  • Prolonged vs Intermittent Infusions of β-Lactam Antibiotics in Adults with Sepsis or Septic Shock : A Systematic Review and Meta-Analysis

    • Importance: There is uncertainty about whether prolonged infusions of β-lactam antibiotics improve clinically important outcomes in critically ill adults with sepsis or septic shock. Objective: To determine whether prolonged β-lactam antibiotic infusions are associated with a reduced risk of death in critically ill adults with sepsis or septic shock compared with intermittent infusions. Data Sources: The primary search was conducted with MEDLINE (via PubMed), CINAHL, Embase, Cochrane Central Register of Controlled Trials (CENTRAL), and ClinicalTrials.gov from inception to May 2, 2024. Study Selection: Randomized clinical trials comparing prolonged (continuous or extended) and intermittent infusions of β-lactam antibiotics in critically ill adults with sepsis or septic shock. Data Extraction and Synthesis: Data extraction and risk of bias were assessed independently by 2 reviewers. Certainty of evidence was evaluated with the Grading of Recommendations Assessment, Development and Evaluation approach. A bayesian framework was used as the primary analysis approach and a frequentist framework as the secondary approach. Main Outcomes and Measures: The primary outcome was all-cause 90-day mortality. Secondary outcomes included intensive care unit (ICU) mortality and clinical cure. Results: From 18 eligible randomized clinical trials that included 9108 critically ill adults with sepsis or septic shock (median age, 54 years; IQR, 48-57; 5961 men [65%]), 17 trials (9014 participants) contributed data to the primary outcome. The pooled estimated risk ratio for all-cause 90-day mortality for prolonged infusions of β-lactam antibiotics compared with intermittent infusions was 0.86 (95% credible interval, 0.72-0.98; I2= 21.5%; high certainty), with a 99.1% posterior probability that prolonged infusions were associated with lower 90-day mortality. Prolonged infusion of β-lactam antibiotics was associated with a reduced risk of intensive care unit mortality (risk ratio, 0.84; 95% credible interval, 0.70-0.97; high certainty) and an increase in clinical cure (risk ratio, 1.16; 95% credible interval, 1.07-1.31; moderate certainty). Conclusions and Relevance: Among adults in the intensive care unit who had sepsis or septic shock, the use of prolonged β-lactam antibiotic infusions was associated with a reduced risk of 90-day mortality compared with intermittent infusions. The current evidence presents a high degree of certainty for clinicians to consider prolonged infusions as a standard of care in the management of sepsis and septic shock.     • Electric vehicle (EV) battery packs contain several parallel and series-connected cells and variations in leakage currents and cell characteristics result in heterogeneous discharge rates among cells, thus limiting the total energy delivery of the pack. Battery-integrated modular multilevel converters (BI-MMCs) increase the controllability of cells thereby improving the energy utilization of the battery pack. Design optimization for BI-MMC with phase-shifted modulation (PSPWM) showed that submodule (SM) DC-link capacitors designed to bypass the switching frequency components result in minimum total losses. However, this requires a large DC-link capacitor bank, which increases the system cost. An alternative modulation technique, nearest level modulation (NLM), characterized by low semiconductor switching frequency, is often preferred for MMCs with many SMs. The first contribution is an experimental loss comparison in an SM of a BI-MMC with PSPWM and NLM. The second contribution is investigating the impact of the size of DC-link capacitors on battery and capacitor losses for NLM. The experiments showed that the battery and capacitor losses are independent of the DC-link capacitor size when using NLM. Furthermore, NLM has lower total losses but higher battery losses than PSPWM. A single-phase 4-SM BI-MMC is used as the experimental platform for the comparison.

  • A microporous polymer based on nonconjugated hindered biphenyls that emits blue light

    • Microporous organic polymers that have three-dimensional connectivity stemming from monomers with tetrahedral or tetrahedron-like geometry can have high surface areas and strong fluorescence. There are however few examples of such polymers based on hindered biaryls, and their fluorescence has not been studied. Hypothesizing that the contortion in a hindered biphenyl moiety would modulate the optical properties of a polymer built from it, we synthesized a meta-enchained polyphenylene from a 2,2ʹ,6,6ʹ-tetramethylbiphenyl-based monomer, in which the two phenyl rings are nearly mutually perpendicular. The polymer was microporous with SBET = 495 m2 g−1. The polymer absorbed near-UV light and emitted blue fluorescence despite the meta-enchainment that would have been expected to break the conjugation. A related copolymer, synthesized from 2,2ʹ,6,6ʹ-tetramethylbiphenyl-based and unsubstituted biphenyl-based monomers, was microporous but not fluorescent. © The Author(s) 2024.

  • Patient safety of adjunct pre-operative intravenous S-ketamine for pain relief in third molar surgery - a randomised, placebo-controlled, double-blind trial

    • Purpose: To study patient safety in third molar surgery, where two different doses of S-ketamine were administered for pain relief and compared to a placebo (saline). The primary focus was capillary oxygen saturation of the blood (SpO(2)) and secondarily, alterations in respiratory rate, blood pressure, pulse or adverse events. Methods: One hundred and sixty-eight subjects were included in a randomised, placebo-controlled, double-blind trial. The two subanaesthetic study drugs were low-dose S-ketamine (0.125 mg/kg) and high-dose S-ketamine (0.25 mg/kg). Every patient was sedated with midazolam prior to infusion of the investigational drug. The teeth were surgically removed according to a routine clinical procedure, under local anaesthesia. Results: Primary end-point for the safety aspects was capillary oxygen saturation (SpO(2)) after administration of the investigational drug was finished. A significant difference was found between the placebo and the high-dose group at that point (p = .021), with a decrease of saturation in the high-dose group. The lowest saturation and the number of registrations of SpO(2) <90% did not show any difference between groups. Oxygen supplementation was given in circa 40% of the cases with no differences between the intervention groups. No other significant differences between groups regarding saturation or respiratory rate were noted. Conclusion: In this study, it was safe to use adjunct preoperative single-dose intravenous S-ketamine 0.25 mg/kg body weight for pain relief, in midazolam-sedated patients receiving third molar surgery. There were no serious adverse events or symptoms of overdose nor any clinically relevant effects on circulatory or respiratory parameters.

  • Studies of the Grid Connection of Offshore Renewable Energy Sources : Technologies and Simulations

    • A substantial increase in renewable energy sources connected to the electrical grid is imperative to achieve net-zero emissions from the electricity sector. Marine energy sources, like marine current power and wave power, have the potential to significantly contribute to the increase of electricity from renewable energy sources. A crucial aspect of enabling marine energy utilization is the development of electrical systems for offshore renewable energy. Hence, this thesis addresses challenges regarding the grid connection of offshore renewable energy.Two important questions for offshore renewable energy are how to construct the offshore electrical grid and how to transmit the power to the shore. This thesis provides a review of AC and DC collection grid topologies and compares HVAC and HVDC transmission for offshore applications. It is concluded that HVDC is the preferred technology for transmission distances exceeding 50 to 100 km.Regardless of the configuration of the offshore collection grid, the energy converters must be connected to the collection and distribution grid. Uppsala University has deployed a marine current energy converter in the river Dalälven in Söderfors, Sweden. The grid connection system at the test site is based on a back-to-back converter technology. In the thesis, a simulation model of the grid connection system of the energy converter is presented. The simulation model is used to evaluate MPPT methods for marine current power. An advanced hydrodynamic model based on a two-dimensional free vortex method is utilized for this purpose. Additionally, a low-complexity hydrodynamic model is incorporated into the simulation model to assess electrical grids for marine current energy. One AC and one DC collection grid, each comprising five marine current energy converters, are compared. Furthermore, three DC collection grids, each with ten marine current energy converters, are assessed and compared.The grid connection system at the Söderfors test site includes an LC filter connected to a power transformer. A novel transfer function is derived for this system, and the transfer function is verified with simulations and experimental investigations. It is shown that the derived transfer function accurately captures the frequency response of the experimental system.

    • Several technological challenges delay the adoption of electrified powertrains in the heavy-duty transport sector. For fuel-cell hybrid electric trucks, key issues include slow cold start, reduced cooling power during high ambient temperatures, and uncertainties regarding durability. In addition, the engineers must handle the complexity of the system. In this article, a Matlab/Simulink library is introduced, which has been developed to aid engineers in the design and optimization of energy management systems and strategies of this complex system that consider mechanical, electrochemical, and thermal energy flows. The library is introduced through five example vehicle models, and through case studies that highlight the various kinds of analysis that can be performed using the provided models. All library code is open source, open for commercial use, and runs in Matlab/Simulink without any need for external libraries.

  • A Physics-Based Dynamic Model for the Rolling Resistance Considering Thermal States and Conditions

    • Planning for charging in transport missions is vital when commercial long-haul vehicles are to be electrified. In this planning, accurate range prediction is essential so the trucks reach their destinations as planned. The rolling resistance significantly influences truck energy consumption, often considered a simple constant or a function of vehicle speed only. This is, however, a gross simplification, especially as the tire temperature has a significant impact. At 80 km/h, a cold tire can have three times higher rolling resistance than a warm tire.A temperature-dependent rolling resistance model is proposed. The model is based on thermal networks for the temperature at four places around the tire. The model is tuned and validated using rolling resistance, tire shoulder, and tire apex temperature measurements with a truck in a climate wind tunnel with ambient temperatures ranging from -30 to 25 °C at an 80 km/h constant speed. Dynamic tire simulations were conducted using a heat transfer model, considering road, ambient, shoulder, and apex temperatures. The simulation results were compared with measured data for ambient, shoulder, and apex temperatures, and the model captures both time constants and stationary levels. The resulting model can predict the dynamics of the rolling resistance and will, therefore, give a more accurate prediction when tires are cold and warming up. Driving range simulations of a long haulage battery-electric truck have also been conducted demonstrating how the range changes with varying ambient temperatures as well as the influence a snapshot consumption has on range estimation.

    • Crystallographic investigations of eight homoleptic N,N '-dimethylpropyleneurea (dmpu) coordinated metal ions in the solid state, [Mg(dmpu)(5)]I-2 (1), [Ca(dmpu)(6)]I-2 (2), [Ca(dmpu)(6)](ClO4)(2) (3), [Ca(dmpu)(6)](CF3SO3)(2) (4), [Sr(dmpu)(6)](CF3SO3)(2) (5), [Ba(dmpu)(6)](CF3SO3)(2) (6), [Sc(dmpu)(6)]I-3 (7), and [Pr(dmpu)(6)]I(I-3)(2) (8), and the complex [CoBr2(dmpu)(2)] (9) as well as the structures of the dmpu coordinated calcium, strontium, barium, scandium(III) and cobalt(II) ions and the cobalt(II) bromide complex in dmpu solution as determined by EXAFS are reported. The methyl groups in the dmpu molecule are close to the oxygen donor atom, causing steric restrictions, and making dmpu space-demanding at coordination to metal ions. The large volume required by the dmpu ligand at coordination contributes to crowdedness around the metal ion with often lower coordination numbers than for oxygen donor ligands without such steric restrictions. The crowdedness is seen in M & ctdot;H distances equal to or close to the sum of the van der Waals radii. To counteract the space-demand at coordination, the dmpu molecule has an unusual ability to increase the M-O-C bond angle to facilitate as large coordination numbers as possible. M-O-C bond angles in the range of 125-170 degrees are reported depending on the crowdedness caused by the coordination figure and the M-O bond distance. All reported structures of dmpu coordinated metal ions in both the solid state and dmpu solution are summarized to study the relationship between the M-O-C bond angle and the crowdedness around the metal ion. However, highly symmetric complexes seem to be favoured in the solid state due to favourable lattice energies. As a result, the dmpu coordinated lanthanoid(III) ions are octahedral in the solid state, while they, except lutetium, are seven-coordinate in the dmpu solution.

  • Health-related quality of life among adult patients with cancer in Uganda : a cross-sectional study

    • ObjectivesThe study aimed to investigate the prevalence and factors associated with poor health-related quality of life in adults with cancer in Uganda.MethodsThis cross-sectional study surveyed 385 adult patients (95% response rate) with various cancers at a specialised oncology facility in Uganda. Health-related quality of life was measured using the EORTC QLQ-C30 in the Luganda and English languages. Predetermined validated clinical thresholds were applied to the instrument in order to identify patients with poor health-related quality of life, that is, functional impairments or symptoms warranting concern. Multivariable logistic regression was used to identify factors associated with poor health-related quality of life in six subscales: Physical Function, Role Function, Emotional Function, Social Function, Pain and Fatigue.ResultsThe mean age of the patients was 48 years. The majority self-reported poor functioning ranging between 61% (Emotional Function) to 79% (Physical Function) and symptoms (Fatigue 63%, Pain 80%) at clinically concerning levels. These patients were more likely to be older, without formal education and not currently working. Being an inpatient at the facility and being diagnosed with cervical cancer or leukaemia was a predictor of poor health-related quality of life.ConclusionImprovement of cancer care in East Africa requires a comprehensive and integrated approach that addresses various challenges specific to the region. Such strategies include investment in healthcare infrastructure, for example, clinical guidelines to improve pain management, and patient education and support services.

    • Online acoustic emission (AE) sensing is a nondestructive method that has the potential to be an indicator of battery health and performance. Rechargeable batteries exhibit complex mechano-electrochemical behaviors during operation, such as electrode expansion/contraction, phase transition, gas evolution, film formation, and crack propagation. These events emit transient elastic waves, which may be detected by a piezoelectric-based sensor attached to the battery cell casing. Research in this field is active and new findings are generated continuously, highlighting its potential and importance of further research and development. This Review provides a comprehensive analysis of AE sensing in rechargeable batteries, aiming to describe the underlying mechanisms and potential applications in battery monitoring and diagnostics. This Review summarizes recent progress and discusses future perspectives in applying online acoustic emission sensing as a non-destructive method for monitoring rechargeable batteries.
