    • Abstract: The aim of this chapter was to analyze associations between measures of cortisol in salivaand mental health and to see if divergent results were functions of the methods used. Measures ofmental health outcome included Major Depressive Disorder (MDD), symptoms of depression, andsymptoms of anxiety, Burnout (BO), and Vital Exhaustion (VE). Only studies on otherwise healthyindividuals were included. Cortisol measures were grouped into single time point measures, measuresof deviations, laboratory test responses, Area Under the Curve (AUC), and response to dexamethasone.Some consistency is seen for MDD, mainly higher mean levels. The results regarding single measuresand depressive mood are less consistent, but the overall picture for depression shows poorer diurnaldeviation and response to stress. Inconsistency among papers studying depression seems to be relatedmainly to the study population. Very few significant findings were found for anxiety, therefore cortisoldoes not seem to be strongly related to anxiety. Most of the statistical analysis does not show asignificant relationship between BO and cortisol, and when these are present, the results areinconsistent. One explanation seems to be the measures of BO used, probably due to the differentconceptual basis for BO. VE measured using the Maastricht Questionnaire seems to be related to apoorer cortisol response to stress and poorer diurnal deviation. The coexistence of BO and VE in manystudies does make it difficult to conclude how the different concepts are related to cortisol. However, aninteresting difference appeared between MDD and VE in response to dexamethasone administration,showing lower suppression in MDD patients and higher suppression in VE patients. A generalconclusion for all mental health measures is that a large proportion of non-significant findings are dueto low power and few sampling days combined with low contrasts between study groups and withinstudy populations. Generally, deviation measures such as diurnal deviation seem to be more validmeasures compared with single measures to capture possible changes in the hypothalamus-pituitaryadrenal axis, measured using salivary cortisol.

    • Medicine today uses a wide range of biomaterials, most of which make contact with blood permanently or transiently upon implantation. Contact between blood and nonbiological materials or cells or tissue of nonhematologic origin initiates activation of the cascade systems (complement, contact activation/coagulation) of the blood, which induces platelet and leukocyte activation.Although substantial progress regarding biocompatibility has been made, many materials and medical treatment procedures are still associated with severe side effects. Therefore, there is a great need for adequate models and guidelines for evaluating the blood compatibility of biomaterials. Due to the substantial amount of cross talk between the different cascade systems and cell populations in the blood, it is advisable to use an intact system for evaluation.Here, we describe three such in vitro models for the evaluation of the biocompatibility of materials and therapeutic cells and tissues. The use of different anticoagulants and specific inhibitors in order to be able to dissect interactions between the different cascade systems and cells of the blood is discussed. In addition, we describe two clinically relevant medical treatment modalities, the integration of titanium implants and transplantation of islets of Langerhans to patients with type 1 diabetes, whose mechanisms of action we have addressed using these in vitro models.

    • Spinal cord injury (SCI) is one of the leading causes of disability in Military personnel for which no suitable therapeutic strategies are available till today. Thus, exploration of novel therapeutic measures is highly needed to enhance the quality of life of SCI victims. Previously, topical application of BDNF and GDNF in combination over the injured spinal cord after 90min induced marked neuroprotection. In present investigation, we added CNTF in combination with BDNF and/or GDNF treatment to examine weather the triple combination applied over the traumatic cord after 90 or 120min could thwart cord pathology. Since neurotrophins attenuate nitric oxide (NO) production in SCI, the role of carbon monoxide (CO) production that is similar to NO in inducing cell injury was explored using immunohistochemistry of the constitutive isoform of enzyme hemeoxygenase-2 (HO-2). SCI inflicted over the right dorsal horn of the T10-11 segments by making an incision of 2mm deep and 5mm long upregulated the HO-2 immunostaining in the T9 and T12 segments after 5h injury. These perifocal segments are associated with breakdown of the blood-spinal cord barrier (BSCB), edema development and cell injuries. Topical application of CNTF with BDNF and GDNF in combination (10ng each) after 90 and 120min over the injured spinal cord significantly attenuated the BSCB breakdown, edema formation, cell injury and overexpression of HO-2. These observations are the first to show that CNTF with BDNF and GDNF induced superior neuroprotection in SCI probably by downregulation of CO production, not reported earlier.

    • X-ray microanalysis conducted using the scanning electron microscope is a technique that allows the determination of chemical elements in bulk or semi-thick specimens. The lowest concentration of an element that can be detected is in the order of a few mmol/kg or a few hundred parts per million, and the smallest amount is in the order of 10(-18) g. The spatial resolution of the analysis depends on the thickness of the specimen. For biological specimen analysis, care must be taken to prevent displacement/loss of the element of interest (usually ions). Protocols are presented for the processing of frozen-hydrated and freeze-dried specimens, as well as for the analysis of small volumes of fluid, cell cultures, and other specimens. Aspects of qualitative and quantitative analysis are covered, including limitations of the technique.
