widely from the rest of the spine. During the last two decades standardised diagnostic and therapeutic algorithms have been defined for most of the craniocervical pathologies often occurring in combination with an underlying disease requiring surgical intervention as well. On the other hand there are some very rare phathological alterations: about 20% of the patients suffering from neurofibromatosis type I develop spinal deformities. These are mostly found in the thoracic and lumbar spine (dystrophic/non-dystrophic type). In rare cases the dystrophic neurofibromatosis type I involves the upper cervical spine leading to bizarre deformities endangering the spinal cord. An aggressive, timely and combined operative therapy is necessary. Patients with Down syndrome should be investigated regularly for affections of the upper cervical spine. Though only in about 1% of all patients with Down syndrome do instabilities require surgical intervention, the upper cervical spine should be screened on a regular basis, since neurological changes due to the pathognomy of the underlying disease often remain undetected for a long time. The operative therapy of the instable os odontoideum in Down syndrome follows the general principles of this pathoanatomical variation. Even though the Klippel-Feil syndrome is generally not linked with neuropathological findings, rare associated deformities of the upper cervical spine should be excluded by proper diagnostic procedures.
