  • The roles of quality departments and their influence on business results

    • This study explores the various roles of quality departments and investigates whether their roles have different influence on business results. Based on a survey of quality managers in 211 Swedish organisations, the analysis identifies four roles of quality departments: firefighters, auditors, process improvers, and orchestrators. The roles vary in their predominant adoption of Quality Management practices ranging from a narrow scope focusing on quality management systems to a broader scope, based on multiple practices. An analysis was performed to identify how each of the identified roles influences business results. The results show that quality departments with a broad focus, combining both explorative and exploitative quality practices, contribute the most to business results.

  • Value-based healthcare translated: A complementary view of implementation

    • Background: Interest in the implementation of various innovations (e.g. medical interventions and organizational approaches) has increased rapidly, and management innovations (MIs) are considered particularly complex to implement. In contrast to a traditional view that innovations are implemented, some scholars have promoted the view that innovations are translated into contexts, a view referred to as translation theory. The aim of this paper is to investigate how a translation theory perspective can inform the Consolidated Framework of Implementation Research (CFIR) to increase understanding of the complex process of putting MIs into practice. The empirical base is a two-year implementation of the MI Value-Based Health Care (VBHC) to a psychiatric department in a large Swedish hospital. Methods: In this longitudinal case study, a qualitative approach was applied using an insider researcher with unique access to data, who followed the implementation starting in 2015. Data sources includes field notes, documents, and audio recordings of meetings and group reflections which were abridged into an event data file structured by CFIR domains. In a joint analysis, an outsider researcher was added to strengthen the analysis and mitigate potential bias. Results: Two themes were identified, for which CFIR did not satisfactorily explain the findings. First, the intervention characteristics (i.e. the content of the MI) were modified along the process and, second, the process did not follow predefined plans. However, the project was still perceived to be successful by internal and external stakeholders. Conclusions: The paper proposes three ways in which translation theory can inform CFIR when applied to MIs: 1) strength of evidence is not as important for MIs as for medical and technical innovations; 2) adaptability of the MI can be emphasized more strongly, and 3) it can be more fruitful to view implementation as a dynamic process rather than seeing it as a matter of planning and execution. For managers, this implies encouragement to seize the opportunity to translate MIs to fit their organization, rather than to aim to be true to an original concept.

    • Inter-organizational collaboration and co-production are often kept separated, despite increasing evidence that collaborations require the active contribution of citizens. In this chapter we argue that citizen-centeredness is necessary in so-called resource integration among multiple actors (whether organization representatives or citizens). Building on two action research cases in a Swedish cancer context, it is suggested that the life event of having cancer and the lived experience as inhabitant in a segregated area are pivotal as resources to identify actors that can make a difference. Once actors are identified, relations may be established and maintained through integration and combination of their tangible and intangible resources targeting the citizens broader life situation. The action researcher may both facilitate such resource integration as well as contributing with their own specific knowledge in order to contribute to change for the better.

  • A Physician-Led and Learning-Driven Approach to Regional Development of 23 Cancer Pathways

    • Sweden has one of the best cancer care systems in the world when it comes to survival. However, prognoses shows that by 2030 the number of men with cancer will increase by almost 130% compared to today – the corresponding increase for women is around 70%. Approximately half of this increase can be explained by population changes, i.e. population size and age structure. Due to this future challenge, a national cancer strategy has been created. We will present some strategies that the Regional Cancer Centre West is working on to face the future challenge. A central part of this work is to create patient-focused, coherent regional care pathways for patients with malignant diseases. For this purpose, 30 experienced senior physicians, specialized in different tumour diseases, have been appointed as regional process (pathway) owners. Together, they share a regional responsibility for 23 cancer pathways, and nine support processes.

    • The study describes different applications of process orientation and associates the applications with organisational performance and inhibiting factors. The objective of the study is to analyse potential differences between the various applications of process orientation and thus to empirically validate what organisations actually do when managing their processes. The article uses survey data from 183 employees of various healthcare facilities in one region in Sweden. Four significantly different user groups of process orientation existed in the studied population. These were Fumblers, Talkers, Mappers and Organisers. The Organisers perceived the greatest effect on organisational performance, slightly greater than the Talkers. The Mappers perceived a significantly lower effect. The results also show that the perceived resistance from inhibiting aspects decreases the more visible and structural the application of the process orientation is, i.e. Mappers and Organisers. The linguisticand cognitive approach that the Talkers have taken has been perceived as fruitful in terms of organisational performance. We argue that the suggested classification ofprocess orientation contributes to a better understanding among practitioners by showing that different applications of process orientation exist and by showing whateffects can be expected from these applications. By not seeing process orientation as one single idea or application, it is our intention that this study contribute to a broader view of process orientation.

  • The role of quality managers in contemporary organisations

    • A key question for firms nowadays is how to organise work with quality management. This naturally includes the role of the quality managers and it appears as if the profession of quality managers is at a crossroads. Alternatives are that the role of a quality manager broadens to include quality at a strategic level in the firm and that Six Sigma specialists and Lean Production managers drive the development of quality management in the future. In this paper, we present the results from a survey of 212 quality managers in Swedish organisations. The purpose is to contribute to an understanding of how the management of quality is designed and practised in contemporary organisations. This study shows that a quality manager's operational responsibility is quite narrowly defined. The agenda of the quality manager is mainly related to quality standards, such as ISO 9000 and environmental management systems. These programmes frame the work of the quality manager, which in many cases leaves other programmes, such as Six Sigma and Lean Production, to other departments or parallel improvement structures.

  • User involvement and value co-creation in well-being ecosystems

    • PURPOSE: This article aims to examine how users' involvement in value co-creation influences the development and orchestration of well-being ecosystems to help tackle complex societal challenges. This research contributes to the public management literature and answers recent calls to investigate novel public service governances by discussing users' involvement and value co-creation for novel well-being solutions. DESIGN/METHODOLOGY/APPROACH: The authors empirically explore this phenomenon through a case study of a complex ecosystem addressing increased well-being, focussing on the formative evaluation stage of a longitudinal evaluation of Sweden's first support centre for people affected by cancer. Following an abductive reasoning and action research approach, the authors critically discuss the potential of user involvement for the development of well-being ecosystems and outline preconditions for the success of such approaches. FINDINGS: The empirical results indicate that resource reconfiguration of multi-actor collaborations provides a platform for value co-creation, innovative health services and availability of resources. Common themes include the need for multi-actor collaborations to reconfigure heterogeneous resources; actors' adaptive change capabilities; the role of governance mechanisms to align the diverse well-being ecosystem components, and the engagement of essential actors. RESEARCH LIMITATIONS/IMPLICATIONS: Although using a longitudinal case study approach has revealed stimulating insights, additional data collection, multiple cases and quantitative studies are prompted. Also, the authors focus on one country but the characteristics of users' involvement for value co-creation in innovative well-being ecosystems might vary between countries. PRACTICAL IMPLICATIONS: The findings of this study demonstrate the value of cancer-affected individuals, with "lived experiences", acting as sources for social innovation, and drivers of well-being ecosystem development. The findings also suggest that participating actors in the ecosystem should utilise wider knowledge and experience to tackle complex societal challenges associated with well-being. SOCIAL IMPLICATIONS: Policymakers should encourage the formation of well-being ecosystems with diverse actors and resources that can help patients navigate health challenges. The findings especially show the potential of starting from the user's needs and life situation when the ambition is to integrate and innovate in fragmented systems. ORIGINALITY/VALUE: The proposed model proposes that having a user-led focus on innovating new solutions can play an important role in the development of well-being ecosystems.

    • ‘Travelling ideas’ denotes the fact that virtually identical management ideas crop up more or less simultaneously in similar organizations. Røvik, who recently launched his ‘virus-inspired theory’ approach, seeks to pinpoint the possibility that the encounter between idea and practice may be lasting, ideas may affect practice, and leaders in organisations may play a more active role than the one often depicted in management fashion theory. This article is a contribution to his virus-inspired theory. It focuses on processes in their pragmatic form, as value-creating flows in organisations. We have studied Process Management (PM) as an idea, and how it has penetrated and flourished in the Swedish Customs Service. We argue that the empirical test in this article supports Røvik's belief that a virus-inspired theory gives a richer picture than the fashion theory. Through our analysis we show that four of our six hypotheses, derived from the virus-inspired theory, are strengthened by the empirical story from the Customs Service.
