  • Digitalisering och social exklusion : Om medborgares användning av och attityder till Arbetsförmedlingens digitala tjänster

    • This research report focuses on the users of e-government in a social science perspective. Our aim is to study how different social groups, registered at the Swedish Public Employment Service, relate to the internet, the agency and the services offered on its website (www.ams.se). The field of e-government research is dominated by studies that centre attention on the supply side (videlicet research investigating the entrance of IT in organizations and the implications that new technology have to these), while usercentred research (demand side) is still scarce. Our study, focusing on how citizens relate to the internet in general and e-government in particular, therefore helps to bridge a knowledge gap within the field. Our survey is based on a questionnaire sent to 2 000 randomly selected persons, all registered at the Swedish Public Employment Service. Of these, 762 job seekers responded, which gives us a frequency rate of 40 percent. The questions asked were related to the job seekers’ usage of and attitudes towards the internet in general and the agency’s webpage in particular, but also to their attitudes to the Swedish Public Employment Service. The main results show that social factors, particularly education, play a major role for the job seekers’ ability to use the web based services offered by the agency. People with a lower educational level are less inclined to use the agency’s website, and at the same time they experience the site as more complicated to use. We also found a strong link between the relations to the internet (access, usage, experience and attitudes) and the relations to the agency’s website. Those with advantaged internet relations – mostly well educated people, white collars and people living in bigger cities – also use the agency’s website more diligently and tend to have more positive attitudes towards it (and vice versa). Thus, its necessary to talk in terms of digitally well equipped and less well equipped groups. The unequal relations to the internet in general and the agency’s website in particular not only indicate that e-government is more suitable for the digitally well equipped, but that it in fact exclude those with less digital resources. This new kind of exclusion has great implications for the job seekers’ possibilities to enter the labour market, and to act their role as citizens. If e government also means a reformation of the citizen role – in the sense of increased individual responsibility towards the government - not bridging the digital divide will carry even more exclusion to those that’s already excluded.

  • Olika publiker, olika livsstilar : Om idrott, kultur och regional utveckling

    • I föreliggande rapport studeras vilka sociala värden som elitidrotten och kulturen genererar för invånarna i landsortspräglade län som Blekinge, Halland, Kalmar och Kronoberg. Studien syftar till att lämna ett bidrag till den samhällsvetenskapliga idrottsforskningen, och ge ökad kunskap om idrottens och kulturens betydelse för den regionala utvecklingen. Studiens material vilar på en postenkät skickad till 1500 slumpmässigt utvalda personer i respektive län. Det sammanlagda antalet besvarade enkäter uppgick till 3181 stycken, vilket ger en nettosvarsfrekvens på 54 procent. Rapporten visar bland annat att både idrott och kultur värderas relativt högt och konsumeras i relativt stor utsträckning. Beträffande idrott konsumeras främst breddidrottsliga evenemang, och vad gäller kultur besöks bibliotek och biografer oftare än exempelvis teatrar. Studien visar också att de kultur- respektive idrottsintresserade skiljer sig åt vad gäller demografi, men även vad gäller livsstil. De idrottsintresserade har en mer folklig och lantlig prägel, medan de kulturintresserade är att betrakta som en välutbildad kosmopolitisk grupp. Med tanke på de sistnämndas ökade rörlighet är satsningar på kultur ej att förringa. Denna typ av satsningar kan få denna resursstarka grupp att dels stanna kvar i landsorten, dels flytta in till densamma.

    • The aim of this study was to describe a nurse-led rheumatology clinic’s impact on empowering patients with rheumatoid arthritis (RA). Rheumatoid arthritis is a chronic, inflammatory disease that attacks many joints, causing considerable functional restrictions for patients. Consequently, these patients are dependent on a wide variety of health-care services. A descriptive, qualitative design inspired by phenomenography was chosen. The conceptions were collected through interviews with 16 strategically selected patients with RA. Three descriptive categories comprising eight conceptions emerged: teaching (gaining insight and receiving information), regular review (receiving security, realizing regularity, and achieving accessibility), and attention (getting a holistic assessment, receiving coordinated care, and getting sufficient time). A nurse-led rheumatology clinic can be a source for empowering patients with RA to adopt new stances to alternative actions and achieve a higher level of faith in their own abilities.

  • Factors influencing nurse supervisor competence : a critical incident analysis study

    • The aim of the study was to identify factors related to critical incidents that influence the competence of nurse supervisors. Nurse supervisors require considerable competence in order to help supervisees to reflect on their clinical work and to interpret the needs of the patient. A qualitative approach involving the critical incident technique was used. Critical incidents were collected by means of self-reports from 25 nurse supervisors. Two main areas emerged: a professional and a personal stance. The professional stance described the nurse supervisors’ awareness of the importance of creating a secure learning environment and facilitating reflection. The supervisors structured the material and created awareness of fundamental nursing values. The second main area, personal stance, described the nurse supervisors’ behaviour when they gave the participating nurses the opportunity to work through the experiences gained in the daily provision of nursing care. Although they experienced lack of self-assurance during the supervision session, they also expressed security regarding their own performance as nurse supervisors. Nurse supervisors need to include more nursing theory and focus on the nursing process as well as being aware of their own shortcomings and resources. One way for the supervisior to scrutinize his/her actions is to discuss and examine them with a more experienced nurse supervisor colleague.

    • A theory based on the demography of top management teams is used to explain membership turnover in two Swedish business groups, network analysis being used to define group membership. The results suggest these business groups possess a combination of financial and industrial experience as a group resource and the socialising strategy of control as a force counteracting the conflict-producing force of heterogeneity. An organisational demographic perspective focusing on opposing forces of heterogeneity and homogeneity is developed. It is shown that the perspective can be applied both to formal organisations and to informal ones such as networks.

    • Background: It has become important to measure long-term effects and quality of life in survivors of childhood cancer. The Minneapolis- Manchester Quality of Life (MMQL) instrument has been proven to better capture the quality of life (QoL) perspective of health than other instruments. The instrument has age appropriate versions and is therefore favourable for longitudinal studies of QoL of children surviving from cancer. The aim of this study was to evaluate the psychometric properties of the Swedish version of MMQL-Youth Form and the Adolescent Form focusing on: 1) face and content validity 2) the internal consistency and 3) the test-retest reliability. Methods: The sample consisted of 950 pupils (11-16 years old) from 7 schools in the western Sweden who completed the questionnaire. For the test-retest evaluation 230 respondents completed the questionnaire two weeks later. Results: Face and content validity was supported and internal consistency was found to be acceptable for the total scale for both the MMQL-Youth Form (8-12 years of age) and the Adolescent Form (13-20 years of age). Test-retest reliability for the MMQL-Youth Form was moderate for 50% of the items and good for the remaining. For the MMQL-Adolescent Form the test-retest showed moderate or good agreement for 80% of the items and fair for 20%. Conclusions: The result indicated that the Swedish version of the MMQL-Youth Form and Adolescent Form was valid and reliable in a sample of healthy children in a Swedish context. It is recommended to test the instrument among diverse samples of children such as survivors of childhood cancer in order to validate its usefulness in research and clinical settings.

    • PURPOSE: This study describes the quality of life (QOL) and uncertainty in patients who have an implantable cardioverter defibrillator (ICD) and predicts QOL at long-term follow-up. METHODS: Long-term follow-up was defined as 6.9 years +/- 1 year (range 4.11-8.7 years). QOL was measured with the Quality of Life Index, and uncertainty was measured with the Mishel Uncertainty in Illness Scale. RESULTS: The overall QOL and health/functioning were unchanged over time. QOL in the socioeconomic (P = .002) and psychologic/spiritual domains (P = .012) decreased in the first year. From baseline to long-term follow-up, the QOL in the family domain (P = .011) and uncertainty (P = .002) decreased. Uncertainty was a predictor of low QOL. CONCLUSION: QOL was reasonably good 6.9 years post-ICD implantation. Patients felt less uncertain once they had passed the first year of their illness.

    • In Europe, cardiovascular nursing (CVN) is a young branch of nursing science. The explicit knowledge contained in CVN dissertations has, so far, not been studied in Europe, and this is especially true in the case of Sweden. Accordingly, the aim of this literature study was to describe the status of and compare trends in Swedish dissertations in the area of CVN in terms of organisational structure, approach, research strategy, social orientation and socio-demographic aspects. The literature search resulted in 29 dissertations and a 26-item questionnaire that illuminated the problem areas. Most dissertations were produced in the universities of Göteborg, Halmstad and Linköping; a minority had a nurse as main supervisor; rehabilitation was the most common CVN approach; very few of the dissertations had an experimental design; and the majority was hospital-based. The main trends were (A) an increase in dissertations that were written during the last 6 years, (B) an increased number of nurses as main supervisors as well as publication in nursing journals, (C) an increase in hospital care settings while a decrease in community settings, and finally, (D) an increased number of dissertations addressing the issues of tertiary prevention as well as focusing on patients and next-of-kin as target groups. An important implication is to stimulate nurse-led interventions at all preventative levels in order to maintain or improve the cardiac health of both healthy and sick individuals, but also in order to visualize CVN and distinguish it from cardiology.

  • Mental health professional support in families with a member suffering from severe mental illness : a grounded theory model

    • The aim of this study was to develop a model of mental health professional (MHP) support based on the needs of families with a member suffering from severe mental illness (SMI). Twelve family members were interviewed with the focus on their needs of support by MHP, then the interviews were analyzed according to the grounded theory method. The generated model of MHP support had two core categories: the family members’ process from crisis to recovery and their interaction with the MHP about mental health/illness and daily living of the person with SMI. Interaction based on ongoing contact between MHP and family members influenced the family members’ process from crisis towards recovery. Four MHP strategies – being present, listening, sharing and empowering – met the family members’ needs of support in the different stages of the crisis. Being present includes early contact, early information and protection by MHP at onset of illness or relapse. Listening includes assessing burden, maintaining contact and confirmation in daily living for the person with SMI. Sharing between MHP and family members includes co-ordination, open communication and security in daily living for the person with SMI. Finally, the MHP strategy empowering includes creating a context, counselling and encouraging development for the family members. The present model has a holistic approach and can be used as an overall guide for MHP support in clinical care of families of persons with SMI. For future studies, it is important to study the interaction of the family with SMI and the connection between hope, coping and empowerment.
