  • Cancer patients' experiences of nurses' behaviour and health promotion activities : a critical incident analysis

    • Patients with head and neck cancer report several disease- and health-related problems before, during and a long time after completed treatment. Nurses have an important role in educating/supporting these patients about/through the disease and treatment so that they can attain well-being. This study describes the cancer patients' experiences of nurses' behaviour in terms of critical incidents after nurses had given them care to promote health. The study had a qualitative, descriptive design and the method used was the critical incident technique. Twenty-one informants from the Nordic countries diagnosed with head and neck cancer were strategically selected. It was explained to the informants what a critical incident implies before the interviews took place; this was defined as a major event of great importance, an incident, which the informants still remember, due to its great importance for the outcome of their health and well-being. The nurses' behaviour was examined, and critical incidents were involved in 208 cases-150 positive and 58 negative ones-the number of incidents varying between three and 20 per informant. The nurses' health promotion activities or lack of such activities based on the patients' disease, treatment and symptoms, consisted of informing and instructing the patients as well as enabling their participation. Personal consideration and the nurses' cognisance, knowledge, competence, solicitude, demeanour and statements of understanding were found to be important. Continuous health promotion nursing interventions were of considerable value for the majority of this group of cancer patients. Oncology nurses could reconfirm and update the care of head and neck cancer patients by including health promotion activities in individual care plans. By more frequent use of health promotion models, such as the empowerment model, the nurses could identify and focus on those individuals who needed to alter their life-style as well as tailor their approach towards these patient by setting goals for well-being and a healthy life-style.

    • Political scientists seldom translate system terminology into systems analysis. This article uses Polity IV data to probe system dynamics for studies of the global diffusion of democracy from 1800 to 2000. By analogy with the Bass model of diffusion of innovations (1969), as translated into system dynamics by Sterman (2000), the dynamic explanation proposed focuses on transitions to democracy, soft power, and communication rates on a global level. The analysis suggests that the transition from democratic experiences (“the soft power of democracy”) can be estimated from the systems dynamics simulation of an extended Bass model. Soft power, fueled by the growth in communications worldwide, is today the major force behind the diffusion of democracy. Our findings indicate the applicability of system dynamics simulation tools for the analysis of political change over time in the world system of polities.

  • Automation and the imbrication of human and material agency : A sociomaterial perspective

    • Automation is projected to transform many industries and work domains and enable both increased levels of safety and efficiency by reallocating many of the functions traditionally performed by operators. However, research on the relation between automation and work practice is lagging and needs to be further explored in order to ground the debate and design of automated work on a sound empirical basis reflecting work in actual organizational settings. In particular, research is needed that offers rich naturalistic representations of human automation interaction that accounts for the mutual shaping of human and material agency over time. The ethnographic workplace study reported in this paper draws on the sociomaterial practice perspective and on the theory of imbrication to analyze a case in which an automatic speed regulation system was installed onboard five large passenger ferries in order to improve the energy efficiency of the execution of voyages. The results show how the adoption, appropriation and use of automated technologies is inextricable from the local patterns of social interaction and collaboration. The study contributes with a deeper understanding of the relation and entanglement of the social and technological elements in human automation interaction.

    • Epidemiological research has so far failed to explain the high incidence of occupational disorders among home care workers (HCWs) and the great differences in organizational incidence rate. A qualitative approach may contribute to a deeper understanding of work group reasoning and handling in a more contextual manner. The aim of this grounded theory study was to gain a deeper understanding of the main concern in the processes of recognizing, communicating and reporting occupational disorders among HCWs. Focus group interviews were conducted with 40 HCWs in 9 focus groups. The selected municipalities represented variations in municipality type and incidence rate of occupational disorders. Making it work in the frontline was identified as the core category explaining that the perceived work situation in home care work was the main concern but interacted with work-group socialising processes as well as with the communicability and derivability of the occupational disorder when defining and reporting occupational disorders. Complex problems could be reformulated and agreed within the workgroup to increase communicability. Described significances for reporting/non-reporting were related to financial compensation, to a part of organizational political game or to an existential uncertainty, i.e. questioning if it belonged to their chosen work and life. Our conclusion is that working situation and work group attitudes have importance for reporting of occupational disorders. To support work-related health for HCWs, integrating communication should be developed about work-related challenges in work situation, as well as about attitudes, culture and efficiency within work-group.

    • PurposeThe purpose of this paper is to understand how organisational culture influences the intentions of primary care staff members (PCSM) to engage in research and development (R&D).Design/methodology/approachThe participants (n = 30) were PCSM employed in a care centre in south-western Sweden. The study had an observational design with an ethnographic approach. The data were collected by means of observations, interviews and analysis of documents.FindingsThe results revealed the perceptions of PCSM in two domains, research and clinical practice, both of which existed at three different cultural levels: visible (structures and policy), semivisible (norms and values) and invisible (taken-for-granted attitudes).Research limitations/implicationsIt is difficult to conduct a purely objective ethnographic study because the investigation is controlled by its context. However, it is necessary to highlight and discuss the invisible level to improve understanding of negative attitudes and preconceptions related to the implementation of R&D in the clinical setting. Practical implications - By highlighting the invisible level of culture, the management of an organisation has the opportunity to initiate discussion of issues related to concealed norms and values as well as attitudes towards new thinking and change in the primary health context.Originality/valueThis paper is one of the very few studies to investigate the influence of organisational culture on the intentions of PCSM to engage in R&D.

    • Att gå från att vara ungdom till att bli vuxen kan innebära en rad förändringar, som att flytta hemifrån, börja studera eller arbeta, skapa nya relationeroch kanske träffa en partner. Men det handlar också om att möta högrekrav på att fatta egna beslut och ta ansvar för beslutens konsekvenser.Med andra ord: att etablera sig som vuxen innebär såväl möjligheter som utmaningar för unga människor. Detta gäller givetvis också för ungasom lever med olika typer och grader av funktionsnedsättning. Den här kunskapsöversikten redovisar forskningsbaserad kunskap om övergången från ungdoms- till vuxenliv för personer med funktionsnedsättning.

  • Prohibiting Sex Purchasing and Ending Trafficking : The Swedish Prostitution Law

    • The Swedish prostitution law from 1999, now followed by Norway and Iceland, criminalized the purchaser and decriminalized the prostituted person. This is analyzed as a cogent state response under international trafficking law, particularly to the obligations set forth in the United Nation’s Trafficking Protocol from 2000. The Protocol states that a person is regarded a trafficking victim when, e.g., someone abuses her “position of vulnerability” in order to exploit her. International jurisprudence and social evidence strongly suggest that prostitution, as practiced in the world, usually satisfies this definition. Further, the Protocol urges states to reduce the demand for prostitution and to protect and assist victims, for instance by adopting laws deterring purchasers of sex, and by supporting those exploited in prostitution. Policy makers, such as the U.S. Department of State, are criticized for taking an inadequate position in face of the growing evidence from the Swedish law's impact.The article shows that Sweden has significantly reduced the occurrence of trafficking in Sweden compared to neighboring countries. It also scrutinizes some misinformation of the law's impact, showing for instance that claims alleging a more dangerous situation for those still in prostitution after 1999 were unfounded. In addition, the article addresses remaining obstacles to the law's effective implementation, arguing that in order to realize the law's full potential to support escape from trafficking, the civil rights of prostituted persons under current law should be strengthened to enable them to claim damages directly from the purchasers for the harm to which they have contributed, and for the violation of the prostituted persons' equality and dignity - a position now recognized by the government to some extent by clarifying amendments made in 2011.

    • Fitness center activities have gained popularity. One central role fitness staff members have is to help participants to become or maintain health and fitness. In recent years, a new phenomenon – orthorexia nervosa, which M.D. Bratman coined in the late 1990s – has emerged. He termed orthorexia as "a fixation on eating healthy food". Research has mentioned that participants in sports and fitness activities may be at higher risk of becoming orthorexic. Since the late 1990s, orthorexia has been noticed, also in Swedish newspaper articles. In so doing, the 'condition' has evolved to include fanatic exercise and eating behaviours. Fitness professionals might thus work with individuals that are considered to suffering from orthorexia. The purpose of this presentation is to elucidate and problematize ethical issues that are raised during the research that deals with personal trainers and group fitness instructors talk of health and orthorexia. The material will consist of four focus groups with 18 fitness professionals that work in Swedish fitness centers. It will be carried out as a qualitative study. This study raises ethical issues, including for instance: a) by focusing and giving attention to orthorexia, the study may contribute to categorizing "the problem", as well as enlarge it. This paradoxical situation as the aim is to contribute scientific knowledge that can problematize orthorexia; b) the challenge to manage and balance a critical approach without causing harm (e.g. introduce orthorexia to fitness professionals who might have an erroneous perspective of and/or have little knowledge about orthorexia) (cf. Halse & Honey, 2005).
