  • Transitions across cognitive states and death among older adults in relation to education: A multistate survival model using data from six longitudinal studies

Introduction: This study examines the role of educational attainment, an indicator of cognitive reserve, on transitions in later life between cognitive states (normal Mini-Mental State Examination (MMSE), mild MMSE impairment, and severe MMSE impairment) and death. Methods: Analysis of six international longitudinal studies was performed using a coordinated approach. Multistate survival models were used to estimate the transition patterns via different cognitive states. Life expectancies were estimated. Results: Across most studies, a higher level of education was associated with a lower risk of transitioning from normal MMSE to mild MMSE impairment but was not associated with other transitions. Those with higher levels of education and socioeconomic status had longer nonimpaired life expectancies. Discussion: This study highlights the importance of education in later life and that early life experiences can delay later compromised cognitive health. This study also demonstrates the feasibility and benefit in conducting coordinated analysis across multiple studies to validate findings.

    • Cognitive reserve helps mitigate the impact of pathology on the clinical expression of dementia. Education and occupational complexity are considered as contributors to reserve, although it has been argued that cognitive reserve is likely formed over the life-course. A life-course model of cognitive reserve in dementia risk has not yet been tested. We apply a life-course model and examine if school grades around age 10, formal educational attainment, and lifetime occupational complexity affect dementia incidence in inpatient registers.

  • Markers of olfactory dysfunction and progression to dementia : A 12-year population-based study

    • Introduction: We evaluated markers of olfactory dysfunction (OD) for estimating hazard of dementia in older adults.Methods: Mild (hyposmia) and severe (anosmia) OD was classified in a population-based study of dementia-free persons (SNAC-K; n = 2473; mean age = 70 years) using the Sniffin sticks odor identification task. Combined variables were created for objective and subjective OD and for OD and APOE status. Hazard of dementia across 12 years was estimated with Cox regression.Results: OD was associated with increased hazard of dementia (2.01; 95% confidence interval [CI] 1.60-2.52), with the strongest association for anosmia (2.92; 95% CI 2.14-3.98). Results remained consistent after adjusting for potential confounders and across age and sex subgroups. APOE ε4 carriers with anosmia had the highest hazard of dementia (ε4: 6.95; 95% CI 4.16-11.62; ε4/ε4: 19.84; 95% CI 6.17-63.78).Discussion: OD is associated with increased risk of dementia, especially severe impairment in combination with genetic risk of Alzheimer's disease.

    • Background: Olfactory impairment has been associated with both cognitive impairment and dementia and Alzheimer’s disease (AD). This study aimed to investigate the association between olfactory dysfunction (OD) and change trajectories in different cognitive domains in aging. Method: Participants (n = 2473, mean age = 72 years, 61% female) from the population-based Swedish National study on Aging and Care-Kungsholmen (SNAC-K) were repeatedly assessed with tasks measuring episodic memory, semantic memory, verbal fluency, and perceptual speed across 12 years. OD was measured at baseline and participants were categorized as normosmic, hyposmic, or anosmic based on the Sniffin’ Sticks odor identification task. Linear mixed-effects models were used to assess the associations between baseline OD and rates of cognitive decline. Result: OD was related to poorer baseline performance and faster rates of decline during follow-up in all examined domains, as well as in global cognition. Associations were generally more pronounced for anosmia compared to hyposmia. Conclusion: Olfactory impairment is associated with accelerated decline in aging across a wide range of cognitive domains.

  • Association of diabetes with stroke and post-stroke dementia : A population-based cohort study

    • Introduction: The impact of prediabetes and diabetes on stroke and the development of dementia after a stroke remain unclear.Methods: A total of 2655 dementia-free participants (including a stroke-free cohort and a prevalent stroke cohort) were followed-up for 12 years. Dementia and post-stroke dementia were determined by clinical examinations and national registry data. Diabetes was ascertained via medical examination, medication use, medical records, or glycated hemoglobin (HbA1c) >= 6.5%. Prediabetes was defined as H bA1c >= 5.7% in diabetes-free participants.Results: In the stroke-free cohort, 236 participants developed ischemic stroke, and 47 developed post-stroke dementia. Diabetes was associated with ischemic stroke (hazard ratio [HR] 1.76, 95% confidence interval [CI] 1.16 to 2.67) and post-stroke dementia (HR 2.56, 95% CI 1.04 to 6.25). In the prevalent stroke cohort, diabetes was also related to dementia risk. Prediabetes was not significantly related to stroke or post-stroke dementia.Discussion: Diabetes, but not prediabetes, is associated with an increased risk of ischemic stroke and post-stroke dementia.

    • Recent research indicates that sleep may play a role in the development of dementia. However, few studies have been conducted in the oldest-old ages. The aim of this study was to examinethe association between subjective sleep disturbance and incidentdementia in a population-based sample of 85-year-olds without dementia at baseline, followed until their death. Methods:Arepresen-tative sample of 85-year-olds (N¼494, 144 men, 350 women) were examined with comprehensive neuropsychiatric examinations. 147 individuals were excluded due to dementia at baseline, two subjects lacked data on subjective sleep disturbance, leaving 345 individuals. Sleep disturbance was assessed with a question regarding a subjective experience of decreased sleep during the last month while dementia during follow-up was diagnosed by geriatric psychiatrists according to DSM-III-R. Results: Subjective sleep disturbance at baseline wasnot related to incident dementia during follow-up (hazard ratio, 1.10; 95% CI, 0.77-1.59). Conclusions: We found in populations-based sample of 85-year-olds that subjective sleep disturbance was not related to incident dementia during follow-up. Future research need to elucidate whether sleep disturbance affect older individuals differently in younger compared to older ages.

  • Relationship between marital and parental status and risk of dementia and Alzheimer's disease

    • Background: There is increasing evidence that social network factors may affect risk of developing dementia. The objective of the present study was to examine the association between marital and parental status separately and their combined effect on the risk of incident dementia and Alzheimer's disease (AD). Methods: A total of 1707 members of a population-based prospective cohort study aged 65 and over were followed for an average period of 9.5 years. During follow-up, 393 participants were diagnosed with dementia, including 221 of Alzheimer's disease (AD). Age, sex, education, smoking, vascular diseases, depressive symptoms, and stressful negative life events were used as covariates. Results: Cox logistic regression revealed that unmarried have a greater incidence of dementia and AD compared to married. Participants that were childless were also more likely to develop dementia and AD than those who were parents. Examining the combined effects of marital and parental status revealed that the already negative effect of being unmarried was further enhanced if the individuals were also childless. Conclusions: Our findings suggest that both marital and parental statuses are associated with risk of dementia and that the effect of marital status need to be examined while taking into account parenthood. Further studies are needed to confirm our finding and to explore the mechanisms underlying this association.

  • Hippocampal Integrity in Swedish Women with Bilateral Salpingo-oophorectomy prior to Natural Menopause

    • Background:Oophorectomy prior to natural menopause places women at increased risk of dementia and/or Alzheimer's disease (AD). Recent findings from our Toronto group reveal a negative association between oophorectomy prior to natural menopause and verbal memory in middle-aged women. We have also found a positive association between estrogen levels and verbal recall. Taken together, these findings support previous work suggesting that oophorectomy, leading to reduced levels of estrogens, is detrimental to verbal memory. Estrogen withdrawal has also been correlated with reduced hippocampal volume and reduced hippocampal resting functional connectivity (FC), both early AD biomarkers. Thus, we wondered whether hippocampal volume and resting functional connectivity would be reduced in women with oophorectomy prior to natural menopause.Methods:In order to determine this, we recruited healthy, Swedish women (30 and 55 years) with the breast cancer mutation gene (BRCA1/2) who had a bilateral salpingo-oophorectomy (BSO) prior to natural menopause. Most women were between 1–7 years post-BSO and at least 6 months post-cancer treatment or had not had cancer. Using magnetic resonance imaging (3T scanner, Phillips) we measured functional resting state over 10 minutes and volume with a T1 structural scan. We collected urine in order to determine estrogen and progesterone levels.Results:We hypothesize that women with BSO will have structural and functional hippocampal changes compared to age matched controls. We predict that women with BSO will have smaller hippocampal volumes and reduced hippocampal FC. We further predict that lower levels of estrogens will correlate with these brain changes. Neuroimaging and endocrine analyses are ongoing.Conclusions:AD affects women in greater numbers and one possibility is that oophorectomy prior to natural menopause contributes to these numbers. Determining whether or not these women show the earliest biomarkers for AD will increase our understanding of estrogen withdrawal's effects on brain health as well as its importance for healthy brain aging. Importantly, results of this study will inform us on the early brain changes in a population at greater risk of AD.

    • INTRODUCTION: The pace of innovation has accelerated in virtually every area of tau research in just the past few years. METHODS: In February 2022, leading international tau experts convened to share selected highlights of this work during Tau 2022, the second international tau conference co-organized and co-sponsored by the Alzheimer's Association, CurePSP, and the Rainwater Charitable Foundation. RESULTS: Representing academia, industry, and the philanthropic sector, presenters joined more than 1700 registered attendees from 59 countries, spanning six continents, to share recent advances and exciting new directions in tau research. DISCUSSION: The virtual meeting provided an opportunity to foster cross-sector collaboration and partnerships as well as a forum for updating colleagues on research-advancing tools and programs that are steadily moving the field forward.
