  • Towards a better understanding and new therapeutics of osteopetrosis.

    • Lack of or dysfunction in osteoclasts result in osteopetrosis, a group of rare but often severe, genetic disorders affecting skeletal tissue. Increase in bone mass results in skeletal malformation and bone marrow failure that may be fatal. Many of the underlying defects have lately been characterized in humans and in animal models of the disease. In humans, these defects often involve mutations in genes expressing proteins involved in the acidification of the osteoclast resorption compartment, a process necessary for proper bone degradation. So far, the only cure for children with severe osteopetrosis is allogeneic hematopoietic stem cell (HSC) transplantation but without a matching donor this form of therapy is far from optimal. The characterization of the genetic defects opens up the possibility for gene replacement therapy as an alternative. Accordingly, HSC-targeted gene therapy in a mouse model of infantile malignant osteopetrosis was recently shown to correct many aspects of the disease.

    • Infertility is an increasing problem all over the world, and it has been estimated that 10 - 15% of couples in fertile age have fertility problems. Likewise induced unsafe abortion is a serious threat to women's health. Despite advances made in assisted reproduction techniques, little progress has been made in increasing the success rate during fertility treatment. This document describes a wide range of projects carried out to increase the understanding in the field of embryo implantation research. The 'Fruitful' research network was created to encourage collaborations within the consortium and to describe our different research potentials to granting agencies or private sponsors.   • Occupational therapy research on assistive technology and physical environmental issues: a literature review.

    • Background. To determine future directions for research in the area of assistive technology and physical environmental issues, it is important to have an understanding of prior research. Purpose. This literature review examined how assistive technology and physical environmental issues have been studied in the research published in international peer-reviewed occupational therapy journals. Method. Five recent volumes of nine journals were manually searched utilizing specific criteria. The publications were classified according to their perspective, application of the Person-Environment-Occupation (PEO) model, and the research design. Results. Both research fields demonstrated use of different research methods and they displayed equal needs with regard to improved research methodologies. Practice Implications. There are a lack of studies involving all three PEO components indicating a lack of research in occupational performance issues. Further research

  • Treatment of HIV infection: Swedish recommendations 2009.

    • On 4 previous occasions, in 2002, 2003, 2005 and 2007, the Swedish Medical Products Agency (Lakemedelsverket) and the Swedish Reference Group for Antiviral Therapy (RAV) have jointly published recommendations for the treatment of HIV infection. In November 2008, an expert group under the guidance of RAV once more revised the guidelines, of which this is a translation into English. The most important updates in the present guidelines include the following: (a) treatment initiation is now recommended at a CD4 cell count of approximately 350/mu l; (b) new recommendations for first-line therapy: abacavir/lamivudine or tenofovir/emtricitabine in combination with efavirenz or a boosted protease inhibitor (PI/r); (c) an increased focus on reducing the use of antiretroviral drugs that may cause lipoatrophy; (d) an emphasis on quality assurance of HIV care through the use of InfCare HIV; (e) considerably altered recommendations for the initiation of antiretroviral therapy in children. All infants (<1 y) should start antiretroviral therapy, regardless of immune status. Also, absolute CD4+ cell counts, rather than percentage, may be used to guide treatment initiation in children above the age of 5 y.

  • Function and dysfunction of the colon and anorectum in adults : Working team report of the Swedish Motility Group (SMoG)

    • Symptoms of fecal incontinence and constipation are common in the general population. These can, however, be unreliably reported and are poorly discriminatory for underlying pathophysiology. Furthermore, both symptoms may coexist. In the elderly, fecal impaction always must be excluded. For patients with constipation, colon transit studies, anorectal manometry and defecography may help to identify patients with slow-transit constipation and/or pelvic floor dysfunction. The best documented medical treatments for constipation are the macrogols, lactulose and isphagula. Evolving drugs include lubiprostone, which enhances colonic secretion by activating chloride channels. Surgery is restricted for a highly selected group of patients with severe slow-transit constipation and for those with large rectoceles that demonstrably cause rectal evacuatory impairment. For patients with fecal incontinence that does not resolve on antidiarrheal treatment, functional and structural evaluation with anorectal manometry and endoanal ultrasound or magnetic resonance (MR) of the anal canal may help to guide management. Sacral nerve stimulation is a rapidly evolving alternative when other treatments such as biofeedback and direct sphincter repair have failed. Advances in understanding the pathophysiology as a guide to treatment of patients with constipation and fecal incontinence is a continuing important goal for translational research. The content of this article is a summary of presentations given by the authors at the Fourth Meeting of the Swedish Motility Group, held in Gothenburg in April 2007.

  • Annotated checklist of fungus gnats from Sweden (Diptera : Bolitophilidae, Diadocidiidae, Ditomyiidae, Keroplatidae and Mycetophilidae)

    • The first modern Swedish checklist of fungus gnats of the families Bolitophilidae, Diadocidiidae, Ditomyiidae, Keroplatidae and Mycetophilidae is presented. The checklist results from a comprehensive literature research as well as treatment of a considerable determined and undetermined material from Swedish museum collections and recent material from several sources. To get better grounds for judging many old and doubtful records from Sweden, a nearly complete review of fungus gnats from the J. W. Zetterstedt collection at the Museum of Zoology in Lund has been undertaken and identifications corrected where needed. In order to preserve nomenclatural stability three species names are reinstated, all with priority over junior synonyms, viz. Orfelia lugubris (Zetterstedt, 1851) comb. n. = Orfelia tristis (Lundström, 1911) syn. n., Ectrepesthoneura ovata Ostroverkhova, 1977 = Ectrepesthoneura bucera Plassmann, 1980 syn. n., and Dynatosoma thoracicum (Zetterstedt, 1838) sens. auct. nec Zaitzev = Dynatosoma norwegiense Zaitzev & Økland, 1994 syn. n. We further find the holotype of Mycetophila grisea Zetterstedt, 1852 to be identical to Mycetophila perpallida Chandler, 1993, but retain Mycetophila perpallida pending a review of other possible synonyms with precedence. Ectrepesthoneura pubescens (Zetterstedt, 1860) is found to be a synonym of Ectrepesthoneura unifurcata (Zetterstedt, 1860) comb. n. syn. n. where the first name is retained as valid due to page priority. Four names described by Zetterstedt remain nom. dub., viz.: Mycomya diluta (Zetterstedt, 1860), Mycomya (Mycomya) notata (Zetterstedt, 1860), Brevicornu fusculum (Zetterstedt, 1838), and Brevicornu obscurellum (Zetterstedt, 1838) comb. n. Stigmatomeria obscura (Winnertz, 1863) sp. restit. is found to be a distinct species separate from Stigmatomeria crassicornis (Stannius, 1831). New combinations are suggested for Exechia praedita Plassmann, 1976 = Exechiopsis (Xenexechia) praedita (Plassmann, 1976) comb. n., and for Rhymosia pectinacea Ostroverkhova, 1979 = Pseudexechia pectinacea (Ostroverkhova, 1979) comb. n. Having removed 17 published species names with doubtful or wrong records (listed at the end) we find that the published sources comprise altogether 585 species. 137 species are reported for the first time in Sweden, bringing the total up to 722. Additional 36 species considered to be new to science are mentioned for completeness, but will be further presented and described elsewhere. Hence, at present the known fauna (including claimed undescribed species) comprises 758 species in 83 genera; 25 species of the family Bolitophilidae, 5 of the family Diadocidiidae, 3 of the family Ditomyiidae, 45 of the family Keroplatidae, 679 of the family Mycetophilidae and 1 in the unplaced genus Sciarosoma Chandler. For each species a full account of published Swedish sources with pagination, all synonyms used in Sweden, and the currently known distribution in the Swedish provinces as well as the known world distribution is given. The Swedish species of fungus gnats are generally very widespread: 47% being widespread in Europe, 43% being distributed in the Eastern Palaearctic Region, and 25% having a Holarctic distribution. We find significantly higher species richness in the boreal forests of northern Sweden, but only 3% of the species so far described are known only from the Nordic biogeographical region.

    • Background: Residential wood combustion is now recognized as a major particle source in many developed countries, and the number of studies investigating the negative health effects associated with wood smoke exposure is currently increasing. The combustion appliances in use today provide highly variable combustion conditions resulting in large variations in the physicochemical characteristics of the emitted particles. These differences in physicochemical properties are likely to influence the biological effects induced by the wood smoke particles. Outline: The focus of this review is to discuss the present knowledge on physicochemical properties of wood smoke particles from different combustion conditions in relation to wood smoke-induced health effects. In addition, the human wood smoke exposure in developed countries is explored in order to identify the particle characteristics that are relevant for experimental studies of wood smoke-induced health effects. Finally, recent experimental studies regarding wood smoke exposure are discussed with respect to the applied combustion conditions and particle properties. Conclusion: Overall, the reviewed literature regarding the physicochemical properties of wood smoke particles provides a relatively clear picture of how these properties vary with the combustion conditions, whereas particle emissions from specific classes of combustion appliances are less well characterised. The major gaps in knowledge concern; (i) characterisation of the atmospheric transformations of wood smoke particles, (ii) characterisation of the physicochemical properties of wood smoke particles in ambient and indoor environments, and (iii) identification of the physicochemical properties that influence the biological effects of wood smoke particles.

  • On neural plasticity, new neurons and the postischemic milieu: An integrated view on experimental rehabilitation

    • This review discusses actual and potential contributors to functional improvement after stroke injuries. Topics that will be covered are neuronal re-organization and sprouting, neural stem/progenitor cell,activation and neuronal replacement, as well as the neuronal milieu defined by glia, inflammatory cells and blood vessel supply. It is well established that different types of neuronal plasticity ultimately lead to post-stroke recovery. However, an untapped potential which only recently has started to be extensively explored is neuronal replacement through endogenous or exogenous resources. Major experimental efforts are needed to achieve progress in this burgeoning area. The review stresses the importance of applying neurodevelopmental principles as well as performing a characterization of the role of the postischemic milieu when studying adult brain neural stem/progenitor cells. Integrated and multifaceted experimentation, incorporating actual and possible poststroke function modulators, will be necessary in order to determine future strategies that will ultimately enable considerable progress in the field of neurorehabilitation.   • Bacterial vaginosis - Transmission, role in genital tract infection and pregnancy outcome: An enigma

    • Whether bacterial vaginosis (BV) is acquired from an endogenous or an exogenous source is subject to controversy. Despite findings of an association between sexual behaviour and BV, some data indicate that BV is not a sexually transmitted infection in the traditional sense, while other data indicate that BV is an exogenous infection. A third aspect of BV is its tendency to go unnoticed by affected women. All of this will have a strong impact on how physicians view the risks of asymptomatic BV This review focuses on whether or not BV should be regarded as a sexually transmitted infection (STI), its role in postoperative infections and pelvic inflammatory disease (PID), and on whether or not treatment of BV during pregnancy to reduce preterm delivery should be recommended. The reviewed studies do not lend unequivocal support to an endogenous or exogenous transmission of the bacteria present in BV For women undergoing gynaecological surgery such as therapeutic abortion, the relative risk of postoperative infection is clearly elevated (approx. 2.3-2.8). A weaker association exists between BV and pelvic inflammatory disease. Data on treatment of BV as a way of reducing preterm delivery are inconclusive and do not support recommendations for general treatment of BV during pregnancy. The discrepant associations between BV and preterm birth found in recent studies may be explained by variations in immunological response to BV. Genetic polymorphism in the cytokine response - both regarding the TNF alleles and in interleukin production - could make women more or less susceptible to BV, causing different risks of preterm birth. Thus, studies on the vaginal inflammatory response to microbial colonization should be given priority. Copyright © APMIS 2005.
