  • Indications of change in life perspective among women with breast cancer admitted to complementary care

    • In this study, qualitative content analysis was used in order to understand the reported changes of life perspective in interviews with 59 women with breast cancer who were admitted to complementary care. The aim of this research was to study women's perceived consequences as well as perceived causes of breast cancer and to explore aspects of importance to the women. The material was collected in semistructured interviews from women with breast cancer at different stages of the disease. The women received complementary care at an anthroposophic clinic in Sweden. Findings showed that these women's view of their relationships with others grew more valuable. Their self-confidence and experience of strength improved, and they regarded life as being more enriched. A change in their disposition towards becoming more fragile and low-spirited was experienced as a hardship by the women. An interesting finding was that the patients described the aetiology of the disease from several interacting perspectives, which also affected their ideas of how to achieve well-being and health. The findings support the view that changes of both benefit and harm are present in the experience of breast cancer.

  • Living with breast cancer - a challenge of expansive and creative forces

    • The aim of this qualitative case study is to obtain a deeper and more profound understanding of the life world of women living with breast cancer focusing particularly on changes in life perspective. The study is based on a series of interviews carried out within the space of one year and involving four women with breast cancer; each woman was interviewed four times. The participants were between 42 and 54 years of age; three of the four interviewed were in an advanced stage, with metastasis or recurrent breast cancer. There was an increased awareness of the relationship between life and death, which constituted a disclosure rather than an actual change in life perspective. The four women were 'opening up' to the beauty and the essentials in life and experienced an increased desire to live their life in accordance with their own values. Their revitalised view of life increased their desire for authenticity. When it proved impossible to live in accordance with new insights the women were particularly frustrated. From a caring perspective our findings suggest that an awareness of patients' increased openness to their own needs and desires is an important resource in the healing and rehabilitative process of breast cancer patients. The paradoxes and the struggles involved disguise a hidden potential for health.

  • The face of suffering among women with breast cancer - being in a field of forces

    • Through qualitative interviews, the suffering experiences of women with breast cancer and their significant others were disclosed. Seventeen women with different stages of breast cancer and 16 significant others from 4 different care cultures in Sweden and Finland participated. Five of the women had advanced metastatic breast cancer, and 12 had a localized disease. Mean age was 48 years. As a methodology, a team approach, inspired by the Vancouver School of Doing Phenomenology, was used. The findings elucidate how the suffering experience touched the women's inner existence and values. This can metaphorically be described as a "field of force" and affected everything in the women's lives, including their views of themselves and their relationships. Existential questions were raised about life and death and the meaning of life. In their suffering, the women's dependency upon significant others, as well as healthcare personnel, was prominent. Suffering related to healthcare was a strong theme. Different faces of suffering related to breast cancer may still be unknown by healthcare professionals working in cancer care.

    • The aim of this study was to review literature on how the lived experience of breast cancer and suffering was described in nursing/caring literature between 1990 and 2000, and to interpret and discuss the result from the perspective of suffering. The frequent reports of changing courses, called by various names, such as transition, transformation, transcendence, and finding meaning, werefound as paths whereby the person regained integrity, balance, and wholeness. When findings were interpreted from the perspective of suffering and an ontological health model, actions, values, and existential concerns were understood as both expedients for alleviating suffering and a sign of the patient's inner struggle.

    • Denna rapport är en del av slutrapporteringen från två av projekten i programmet ”Miljöstrategiska verktyg”. Projekten är Utveckling av verktyg för strategisk miljöbedömning och medborgarmedverkan (MiSt-projekt 1) samt Scenariometoder i strategisk miljöbedömning (MiSt-projekt 7). I samarbete med Finspångs kommun har ett gemensamt projekt utvecklats för att ta fram en ny energiplan i kommunen. Programmet "Miljöstrategiska verktyg", MiSt, är ett tvärvetenskapligt forskningsprogram finansierat av Naturvårdsverket. Programmet leds från Blekinge Tekniska Högskola. I programmet studeras verktyg som kan underlätta miljöbedömning i strategiskt beslutsfattande på olika nivåer, från nationell till lokal. Förståelsen och basen för utveckling av verktyg och rekommendationer om verktyg i olika planerings- och beslutsprocesser baseras på empirisk forskning på fall inom flera sektorer. Målen för MiSt-programmet: • Kritisk undersökning av verktygens funktion • Teoribaserad förståelse av deras verkan • Utveckling av råd om effektiv användning av verktyg och kombinationer avverktyg

    • The aim was to study the perceived quality of life/life satisfaction in a sample of women with breast cancer who were treated in a hospital with alternative/complementary care and the same variables in individually matched patients who received only conventional medical treatment. A non-randomized controlled trial design with repeated measurements was used. Sixty women with breast cancer treated with anthroposophic medicine (ABCW) and 60 with conventional medicine (CBCW) were included and 36 matched pairs took part on all occasions. The quality of life was measured by the EORTC QLQ-C30 and the Life Satisfaction Questionnaire (LSQ). The comparisons were calculated as effect sizes (ES). The women in the ABCW group reported small or moderate effects, expressed as ES, on their quality of life/life satisfaction compared to their matched "twins" in the CBCW group at the 1-year follow-up in 15 out of 21 scales/factors. It was concluded that the women who had chosen anthroposophic care increased their perceived quality of life/life satisfaction according to the methodology of the study.

    • The aim of the present study, which is part of a major clinical controlled study of the life situation of women with breast cancer, was to compare two groups of women concerning perceived quality of life and coping. The women were treated in two different cancer treatment programs: complementary treatment, which included anthroposophic therapy, and conventional cancer treatment. A total of 120 women were included, 60 women treated with anthroposophic medicine, and 60 individually matched women treated with conventional medicine only. Quality of life was measured by the European Organization for Research and Treatment of Cancer, Quality of Life Questionnaire, Core 30, and the Life Satisfaction Questionnaire. Coping was measured by the Mental Adjustment to Cancer scale. The results showed that the women who chose anthroposophic therapy perceived their quality of life to be lower on admission to the hospital and showed more anxious preoccupation than the women in conventional medicine. It can be concluded that, due to the careful matching procedure, the women in the two groups are comparable in a medical sense but not from the perspective of quality of life and coping.

  • A controlled empirical evaluation of a requirements abstraction model

    • Requirement engineers in industry are faced with the complexity of handling large amounts of requirements as development moves from traditional bespoke projects towards market-driven development. There is a need for usable and useful models that recognize this reality and support the engineers in a continuous effort of choosing which requirements to accept and which to dismiss off hand using the goals and product strategies put forward by management. This paper presents an evaluation of such a model that is built based on needs identified in industry. The evaluation's primary goal is to test the model's usability and usefulness in a lab environment prior to large scale industry piloting, and is a part of a large technology transfer effort. The evaluation uses 179 subjects from three different Swedish Universities, which is a large portion of the university students educated in requirements engineering in Sweden during 2004 and 2005. The results provide a strong indication that the model is indeed both useful and usable and ready for industry trials. © 2006 Elsevier B.V. All rights reserved.

    • Den här rapporten presenterar resultat från forskningsprojektet SEAMLESS, Strategic Assessment and Management in Local authoritiES in Sweden. Syftet med SEAMLESS var att studera konceptuella och praktiska kopplingar mellan verktyg för strategisk miljöplanering, miljöbedömning och miljöledning, speciellt mellan Miljöbedömning av planer och program (baserat på EG-direktivet 2001/42/EC) samt Miljöledningssystem (som utgår från huvudprinciperna i ISO 14001/EMAS) i kommuner. Utgångspunkten för studien var att kommuner är viktiga aktörer på miljöområdet eftersom de fattar och genomför en stor mängd beslut som direkt eller indirekt har en inverkan på miljön. Ett stort antal olika system och verktyg har utvecklats för att stötta kommunernas miljöarbete. Genom att ha ett integrerat angreppssätt finns det goda möjligheter för ett mer verkningsfullt miljöarbete.SEAMLESS-projektet har genomförts i fem delsteg. Först gjordes en litteraturgenomgång och en undersökning av internationell praxis vad gäller kopplingar mellan miljöbedömningar av planer och program samt miljöledningssystem. I projektets andra steg utformades en konceptuell modell av möjliga kopplingar mellan verktygen. Sedan genomfördes fallstudier i sex svenska kommuner för att få en bättre förståelse för hur verktygen används i praktiken samt vilka kopplingar som finns mellan dem. I projektets fjärde steg analyserades resultaten från de tre första stegen och utifrån detta utvecklades och genomfördes en pilotstudie. I projektets sista steg formulerades generella rekommendationer för ett integrerat angreppssätt för verktyg för strategisk planering, bedömning och management.Teoretiskt skulle lokala strategier för hållbar utveckling kunna gynnas av influenser från några av huvudprinciperna i miljöledningssystem och miljöbedömning av planer och program. Men att förverkliga detta i praktiken är en utmaning som kräver ett brett systemperspektiv och en viss mognad inom organisationen. När SEAMLESS-projektet genomfördes fanns det varken behov eller möjligheter för denna typ av integration i de kommuner som studerades.SEAMLESS-projektets huvudkoncept tool technique och tool practice användes för att förklara och analysera de förutsättningar, möjligheter och svårigheter som ett integrerat angreppssätt för med sig. Tool technique fokuserar de tekniska aspekterna av verktygsanvändning och tar inte hänsyn till den verklighet och sammanhang som verktyget ska användas i. För att få ett verktyg att fungera i praktiken krävs denna hänsyn, något som vi i SEAMLESS definierar som tools practice.Verktygsintegrering är ett annat viktigt koncept inom SEAMLESS, där tre viktiga stadier av integrering identifierats: formell integration, lärande integration och interaktiv integration. Vid ett för stort fokus på formell integration (som är nära kopplat till tool technique), riskerar möjliga kopplingar mellan de integrerade verktygen att gå förlorad eftersom liten eller ingen hänsyn tas till sammanhanget där de ska implementeras. För att möjliggöra integrering krävs lärande integration. Interaktiv integration är svår att uppnå och det är osäkert om detta stadium är eftersträvansvärt eftersom det riskerar att bli för komplext och resurskrävande för att vara effektivt.Baserat på resultat och analys har SEAMLESS-projektet resulterat i en uppsättning rekommendationer. Slutsatserna är att integration inte bör vara ett självändamål; att verktygsintegrering bör fokusera att minska gapet mellan olika professionella kulturer; samt att ett brett systemperspektiv är nödvändigt.

  • Family suffering related to war experiences : An interpretative synopsis review of the literature from a caring science perspective

    • The aim of this study was to conduct a synopsis review of findings from families' experiences related to combat and war events and to interpret these findings from the perspective of theories of suffering. The method used in the study was a synopsis review of 12 articles dealing with family suffering related to war or combat experiences and an interpretation of the articles from a caring science perspective. Findings from the synopsis review were that the dominant part of the articles viewed suffering in general and post-traumatic stress disorders (PTSD) in particular from a medical, psychiatric or psychologically behaviouristic perspective. PTSD and other distress-related conditions were mainly described in terms of their symptoms and dealt with in terms of pathology. The interpretation of the articles from a caring science perspective generated three significant themes: first, interdependence as a spiritual dimension of dependence, secondly, familial communion as sharing moral and spiritual values and thirdly, familial suffering visualized by compassion. The study's conclusion is that, from a caring science perspective, the appearance of family suffering should be comprehended in terms of expressed compassion and that any disturbance within familial communion is likely to have an emotional impact on all of the family members, as a result of their interdependence. © 2006 The Authors.
