    • Private and public eldercare Swedish care workers’ opinions on work environment and local government This article reports findings from a mail survey sent to all local politicians and employees in publicly financed eldercare services in eight Swedish municipalities in the year 2003 (response rate 72 percent among politicians and 66 percent among employees). The present article compares publicly (n=3046) and privately employed (n=498) care workers. Very few differences in the assessment of the work environment were found between publicly and privately employed care workers when controlling for sex, age, working hours, length of current employment and municipality. The conclusion is that private and public care workers experience no systematic, significant or crucial differences in work environment. By studying other possible aspects that could be related to private vs. public employment, it was found that in comparison with public care workers, privately employed care workers attributed significantly less influence to local politicians on issues such as the organization of care and work environment, municipal finances and quality of care services. Among public employees there was a consistent and comparatively strong negative opinion regarding out-sourcing of eldercare services, whereas private employees had a neutral or slightly positive opinion. Among private employees, the most positive opinion was found among those who had the most favourable perception of their work environment (with no significant relation to how they perceive the role of local politicians). On the other hand, no significant relation between work environment and opinion regarding out-sourcing was detected among public care workers. Most negative towards out-sourcing were those public employees who perceived local politicians as having a strong influence on the work environment. A tentative interpretation of these results is that private care workers with »good« working conditions regard their work environment as a result of out-sourcing, whereas public care workers with »poor« working conditions seem to have little hope that out-sourcing will improve their situation. Public care workers rather seem to hope that local politicians will play a more active role.

    • This article aims to contribute to comparative welfare state research by analysing the everyday work life of long-term care facility workers in Canadaand Sweden. The study’s empirical base was a survey of fixed and open-ended questions. The article presents results from a subset of respondents (care aides and assistant nurses) working in facilities in three Canadian provinces (n = 557) and across Sweden (n = 292). The workers’ experiences were linked to the broader economic and organisational contexts of residential care in the two jurisdictions.We found a high degree of country-specific differentiation of work organisation:Canada follows a model of highly differentiated task-oriented work, whereasSweden represents an integrated relational care work model. Reflecting differences in the vertical division of labour, the Canadian care aides had more demanding working conditions than their Swedish colleagues. The consequences of these models for care workers, for older people and for their families are discussed.

  • Omsorgsarbetets vardag och villkor i Sverige och Danmark : Ett feministiskt kritiskt perspektiv

    • The present study analyses the welfare state as employer by studying eldercare workers’ experiences of their work in Sweden and Denmark.  The Nordic welfare states are often described as potentially women-friendly due to the availability of publicly provided services that enable women to combine paid work and caring responsibilities.  Whereas this might be empowering for a large group of women, paid care workers are often neglected in the discussion. The theoretical point of departure is Nancy Fraser’s dualistic model of gender justice, which encompasses redistribution (of material resources) and recognition (in the form of social status).By utilising survey-data (NORDCARE) on Swedish (n=532) and Danish (n=732) eldercare workers, the study shows that care recipients have larger needs and working conditions are more arduous in Sweden. However, in both countries workers report deficiencies with regard to insufficient resources, such as lack of staff, limited opportunities for development and training, and lack of necessary equipment for lifting service users, of support from managers and of reasonable time for the tasks to be performed. The pressure at work makes the care workers feel inadequate in relation to quality of care they are able to offer.The differences in job strain between the countries turn out to be of little importance when the care workers’ experiences of bodily and mental fatigue are compared. The bodies of the care workers are their main working tool and thus the bearer of the working conditions. More than 60 per cent of the respondents state that they often are physically tired after the day's work, and two-fifths of the respondents have seriously considered leaving their job during the past year. In both countries, the experience of physical and mental fatigue and the number of sick days over the past year are important factors behind thoughts about quitting the job.Using a dualistic model of gender justice, where redistribution and recognition are theorised as overlapping analytical dimensions, the results are interpreted as continuous organisational shortcomings which make the care workers’ everyday work invisible, and in the long run imply a risk for their health.

    • Care services for elderly and disabled persons: different conditions, different trends? Care services for disabled and elderly persons are essential parts of the Nordic welfare states. While these services are often not separated in statistics and research, a comparison of the two services reveals substantial differences in the case of Sweden. This article focuses the recent developments of Swedish elder care and disability care; the changing roles of the state, the family and the market; and the consequences of the changes for all parties concerned: the elderly and disabled persons, their next of kin and the care staff. The analysis shows that the financial resources for elder care have declined in relation to the increasing number of old people, while the resources for disability care have increased substantially. The coverage of services has decreased among elderly people and increased among the younger. The care workers in elder care and disability care report very different working conditions regarding their workload and the possibility to meet the needs of the care recipients. Private providers (mainly for-profit companies) of publicly financed care services have enlarged their share in both services, but are far more common in disability care than in elder care. The boundary line between formal and informal care has changed in both groups, but partly in opposite directions. There is a trend of informalisation among elderly people with larger as well as smaller care needs, especially among elderly with lower education. Among less disabled younger persons there is a similar trend of informalisation, while there is an opposite trend towards more formal care services among disabled persons with large needs of assistance and support. In relation to the Nordic welfare state model, care services for elderly and for disabled people in Sweden seem to be moving in different directions. An increasing number of disabled people with extensive care needs can lead their lives with greater autonomy and less family dependency than previously. Among frail elderly people, decreasing public support and increasing, often coerced, family dependency, might instead be a sign of a departure from the Nordic model.

    • The purpose of this thesis is to analyse the work situation in Swedish homecare from the perspective of care workers. In recent decades, the eldercare sector has faced challenges related to organisational changes and difficulties with retaining and recruiting staff. Due to an ageing population, increasing needs for care and a decline in residential care there has been an increased focus on ageing-in-place policies. Homecare is thus an increasingly important part of eldercare, both in Sweden and in other Western countries.The thesis consists of four studies that explore and analyse different aspects of the work situation in Swedish homecare. The empirical material consists of both quantitative and qualitative data. The quantitative part of the thesis derives from survey-data (Nordcare) from 2005 and 2015, whilst the qualitative material consists of 11 group interviews with 64 homecare workers. Theoretically, the thesis draws upon feminist perspectives on care work. In order to deepen the analysis of the work situation in homecare, the thesis joins work-life oriented research with feminist care research. The results are related to and analysed against the backdrop of recent structural changes and reorganisations in the eldercare sector.Study I shows that the work situation in Swedish home care has deteriorated between 2005 and 2015, both regarding intensification of work and the occurrence of work-related problems. For example, workers in 2015 reported meeting a larger number of recipients per day, receiving less support from their supervisor and colleagues, and having considerably less autonomy to affect the planning of their daily work. Study II illustrates geographical differences regarding homecare workers experiences of their work situation. Staff in larger cities experience worse working conditions and more mental strain, but better employments conditions, work-time arrangements and work-life balance, compared to those working in rural areas. Study III shows that homecare workers in Sweden experience a high level of multidimensional precariousness, and that several of the problematic working conditions are associated with physical strain, mental strain, poor work-life balance and considerations to quit the job. The main results of study IV are that the workers attribute their experiences of time pressure to the compression of time, the control of time and the unpredictability of time. Strategies adopted by the workers to handle time pressure include working unpaid hours, maximising efficiency, and reallocating time. Thus, the workers mainly adopt individual and short-term strategies to deal with time pressure, with potential negative consequences for both the workers and the care recipients.Overall, this thesis shows that the work situation in Swedish homecare has deteriorated over time and is characterised by several problematic employment- and working conditions, over a range of dimensions, with negative consequences for the workers’ health and wellbeing. The accumulation of work-related problems and multidimensional precariousness experienced by the workers strongly indicate that there is no easy solution to the issues experienced by the homecare workers, calling for wide-ranging political and organisational responses.

    • Even though nursing home care in Sweden and Canada takes place in different care contexts and utilizes different organisational models, in both countries, the traditional image of an ideal care worker is characterized by femininity and whiteness. However, today, in both countries, the care workforce is becoming far more diverse, with increasing numbers of white men, and non-white women and men.The overarching aim of this thesis is to study the meaning of gender and skin colour in the everyday life in Swedish and Canadian nursing homes. The study, based on observations and interviews, focuses on how organizational conditions contribute to creating, maintaining and challenging notions of gender and skin colour.The thesis consists of four articles, analysing the following questions:How is gender, in interaction with ethnicity and sexuality, expressed by women and men care workers in a Swedish nursing home? How do organizational conditions shape and influence men’s positions and their possibilities to be integrated and accepted as care workers in two Canadian nursing homes characterized by different care models?  How do organizational conditions impact on care workers’ strategies to handle skin colour, racism and language problems in Swedish and Canadian nursing homes characterized by different scope for care workers to exercise discretion? How do non-white men born outside Europe describe their experiences of working in Swedish nursing homes, and what does it mean for them to have a body that differs from the traditional image of a care worker?Overall, the findings demonstrate that an interaction of gender and skin colour shapes the workers’ experiences and position in the everyday life of care. The most profound impact is on those who deviate the most from the normative care worker, non-white men care workers. Significantly, the results also show that the ability for the organization to handle diversity is highly dependent on whether or not the workers are able to exercise discretion in their daily work. If these organizational conditions do not exist, there is a high risk that men and non-white workers will be seen as problematic by both co-workers and residents.In order to mitigate these risks and manage the increasing diversity of those working in nursing homes, it is essential to recognize the importance of organizational conditions that can create and ensure acceptance and integration of care workers from diverse backgrounds. Therefore, questions about gender and skin colour must be recognized as a priority for management, an organizational matter that should not be left to the individual or the work group to handle.
