  • EPUAP classification system for pressure ulcers : european reliability study

    • Aim. This paper is a report of a study of the inter-observer reliability of the European Pressure Ulcer Advisory Panel pressure ulcer classification system and of the differential diagnosis between moisture lesions and pressure ulcers. Background. Pressure ulcer classification is a valuable tool to provide a common description of ulcer severity for the purposes of clinical practice, audit and research. Despite everyday use of the European Pressure Ulcer Advisory Panel system, its reliability has been evaluated in only a limited number of studies. Methods. A survey was carried out between September 2005 and February 2006 with a convenience sample of 1452 nurses from five European countries. Respondents classified 20 validated photographs as normal skin, blanchable erythema, pressure ulcers (four grades), moisture lesion or combined lesion. The nurses were familiar with the use of the European Pressure Ulcer Advisory Panel classification scale. Results. Pressure ulcers were often classified erroneously (kappa = 0.33) and only a minority of nurses reached a substantial level of agreement. Grade 3 lesions were regularly classified as grade 2. Non-blanchable erythema was frequently assessed incorrectly as blanchable erythema. Furthermore, the differential diagnosis between moisture lesions and pressure ulcers appeared to be complicated. Conclusion. Inter-observer reliability of the European Pressure Ulcer Advisory Panel classification system was low. Evaluation thus needs to focus on both the clarity and complexity of the system. Definitions and unambiguous descriptions of pressure ulcer grades and the distinction between moisture lesions will probably enhance clarity. To simplify the current classification system, a reduction in the number of grades is suggested.

  • Criterion-related validity of the last 7-day, short form of the International Physical Activity Questionnaire in Swedish adults

    • OBJECTIVE: To examine the validity of the short, last 7-day, self-administered form of the International Physical Activity Questionnaire (IPAQ). DESIGN: All subjects wore an accelerometer for seven consecutive days and completed the IPAQ questionnaire on the eighth day. Criterion validity was assessed by linear regression analysis and by modified Bland-Altman analysis. Specificity and sensitivity were calculated for classifying respondents according to the physical activity guidelines of the American College of Sports Medicine/Centers for Disease Control and Prevention. SETTING: Workplaces in Uppsala, Sweden. SUBJECTS: One hundred and eighty-five (87 males) participants, aged 20 to 69 years. RESULTS: Total self-reported physical activity (PA) (MET-min day(-1)) was significantly correlated with average intensity of activity (counts min(-1)) from accelerometry (r = 0.34, P < 0.001). Gender, age, education and body mass index did not affect this relationship. Further, subcomponents of self-reported PA (time spent sitting, time in PA, time in moderate and vigorous activity (MVPA)) were significantly correlated with objectively measured PA (P < 0.05). Self-reported time in PA was significantly different from time measured by accelerometry (mean difference: -25.9 min day(-1); 95% limits of agreement: -172 to 120 min day(-1); P < 0.001). IPAQ identified 77% (specificity) of those who met the current PA guidelines of accumulating more than 30 min day(-1) in MVPA as determined by accelerometry, whereas only 45% (sensitivity) of those not meeting the guidelines were classified correctly. CONCLUSIONS: Our results indicate that the short, last 7-days version of the IPAQ has acceptable criterion validity for use in Swedish adults. However, the IPAQ instrument significantly overestimated self-reported time spent in PA. The specificity to correctly classify people achieving current PA guidelines was acceptable, whereas the sensitivity was low.

To investigate the astrometric effects of stellar surface structures as a practical limitation to ultra-high-precision astrometry (e.g. in the context of exoplanet searches) and to quantify the expected effects in different regions of the HR-diagram. Methods. Stellar surface structures (spots, plages, granulation, non-radial oscillations) are likely to produce fluctuations in the integrated flux and radial velocity of the star, as well as a variation of the observed photocentre, i.e. astrometric jitter. We use theoretical considerations supported by Monte Carlo simulations (using a starspot model) to derive statistical relations between the corresponding astrometric, photometric, and radial velocity effects. Based on these relations, the more easily observed photometric and radial velocity variations can be used to predict the expected size of the astrometric jitter. Also the third moment of the brightness distribution, interferometrically observable as closure phase, contains information about the astrometric jitter. Results. For most stellar types the astrometric jitter due to stellar surface structures is expected to be of the order of 10 micro-AU or greater. This is more than the astrometric displacement typically caused by an Earth-size exoplanet in the habitable zone, which is about 1-4 micro-AU for long-lived main-sequence stars. Only for stars with extremely low photometric variability (< 0.5 mmag) and low magnetic activity, comparable to that of the Sun, will the astrometric jitter be of the order of 1 micro-AU, sufficient to allow the astrometric detection of an Earth-sized planet in the habitable zone. While stellar surface structure may thus seriously impair the astrometric detection of small exoplanets, it has in general a negligible impact on the detection of large (Jupiter-size) planets and on the determination of stellar parallax and proper motion. From the starspot model we also conclude that the commonly used spot filling factor is not the most relevant parameter for quantifying the spottiness in terms of the resulting astrometric, photometric and radial velocity variations.

  • Sexual patterns of prebreeding energy reserves in the common frog Rana temporaria along a latitudinal gradient

    • The ability to store energy is an important life history trait for organisms facing long periods without energy income, and in particular for capital breeders such as temperate zone amphibians, which rely on stored energy during reproduction. However, large scale comparative studies of energy stores in populations with different environmental constraints on energy allocation are scarce. We investigated energy storage patterns in spring (after hibernation and before reproduction) in eight common frog Rana temporaria populations exposed to different environmental conditions along a 1600 km latitudinal gradient across Scandinavia (range of annual activity period is 3-7 months). Analyses of lean body weight (eviscerated body mass), weight of fat bodies, liver weight, and liver fat content, showed that 1) post-hibernation/pre-breeding energy stores increased with increasing latitude in both sexes, 2) males generally had larger energy reserves than females and 3) the difference in energy stores between sexes decreased towards the north. Larger energy reserves towards the north can serve as a buffer against less predictable and/or less benign weather conditions during the short activity period, and may also represent a risk-averse tactic connected with a more pronounced iteroparous life history. In females, the continuous and overlapping vitellogenic activity in the north may also demand more reserves in early spring. The general sexual difference could be a consequence of the fact that, at the time of our sampling, females had already invested their energy into reproduction in the given year (i.e. their eggs were already ovulated), while the males' main reproductive activities (e.g. calling, mate searching, sexual competition) occurred later in the season.

  • Effects of influenza A virus infection on migrating mallard ducks

    • The natural reservoir of influenza A virus is waterfowl, particularly dabbling ducks (genus Anas). Although it has long been assumed that waterfowl are asymptomatic carriers of the virus, a recent study found that low-pathogenic avian influenza (LPAI) infection in Bewick's swans (Cygnus columbianus bewickii) negatively affected stopover time, body mass and feeding behaviour. In the present study, we investigated whether LPAI infection incurred ecological or physiological costs to migratory mallards (Anas platyrhynchos) in terms of body mass loss and staging time, and whether such costs could influence the likelihood for long-distance dispersal of the avian influenza virus by individual ducks. During the autumn migrations of 2002-2007, we collected faecal samples (n = 10 918) and biometric data from mallards captured and banded at Ottenby, a major staging site in a flyway connecting breeding and wintering areas of European waterfowl. Body mass was significantly lower in infected ducks than in uninfected ducks (mean difference almost 20 g over all groups), and the amount of virus shed by infected juveniles was negatively correlated with body mass. There was no general effect of infection on staging time, except for juveniles in September, in which birds that shed fewer viruses stayed shorter than birds that shed more viruses. LPAI infection did not affect speed or distance of subsequent migration. The data from recaptured individuals showed that the maximum duration of infection was on average 8.3 days (s.e. 0.5), with a mean minimum duration of virus shedding of only 3.1 days (s.e. 0.1). Shedding time decreased during the season, suggesting that mallards acquire transient immunity for LPAI infection. In conclusion, deteriorated body mass following infection was detected, but it remains to be seen whether this has more long-term fitness effects. The short virus shedding time suggests that individual mallards are less likely to spread the virus at continental or intercontinental scales.
