    • The purpose of this study was to explore, by qualitative methods, the meaning of equine-assisted social work (EASW) both to young women with self-harm problems and their staff in residential treatment. Data were collected by in-depth interviews with eight staff members and nine clients. Human-horse interaction was observed subsequently in three of the staff and four of the clients. Based on the narratives of the staff members and the clients, who agreed, the horse’s ability to read human emotions made the staff and clients aware of their own emotions. In order not to alienate the horse or make the situation dangerous, which intensified their sense of being in the present, both the clients and staff had to regulate their emotions. As a result the resistance for change decreased, made the participants more likely to regulate their emotions and change behavior rather than avoid the situation or the information. The horse seemed to set the framework for the interaction between the staff and young women. The fact that even the staff could fail in the interaction with the horse, affected the interaction between staff and clients making it more informal and dynamic. A relationship based on empathy, trust, respect and negotiation, where clients shared private matters, resulted in the perception of a more authentic relationship. Further research is needed to see which conditions are favourable for achieving an authentic relationship and which conditions possibly fail.

  • Mentalizing and Emotional Labor Facilitate Equine-Assisted Social Work with Self-harming Adolescents

    • This article explores equine-assisted social work (EASW). Horses' capacities to mirror human emotions create possibilities for authentic relationships between clients and staff. This study examines what eases or counteracts the horse's capacity to facilitate relationships perceived by humans to be authentic. Video recordings of the human-horse interactions of three staff members and four female self-harming clients aged 15–21 years in a residential treatment facility were analyzed. The findings show that if the staff gave instructions and advice similar to traditional equestrian sports in combination with viewing the horse as an object, EASW is not facilitated. EASW seems to be facilitated when the horse is perceived as a subject by both staff and clients, provided that the staff gave meaning to the horse's behavior. The staff needed to highlight empathy for the horse when the horse is not able to fulfill its task without adding depth to the client's performance, to avoid raising defense mechanisms. The essence of EASW were perceived as eased by staff members when they focus on the client's emotions and help the client understand that the horse is acting in response to the client's and the staff's behavior through mentalizing and enacting emotional labor in regarding the horse as a subject. The results indicate the need for higher demands on staff members in order to facilitate EASW. Depending on whether the staff and the clients focus on performance or on emotions, different positive or negative outcomes on communication, self-confidence, self-esteem, and self-image will be likely to emerge.

  • Higher- and lower-order personality traits and cluster subtypes in social anxiety disorder

    • Social anxiety disorder (SAD) can come in different forms, presenting problems for diagnostic classification. Here, we examined personality traits in a large sample of patients (N = 265) diagnosed with SAD in comparison to healthy controls (N = 164) by use of the Revised NEO Personality Inventory (NEO-PI-R) and Karolinska Scales of Personality (KSP). In addition, we identified subtypes of SAD based on cluster analysis of the NEO-PI-R Big Five personality dimensions. Significant group differences in personality traits between patients and controls were noted on all Big Five dimensions except agreeableness. Group differences were further noted on most lower-order facets of NEO-PI-R, and nearly all KSP variables. A logistic regression analysis showed, however, that only neuroticism and extraversion remained significant independent predictors of patient/control group when controlling for the effects of the other Big Five dimensions. Also, only neuroticism and extraversion yielded large effect sizes when SAD patients were compared to Swedish normative data for the NEO-PI-R. A two-step cluster analysis resulted in three separate clusters labelled Prototypical (33%), Introvert-Conscientious (29%), and Instable-Open (38%) SAD. Individuals in the Prototypical cluster deviated most on the Big Five dimensions and they were at the most severe end in profile analyses of social anxiety, self-rated fear during public speaking, trait anxiety, and anxiety-related KSP variables. While additional studies are needed to determine if personality subtypes in SAD differ in etiological and treatment-related factors, the present results demonstrate considerable personality heterogeneity in socially anxious individuals, further underscoring that SAD is a multidimensional disorder.

    • International opportunities are the catalysts of SME internationalization. Our knowledge regarding the role of international opportunities in internationalization is undisputable, yet our understanding of the underlying mechanisms of international opportunity development remains inadequate. In this paper, we seek to address this shortcoming in the literature by answering the following research questions: how do internal and external factors influence the international opportunity scouting of SMEs, and what are the effects of the international opportunity scouting strategy in the internationalization of the firm? From a case study of nine Swedish SMEs, the authors develop propositions on the interplay between the theoretical constructs: international experience, network structure, international opportunity scouting, and international opportunity novelty. We propose that the greater the international experience of the SME, the more systematically the SME will scout for international opportunities; the more closed the network the SME is embedded in, the more systematically the SME will scout for international opportunities; and the more systematically the SME scouts for international opportunities, the lower the novelty of the opportunities identified by the SME. These findings come with implications for theory as well as for SME managers.

    • This paper studies the performance consequences of the speed of SME internationalization. The authors identify three research gaps: few studies treat speed as an independent variable; most studies analyze speed only until internationalization starts; and, finally, studies have paid little attention to the multidimensionality of the speed concept. The authors seek to address these gaps and to contribute to the literature on the dynamics of internationalization by developing three measures of internationalization speed, which capture its multidimensionality. Building on the theories of learning advantage of newness and time compression diseconomies, the study presents three hypotheses on speed’s effect on performance, and the theoretically derived research model is tested on a sample of 183 SMEs visited on site. The analysis demonstrates that the speed of a firm’s increase in the breadth of its international markets has a positive but curvilinear effect on firm performance. It also demonstrates that the speed of a firm’s increase in commitment of foreign resources has a negative but curvilinear effect on the performance of the firm. These results have implications both for scholars interested in the dynamics of firm internationalization and for SME managers

  • Self reported fear of childbirth and its association with women´s birth experience and mode of delivery : a longitudinal population-based study

    • ObjectiveTo explore fear of childbirth (FOC) during pregnancy and one year after birth and its association to birth experience and mode of delivery.DesignA longitudinal population-based study.PopulationPregnant women who were listed for a routine ultrasound at three hospitals in the middle-north part of Sweden.MethodDifferences between women who reported FOC and who did not were calculated using risk ratios with a 95% confidence interval. In order to explain which factors were most strongly associated to suffer from FOC during pregnancy and one year after childbirth, multivariate logistic regression analyses were used.ResultsFOC during pregnancy in multiparous women was associated with a previous negative birth experience (RR 5.1, CI 2.5–10.4) and a previous emergency caesarean section (RR 2.5, CI 1.2–5.4). Associated factors for FOC one year after childbirth were: a negative birth experience (RR 10.3, CI 5.1–20.7), fear of childbirth during pregnancy (RR 7.1, CI 4.4–11.7), emergency caesarean section (RR 2.4, CI 1.2–4.5) and primiparity (RR 1.9, CI 1.2–3.1).ConclusionFOC was associated with negative birth experiences. Women still perceived the birth experience as negative a year after the event. Women's perception of the overall birth experience as negative seems to be more important for explaining subsequent FOC than mode of delivery. Maternity care should focus on women's experiences of childbirth. Staff at antenatal clinics should ask multiparous women about their previous experience of childbirth. So that FOC is minimized, research on factors that create a positive birth experience for women is required.

  • Study of patient information after percutaneous coronary intervention (SPICI) : should prevention programmes become more effective?

    • Aims: This cross-sectional observational study was designed to evaluate the uptake and outcome of patient education after percutaneous coronary intervention (PCI).Methods and results: A questionnaire containing 41 items was handed out to consecutive patients from randomly selected Swedish hospitals after PCI. Questions concerned the patient's attribution of the cause of the cardiac event, perception of the information provided by physicians and nurses, and a self-assessment of changes in lifestyle post PCI regarding tobacco, physical activity, food habits and stress. Replies were obtained from 1,073 patients (reply rate 67%). Non-modifiable risk factors (age, heredity) were attributed a higher rate as the cause of disease compared to modifiable factors (smoking, physical activity, food habits). Most patients (67%) perceived they were cured, and 38% perceived from the given information that there was no need to change their habits. A mere 27% reported that they still had cardiovascular disease and needed behavioural change. After PCI, 16% continued to use tobacco; half of these were offered smoking cessation support. In spite of an 80% referral rate to cardiac rehabilitation, one out of two patients did not enrol. Fewer than half were regularly physically active. Nutritional counselling was provided to 71%, but only 40% changed food habits. Stress management programmes were rarely provided.Conclusions: Current preventive practice scarcely meets the challenge posed by the progress in modern invasive cardiology. The Study of Patient Information after percutaneous Coronary Intervention (SPICI) motivates an in-depth revision and adaptation of cardiac rehabilitation programmes in order to improve patient understanding of the disease, and to support greater compliance with a cardioprotective lifestyle.
