    • Private and public eldercare Swedish care workers’ opinions on work environment and local government This article reports findings from a mail survey sent to all local politicians and employees in publicly financed eldercare services in eight Swedish municipalities in the year 2003 (response rate 72 percent among politicians and 66 percent among employees). The present article compares publicly (n=3046) and privately employed (n=498) care workers. Very few differences in the assessment of the work environment were found between publicly and privately employed care workers when controlling for sex, age, working hours, length of current employment and municipality. The conclusion is that private and public care workers experience no systematic, significant or crucial differences in work environment. By studying other possible aspects that could be related to private vs. public employment, it was found that in comparison with public care workers, privately employed care workers attributed significantly less influence to local politicians on issues such as the organization of care and work environment, municipal finances and quality of care services. Among public employees there was a consistent and comparatively strong negative opinion regarding out-sourcing of eldercare services, whereas private employees had a neutral or slightly positive opinion. Among private employees, the most positive opinion was found among those who had the most favourable perception of their work environment (with no significant relation to how they perceive the role of local politicians). On the other hand, no significant relation between work environment and opinion regarding out-sourcing was detected among public care workers. Most negative towards out-sourcing were those public employees who perceived local politicians as having a strong influence on the work environment. A tentative interpretation of these results is that private care workers with »good« working conditions regard their work environment as a result of out-sourcing, whereas public care workers with »poor« working conditions seem to have little hope that out-sourcing will improve their situation. Public care workers rather seem to hope that local politicians will play a more active role.

    • This article aims to contribute to comparative welfare state research by analysing the everyday work life of long-term care facility workers in Canadaand Sweden. The study’s empirical base was a survey of fixed and open-ended questions. The article presents results from a subset of respondents (care aides and assistant nurses) working in facilities in three Canadian provinces (n = 557) and across Sweden (n = 292). The workers’ experiences were linked to the broader economic and organisational contexts of residential care in the two jurisdictions.We found a high degree of country-specific differentiation of work organisation:Canada follows a model of highly differentiated task-oriented work, whereasSweden represents an integrated relational care work model. Reflecting differences in the vertical division of labour, the Canadian care aides had more demanding working conditions than their Swedish colleagues. The consequences of these models for care workers, for older people and for their families are discussed.

    • Care services for elderly and disabled persons: different conditions, different trends? Care services for disabled and elderly persons are essential parts of the Nordic welfare states. While these services are often not separated in statistics and research, a comparison of the two services reveals substantial differences in the case of Sweden. This article focuses the recent developments of Swedish elder care and disability care; the changing roles of the state, the family and the market; and the consequences of the changes for all parties concerned: the elderly and disabled persons, their next of kin and the care staff. The analysis shows that the financial resources for elder care have declined in relation to the increasing number of old people, while the resources for disability care have increased substantially. The coverage of services has decreased among elderly people and increased among the younger. The care workers in elder care and disability care report very different working conditions regarding their workload and the possibility to meet the needs of the care recipients. Private providers (mainly for-profit companies) of publicly financed care services have enlarged their share in both services, but are far more common in disability care than in elder care. The boundary line between formal and informal care has changed in both groups, but partly in opposite directions. There is a trend of informalisation among elderly people with larger as well as smaller care needs, especially among elderly with lower education. Among less disabled younger persons there is a similar trend of informalisation, while there is an opposite trend towards more formal care services among disabled persons with large needs of assistance and support. In relation to the Nordic welfare state model, care services for elderly and for disabled people in Sweden seem to be moving in different directions. An increasing number of disabled people with extensive care needs can lead their lives with greater autonomy and less family dependency than previously. Among frail elderly people, decreasing public support and increasing, often coerced, family dependency, might instead be a sign of a departure from the Nordic model.

    • This thesis examines the extent of family care for older people, primarily filial care, and the costs of caring in the Swedish welfare state. Costs of caring are understood as the negative effects of caregiving, primarily on the caregivers’ working life. The analysis is inspired by feminist theories on the importance of welfare state provisions for care for women’s citizenship, including personal autonomy and economic independence.The main aims of this thesis are twofold. The first is to explore the extent and development of family care for older persons in Sweden, primarily filial care, and the consequences of caregiving for well-being and working life. The second is to explore how older persons’ family members have been represented and the possible consequences of these representations for the development of publicly financed eldercare services and other forms of support for family carers, as well as for family members’ living conditions.The thesis consists of four studies. The first reviews the literature concerning the extent and consequences of family caregiving for older persons and the welfare state’s policy responses to older people’s care needs. The second study analyses how older persons’ family members and their role in eldercare have been represented in Swedish eldercare policy since the 1950s. The third study analyses surveys to explore changes during the 2000s in the role of the family, the public sector and the market in providing care for older persons in Sweden. The fourth study is a survey analysis of the extent, content and consequences of filial care among middle-aged women and men in Sweden in 2013.The policy analysis found that the expansion of eldercare was motivated solely in relation to older persons’ needs; thus working daughters’ needs of eldercare have been a blind spot in Swedish eldercare policy.Since 2000, every fourth residential care bed has disappeared and the increase in homecare services did not fully compensate for the decline, resulting in a significant increase in filial care in all social groups, and among both sons and daughters. Daughters of older persons with shorter education, however, remained the primary providers of filial care.Both daughters and sons are affected by caregiving. They suffer to the same extent from difficulties in managing to accomplish their work tasks and taking part in meetings, courses and travels. They are also equally likely to reduce their working hours and to quit their job. It is however clearly more common that daughters experience mental and physical strain, difficulties in finding time for leisure and reduced ability to focus on their job. Although more daughters than sons retire earlier than planned due to filial care, this is very rare.Managerial care (handling contacts with health and eldercare services) has a more salient role in a welfare state such as Sweden, with generously provided care services, less intense filial care and high employment rates among both sexes. The high labour force participation however makes middle aged children more vulnerable when their parents’ care arrangement does not work. The decline in eldercare services since 1980 has reinforced co-ordination problems in health and eldercare services. The managerial care required to handle this development, while living up to the demands of work and family life, stands out as especially demanding for the well-being and working lives of daughters.

    • Market-oriented restructurings of long-term care policies contribute significantly to the aggravation of care workers’ situations. This article focuses on the effects of broader long-term care policy developments on market-oriented reforms. Germany, Japan and Sweden are three countries that have introduced market-oriented reforms into home-based care provision embedded in distinct long-term care policy developments. Conceptually, this article draws on comparative research on care to define the institutional dimensions of long-term care policies. Empirically, the research is based on policy analyses, as well as on national statistics and a comparative research project on home-care workers in the aforementioned countries. The findings reveal the mediating impact of the extension and decline of long-term public care support and the corresponding development of the care infrastructure on both the restructuring of care work and the assessments of the care workers themselves.

  • Marketization in Long-Term Care : A Cross-Country Comparison of Large For-Profit Nursing Home Chains

    • This article presents cross-country comparisons of trends in for-profit nursing home chains in Canada, Norway, Sweden, United Kingdom, and the United States. Using public and private industry reports, the study describes ownership, corporate strategies, costs, and quality of the 5 largest for-profit chains in each country. The findings show that large for-profit nursing home chains are increasingly owned by private equity investors, have had many ownership changes over time, and have complex organizational structures. Large for-profit nursing home chains increasingly dominate the market and their strategies include the separation of property from operations, diversification, the expansion to many locations, and the use of tax havens. Generally, the chains have large revenues with high profit margins with some documented quality problems. The lack of adequate public information about the ownership, costs, and quality of services provided by nursing home chains is problematic in all the countries. The marketization of nursing home care poses new challenges to governments in collecting and reporting information to control costs as well as to ensure quality and public accountability.
