Supradiaphragmatic Presentation of Non‐Hodgkin's Lymphoma in Men Occupationally Exposed to Organic Solvents

- ABSTRACT. Using a standardized interview schedule, occupational exposure to organic solvents was investigated in 61 consecutive male patients with the diagnosis of non‐Hodgkin's lymphoma. When exposure was defined as handling of organic solvents every working day for at least one year, 23 patients were exposed and 38 non‐exposed. Of 15 patients with clinical stage I‐II above the diaphragm, 12 (80%) were occupationally exposed to organic solvents. Only 1 (11%) of 9 patients with clinical stage I‐II disease located below the diaphragm was exposed. Thirty‐seven patients had clinical stage III‐IV disease at the time of diagnosis and 10 (27%) of these were exposed. The predominance of exposed patients in the group with stage I‐II disease above the diaphragm is significant and the results indicate a relation between exposure to organic solvents and supradiaphragmatic presentation of non‐Hodgkin's lymphoma. No correlation was found between exposure and the morphology of the lymphomas. 1981 Association for the Publication of the Journal of Internal Medicine

The contralateral breast at reconstructive surgery after breast cancer operation--a histopathological study

- The present study concerns 73 patients with known unilateral breast carcinoma. Thirty of the patients had a primary invasive carcinoma removed and at a later operation contralateral subcutaneous mastectomy with implantation of a prosthesis. This was performed with or without ipsilateral breast reconstruction. Forty-three of the cases had an in situ carcinoma found by local excision, whereafter bilateral subcutaneous mastectomy was performed in 38 cases. Five cases had already had an ipsilateral mastectomy and contralateral subcutaneous mastectomy was performed. The histological examination of the subcutaneous mastectomy specimens was extensive with breasts cut into 3-5 mm slices, which were embedded and cut in large sections and cut in large sections allowing us to map all lesions. 42.5 per cent of the contralateral breasts contained invasive or in situ carcinoma. In about 70 per cent of the cases other histological lesions, considered more or less precancerous, were found in the contralateral breast. Our results speak in favor of an active approach to the contralateral breast at reconstruction, especially in cases with a long life expectancy after the first carcinoma. It is psychologically comforting to the patient to know that most of the breast gland, which could be the future origin of a new carcinoma, has been removed.

Preleukemic syndrome simulating SLE

- A 70-year-old man presented with symmetrical arthritis and arthralgias, Raynaud's phenomenon, pleurisy, fever, maculopapular erythema, leuko- and thrombocytopenia, anemia, antinuclear antibodies and hypocomplementemia. His bone marrow morphology was normal. During therapy with corticosteroids he developed pulmonary tuberculosis which responded well to tuberculostatic treatment. Approximately one year after onset of his initial symptoms, myeloblasts were seen in the blood and a few weeks later the bone marrow showed a myeloblastic leukemia. The patient did not respond to cytostatic treatment and died six weeks later. Although this patient presented symptoms suggesting the diagnosis of SLE, in retrospect his condition probably represented an unusual type of preleukemic syndrome

Human malignant melanoma heterotransplanted to nude mice

- Five different human malignant melanoma were heterotransplanted subcutaneously to nude mice. When small tissue pieces were used 3 out of 5 tumors grew. Subcutaneous injections of suspended tumor cells were also made, but all failed to take. Metastatic or infiltrative growth was never seen in the mice observed for up to 2.5 months. The successful grafts largely retained the original morphological features. The three successfully transplanted tumors could all be serially transfered with 100% tumor take. In one case passage time was reduced from 40 days to 15 days. As measured with 3H-thymidine incorporation the proliferation rate increased during the passages. These changes might be due to a selection of more rapidly growing tumor cells in the nudes.
