  • Challenges in Elderly Night-Time Care : Dignity 24 Hours a Day in Swedish Elderly Home Care Services?

    • The research study explores a large knowledge gap within Swedish elderly care – namely the challenges of within home care services at night. Despite that home care services is the most common support given to the elderly in Sweden. The few National and International studies that exist on night-time care, have focused on residential care facilities for the elderly. Therefore, focus here is on night-time care in their own homes, given by the night patrol – which we know very little about.In the last decades, organizational transformation has taken place within elderly care towards increased rationalization and marketization. Along with demographical changes, this raises questions of dignified care, equal social rights and access to care. As the elderly are living longer and living in their own homes with severe disability and vulnerability, the pressure on home care services will increase – including at night. Dignity in care has been legislated in the Swedish Social Services Act, but is vaguely implemented, specifically at night.Discussed are results of an interview survey with managers responsible for night-time home care services in 50 Swedish municipalities. How night-time care is organized varies depending on local governance, location and size of municipalities. This indicates challenges for equal access to care, depending on where you live. Care workers experience time pressure at night with many fragile elderly in need of care. How efforts to secure dignity and safety for older people varies over night is therefore important to reveal, not the least from a social equality perspective.

  • Chemosensory perception, symptoms and autonomic responses during chemical exposure in multiple chemical sensitivity

    • Purpose: Multiple chemical sensitivity (MCS) is a prevalent medically unexplained symptom characterized by symptom reactions to everyday chemical exposure below hygienic thresholds. The aim of this study was to investigate the expressions of hyper-reactivity in MCS during whole-body exposure to low concentrations of the odorant n-butanol.Methods: We exposed 18 participants with MCS and 18 non-ill controls to a low concentration of the odorantn-butanol using an exposure chamber. The first 10 min constituted blank exposure, after which then-butanol concentration increased and reached a plateau at 11.5 mg/m3.Results: MCS participants, compared with controls, reported greater perceived odor intensities, more unpleasantness to the exposure and increasing symptoms over time. MCS participants also expressed higher pulse rate and lower pulse rate variability than controls did. No group differences were found for breathing rate or tonic electrodermal activity responses.Conclusions: We conclude that MCS sufferers differ from healthy controls in terms of autonomic responses, symptoms and chemosensory perception during chemical exposure.

    • Chemical intolerance is a medically unexplained affliction that implies deleterious reactions to non-toxic everyday chemical exposure. Sensitization (i.e. increased reactivity to repeated, invariant stimulation) to odorous stimulation is an important component in theoretical explanations of chemical intolerance, but empirical evidence is scarce. We hypothesized that (1) individuals who sensitize to repeated olfactory stimulation, compared with those who habituate, would express a lower blood oxygenated level dependent (BOLD) response in key inhibitory areas such as the rACC, and higher signal in pain/saliency detection regions, as well as primary and/or secondary olfactory projection areas; and (2) olfactory sensitization, compared with habituation, would be associated with greater self-reported chemical intolerance. More-over, we assessed whether olfactory sensitization was paralleled by comparable trigeminal processing - in terms of perceptual ratings and BOLD responses. We grouped women from a previous functional magnetic imaging study based on intensity ratings of repeated amyl acetate exposure over time. Fourteen women sensitized to the exposure, 15 habituated, and 20 were considered "intermediate" (i.e. neither sensitizers nor habituaters). Olfactory sensitizers, compared with habituaters, displayed a BOLD-pattern in line with the hypothesis, and reported greater problems with odours in everyday life. They also expressed greater reactions to CO2 in terms of both perceived intensity and BOLD signal. The similarities with pain are discussed.

  • Late stage pine litter decomposition : Relationship to litter N, Mn, and acid unhydrolyzable residue (AUR) concentrations and climatic factors

    • The aim of this paper is to evaluate relationships between decomposition rates of Scots pine (Pinus sylvestris) and lodgepole pine (Pinus contorta var. contorta) needle litter in the late stage of decomposition (>30% accumulated mass loss), and the progressively changing concentrations of manganese (Mn), nitrogen (N), and acid unhydrolyzable residue (AUR), as well as mean annual temperature (MAT) and mean annual precipitation (MAP). Using available long-term decomposition studies on pine needle litter in a climate gradient in Sweden, we calculated annual mass loss and related to concentrations of Mn, N, and AUR at the start of each one-year period as well as to MAT and MAP. We investigated these relationships for (i) all data on annual mass loss combined and (ii) annual mass loss for five different decomposition categories as defined by accumulated mass loss. We found highly significant, negative, and dominant relationships between annual mass loss and N (R2=0.39) and AUR (R2=0.39), a slight but significant positive relationship to Mn (R2=0.08) and a significant negative relationship to MAT (R2=0.06). The relationships were dynamic, and changed with accumulated mass loss. The rate-dampening effect of N decreased to be a rate-enhancing effect at c. 60-80% accumulated mass loss. A similar trend was found for AUR, becoming rate-enhancing at 70-80% accumulated mass loss. For Scots pine needle litter the effect of MAT on mass loss decreased with increasing accumulated mass loss and changed to a rate-dampening effect at c. 50-70% accumulated mass loss. Mn showed a stimulating effect on mass loss rate in all categories whereas MAP showed no effect in this mainly boreal climatic gradient. The current approach indicates a method for detailed studies of rate-regulating factors for litter decomposition.

  • Manganese in the litter fall-forest floor continuum of boreal and temperate pine and spruce forest ecosystems : a review

    • We have reviewed the literature on the role of manganese (Mn) in the litter fall-to-humus subsystem. Available data gives a focus on North European coniferous forests. Manganese concentrations in pine (Pinus spp.) foliar litter are highly variable both spatially and temporally within the same litter species and for the genus Pinus we found a range from 0.03 to 3.7mgg-1. Concentrations were related negatively to site mean annual temperature (MAT) and annual actual evapotranspiration (AET) for pine species litter but not for that of Norway spruce (Picea abies) as a single species. Combined data for several species showed a highly significant relationship to MAT.Manganese peroxidase is an Mn-dependent enzyme, found in white-rot fungi, essential for the degradation of lignin and ligninlike compounds. The decomposition rates of lignified litter tissue (late phase) is positively related to the litter’s Mn concentration. Further, the Mn concentration is positively related to the limit value for decomposition - the higher the Mn concentration the smaller the stable litter fraction. Manganese release from decomposing litter appears at least in part to be species related. Thus was release from pine needle litter significantly faster (p<. 0.001) than that from the Mn-richer litter of Norway spruce. Over Northern Europe concentrations of total Mn in mor humus as well as extractable Mn in the mineral soil increase with decreasing MAT and over a climatic gradient the Mn concentrations in Norway spruce mor increase more with decreasing MAT than in a gradient with Scots pine. Higher Mn concentrations in humus appear to decrease its stability and result in a higher release of carbon dioxide (CO2) and dissolved organic carbon (DOC). We conclude that this may explain (i) the lower amount of carbon (C) in mor layers under Norway spruce as compared to Scots pine as well as the higher amount of C in mineral soil under spruce. The increase in nitrogen (N) concentration in humus, following N fertilization resulted in a decrease in that of Mn. We have found four cases - empirical - with negative interaction between Mn and N; (i) in pine foliar litter fall concentrations of Mn decrease with site MAT whereas those of N increase, (ii) in decomposing late-stage litter with N retarding and Mn stimulating decomposition, (iii) for the stable phase, limit values are related negatively to N and positively to Mn, and (iv) Mn concentrations in humus decrease with MAT whereas those of N increase.

    • PurposeThe aim was to determine the responsiveness and minimal important change (MIC) of the questionnaire ProFitMap-neck that measures symptoms and functional limitations in people with neck pain. The same measurement properties were determined for Neck Disability Index (NDI) for comparison purposes.MethodsLongitudinal data were derived from two randomized controlled trials, including 103 and 120 women with non-specific neck pain, with questionnaire measurements performed before and after interventions. Sensitivity and specificity to discriminate between improved and non-improved participants, based on categorization of a global rating of change scale (GRCS), were determined for the ProFitMap-neck indices and NDI by using area under receiver operator curves (AUC). Correlations between the GRCS anchor and change scores of the questionnaires were also used to assess responsiveness. The change score that showed the highest combination of sensitivity and specificity was set for MIC.ResultsThe ProFitMap-neck indices showed similar responsiveness as NDI with AUC exceeding 0.70 (Range: ProFitMap-neck, 0.74 – 0.83; NDI, 0.75 – 0.86). The MIC in the two samples ranged between 6.6 – 13.6% for ProFitMap-neck indices and 5.2 and 6.3% for NDI. Both questionnaires had significant correlations with GRCS (Spearman’s rho 0.47 – 0.72).ConclusionsValidity of change scores was demonstrated for the ProFitMap-neck indices with adequate ability to discriminate between improved and non-improved participants. Values of minimal important change were presented.

    • Background The influence of various physical exercise frequencies on postural balance and muscle performance among young persons with intellectual disability (ID) is not well understood.Method Cross-sectional data from 26 elite athletes were compared with 37 students at a sports school and to 57 students at a special school, all diagnosed with mild to moderate ID and with different exercise frequencies. Data were also compared with a group of 149 age-matched participants without ID.Results There were no significant differences in postural balance between young ID groups regardless of physical exercise frequency, all of them had however impaired postural balance compared to the non-ID group. The group with high exercise performed better than the other ID groups in the trunk muscle endurance test.Conclusions It appears as if physical exercise frequency don’t improve postural balance but endurance in the trunk muscles for young persons with ID.

    • Background: Chemical intolerance (CI) is a widespread occupational and public health problem characterized by symptoms that reportedly result from low-levels of chemical exposure. The mechanisms behind CI are unknown, however modifications of the chemical senses (rather than toxic processes) have been suggested as key components. The aim of this study was to investigate whether individuals with self-reported CI report more sensory irritation during masked acrolein exposure compared to controls without CI. Methods: Individuals with CI (n = 18) and controls without CI (n = 19) were exposed in an exposure chamber. Each participant took part in two exposure conditions – one with heptane (the masking compound), and one with heptane and acrolein at a dose below previously reported sensory irritation thresholds. The exposures lasted for 60 min. Symptoms and confidence ratings were measured continuously throughout the exposure as were measurements of electrodermal activity and self-reported tear-film break-up time. Participants were blind to exposure condition. Results: Individuals with CI, compared with controls reported greater sensory irritation in the eyes, nose and throat when exposed to acrolein masked with heptane. There was no difference during exposure to heptane. Conclusions: Masked exposure to acrolein at a concentration below the previously reported detection threshold is perceived as more irritating by individuals with CI compared with controls. The results indicate that there is altered trigeminal reactivity in those with CI compared to controls.

    • Chemical intolerance (CI) is a term used to describe a condition in which the sufferer experiences a complex array of recurrent unspecific symptoms attributed to low-level chemical exposure that most people regard as unproblematic. Severe CI constitutes the distinguishing feature of multiple chemical sensitivity (MCS). The symptoms reported by CI subjects are manifold, involving symptoms from multiple organs systems. In severe cases of CI, the condition can cause considerable life-style limitations with severe social, occupational and economic consequences. As no diagnostic tools for CI are available, the presence of the condition can only be established in accordance to criteria definitions. Numerous modes of action have been suggested to explain CI, with the most commonly discussed theories involving the immune system, central nervous system, olfactory and respiratory systems as well as altered metabolic capacity, behavioral conditioning and emotional regulation. However, in spite of more than 50 years of research, there is still a great deal of uncertainties regarding the event(s) and underlying mechanism( s) behind symptom elicitation. As a result, patients are often misdiagnosed or offered health care solutions with limited or no effect, and they experience being met with mistrust and doubt by health care professionals, the social care system and by friends and relatives. Evidence-based treatment options are currently unavailable, however, a person-centered care model based on a multidisciplinary treatment approach and individualized care plans have shown promising results. With this in mind, further research studies and health care solutions should be based on a multifactorial and interdisciplinary approach.
