  • "why do they need to check me?" patient participation through ehealth and the doctor-patient relationship : Qualitative study

    • Background: Roles in the doctor-patient relationship are changing and patient participation in health care is increasingly emphasized. Electronic health (eHealth) services such as patient accessible electronic health records (PAEHRs) have been implemented to support patient participation. Little is known about practical use of PAEHR and its effect on roles of doctors and patients. Objective: This qualitative study aimed to investigate how physicians view the idea of patient participation, in particular in relation to the PAEHR system. Hereby, the paper aims to contribute to a deeper understanding of physicians' constructions of PAEHR, roles in the doctor-patient relationship, and levels and limits of involvement. Methods: A total of 12 semistructured interviews were conducted with physicians in different fields. Interviews were transcribed, translated, and a theoretically informed thematic analysis was performed. Results: Two important aspects were identified that are related to the doctor-patient relationship: roles and involvement. The physicians viewed their role as being the ones to take on the responsibility, determining treatment options, and to be someone who should be trusted. In relation to the patient's role, lack of skills (technical or regarding medical jargon), motives to read, and patients' characteristics were aspects identified in the interviews. Patients were often referred to as static entities disregarding their potential to develop skills and knowledge over time. Involvement captures aspects that support or hinder patients to take an active role in their care. Conclusions: Literature of at least two decades suggests an overall agreement that the paternalistic approach in health care is inappropriate, and a collaborative process with patients should be adopted. Although the physicians in this study stated that they, in principle, were in favor of patient participation, the analysis found little support in their descriptions of their daily practice that participation is actualized. As seen from the results, paternalistic practices are still present, even if professionals might not be aware of this. This can create a conflict between patients who strive to become more informed and their questions being interpreted as signs of critique and mistrust toward the physician. We thus believe that the full potential of PAEHRs is not reached yet and argue that the concept of patient empowerment is problematic as it triggers an interpretation of "power" in health care as a zero-sum, which is not helpful for the maintenance of the relationship between the actors. Patient involvement is often discussed merely in relation to decision making; however, this study emphasizes the need to include also sensemaking and learning activities. This would provide an alternative understanding of patients asking questions, not in terms of "monitoring the doctor" but to make sense of the situation.

    • This paper is an investigation of how the specific design and materiality of the artefact of the boat, as well as the various material, visual, technical and geographical practices at work in the space of Mediterranean Sea, orientate a specific space and produce a selective politics of seeing, saving and framing of bodies on the move. It highlights how the very presence and movement of ‘unseaworthy boats’ in this actively orientated space of the sea brings to the fore the many strategies and techniques that have been employed to make it a space of European control. We argue that this is an active and deadly orientation carried out in an often dispersed number of practices and interventions within a seemingly flat space of water. The paper concludes that border transgressors’ act of moving by boat, with all of the losses involved, both challenge and potentially reorientate European mobility regimes.

  • A low-absorption x-ray energy filter for small-scale applications

    • We present an experimental and theoretical evaluation of an x-ray energy filter based on the chromatic properties of a prism-array lens (PAL). It is intended for small-scale applications such as medical imaging. The PAL approximates a Fresnel lens and allows for high efficiency compared to filters based on ordinary refractive lenses, however at the cost of a lower energy resolution. Geometrical optics was found to provide a good approximation for the performance of a flawless lens, but a field-propagation model was used for quantitative predictions. The model predicted a 0.29 ΔE/E energy resolution and an intensity gain of 6.5 for a silicon PAL at 23.5 keV. Measurements with an x-ray tube showed good agreement with the model in energy resolution and peak energy, but a blurred focal line contributed to a 29% gain reduction. We believe the blurring to be caused mainly by lens imperfections, in particular at the periphery of the lens.

  • A Tunable Energy Filter for Medical X-Ray Imaging

    • A multiprism lens (MPL) is a refractive X-ray lens, and its chromatic properties can be employed in an energy filtering setup to obtain a narrow tunable X-ray spectrum. We present the first evaluation of such a filter for medical X-ray imaging. The experimental setup yields a 6.6 gain of flux at 20 keV, and we demonstrate tunability by altering the energy spectrum to center also around 17 and 23 keV. All measurements are found to agree well with ray-tracing and a proposed geometrical model. Compared to a model mammography system with absorption filtering, the experimental MPL filter reduces dose 13–25% for 3–7 cm breasts if the spectrum is centered around the optimal energy. Additionally, the resolution is improved 2.5 times for a 5 cm breast. The scan time is increased 3 times but can be reduced with a slightly decreased energy filtering and resolution.

    • Recent molecular studies have identified substantial fungal diversity in indoor environments. Fungi and fungal particles have been linked to a range of potentially unwanted effects in the built environment, including asthma, decay of building materials, and food spoilage. The study of the built mycobiome is hampered by a number of constraints, one of which is the poor state of the metadata annotation of fungal DNA sequences from the built environment in public databases. In order to enable precise interrogation of such data – for example, “retrieve all fungal sequences recovered from bathrooms” – a workshop was organized at the University of Gothenburg (May 23-24, 2016) to annotate public fungal barcode (ITS) sequences according to the MIxS-Built Environment annotation standard (http://gensc.org/mixs/). The 36 participants assembled a total of 45,488 data points from the published literature, including the addition of 8,430 instances of countries of collection from a total of 83 countries, 5,801 instances of building types, and 3,876 instances of surface-air contaminants. The results were implemented in the UNITE database for molecular identification of fungi (http://unite.ut.ee) and were shared with other online resources. Data obtained from human/animal pathogenic fungi will furthermore be verified on culture based metadata for subsequent inclusion in the ISHAM-ITS database (http://its.mycologylab.org).

  • Soft Intelligence : Liquids Matter in Compliant Microsystems

    • Soft matter, here, liquids and polymers, have adaptability to a surrounding geometry. They intrinsically have advantageous characteristics from a mechanical perspective, such as flowing and wetting on surrounding surfaces, giving compliant, conformal and deformable behavior. From the behavior of soft matter for heterogeneous surfaces, compliant structures can be engineered as embedded liquid microstructures or patterned liquid microsystems for emerging compliant microsystems.Recently, skin electronics and soft robotics have been initiated as potential applications that can provide soft interfaces and interactions for a human-machine interface. To meet the design parameters, developing soft material engineering aimed at tuning material properties and smart processing techniques proper to them are to be highly encouraged. As promising candidates, Ga-based liquid alloys and silicone-based elastomers have been widely applied to proof-of-concept compliant structures.In this thesis, the liquid alloy was employed as a soft and stretchable electrical and thermal conductor (resistor), interconnect and filler in an elastomer structure. Printing-based liquid alloy patterning techniques have been developed with a batch-type, parallel processing scheme. As a simple solution, tape transfer masking was combined with a liquid alloy spraying technique, which provides robust processability. Silicone elastomers could be tunable for multi-functional building blocks by liquid or liquid-like soft solid inclusions. The liquid alloy and a polymer additive were introduced to the silicone elastomer by a simple mixing process. Heterogeneous material microstructures in elastomer networks successfully changed mechanical, thermal and surface properties.To realize a compliant microsystem, these ideas have in practice been useful in designing and fabricating soft and stretchable systems. Many different designs of the microsystems have been fabricated with the developed techniques and materials, and successfully evaluated under dynamic conditions. The compliant microsystems work as basic components to build up a whole system with soft materials and a processing technology for our emerging society.

  • Desiderata for developmental cognitive architectures

    • This paper complements Ron Sun’s influential Desiderata for Cognitive Architectures by focussing on the desirable attributes of a biologically-inspired cognitive architecture for an agent with a capacity for autonomous development. Ten desiderata are identified, dealing with value systems & motives, embodiment, sensorimotor contingencies, perception, attention, prospective action, memory, learning, internal simulation, and constitutive autonomy. These desiderata are motivated by studies in developmental psychology, cognitive neuroscience, and enactive cognitive science. All ten focus on the ultimate aspects of cognitive development — why a feature is necessary and what it enables — rather on than the proximate mechanisms by which it can be realized. As such, the desiderata are for the most part neutral regarding the paradigm of cognitive science — cognitivist or emergent — that is adopted when designing a cognitive architecture. Where some element of a desideratum is specific to a particular paradigm, this is noted.
