  • I Stay : Swedish Specialist Nurses in the Perioperative Context and Their Reasons to Stay at Their Workplace

    • PURPOSE: To investigate why nurse anesthetists and operating room nurses choose to stay in the same workplace.DESIGN: Qualitative design.METHODS: Individual interviews were conducted with 15 nurse specialists from four hospitals in Sweden. Two men and 13 women aged between 43 and 63 participated. Data were analyzed with systematic text condensation according to Malterud.FINDINGS: Three themes were identified. (1) Organizational stability contributed to low staff turnover, with good spirits between colleagues, representing everyone's equal value and resulting in a feeling of homelikeness. (2) Sustained development in one's own profession. (3) A humane head nurse who was at hand, who was a facilitator, who knew staff members, and eliminated obstacles for them.CONCLUSIONS: In a nonhierarchical and stable organization with a head nurse with caritative leadership skills, a welcoming working environment with opportunities for professional development is created. Thus, nurse specialists choose to stay, contributing to organizational development.

  • Iron status influences trace element levels in human

    • Food is the main source of trace elements for the general Population. The gastrointestinal absorption of certain trace elements, e.g., cadmium, is strongly influenced by iron (Fe) status. This factor may also be relevant for the bioavailability of other trace elements. Therefore, we investigated relationships between Fe status indicators and trace element concentrations in blood and serum of 234 boys and girls at ages 15 and 17 years. Fe status was measured using serum ferritin (S-Ft), soluble transferrin receptor in serum (sTfR), and the ratio sTfR/S-Ft. The trace elements we investigated were, in blood, cadmium, cobalt, copper, zinc, selenium, rubidium, mercury, and lead, and, in serum, cobalt, copper, zinc, selenium, rubidium, tungsten, mercury, and lead. We found inverse correlations between Fe status and blood cadmium, blood or serum cobalt, or blood copper. There were positive correlations between Fe status and mercury concentrations. selenium was positively correlated with sTfR. The relationships between Fe status and lead were equivocal. There were fewer correlations for serum than for blood, but the inverse relationships between Fe status and cobalt were equally strong in serum and blood. We found only occasional, and perhaps spurious, correlations with zinc, rubidium, and tungsten. In conclusion, previous indications that cadmium, cobalt, and copper are absorbed by transport mechanisms similar to that of Fe are supported by this study. Strong positive correlations between Fe status and mercury concentrations remain to be explained.

    • Proteininnehållet i svenskkost är relativt högt i förhållande till internationella rekommendationer. Höga proteinintaghos barn har satts i samband med ökad risk för övervikt. Hos vuxna har ett högt proteinintag,och därmed förhöjda homocysteinnivåer i serum, samtidigt med ett lågt intag av folacin förts fram som riskfaktorer förhjärt–kärlsjukdom. Det finns inga fördelar – men eventuellt nackdelar – med ett proteinintag över 15 energiprocent, eller1,5 g/kg kroppsvikt.

    • The aim of this article is to describe how experince-based knowledge can be made visible by giving some examples of how this has been done in Swedish social welfare services, in collaborations between social services agencies and research and development (R&D) units. These examples will be linked to theories and discussed in relation to different research utilisation models. By using one of these models, it is argued that R&D activities can broaden the concept of evidence-based practice and help brigde the gap between research and practice.

    • Elucidating patients´ experiences of living with chronic progressive hereditary ataxia and the symptomatic treatment with intrathecal baclofen (ITB) is the objective of the current study. A multicenter qualitative study with four patients included due to the rare combination of hereditary ataxia and ITB therapy was designed to elucidate participants' experiences through semi-structured interviews. The transcribed text was analyzed according to content analysis guidelines. Overall we identified living in the present/ taking one day at a time as the main theme covering the following categories: 1) Uncertainty about the future as a consequence of living with a hereditary disease; The disease; 2) Impact on life as a whole, 3) Influence on personal life in terms of feeling forced to terminate employment, 4) Limiting daily activities, and 5) ITB therapy, advantages, and disadvantages. Uncertainty about the future was the category that affected participants' personal life, employment, and daily activities. The participants' experience of receiving ITB therapy was expressed in terms of improved quality of life due to better body position and movement as well as better sleep and pain relief.

  • Physical activity, energy expenditure and their correlates in two cohorts of Swedish subjects between adolescence and early adulthood

    • OBJECTIVE: To assess physical activity and energy expenditure and to identify their correlates during adolescence and early adulthood. DESIGN: In a cohort study, total energy expenditure (TEE) and physical activity level (PAL) were assessed at 15 and 21 y from a 7-day activity diary and predicted BMR. The influences on TEE and PAL of body size, gender, sexual maturity, physical activity habits, sitting time, region, social conditions, employment, food habits, smoking and alcohol intake were examined in multivariate analyses.Subjects:71 male and 89 female subjects, living in two socioeconomically different regions of Sweden, a university region and an industrial region. SETTING: The university city of Uppsala and the industrial town of Trollhättan. RESULTS: At both 15 and 21 y, PAL and TEE were high, with gender, sitting time and physical activity habits as main correlates. At age 21 y, employment and the mother's educational level also appeared as significant correlates. The relations between the main variables and their correlates were more complex than at age 15 y, and the sitting time was reduced and the activity habits were changed. CONCLUSIONS: A reduction of daily sitting appears to be a major reason why high levels of physical activity and energy expenditure were maintained from 15 to 21 y of age in spite of changed and less frequent activity habits during this interval.

  • Different Personality Patterns in Non-Socialized (Juvenile Delinquents) and Socialized (Air Force Pilot Recruits) Sensation Seekers

    • Young delinquents are known to be sensation seekers. Not all sensation seekers become delinquents: many engage in socially accepted activities, such as mountaineering or parachute jumping. The present study compares 47 juvenile delinquents (mean age 17 years) with 18 Swedish air force pilot recruits (mean age 23 years) and 19 conscripts (mean age 18 years) as a control group. Sensation-seeking behaviour, impulsiveness, and psychiatric/psychological vulnerability were measured by the Zuckerman Sensation-Seeking Scales (SSS), the Karolinska Scales of Personality, and the Eysenck Personality Questionnaire. Two separate multivariate analyses of variance were performed, followed up by stepdown analyses to identify those personality scale scores that contributed uniquely. In order to clarify the relationships, the pooled within-group correlations among scales were computed. Juvenile delinquents and pilot recruits were both high in sensation seeking, but on different subscales. Delinquents were high in impulsiveness, somatic anxiety, and extraversion–sociability, and low in socialization, suggesting psychiatric/psychological vulnerability.   • Thalamo-striato-cortical determinants to fatigue in multiple sclerosis

    • BackgroundThe aim was to explore the thalamo-striato-cortical theory of central fatigue in multiple sclerosis (MS) patients with self-reported fatigue. If the theory correctly predicted fatigue based on disruptions of the thalamo-striato-cortical network, we expected altered brain activation in this network in MS participants while performing a complex cognitive task that challenged fatigue.MethodsMS participants with self-reported fatigue were examined by functional magnetic resonance imaging (fMRI) during the performance of a complex working memory task. In this task, cognitive effort was challenged by a parametric design, which modeled the cerebral responses at increasing cognitive demands. In order to explore the theory of central fatigue in MS we also analyzed the cerebral responses by adding perceived fatigue scores as covariates in the analysis and by calculating the functional connectivity between regions in the thalamo-striatocortical network. The main findings were that MS participants elicited altered brain responses in the thalamo-striato-cortical network, and that brain activation in the left posterior parietal cortex and the right substantia nigra was positively correlated to perceived fatigue ratings. MS participants had stronger cortical-to-cortical and subcortical-to-subcortical connections, whereas they had weaker cortical-to-subcortical connections.ConclusionsThe findings of the present study indicate that the thalamo-striato-cortical network is involved in the pathophysiology of fatigue in MS, and provide support for the theory of central fatigue. However, due to the limited number of participants and the somewhat heterogeneous sample of MS participants, these results have to be regarded as tentative, though they might serve as a basis for future studies.

  • Work capacity and health-related quality of life among individuals with multiple sclerosis reduced by fatigue: a cross-sectional study

    • Background: Among individuals diagnosed with the chronic neurologic disease, multiple sclerosis (MS), a majority suffers from fatigue, which strongly influences their every-day-life. The aim of this study was to investigate work capacity and health-related quality of life (HRQoL) in a group of MS patients and also to investigate if work capacity and HRQoL could be predicted by background factors, fatigue, heat sensitivity, cognitive dysfunction, emotional distress or degree of disability. less thanbrgreater than less thanbrgreater thanMethods: A descriptive, cross-sectional, designed survey was undertaken A questionnaire was sent to 323 individuals diagnosed with MS, aged between 20 and 65 years, with physical disability on the expanded disability status score (EDSS) in the interval 0 andgt;= EDSS andlt;= 6.5, living in sterg"tland county in eastern Sweden. Questions on background factors, occupation and work, together with the health-related quality of life short form instrument (SF-36), the fatigue severity scale (FSS), the perceived deficit questionnaire (PDQ) and the hospital anxiety depression scale (HAD) were posed. Associations between variables were analyzed using Pearsons and Spearmans correlations. Differences between groups were tested using the Chi-square test, the Mann Whitney U-test, and the Students t-test. Predictive factors were analyzed using multiple linear and multiple logistic regression analysis. less thanbrgreater than less thanbrgreater thanResults: Of those who completed the questionnaire (n = 257, 79.6%), 59.8% were working. Work capacity was found significantly more among men (p andlt; 0.005), those with a higher level of education (p andlt; 0.001), those reporting less fatigue (p andlt; 0.001), and those having no heat sensitivity (p = 0.004). For work capacity, significant predictors were low physical disability (EDSS), low fatigue, higher level of education, male sex and lower age. Those with work capacity showed significantly higher HRQoL than those who had no work capacity (p andlt; 0.001). Levels of fatigue, cognition and emotional distress were found to be major contributing factors for HRQoL. less thanbrgreater than less thanbrgreater thanConclusions: Work capacity and HRQoL among individuals diagnosed with MS are highly influenced by fatigue which can be considered as a key symptom. Work capacity was influenced by heat-sensitivity, cognitive difficulties and emotional distress and significant predictive factors besides fatigue, were physical disability (EDSS), age, sex, and level of education. Remaining at work also gives a better HRQoL.
