    • In Sweden, clear changes in the care of the elderly have occurred during the 1990s, with fewer people being provided public care, although greater efforts are now directed towards those most in need of help. Elderly people are cared for increasingly in other ways: by the family, by means of market-provided care, and by voluntary and informal means. Differences between municipalities are considerable. A comparative study was conducted in eight Swedish municipalities, four of them characterized by extensive reorganization of home-help services, and the other four constituting a reference group where such changes had not occurred. The aim was to examine processes of setting local priorities and adjustments in a period of marked structural change. Interviews with local politicians, administrators, professionals, and union representatives, and with the elderly themselves, were the main sources of data. The process of determining the extent and content of home care services in the municipalities was found to be a complex process, one involving a number of partly interdependent factors. Restructuring was found to be greatest in the context of a traditionally strong reliance on home-help services, rather than in the context of institutionalized care, where administrative decision-making and a medical and rehabilitative perspective dominated.

    • This study examines how children report abuse, neglect and behavioural problems and what authorities they claim to have had contact with that are legally mandated to report to the Child Welfare Services, e.g. health services and police. It draws data from a longitudinal research programme, LoRDIA, in which four data collections followed adolescents’ development from 12 to 15 years (n = 1884). A total of 61 indicators of self-reported child abuse, neglect and behavioural problems were constructed to identify children with severe exposure so that each indicator would have prompted referral. The main finding is that 445 (25.3%) of the study population reported severe exposure of this magnitude. Among these self-reported severely exposed children (SSE), boys reported higher rates of child neglect and of overall behavioural problems, specifically criminal and other socially destructive behaviour. Poverty and living in single-parent households significantly increase the risk of abuse, neglect, and behavioural problems. Increased risk of neglect and criminal behaviour were found for children studying Swedish as second language. The SSE children more than other children report contact with all authorities with mandatory reporting. Odds ratios of contact were higher in the case of behavioural problems compared to the odds ratios for abuse and neglect.

    • Sweden has a long tradition of providing social care and support to its citizens in their own homes through formal home care, delivered either by home care services or personal assistance. A majority of people given support by formal home care need assistance with personal and intimate care. The focus of this interview study was on exploring care recipients’, care workers’, personal assistants’ and care unit managers’ perceptions and experiences of intimate and personal care in the context of formal home care in Sweden. In total, 57 interviews were conducted with 42 persons. Three themes emerged in the analysis: Personal Hygiene, Personal Sphere, and The Contextual Variability of Intimacy. Interviewees described intimate care as being inseparable from a person’s service needs as a whole. Highlighted was how caring for and washing intimate body parts, intrusion into recipients’ personal spheres, and the need to preserve integrity vary depending on situational, temporal and relational aspects. To safeguard the care recipient’s influence, integrity and dignity in the reception/provision of care work, home care services and personal assistance, it is important to raise awareness of the variation in perceptions of intimate and personal care in education and inhouse training.

    • Medication management in elderly care has become a considerable part of the care work. It involves health care professionals, such as registerednurses, formally delegating their tasks to non-professionals, typicall ycare workers. The aim of this article is to explore the medication management in nursing homes for older people. The focus lies on howcare workers and residents interact in relation to the medication management routines. Participant observations were made in seven nursing homes, and the analysis of the empirical material applied the theoretical concepts of local routine culture and materiality. The nursing home residents may try to resist or negotiate the medication management routines, express their reluctance in words or actions, ask questions or make demands. While routines produce institutional bodies, acts of resistance and negotiation may strengthen a person’s sense of self. Medication management affect the relationship between care workers and residents, by enabling interaction. Looking at medication management as a social phenomenon deepens the understanding of how it affects care work and illustrates that in the context of nursing home care, health care and social care are not separate but rather intertwined.

    • Throughout Europe the most damaging consequences of the coronavirus have fallen disproportionately on older people who live in care homes. This study involves the analysis of secondary data sources relating to deaths, and related harms, in European care homes from seven countries between March and December 2020. The findings are reviewed using the framework of the European Convention on Human Rights to identify examples of human rights violations - namely the right to life, liberty and security, respect for private and family life, and prohibition of torture, and general prohibition of discrimination. A significant contributing factor to the scale and nature of deaths and harms is the abject disregard of older people's human rights. Based on the findings, the authors, a group of social work academics, call for an urgent re-examination of the role of social work in relationship to care homes and the importance of re-engaging with human rights issues for care home residents.

  • Improving evidence-based social work practice with youths exhibiting conduct problems through structured assessment

    • A key task in evidence-based case management of youth is the assessment of research-based risk and protective factors. In the present study we compare assessments of social workers using a structured assessment instrument with assessments of social workers not using such an instrument. Assessments of the exact same case—a vignette about a 14-year-old boy—conducted by 30 social workers using a structured assessment instrument and 30 social workers not using such an instrument were compared. The 60 assessments were also rated by independent researchers and senior social services managers, blind to whether an instrument had been used in the assessments or not. As hypothesized, using a structured assessment instrument resulted in the identification of a greater number of research-based risk and protective factors, and the assessments were rated as better in terms of general adequacy, quality, accuracy and potential treatment effectiveness, than when an instrument was not used. The present study demonstrates that social workers’ assessments of youth become more evidence-based, adequate and potentially more treatment effective when a structured assessment instrument is used as compared to when it is not.

  • Bedtime stories from inside - family practices and affinities in families with incarcerated fathers

    • The present paper aims to explore whether a prison literacy project, Bedtime stories from inside, can contribute to improving and maintaining relationships between incarcerated fathers and their children. The analysis is focused on the concepts "doing family" and "affinities". The material consists of anonymous evaluation forms from the incarcerated fathers (70) and the carers (46). The results show that deeper familial connections are possible when fathers are allowed to interact with their children in a way that includes attending to practicalities like reading fairy tales to their children. The family practices within the project contribute to challenging norms of masculinity and overcoming impulses to shut down emotionally. The project highlights the need for the prison system and social work practice to support families in the hard work of forming and maintaining connections.

    • The provision of intimate and personal care constitutes a challenge for both careworkers and care recipients and is still a neglected area of research. An observational study of the interaction between the careworkers and care recipients in night-time home care services was conducted in a large municipality in Sweden. The results were analysed in light of previous research and theorising on strategies for handling intimacy in intimate care. The study highlights what appears to be a tension between the ways in which the recipient of care is conceptualised as an active consumer of care in present-day guidelines and the strategies chosen on the part of both caregivers and care recipients, when intimacy and integrity is most at stake, and framed as it is by the care recipients' situation of dependency and vulnerability. Home care services night-time was shown to be a case that markedly differs from many other settings of intimate care, but in the interactional routines intimate care came forth as a smooth and minimally obtrusive activity. The careworkers and care recipients engaged in strategies such as disattention, eye-discipline, middle-distance orientation, and objectification, thereby serving the purpose of balancing the transgressions of thresholds of intimacy.

    • This article explores conditions for implementing shared decision-making (SDM) in coordinated individual care planning (CIP) with individuals with complex mental health needs. SDM in CIP are described as central, although such user centred collaboration still remains to be realised. Research underlines the need for a changed way of working, where user expertise is valued and a balance of power is promoted. The aim of the present study is to investigate the conditions for implementing SDM in connection with CIP for and with people with mental illness. To better understand the context and conditions that can promote such an implementation, altogether 15 participants were interviewed in three regions in Sweden within the scope of a stakeholder analysis. Both hindering and supporting factors were identified with respect to an implementation process, such as staff turnover, differences in work culture and committed leadership. Further focus should be directed specifically towards professionals working more closely with CIP and towards in-depth analysis of the construct of culture in terms of implementation processes.
