  • Frailty trajectories in three longitudinal studies of aging : Is the level or the rate of change more predictive of mortality?

    • BACKGROUND: frailty shows an upward trajectory with age, and higher levels increase the risk of mortality. However, it is less known whether the shape of frailty trajectories differs by age at death or whether the rate of change in frailty is associated with mortality.OBJECTIVES: to assess population frailty trajectories by age at death and to analyse whether the current level of the frailty index (FI) i.e. the most recent measurement or the person-specific rate of change is more predictive of mortality.METHODS: 3,689 individuals from three population-based cohorts with up to 15 repeated measurements of the Rockwood frailty index were analysed. The FI trajectories were assessed by stratifying the sample into four age-at-death groups: <70, 70-80, 80-90 and >90 years. Generalised survival models were used in the survival analysis.RESULTS: the FI trajectories by age at death showed that those who died at <70 years had a steadily increasing trajectory throughout the 40 years before death, whereas those who died at the oldest ages only accrued deficits from age ~75 onwards. Higher level of FI was independently associated with increased risk of mortality (hazard ratio 1.68, 95% confidence interval 1.47-1.91), whereas the rate of change was no longer significant after accounting for the current FI level. The effect of the FI level did not weaken with time elapsed since the last measurement.CONCLUSIONS: Frailty trajectories differ as a function of age-at-death category. The current level of FI is a stronger marker for risk stratification than the rate of change.

    • Social isolation and loneliness are often considered to be important and frequent problems in old age. However, the empirical support for this notion is weak. Results from Swedish studies show that rather extensive social networks continue into old age. Only a small minority can be said to be isolated and suffering from feelings of loneliness. This minority, however, reports a lower life satisfaction, which can also produce physical and psychological problems. Increased attention must be directed toward such negative consequences of weak social networks. This is especially important in discussions of how to alter a social situation characterized by loneliness and isolation. It is emphasized that further research within this field must be based on more explicit theories.

  • Overweight and dementia : a time-varying effect

    • Objectives: The negative effects of overweight on cardiometabolic health is well-known. An increasing body of evidence extends the negative effects of midlife overweight to dementia. However, a different picture emerge when overweight is assessed in late life. The time-varying effect of weight status on dementia was evaluated in two prospective Nordic population-based studies. Methods: The participants included in the Swedish Twin Registry self-reported their height and weight in 1963 (mean age 52.5 years). About 25 years later these twins were either included in the SATSA study (50 years and older) or the OCTO-Twin study (80 years and older). Dementia was consequently screened for and diagnosed according to the Diagnostic and Statistical Manual of Mental Disorders (DSM) criteria presently used at the time of diagnoses. The participants weight and height was assessed at baseline of the Finnish Lieto Study (mean age 70.8) and dementia was screened for and diagnosed according to the DSM-IV criteria eight years later. Results: Logistic regression analyses indicated that midlife overweight was associated with a greater risk of all cause dementia, odds ratio 1.55 (95% confidence interval (CI) = 1.18-2.04), when demographic and cardiometabolic risk factors and diseases were controlled for. However, Cox regression analyses indicated that for each unit increase in BMI score in late life, the risk of dementia decreased 8% (hazard ratio = 0.92, 95% CI = 0.87–0.97), when demographic and cardiometabolic risk factors and diseases were controlled for. The association remained significant when individuals who developed dementia during the first four years of follow-up were excluded from the analyses. Conclusions: Our results indicate there might be a time-varying effect of weight status on dementia. Preclinical dementia might blur the association between weight status and dementia in late life. This needs to be further analysed in studies following the same sample over the life course.
