  • Perception of affordable diet is associated with pre-school children's diet diversity in Addis Ababa, Ethiopia : the EAT Addis survey

    • Background: Despite improvements in food access and nutrition security over the last few decades, malnutrition remains a major public health problem. One of the significant contributors to these problems is affordability of nutritious food. This study aimed to examine the association between perceived food affordability and pre-school children's diet diversity in Addis Ababa, Ethiopia.Methods: Cross-sectional data from 2017 to 18 were used for the analysis. A 24-hour dietary recall assessment was done to assess children's dietary diversity (DD). We used a modified operational definition of affordability indicator called perceived affordability of dietary diversity (afford-DD) to evaluate the impact of the food environment in terms of affordability at the household level. A sample (n 4,898) of children aged 6-59 months representative of households in Addis Ababa was randomly selected using a multistage sampling procedure including all districts in the city. Mixed-effects linear regression models were used to assess the association between children's DD and afford-DD.Results: The survey revealed that the mean (standard deviation [SD]) of children's DD was 3.9 [±1.4] while the mean [SD] of afford-DD was 4.6 [±2.1]. Overall, 59.8% of children met the minimum dietary diversity (≥ 4 food groups). White roots and tubers were the most commonly consumed food groups regardless of their affordability. Considerable variations were observed between households that reported the food item affordable and not affordable in consumption of Vitamin A rich vegetables and fruits, meat and fish, egg, and dairy. The children's DD was positively associated with afford-DD after adjusting for maternal education, household wealth status and other relevant confounding. Higher maternal education modified the association between affordability and children's diet diversity.Conclusions: This study suggests higher perceived food affordability was associated with better diet diversity in children. A higher level of maternal education had the potential to mitigate affordability challenges in meeting the children's dietary diversity needs. Our study emphasizes the need for inclusive food programs and nutrition interventions addressing social differences, intensifying efforts to make nutrient-rich diets affordable for the less privileged, and highlights the potential benefits of targeting maternal education in addressing child dietary diversity.

  • Maternal betel quid use during pregnancy and child growth : a cohort study from rural Bangladesh

    • Background: Chewing betel quid (BQ) - a preparation commonly containing areca nut and slaked lime wrapped in betel leaf - is entrenched in South Asia. Although BQ consumption during pregnancy has been linked to adverse birth outcomes, its effect on postnatal growth remains largely unexplored.Objective: We examined the associations of BQ use during pregnancy with children's height-for-age and body mass index-for-age z-scores (HAZ and BAZ, respectively) and fat and fat-free mass along with sex-based differences in association in rural Bangladesh.Methods: With a prospective cohort design, we assessed BQ use among mothers enrolled in the Preterm and Stillbirth Study, Matlab (n = 3140) with a structured questionnaire around early third trimester. Children born to a subset of 614 women (including 134 daily users) were invited to follow-up between October 2021 and January 2022. HAZ and BAZ were calculated from anthropometric assessment, and fat and fat-free mass were estimated using bioelectric impedance. Overall and sex-specific multiple linear regression models were fitted.Results: Growth data were available for 501 children (mean age 4.9 years): 43.3% of them were born to non-users, 35.3% to those using prior to or less-than-daily during the survey, and 21.3% to daily users. No statistically significant associations were observed after adjusting for sex, parity, maternal height and education, and household wealth.Conclusions: There was no effect of BQ use during pregnancy on postnatal growth in this study. Longitudinal studies following up those born to heavy users beyond childhood are warranted for capturing long-term implications of prenatal BQ exposure.

  • High Degree of Desensitization After 1 Year of Early-Life Peanut Oral Immunotherapy : Small Children Oral Immunotherapy (SmaChO) Randomized Controlled Trial

    • Background: The prevalence of peanut allergy is about 2% and mostly lifelong. Studies of oral immunotherapy (OIT) with peanut (the daily oral intake of an initially low and then increasing dose of peanut) often show problematic side effects, but there are indications of better safety and effect in younger children compared with older children and adults.Objective: To determine the safety and effectiveness of peanut OIT with a slow up-dosing strategy and low maintenance dose in children aged 1 to 3 years who were allergic to peanut, through a 1-year interim analysis.Method: In a randomized controlled trial (2:1 ratio), 75 children, median age 31 months (interquartile range [IQR], 23-40 months) were assigned to receive peanut OIT (n = 50) or peanut avoidance (n = 25).Results: In the OIT and avoidance groups, 43 of 50 and 20 of 25 children, respectively, performed the 1-year open oral peanut challenge. A cumulative dose of 750 mg peanut protein after 1 year was tolerated by 72% (36 of 50 children) in the OIT group compared with 4% (1 of 25) in the avoidance group (P < .001). Median tolerated cumulative dose was 2,750 mg (IQR, 275-5,000 mg) peanut protein in the OIT group compared with 2.8 mg (IQR, 0.3-27.8 mg) in the avoidance group (P < .001). Of the doses administered at home during the first year of OIT, 1.4% resulted in adverse events and 79% were mild, and three doses of epinephrine were given at home to two individuals.Conclusion: In children aged 1 to 3 years, peanut OIT with the combination of slow up-dosing and low maintenance dose seems safe and effective after 1 year.

  • Staphylococcus aureus-derived factors promote human Th9 cell polarization and enhance a transcriptional program associated with allergic inflammation

    • T helper (Th) 9 cells, characterized by robust secretion of IL-9, have been increasingly associated with allergic diseases. However, whether and how Th9 cells are modulated by environmental stimuli remains poorly understood. In this study, we show that in vitro exposure of human PBMCs or isolated CD4 T-cells to Staphylococcus (S.) aureus-derived factors, including its toxins, potently enhances Th9 cell frequency and IL-9 secretion. Furthermore, as revealed by RNA sequencing analysis, S. aureus increases the expression of Th9-promoting factors at the transcriptional level, such as FOXO1, miR-155, and TNFRSF4. The addition of retinoic acid (RA) dampens the Th9 responses promoted by S. aureus and substantially changes the transcriptional program induced by this bacterium, while also altering the expression of genes associated with allergic inflammation. Together, our results demonstrate a strong influence of microbial and dietary factors on Th9 cell polarization, which may be important in the context of allergy development and treatment.

  • Dietary diversity and its relation with health among the older population in Thailand

    • The role of dietary diversity (DD) in the health status among older people has been recognised. However, the evidence from lower-and-middle-income countries is scarce. The aim of this thesis was to explore the food choice, and dietary diversity, the associated factors and the health consequences of low dietary diversity among older people in Thailand.Study I was a qualitative study, using focus group discussions and individual interviews of people aged 60 years and older informal caregivers in Samut Sakhon Province, Thailand. A deductive thematic analysis approach was used. Studies II–IV acquired the data from the fifth Thai National Health Examination Survey, which was conducted between 2013 to 2015. Dietary diversity score (DDS) comprised eight food groups, including grains, white roots and tubers; pulse and beans, nuts and seeds; dairy products; meat, poultry, or fish; eggs; vegetables; fruits; and fats and oils. Socio-demographic characteristics included age, sex, educational level, wealth index, place of residence and living arrangement. The risk of cardiovascular disease (CVD) was determined using the Thai CV risk score. The cardiometabolic risk factors included hypertension, diabetes mellitus (DM) and lipid profiles. The mortality data was extracted from the Vital Statistics System in 2021. The data analyses used linear, logistic and proportional hazard regressions, which were adjusted for the complex survey design. We found that older people’s food choices relied on several factors, including health, social factors, the food environment and the economic factors. Regarding DD, a higher educational level, a higher wealth index and living in an urban area had a positive association, whereas living alone had a negative association with DDS. Regarding health, DDS had a negative association with log-Thai CV risk score, hypertension, DM and log-TG levels, whereas DDS was positively associated with TC and LDL-C levels. Additionally, DDS was inversely associated with mortality. The association between DDS and mortality was stronger in people aged 70 years and older. Finally, there was an inverse association between DDS and mortality in the underweight subgroup, while there was a positive association between DDS and mortality in the overweight and obesity sub-groups.In conclusion, this thesis showed the associations between DD and several socio-demographic factors and health outcomes. However, the effect of DD on mortality varied, depending on the individual’s age and BMI. Thus, the nutritional policies aimed at improving DD should be focused on the specific target groups to reduce mortality.

    • Objective: To examine the association between dietary diversity (DD) and mortality among Thai older people and to investigate whether age, sex, and nutritional status modify this association.Methods: The national survey conducted from 2013 to 2015 recruited 5631 people aged > 60 years. Dietary diversity score (DDS) was assessed for the consumption of eight food groups using food frequency questionnaires. The Vital Statistics System provided the data on mortality in 2021. The association between DDS and mortality was analyzed by Cox proportional hazard model and adjusted for the complex survey design. Interaction terms between DDS and age, sex, and BMI were also tested.Results: The DDS was inversely associated with mortality (HR adj 0.98, 95%CI: 0.96–1.00). This association was stronger in people aged > 70 years (HR adj 0.93, 95%CI: 0.90–0.96 for aged 70–79 years, and HR adj 0.92, 95%CI: 0.88–0.95 for aged > 80 years). Inverse association between DDS and mortality was also found in the underweight older population (HR adj 0.95, 95%CI: 0.90–0.99). A positive association was found between DDS and mortality in the overweight/obese group (HR adj 1.03, 95%CI: 1.00–1.05). However, the interaction between the DDS with sex to mortality was not statistically significant.Conclusion:Increasing DD reduces mortality among Thai older people, especially in those above 70, and underweight. In contrast, an increase in DD also meant an increase in mortality among the overweight/obese group. Focus should be placed on the nutritional interventions aimed to improve DD for those 70 and over and underweight to reduce mortality.

  • Hyperglycemia and insulin infusion in pancreatoduodenectomy : a prospective cohort study on feasibility and impact on complications

    • BACKGROUND: Hyperglycemia is a risk factor for postoperative complications but its impact on outcome after pancreatoduodenectomy (PD) is scarcely studied. This prospective cohort study aimed to assess the effect of continuous insulin infusion on postoperative complications and blood glucose, as well as to evaluate the impact of hyperglycemia on complications, after PD. MATERIALS AND METHODS: One hundred patients planned for PD at Skåne University Hospital, Sweden were prospectively included for perioperative continuous insulin infusion and a historic cohort of 100 patients was included retrospectively. Median blood glucose levels were calculated and data on complications were analyzed and compared between the historic cohort and the intervention group as well as between normo- and hyperglycemic patients. RESULTS: Median glucose levels were significantly lower in the intervention group compared to the historic cohort up to 30 days postoperatively (median glucose 8.5 mmol/l (interquartile range 6.4-11) vs. 9.1 mmol/l (interquartile range 6.8-17) ( P =0.007)). No significant differences in complication rates were recorded between these two groups. The incidence of complications classified as Clavien ≥3 was higher in hyperglycemic patients (100 vs. 27%, P =0.024). Among hyperglycemic patients the prevalence of preoperative diabetes was higher compared to normoglycemic patients (52 vs.12%, P <0.001). In patients with a known diagnosis of diabetes, a trend, although not statistically significant, towards a lower incidence of postoperative pancreatic fistula grade B and C, as well as postpancreatectomy hemorrhage grade B and C, was seen compared to those without preoperative diabetes (6.8 vs. 14%, P =0.231 and 2.3 vs. 7.0%, P =0.238, respectively). CONCLUSION: Insulin infusion in the early postoperative phase after PD is feasible in a non-ICU setting and significantly decreased blood glucose levels. The influence on complications was limited. Preoperative diabetes was a significant predictor of postoperative hyperglycemia and was associated with a lower incidence of clinically significant postoperative pancreatic fistula.

    • Aims: Intermittently scanned continuous glucose monitoring (isCGM) systems have not been thoroughly evaluated during in-hospital stay, and there are concerns about accuracy during various conditions. Patients undergoing pancreatoduodenectomy have an increased risk of hyperglycaemia after surgery which is aggravated by parenteral nutrition therapy. This study aims to evaluate glycaemic control and safety during insulin infusion in a surgical non-ICU ward, using a hybrid glucose monitoring approach with isCMG and periodic point-of-care (POC) testing. Methods: We prospectively included 100 patients with a resectable pancreatic tumour. After surgery, continuous insulin infusion was initiated when POC glucose was > 7 mmol/l and titrated to maintain glucose between 7 and 10 mmol/l. Glucose was monitored with isCGM together with intermittent POC, every 3–6 h. Median absolute relative difference (MARD) and hypoglycaemic events were evaluated. Mean glucose was compared with a historic control (n = 100) treated with multiple subcutaneously insulin injections, monitored with POC only. Results: The intervention group (isCGM/POC) had significantly lower POC glucose compared with the historic control group (8.8 ± 2.2 vs. 10.4 ± 3.4 mmol/l, p < 0.001). MARD was 17.8% (IQR 10.2–26.7). isCGM readings were higher than POC measurements in 91% of the paired cases, and isCGM did not miss any hypoglycaemic event. About 4.5% of all isCGM readings were < 3.9 mmol/l, but only six events were confirmed with POC, and none was < 3.0 mmol/l. Conclusions: A hybrid approach with isCGM/POC is a safe and effective treatment option in a non-ICU setting after pancreatoduodenectomy.

  • Adolescent nutrition in rural Bangladesh : Studies on diet, cardiometabolic indicators, physical activity and fitness in the MINIMat cohort

    • Adolescent nutrition is crucial for curbing non-communicable disease burden among transitioning populations in resource-limited settings. This thesis explored the gender and socio-economic stratification of diet, physical activity (PA) and fitness, and the relationship of these modifiable dimensions of nutrition with cardiometabolic risk among adolescents of a birth cohort in rural Bangladesh. Data were collected during the 15-year follow-up of the MINIMat (Maternal and Infant Nutrition Interventions in Matlab) cohort through household survey incorporating a 24-hour dietary recall, accelerometry, fitness tests, anthropometric assessment and plasma assay. Statistical descriptive methods, cluster analysis and regression models were employed.The prevalence of inadequate dietary diversity (DD) was 42.3%. Nearly 83% adolescents consumed at least one ultra-processed or deep-fried item. Adolescents from the poorest and food insecure households were more likely to have inadequate DD. Cluster analysis identified four dietary patterns including one marked by fairly high DD concurrent with greater consumption of unhealthy foods. The odds of consuming unhealthy foods were substantially higher among the boys. The adolescents spent about 69% of the daily awake time being sedentary and another 7% in moderate-to-vigorous activity (MVPA). The girls were more sedentary and engaged in moderate-intensity PA more than boys; whereas total and vigorous-intensity activity (VPA), and fitness were higher among the boys. Higher socio-economic status was associated with higher sedentary time and lower PA and fitness. Small, negative associations of Fish- and Meat-dominant diet clusters with plasma triglyceride were observed among the boys. Sedentary time displayed weak, positive associations with waist circumference (WC) and insulin resistance. Greater MVPA and VPA, and higher muscular fitness were associated with lower levels of cardiometabolic indicators. The associations were notably strong for VPA and upper body muscular fitness, and generally more pronounced among the boys – except those with WC.The findings underscored a clear gender and socio-economic stratification of diet, PA and fitness alongside beneficial impacts of PA and muscular fitness on cardiometabolic risk profile at 15 years of age. VPA represents a potential target for preventive measures tailored to adolescents. Adolescent-responsive policies and continued efforts to alleviate poverty are needed for improving adolescent nutrition in rural Bangladesh.
