    • Objectives: To describethe occurrence of a cavernous hemangioma of the external auditory meatus extending to the middle ear and the retroauricolar region in an adult patient, and to review the relevant literature.Methods: Case report. We report the clinical presentation, imaging studies and surgical procedures used in the study.Results:  The angiographic study showed that the blood supply of the mass originated from the posterior auricular artery. Via a retroauricular approach, this artery was isolated and ligated in order to control the intra-operative bleeding. A canal wall up mastoidectomy with posterior tympanotomy and a tympano-canaloplasty were performed, permitting a complete excision. At 36 months of follow-up, no clinical and radiological signs of recurrence were detected.Conclusion: Surgical removal is the treatment of choice, with rare recurrence of the hemangiomas. Pre-operative evaluation is based on computed tomography (CT scan), but should be followed by magnetic resonance angiography (MR-angio) or intra-arterial digital subtraction angiography (IA-DSA), especially in case of wider lesions.

  • Familial aggregation of tinnitus : a European multicentre study

    • INTRODUCTION AND AIM:Tinnitus is a common condition affecting approximately 20% of the older population. There is increasing evidence that changes in the central auditory system following cochlear malfunctioning are responsible for tinnitus. To date, few investigators have studied the influence of genetic factors on tinnitus. The present report investigates the presence of a familial effect in tinnitus subjects.METHODS:In a European multicentre study, 198 families were recruited in seven European countries. Each family had at least 3 siblings. Subjects were screened for causes of hearing loss other than presbyacusis by clinical examination and a questionnaire. The presence of tinnitus was evaluated with the question "Nowadays, do you ever get noises in your head or ear (tinnitus) which usually last longer than five minutes". Familial aggregation was tested using three methods: a mixed model approach, calculating familial correlations, and estimating the risk of a subject having tinnitus if the disorder is present in another family member.RESULTS:All methods demonstrated a significant familial effect for tinnitus. The effect persisted after correction for the effect of other risk factors such as hearing loss, gender and age. The size of the familial effect is smaller than that for age-related hearing impairment, with a familial correlation of 0.15.CONCLUSION:The presence of a familial effect for tinnitus opens the door to specific studies that can determine whether this effect is due to a shared familial environment or the involvement of genetic factors. Subsequent association studies may result in the identification of the factors responsible. In addition, more emphasis should be placed on the effect of role models in the treatment of tinnitus.

  • Accuracy and precision of direct bone conduction measurements.

    • Variability in Baha® sound processor fittings arise from several sources including the nature of the implant-to-bone transmission and transcranial attenuation in patients with single-sided sensorineural deafness. One method of improving the predictability of Baha fittings is to measure the individual patient's actual bone conduction thresholds via the implant, thereby removing the influence of skin thickness and/or implant location site.One hundred thirty eight adult wearers of the Baha System participated in the study. Direct bone conduction thresholds were obtained through the BC Direct feature of the Cochlear™ Baha Fitting Software combined with the CochlearBaha BP100 sound processor. Test-retest reliability measurement was performed in 58 participants.Improved transmission of sound through the implant rather than transcutaneously through the skin was confirmed. On average, the BC Direct thresholds were closer to the patient's unmasked thresholds than the masked values. In patients with single-sided sensorineural deafness, BC Direct results were elevated compared to the contralateral bone conduction thresholds due to transcranial attenuation. The test-retest reliability for the BC Direct measurements was within ±5 dB, which is in within the accepted variability for audiometric test measurements.Direct bone conduction measurement provides a validated method of comparing the transcutaneous thresholds as measured through audiometry with the percutaneous responses from the Baha sound processor. The Baha fitting based on direct measurements of bone conduction may require less fine-tuning and provide a greater understanding of the variability of the bone conduction sound pathway.

  • Quality of life in patients with tinnitus and sensorineural hearing loss

    • Purpose: The aim of this study was to evaluate the quality of life in patients with tinnitus and SNHL, to investigate patients’ mental and physical health and to measure the level of satisfaction concerning the care experienced by the patients.Methods: Three questionnaires related to patients` physical and psychological health, were mailed to 714 Swedish patients with tinnitus and sensorineural hearing loss.Results: Female patients had significantly higher Tinnitus Handicap Inventory scores than male patients. Patients who were satisfied with the care they obtained had significantly higher PTA. It was found that the satisfied group had significantly lower THI-scores. In the dissatisfied group, 61% of patients had no hearing aids compared to 42% in the satisfied group.Conclusion: Our cohort of patients estimated their life quality and general health at a good level almost 4.5 years after their first report of tinnitus. Another finding was that 47% of patients were not satisfied with the treatment they obtained. In the dissatisfied group, 61% of patients had no hearing aids. Further research is required to find an approach that could motivate patients with both tinnitus and hearing impairment to use hearing aids.
