Det besvärliga våldet : Socialtjänstens stöd till kvinnor som utsatts för våld i nära relationer

- I avhandlingen analyseras hur våldsutsatta kvinnors behov tolkas, omförhandlas och anpassas för att kunna hanteras inom socialtjänstens organisation. Avhandlingens övergripande frågeställningar fokuserar på insatser och behov, betydelsen av socialtjänstens organisering och betydelsen av socialarbetarnas handlingsutrymme. Avhandlingen baseras på kvalitativa analyser av statliga propositioner, intervjuer med socialarbetare och med kvinnor som varit utsatta för våld i nära relationer. Det teoretiska ramverket bygger bland annat på Frasers (1989) teoretiska perspektiv som tar sin utgångspunkt i samhällets tolkningar av människors/gruppers behov av stöd, nyinstitutionell teori och teorier om gatubyråkrater. Avhandlingen visar att socialarbetares tolkningar är centrala aspekter av förhandlingen om hur våldsutsatta kvinnors behov och rätt till stöd ska förstås. Ett viktigt resultat i avhandlingen är att stödet till våldsutsatta kvinnor blir så pass olika. Avhandlingen ger inga svar i kvantitativa termer på hur olikheten är fördelad, men den ger exempel på hur olikheten tar sig uttryck. I kommuner där specialiseringen innebär att socialarbetarna på socialkontoret i första hand utreder behov och fattar beslut om insatser, måste det också finnas adekvata insatser att besluta om. Saknas det så erbjuds inte heller något stöd. Stödet till kvinnor som utsatts för våld i nära relationer blir också olika eftersom socialarbetare ställer olika krav eller sätter upp olika trösklar för att kvinnor ska få stöd. Avhandlingen visar att både gemensam kunskap och gemensam syn på sociala problem är centralt för att samarbetet inom den specialiserade socialtjänstens ska fungera och i längden också för vilket stöd människor kommer att erbjudas.

Families' importance in nursing care : nurses' attitudes - an instrument development

- This article describes the development and testing of a research instrument, Families' Importance in Nursing Care-Nurses' Attitudes (FINC-NA), designed to measure nurses' attitudes about the importance of involving families in nursing care. The instrument was inductively developed from a literature review and tested with a sample of Swedish nurses. An item-total correlation and a first principal component analysis were used to validate the final instrument, including a second principal component analysis to analyze dimensionality, and Cronbach's alpha was used to estimate internal consistency. The instrument consists of 26 items and reveals four factors: families as a resource in nursing care, family as a conversational partner, family as a burden, and family as its own resource. Cronbach's alpha was 0.88 for the total instrument and 0.69 to 0.80 for the subscales. The instrument requires further testing with other nurse populations.

The level of and relation between hope, hopelessness and fatigue in patients and family members in palliative care

- Hope, hopelessness and fatigue are important experiences for patients and family members in palliative care. The aim of the study was to describe the level of and relation between hope, hopelessness and fatigue in patients and family members in palliative care. Eighty-five participants completed the Herth Hope Index, Beck's Hopelessness Scale and rated their level of fatigue from none to severe. The level of hope proved to be significantly lower among the family members than among the patients. For the family members, correlations were found between hope and age (–0.358*), fatigue and hope (–0.439*), hopelessness and age (0.484**), age and fatigue (0.403**) and between hope and hopelessness (–0.723**). For the patients, correlations were found between age and hopelessness (0.555**) and between hopelessness and hope (–0.580**). Efforts to increase the experience of hope and decrease hopelessness and fatigue must include not only the patient but also the family members. (*=P<0.5, **=P<0.01).

Learning about each other : Students' conceptions before and after interprofessional education on a training ward

- In interprofessional work the striving of the members of each profession to establish their own positive in-group identity can be a source of conflict and have a negative effect on care. To counteract this, interprofessional training wards (IPTWs) have been developed in Sweden. The aim of the present study was to investigate similarities and differences in how student nurses, student occupational therapists and student social workers perceived their own and the other two professions before and after clinical education on an IPTW. Sixteen students were interviewed before and after the training on an IPTW in municipal care for older people in Sweden. A coding scheme developed in an earlier study was used in the analysis of the interviews. The findings indicate that there are changes in the students' stereotyped views, enhancing understanding of each other's professions after three weeks' clinical education on the IPTW. In some areas, however, there are still discrepancies between the description of own profession and the others' understanding of this profession that need to be confronted. In interprofessional training during education in social and health care there needs to be a balance between on the one hand the particular professional identity, on the other the shared identity implied by membership of the health-care team focusing on a common goal.

Pathways in end-of-life care for older people : care managers' reasoning

- Seven care managers employed by a large municipality in Sweden were interviewed concerning their reasoning regarding end-of-life care for older people. Data were analyzed using a hermeneutic approach. The results showed that end-of-life care was considered to constitute a small part of the care managers’ work and was something they did not focus on in general when assessing care needs. Two different pathways to death – the natural and the medical – were identified. In the natural pathway, death was invisible and the care was more routine-oriented. In the medical pathway, death was visualised and the care more individualised. Neither of the pathways paid attention to communication or existential needs. Thus, there is a need for a palliative pathway to death based on the philosophy of palliative care, which could provide guidance for care managers and promote opportunities for older people to achieve a dignified dying and death.

- The innovative contributions of third sector organizations (TSOs) to tackle work-related societal challenges are increasingly acknowledged in policy and research, but rarely in Nordic working life studies. The article helps fill this knowledge gap by an empirical mapping of efforts by Swedish TSOs to promote work inclusion among people considered disadvantaged in the regular labor market, due to age, disabilities, origin, etc. Previous studies of social innovation help distinguish their innovativeness in terms of alternative or complementary ways to perceive and promote work inclusion in regard to Swedish labor market policies. By combining various measures for providing and preparing work opportunities, addressing their participants through individualistic and holistic approaches, and manag-ing work inclusion by varying organization, funding, and alliances, the mapped cases seem to innova-tively compensate for government and market failures in the work inclusion domain to some extent, while also being limited by their own voluntary failures.

Students' learning experiences from interprofessional collaboration on a training ward in municipal care

- One way to offer students pursuing health and social care programmes realistic experiences of teamwork is interprofessional training wards where students from different educational programmes learn teamwork by working together. In the present study, a training ward in municipal care for older people was evaluated. Students from occupational therapy, nursing and social work programmes worked together on the ward for 3 weeks to learn with, from and about each other. The aim of the study was to compare students' attitudes towards practice on a training ward before and after and to evaluate goal fulfilment after 3 weeks' interprofessional education on a training ward. An attitude questionnaire was distributed to all students before and after their time on the ward, supplemented with a retrospective goal-fulfilment questionnaire afterwards. The results show that the collaborative, social experience the training ward offers was appreciated by the students and in most respects, met the learning goals set up for the course. The most important learning experience was working together in a real-life setting. However, there are some issues to take into consideration when planning and developing training wards. The setting needs to be realistic and relevant in relation to future roles for all of the student groups involved. The value and purpose of engaging together in basic patient care needs to be the subject of further investigation. When it comes to fostering competent team-workers, training wards seem to be one way forward. But to fully understand the challenges and difficulties involved in planning these wards, the learning achieved must be understood in the context of the setting as a whole, in all its aspects.

- The purpose of this study was to gain in-depth understanding of what it is like for a person with irritable bowel syndrome to be in the patient position in encounters with health care providers. We conducted qualitative interviews with nine individuals. Our analysis, guided by interpretive description, revealed experiences of unsupportive and supportive encounters. Unsupportive encounters were shaped by humiliation, insignificance, and abandonment. Not feeling believed and acknowledged as persons made the participants lose trust in their own experience, which enhanced their unfamiliar mood of being in the world. Supportive encounters were less prominent. These were characterized by openness and acknowledgment of the patient’s lifeworld. Feeling significant and being listened to promoted feelings of being in a partnered, cocreating relationship. It is reasonable to assume that care originating in the patient’s lifeworld would support the patients to use their strength to manage illness and regain familiarity in everyday life.

- Nursing today faces numerous challenges. Societal changes lead to reorganization of health care, changing workloads with sicker patients in hospital and home care, and limited economic resources. The increasing and changing nature of knowledge needed for expert care provision challenges nurses to continually update their competencies. These are issues demanding proactive and dynamic changes in the way nurses conceive their mandates and practice. The aim of the action-research project presented here was to foster improved quality of care for patients with advanced cancer through collaborative endeavours integrating cancer nursing clinical practice, research and education in a knowledge exchange programme. The programme was based on input about caregiving needs from multi-professional staff caring for patients with advanced cancer in a variety of healthcare settings. Undergraduate baccalaureate nursing students were then engaged in literature studies to help address these needs. Results of the studies were communicated back to the involved clinicians in a variety of ways. In this paper, we discuss what we have experienced as opportunities and obstacles in conducting the project, based on our reflections and external evaluations. This is linked to a broader discussion of ways of integrating cancer nursing research, education and practice.

- This article is a critical interrogation of how gender and power figure in Swedish child welfare policy and the discourses on violence in intimate relationships vis-à-vis children exposed to violence. Drawing on feminist violence research, critical childhood studies, and intersectional perspectives, we identify a differentiation with racialised undertones in the understanding of violence as a social problem when related to children’s exposure. While predominately gender-neutral discourses of social heredity and epidemiology run through the material for the seemingly ‘universal’ child, forms of violence ascribed to the presumed cultural Others link to gender, structural power, and sexuality. The article concludes that gendered articulations of violence are restricted yet pivotal if children’s exposure is to be linked to issues of inequality and power. However, when gendering interlinks with racialisation, problematic differentiations of violence, childhoods, and children are produced.
