    • Purpose: This study investigates how contracts with OHS-providers in Sweden are established and implemented  to explore i) the procurement process ii) contractual terms and conditions and iii) processes for implementing and evaluating the services provided.Methods: Review and analysis of 17 OHS contracts using both quantitative and qualitative approaches. Follow-up interviews were conducted with Human Resource managers, management, health and safety representatives and OHS professionals.Results: Contracts with OHS providers were mainly drawn up by HR departments. First-line managers or health and safety representatives were not involved. The contracts were not integrated with the companies’ occupational health and safety management. The organisations lacked knowledge on when or why to use services from their OHS provider, which promoted reactive rather than preventive interventions. Terms and conditions of contracts were found to be quite irrelevant to what services that were actually utilised.Conclusions: Important factors in creating conditions for a more preventive and group-oriented use of OHS expertise could be 1) the inclusion of first-line managers and health and safety representatives in needs analysis and implementation processes, 2) the definition of relevant, achievable and measurable goals regarding the collaboration and 3) the arrangement of regularly meetings with the OHS-provider.

  • Minimizing contrast media dose in CT pulmonary angiography with high-pitch technique

    • OBJECTIVES: To perform CT pulmonary angiography (CTPA) using a minimal amount of iodinated contrast media.METHODS: 47 patients (25 females) with mean age 69 years (range 41-82 years) referred for contrast-enhanced chest CT were prospectively included in this Phase IV clinical drug trial. All participants underwent a study specific CTPA in addition to the chest CT. The participants received 80 mg I/kg body weight Iohexol contrast media using a preparatory saline bolus, a dual flow contrast/saline bolus and a saline flush, and a scanner protocol with 80 kVp dual source high-pitch mode. Three readers independently assessed the image quality on the 3-point scale non-diagnostic, adequate or good-excellent image quality. Additionally, the pulmonary arterial contrast opacification was measured.RESULTS: On average, the patients received 16.8 ml Iohexol 350 mg I/mL (range 12-20 ml). Mean patient weight was 71 kg (range 50-85 kg). Identically for all readers, pulmonary embolism (PE) was detected in 1/47 participants. The median number of examinations visually scored concerning pulmonary embolism as good-excellent was 47/47 (range 44-47); adequate 0/47 (0-3) and non-diagnostic 0/47 (range 0-0). The proportion adequate or better examinations was for all readers 47/47, 100% [95% confidence interval 92-100%]. The mean attenuation ± standard deviation in the pulmonary trunk was 325 ± 72 Hounsfield unit (range 165-531 Hounsfield unit).CONCLUSIONS: Diagnostic CTPA with 17 ml contrast media is possible in non-obese patients using low kVp, high pitch and carefully designed contrast media administration.ADVANCES IN KNOWLEDGE: By combining several procedures in a CTPA protocol, the contrast media dose can be minimized.

    • BACKGROUND: Employers are required to get expert advice whenever needed to ensure a safe work environment. Providers of Occupational Health Services (OHS) could be such experts, but their services are usually used to provide health-related support to individuals, not preventive Occupational Health and Safety Management (OHSM) or other group-focused interventions. OBJECTIVE: To investigate how contracts with OHS providers in Sweden are established and implemented. METHODS: Written OHS contracts were reviewed, and follow-up interviews were conducted with Human Resource (HR) managers, management, safety representatives, and OHS professionals in seven organizations. RESULTS: Generally, the HR departments drew up the contracts with the OHS providers. The contracts were not integrated with the companies' occupational health and safety management. Managers lacked knowledge on how to utilize services offered by their OHS provider. Terms and conditions of contracts were found to be inconsistent with services actually utilized. CONCLUSIONS: The procurement and implementation process promotes reactive rather than preventive interventions. Employers should include managers and safety representatives in procurement- and implementation processes and define relevant and measurable goals regarding the collaboration.

  • How management groups' reason when deciding to use a model focusing on systematic work environment management : Experiences from Swedish municipalities

    • Purpose The purpose of this paper is to explore and describe the perspective of the management group regarding how they reasoned when deciding to engage in a model focussing on systematic work environment management, and what motives that influenced their decision. Design/methodology/approach This qualitative study with semi-structured interviews includes 18 representatives from the management groups in 18 Swedish municipalities. Data were analysed with a constant comparative method. Findings The participants described two aspects that were of importance when making the decision; establishing commitment before making the decision and establishing strategies to legitimise the decision. Furthermore, they expressed motives that were linked both to their individual expectations and wishes and to policies and facts in their organisations. The participants experienced the model as a valuable tool in their organisations to increase employee participation and to provide structured support to their first-line managers. Practical implications - The managers' motives were linked to individual expectations and external directives. These were often intertwined and influenced their decisions. When implementing this type of model, it is important to discuss decisions in a larger group to avoid building an organisational initiative on one person's expectations. Furthermore, it is important to support the management's work to establish commitment for the model in the municipal organisation. Originality/value This study adds to knowledge of the complexity of deciding and implementing models to support systematic work environment management in organisations.

  • Gonadotropin-releasing hormone agonist against severe aggression in autism

    • Aggression in patients with autism spectrum disorder (ASD) presents an important therapeutic challenge. Conventional treatment appears to be inadequate in a number of cases. The occurrence of severe aggressive symptoms since the inception of adolescence in a male patient with ASD suggested a hormonal influence by androgens. Conventional treatment with antipsychotic and antiepileptic drugs and benzodiazepines was ineffective. A subcutaneous long-acting gonadotropin-releasing hormone agonist (GnRH agonist) injection was given on a monthly basis resulting in a substantial improvement in his aggressive behaviour and renewed socialisation.

    • The BCJR algorithm is an important channel decoding method. We extend it to trellis rate-distortion data compression. Beginning from source coding principles, the derivation of the algorithm avoids channel coding or soft output ideas. The encoder does not use entropy coding; equiprobable reproducer letters are emphasized since these maximize entropy. The BCJR method is demonstrated by tests of a tailbiting variant. It performs much better than the ordinary Viterbi algorithm for short and medium blocks. However, the improvement stems from tailbiting; the role of the BCJR is to achieve tailbiting in a relatively simple way. Some issues that arise with tailbiting are explored. It is shown that there is an optimal trellis state size for each block length.

    • Some case reports have shown that patients suffering from obsessive-compulsive disorder can be effectively treated with anti-androgenic pharmacological agents with various modes of action. The most effective group of such agents is the long-acting analogues of the gonadotropin-releasing hormone. This investigation was undertaken in order to further elucidate the possibility of using such powerful anti-androgenic agents in the treatment of obsessive-compulsive disorder. Six male patients, all suffering from therapy-resistant obsessive-compulsive disorder, were included in a 48 weeks open-label trial of the long-acting GnRH-analogue triptorelin. Every other week, the patients rated the severity of obsessive-compulsive disorder symptoms by means of a visual analogue scale. During the course of the trial, five out of six patients experienced a considerable improvement. The finding gives further support to the contention that anti-androgenic agents are effective in the treatment of obsessive-compulsive disorder.
