  • Therapeutic alliance : exploration of the construct validity of the concept
  • 2014
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • Background: Repeated meta-analyses have established that there exists a robust association between therapeutic alliance and outcome at end of treatment; still, there is a lack of coherent theory regarding the concept and lack of agreement on how to interpret and understand research results. Further support of the construct validity of the concept is needed. Aim: The aim of the present thesis was to explore the construct validity of the concept therapeutic alliance. Paper I aimed at studying the adaptation of an established alliance measure to a group psychotherapy setting. Paper II aimed at exploring the association between alliance and outcome in treatment for young adults. Paper III aimed at testing prediction and moderation of early alliance in psychodynamic psychotherapy (ORP) and dialectical behavior therapy (DBT). Paper IV aimed at predicting growth of alliance from relational personality variables. Methods: Paper I is based on data from a randomized controlled study on patients on sick leave diagnosed with work related depression and/or stress reactions participating in psychodynamic group psychotherapy (n = 18). Paper II and IV are based on data from a prospective, naturalistic outcome study on young adults receiving long-term psychoanalytic psychotherapy (n = 115, n = 79). Paper III is based on data from a randomized controlled study on female, suicidal patients diagnosed with borderline personality disorder (n = 59). Analyses were performed on data for patients for whom patients and/or therapists had filled out relevant questionnaires. Results: Study I: Reliability for the alliance questionnaire was acceptable. Average alliance was associated with outcome, while dismissing attachment, level of interpersonal problems, problems being exploitable, and problems being overly nurturant were associated with growth of alliance. Dismissing attachment was also predictive of early alliance. Paper II: Therapist-rated alliance was negatively associated with symptom change in patients with high levels of initial suffering. Paper III: Patients and therapists in DBT rated early alliance higher than their counterparts in ORP. Somatic anxiety moderated discrepancy in therapist/patient-rated alliance between the two treatments. Study IV: Two measures of object and self-representations predicted growth of patient-rated alliance, while interpersonal problems within the friendly-submissive domain predicted fluctuations in therapist-rated alliance. Conclusion: The present thesis found some support for the construct validity of the concept therapeutic alliance. However, the lack of coherent theory of the concept makes it difficult to interpret diverging results other than from a clinical perspective.

  • Long-term outcome and post-treatment effects of psychoanalytic psychotherapy with young adults
  • 2010
  • Ingår i: Psychology and Psychotherapy. - : Wiley-Blackwell. - 1476-0835 .- 2044-8341. ; 83:Pt 1, s. 27-43
  • Tidskriftsartikel (refereegranskat)abstract
    • OBJECTIVES: The short- and long-term effects of open-ended, long-term psychoanalytic psychotherapy for young adults were investigated. Possible changes during the year and a half follow-up, as well as predictors of change, were explored.DESIGN: Patients aged 18-25 years who accepted the offered psychoanalytic individual or group psychotherapy were included. Patients filled out questionnaires and were interviewed at intake, termination, and follow-up. Alliance data were collected after the second session of psychotherapy proper.METHODS: The primary outcome measures were the Symptom Checklist-90 and the Inventory of Interpersonal Problems. The Helping Alliance Questionnaire-II was used to measure alliance. Mixed model ANOVAs were used to analyse changeover time and prediction of change in relation to gender, treatment format, treatment duration, and in individual psychotherapy, therapist- and patient-rated alliance.RESULTS: All outcome measures changed significantly from intake to follow-up. None changed significantly during the follow-up period, but there was a tendency towards recurring symptoms and an improvement in one of the object relational measures during the follow-up. The latter was the only outcome measure that did not change significantly during treatment. Lower therapist-rated alliance was predictive of greater change in psychiatric symptoms for patients with high levels of symptoms at intake.CONCLUSIONS: The long-term effectiveness of psychoanalytic psychotherapy for young adults was supported. Low therapist-rated alliance implies that the therapists have identified problematic interactions, which might have mobilized their effort to solve the problems. Further research on cases reporting no gain or even deterioration is needed.

  • Electrochemistry of Heme Containing Enzymes - Fundamentals and Applications
  • 2000
  • Doktorsavhandling (övrigt vetenskapligt/konstnärligt)abstract
    • The thesis is a bioelectrochemical study of heme enzymes, mainly peroxidases and cellobiose dehydrogenases (CDH). It covers both fundamental and applied aspects, viz. studies of the direct electron transfer and kinetics of enzymes on electrodes, and the development of amperometric enzyme-based biosensors. The kinetics of the direct and mediated electron transfer of a wide range of peroxidases (both native and genetically engineered peroxidases) were investigated on graphite and gold electrodes using rotating disk electrode methodology. By genetic engineering of horseradish peroxidase some additional knowledge about the nature of direct ET was obtained. Some new approaches for oriented immobilisation of peroxidase were also investigated. Direct electron transfer of the multi-cofactor enzyme CDH was studied on gold and graphite electrodes with techniques such as voltammetry and spectroelectrochemistry. CDH can be cleaved into its two subunits, one containing the heme cofactor and the other the flavin cofactor, these two subunits were studied individually at the electrode surface to prove that the direct electron transfer proceeds via the heme group. Two biosensors for determination of (di-)phenolic compounds were developed, one using peroxidase and the other using CDH as the biological component.
