  • Like ticking time bombs. Improvising structural competency to ‘Defuse’ the exploding of violence against emergency care workers in Italy

    • While violence against health care workers is being progressively recognised as a serious problem in the healthcare industry, it remains an under-studied area of enquiry in global public health. Anthropologists have long observed that violence toward patients is tied to institutional care practises in multiple ways, including repression, misrecognition and silencing. But research on health care staff’s experience of violence is still lacking. This article aims to address this literature gap by providing research on the daily experience of vulnerability to violence that health care providers face during their work. To do so, the paper ethnographically explores the effects and perception of violence against health care workers in an emergency department (ED) in northern Italy, a place with a dramatic escalation of violent incidents. The article illustrates how the ED staff attended to the experience of suffering of potentially violent patients. In so doing, ED professionals shifted the responsibility of violence against them from violent individuals to violent structures shaping health inequities. The paper thus argues that ED professionals display a structural competence perspective when dealing with violence.

    • Mistrust is increasingly a daily reality of healthcare delivery worldwide. Yet it remains understudied as a form of relationship and a force in its own right. I address this gap through the ethnography of an Italian Emergency Department (ED), where conflicts have increased since the 2008 financial crisis. I show how mistrust does not result in a breakdown of healthcare interactions. Rather, mistrust is used in ambivalent care relationships to negotiate the roles, the risks, and the power that patients and staff are willing to entrust to others. Mistrust manifests in risk management strategies within relationships of “mistrustful dependency.”.

  • The Negotiation of Urgency: Economies of Attention in an Italian Emergency Room

    • Urgency in a hospital Emergency Room (ER) is not a self-evident state. Urgency is made, by establishing priorities, distributing attention and material resources, and deciding who and what needs to be attended to first – and, simultaneously, who and what has to wait. The process of determining urgency is known as “triage” (from the French verb, trier, “to choose”). This thesis is about the vicissitudes of triage in an Italian ER. Based on one year of ethnographic fieldwork, the thesis explores what happens when urgency is at stake; when it is contested and caught up between different, and frequently conflicting, perspectives. It explores how urgency is determined in practice, and shows how triage always is a vulnerable process of negotiation guided by economies of attention. How is urgency actually shaped in interactions between patients, their families and friends, and the ER staff? The different chapters explore how time in the ER is created through shifting registers of attention, and how attention in the ER is affected by widespread economic and social precarity, and neoliberal national policies of governance. It discusses how triage increasingly is structured by attitudes of mistrust; and also by potential or real outbreaks of violence. Addressing the particular positioning of the ER as a thick space of conjunction between neoliberal state politics and people's increasing need for care and recognition, the thesis aims to contribute to medical anthropology literature by analyzing triage not as a neutral medical way of sorting, but as a practice that actively creates difference. It explores both the limits of triage, and how those limits can spark improvisation and creative reinvention.

    • This article investigates the cross-linguistic comparability of the newly developed lexical assessment tool Cross-linguistic Lexical Tasks (LITMUS-CLT). LITMUS-CLT is a part the Language Impairment Testing in Multilingual Settings (LITMUS) battery (Armon-Lotem, de Jong & Meir, 2015). Here we analyse results on receptive and expressive word knowledge tasks for nouns and verbs across 17 languages from eight different language families: Baltic (Lithuanian), Bantu (isiXhosa), Finnic (Finnish), Germanic (Afrikaans, British English, South African English, German, Luxembourgish, Norwegian, Swedish), Romance (Catalan, Italian), Semitic (Hebrew), Slavic (Polish, Serbian, Slovak) and Turkic (Turkish). The participants were 639 monolingual children aged 3;0-6;11 living in 15 different countries. Differences in vocabulary size were small between 16 of the languages; but isiXhosa-speaking children knew significantly fewer words than speakers of the other languages. There was a robust effect of word class: accuracy was higher for nouns than verbs. Furthermore, comprehension was more advanced than production. Results are discussed in the context of cross-linguistic comparisons of lexical development in monolingual and bilingual populations.

  • Autism and the Question of the Human

    • The article explores how normative notions of emotions and interaction are active in constructions of the categories of “human” and “animal” in different discourses about autism: scientific and autobiographical. In the scientific discourse of autistic emotionality, a deficit perspective of autism is central. The general affective deficit discourse relies on normative discursive notions of “humanity” or “human emotionality.” Thus, neurotypicals are produced as real “humans” and neurotypical emotionality as “normal” human emotionality. This human normativity is challenged in the Swedish autobiographical texts by Gunilla Gerland (b. 1963), Iris Johansson (b. 1945) and Immanuel Brändemo (b. 1980). Along with American authors of autobiographies about autism, such as Temple Grandin’s Thinking in Pictures (1995) and Dawn Prince-Hughes’ Songs of the Gorilla Nation (2004) they destabilize the categories of “human” and “animal” by identifying with nonhuman animals, describing themselves as such, or feeling disqualified as real humans.

Men hälso- och sjukvården fungerar inte som den ska. Arbetsmiljön är osund, sjuksköterskor lämnar yrket, vården är ojämlik och svårt sjuka patienter får vänta i långa köer. Detta gör att människors liv, hälsa och värdighet står på spel.På spaning efter etik uppmärksammar tre nyckelgrupper av aktörer som genom sina beslut har ett stort inflytande över vårdens villkor: politiker, opolitiska chefstjänstemän och vårdgivarnas högsta chefer i landstingets ledningsorganisation.     • There are an increasing number of medical research studies involving children, including many long-term birth cohort studies. Involving children raises many issues, and little is known about children's own views. This study explored children's views (N = 5,851) on participation in a long-term screening study for type 1 diabetes. The results show that children 10 to 13 years of age have in general a positive attitude to pediatric research and emphasized trust in researchers. The children stressed the importance to receive information and to be involved in decisions. The children also reported feeling concerned about blood sampling and disease risk. Researchers involved in long-term pediatric research need to address these issues to promote involvement and decrease worry.

    • BackgroundEthical analyses of disorders of consciousness traditionally focus on residual awareness. Going one step further, this paper explores the potential ethical relevance of the unawareness retained by patients with disorders of consciousness, focusing specifically on the ethical implications of the description of the unconscious provided by recent scientific research.MethodsA conceptual methodology is used, based on the review and analysis of relevant scientific literature on the unconscious and the logical argumentation in favour of the ethical conclusions.ResultsTwo conditions (experiential wellbeing and having interests) that are generally considered critical components in the ethical discussion of patients with disorders of consciousness might arguably be both conscious and unconscious.ConclusionsThe unconscious, as well as consciousness, should be taken into account in the ethical discussions of patients with disorders of consciousness.

    • The aim of this study was to describe nurses' conceptions of decision making with regard to life-sustaining treatment for dialysis patients. Semistructured interviews were conducted with 13 nurses caring for such patients at three hospitals. The interview material was subjected to qualitative content analysis. The nurses saw decision making as being characterized by uncertainty and by lack of communication and collaboration among all concerned. They described different ways of handling decision making, as well as insufficiency of physician-nurse collaboration, lack of confidence in physicians, hindrances to patient participation, and ambivalence about the role of patients' next of kin. Future research should test models for facilitating communication and decision making so that decisions will emerge from collaboration of all concerned. Nurses' role in decision making also needs to be discussed.

    • IntroductionModern sports equipment is nowadays manufactured industrially according to globally accepted and standardized models, but traditionally tools for play and games were prepared from materials found in the local environment. The objective of this article is to investigate various aspects of Sami local knowledge about organisms used for their material culture of traditional sports and games in northern Fennoscandia (Sapmi). What functions did the surrounding biota have in the production of equipment used in sports and games?MethodsA qualitative method was used; the ethnographic literature and travel narratives have been analyzed particularly for descriptions and notes on traditional games, toys, and sports.ResultsBefore the turn of the twentieth century, bats, balls, and skis were seldom produced in factories, but by children and adults who utilized available materials from the surrounding environment. The manufacture of tools for play and games was characterized by a rich creativity in the use of various biological and natural resources. A wide range of such resources is presented in this article, among them the bracket fungus Fomitopsis betulina, used for making balls, reindeer antlers utilized for lassoing contests, and pine bark painted with reindeer blood, prepared for playing cards. We also highlight how tools usually associated with means of transport could switch functions and serve playful and competitive purposes, such as skis made of compression pine or walking sticks of birch: The former were used in skiing races, and the latter appeared in fencing competitions.ConclusionThe industrialization of the material culture of sports has been contributed to a loss of local knowledge and familiarity with locally available organic stuffs for producing equipment for play and games. By reconnecting with previous knowledge of traditional games, we discover a potentially new direction for modern sports and games, shifting from globalization to environmentalization. Such an environmentalization could permit the local environmental context define the content, meaning and structure of sports, and simultaneously enrich both sports and outdoor life.
