  • Health-related quality of life and everyday life for children with right ventricular outflow tract anomalies. Perspectives from children and parents.

    • Background: Children with right ventricular outflow tract (RVOT) anomaliestypically undergo surgical repair during their first months of life and thereafter they will need one or more reoperations through their life.Aims: The overall aim was to describe health-related quality of life (HRQoL) forchildren with congenital heart disease (CHD) with a special focus on children withRVOT anomalies and to explore children’s and their parents’ experiences ofeveryday life during assessment that could result in a new heart surgery.Methods: Two quantitative studies with HRQoL assessment for 337 children withCHD and 97 children with RVOT anomalies were performed. Another twoqualitative interview studies were conducted, in which nine children with RVOTanomalies and their parents were interviewed at three time points (at start of thepreoperative assessment, at the assessment decision and around 11 months after the decision).Results: Children with three or more surgeries had lower HRQoL than children with fewer heart surgeries. Children with RVOT anomalies rated their lowest HRQoL for cognitive function, as did their parents. The agreement for HRQoL between children with RVOT anomalies and their parents was strong for 13 of 22 items. The thematic analysis of the interviews with children resulted in three themes and eight subthemes: Me and my heart disease explores the children’s experiences of their heart disease; Being me explores their sense of self; Being placed in someone else’s hands describes how the assessment was a safety net for the child, at least until the decision for heart surgery. The interviews with parents resulted in five themes: The heart surgery keeps my child alive illuminates’ parents’ experiences related to the assessment: Everyday struggles illuminates the struggles parents had to face on behalf of their child in everyday life; Unconditional love, Trust in life and Togetherness illuminates the ways in which the parents gained inner strength in their everyday lives.Conclusion: By inviting both the child and their parents to participate in the child’s care, individually tailored support can be identified and given.

  • “I Dread the Heart Surgery but it Keeps My Child Alive”—Experiences of Parents of Children with Right Ventricular Outflow Tract Anomalies during the Assessment for Cardiac Reoperation

    • Background: Parents of children with complex right ventricular outflow tract (RVOT) anomalies are con-fronted with their child’s need for heart surgery early in life and repeated reoperations later on. Preoperative assessment needs to be performed whenever an indication for reoperation is suspected. The aim was to illuminate the experiences of parents of children diagnosed with RVOT anomalies, in particular, how they experience their child’s heart disease and everyday life during the assessment and after the decision on whether to perform a reo-peration. Method: Individual interviews (n = 27) were conducted with nine parents on three occasions between 2014 and 2016 and analyzed using reflexive thematic analysis. Results: The analysis resulted in the following five main coexisting themes: The heart surgery keeps my child alive illuminates parents’ experiences during and after the assessment and emphasizes that heart surgery, although dreaded, is central for their child’s survival; Everyday struggles illuminates the different struggles parents had to face to ensure that their child would be in the best possible condition; the remaining three themes, Unconditional love, Trust in life, and Togetherness, illuminate the ways in which the parents gained inner strength and confidence in their everyday lives. Conclusion: Although the parents were grateful for the assessment and had learned to navigate among the fears it aroused, they experi-enced several distressing situations during the assessment process that should be addressed. By inviting both the parents and their child to participate in the child’s care, individualized support can take into account the needs of both parents and child.

    • Many children with complex right ventricle outflow tract anomalies such as Tetralogy of Fallot, common arterial trunk, and pulmonary atresia with ventricular septal defect require repeated heart surgeries early in life, but also later throughout their lifetime, thereby emphasising the importance of careful life-long follow-up. The need for repeated heart surgeries during childhood is recognised as a heavy burden on the child. Optimising the time point for re-intervention is important, since delaying re-intervention for these children can lead to complications such as ventricular arrhythmias, heart failure, and death. To this purpose, thorough pre-operative assessment (henceforth named as assessment) including clinical examination, echocardiography, MRI, and exercise test need to be performed whenever the indication for reoperation is suspected. It is likely to believe that children who are going through this kind of assessment that may lead to heart surgery need extra support. According to previous research, children with complex heart disease fear for the possibility of surgery and the thought of future repeated heart surgery is associated with anxiety. This might have an impact on children's everyday life and in research nowadays involving children with CHD, the focus has changed from survival to how these children experience their everyday life. Earlier studies have shown that they experience physical activities limitation and feelings of isolation, but no study has yet studied how children with right ventricle outflow tract anomalies experience their everyday life. To be able to provide support, studies are needed to explore how these children experience the period from assessment to decision, as well as the months thereafter. Therefore, the aim of this study was to explore how children diagnosed with complex right ventricle outflow tract anomalies experience their heart disease and their everyday life during the assessment and after the decision on whether to perform a new cardiac surgery.

    • This dissertation focuses on two distinct areas of study that do not have a predetermined or necessary connection to each other. The first area concerns issues of more immediate relevance to the cultural history of the Icelandic sagas, whereas the second area concerns issues of more general relevance to cultural memory research (Cultural Memory Studies, Kulturelles Gedächtnis) and what the author perceives as a deficiency or “blind spot” in this theory. The latter area, in its turn, falls into two distinct yet related areas of investigation (or critique).In the field of Cultural Memory Studies – finding its most pregnant expression in the work of Jan and Aleida Assmann – there is a consistent and detrimental tendency to underestimate oral tradition. Much of this tendency can be attributed to the Assmann’s abidance by a written/textual norm. Because of this they have had difficulties handling mechanisms of oral transmission, which are often overlooked, misinterpreted, or underestimated. A telling example of this is the discussion of cultural storage and its importance for stability, creativity, and development within a given memory tradition, particularly prominent in the work of Aleida Assmann. Cultural memory theory particularly undervalues oral tradition when it comes to oral prose tradition. That is, the form of oral tradition that has the least in common with written cultural forms of expression. Cultural memory theory thus appears remarkably unable to account for the cultural dynamics of oral traditions as well as having difficulty handling texts with oral origins in a longer diachronic perspective. In the present investigation, this is considered a “blind spot” in the theory of cultural memory.Since this area of study has been largely unexplored within cultural memory research, the empirical basis for the development of such a theory is limited in scope. Nonetheless, the intention in this dissertation is to outline a theory of channels for the transmission of cultural memory in oral prose tradition, and to initiate a critical discussion on how well the existing framework within cultural memory studies is able to accommodate such a theory. What makes these new insights possible is the main empirical material that is taken as a starting point, namely the Icelandic sagas, which will be put to work as an empirical corrective, through the identification of traces of such processes in the saga texts (which are here perceived as a type of orally derived texts).The thesis maintained throughout this dissertation is strictly twofold. The first part is about establishing a fundamental theory of cultural memory processes in oral narrative tradition (“oral prose”). Above all, it is about identifying the oral transmission processes that have established channels for cultural memory transmission. I argue that these processes are partially analogous to those found in written transmission. These processes also entail certain types of stabilizing contextual factors that may be relevant in this context. The second part is about supplementing the memory theory’s view of cultural transmission and reproduction with less “fossilized” and more “living” types of transmission. This supplementary aim, in turn, requires a thorough discussion of several central terms in cultural memory theory. As a result, such an analysis has to involve the establishment of a diachronic cultural historical perspective on the saga literature. This ambition is part of a broader and somewhat loosely formulated goal of the dissertation.

    • This chapter aims to reflect about the power that is exercised in autobiographical accounts. Criminal policy ultimately shows how everyday life, identities, and biographies are constituted. The criminal policy of the Swedish welfare state was founded on the ideology of treatment and social engineering. Looking at the prison as a training camp helps me to see how the norms are impressed on people, the criminal identity is formed, and the frameworks for the biography are outlined. The internal punishments that occur in prisons, for example, infrequently lead to promotion in the criminal identity formation. The main instrument of power in the construction of outsiderhood was the biographies compiled of the inmates. The role of imprisonment in controlling and readjusting a small, deviant criminal minority is insignificant. The often punishment is used, the less effective it becomes. It erases all forms of moral responsibility in the person to whom it is applied.

  • Impact of protease and lipase activities on quality of Swedish raw milk

    • An increasing demand for dairy products with long shelf-life is foreseen. A limiting factor for maintaining high quality of these products is the activity of spoilage enzymes during storage. Lipase and protease activities in Swedish raw milk at farm and dairy level were investigated, analysing milk samples from three geographical regions and two seasons. Lipase activity in milk at farm level was affected by regional and seasonal variations, whereas at dairy level only season had an effect. Lipase activity was positively correlated with ionic calcium. For protease activity, no effect of either region or season was seen. Degradation products, e.g., free fatty acids, peptides and plasmin proteolysis products, varied differently between season and geographical origin at both farm and dairy level. The results indicate that lipase and protease activities are important for raw milk quality, while ionic calcium might be a future indicator for milk fat stability.

  • Health-Related Quality of Life in Children With Earlier Surgical Repair for Right Ventricular Outflow Tract Anomalies and the Agreement Between Children and Their Parents

    • Background: Children diagnosed with right ventricle outflow tract (RVOT) anomalies require surgical repair early in life, reoperations and lifelong follow-up. The aim is to comprehensively describe their health related quality of life (HRQoL) and to assess the agreement in this regard between children and parents. Methods and Results: Child- and parent-reported HRQoL was assessed in 97 children aged 8–18 years using three different HRQoL questionnaires. The mean age was 12.9 ± 3 years. The mean total score for the child report was lower in the PedsQL Cardiac Module than in the PedsQL 4.0 and DISABKIDS (p ≤ 0.001). The mean score for each domain in PedsQL Cardiac Module ranged between 67 (cognitive function) and 79 (physical appearance), and between 72 (school function) and 82 (physical and social function) in PedsQL 4.0. Nearly half of the children reported problems with shortness of breath during physical activity. In the PedsQL Cardiac Module the child-parent agreement was strong for 13 of 22 items. Conclusion: HRQoL problems as perceived by children with RVOT anomalies are best identified with the PedsQL Cardiac Module and relate mostly to cognitive and physical functioning. The agreement findings suggest the need to take into account both child- and parent reports in the assessment of HRQoL.
