    • Nomenclatural type definitions are one of the most important concepts in biological nomenclature. Being physical objects that can be re-studied by other researchers, types permanently link taxonomy (an artificial agreement to classify biological diversity) with nomenclature (an artificial agreement to name biological diversity). Two proposals to amend the International Code of Nomenclature for algae, fungi, and plants (ICN), allowing DNA sequences alone (of any region and extent) to serve as types of taxon names for voucherless fungi (mainly putative taxa from environmental DNA sequences), have been submitted to be voted on at the 11th International Mycological Congress (Puerto Rico, July 2018). We consider various genetic processes affecting the distribution of alleles among taxa and find that alleles may not consistently and uniquely represent the species within which they are contained. Should the proposals be accepted, the meaning of nomenclatural types would change in a fundamental way from physical objects as sources of data to the data themselves. Such changes are conducive to irreproducible science, the potential typification on artefactual data, and massive creation of names with low information content, ultimately causing nomenclatural instability and unnecessary work for future researchers that would stall future explorations of fungal diversity. We conclude that the acceptance of DNA sequences alone as types of names of taxa, under the terms used in the current proposals, is unnecessary and would not solve the problem of naming putative taxa known only from DNA sequences in a scientifically defensible way. As an alternative, we highlight the use of formulas for naming putative taxa (candidate taxa) that do not require any modification of the ICN.

  • Molecular oxygen in the rho Ophiuchi cloud

    • Context: Molecular oxygen, O2, has been expected historically to be an abundant component of the chemical species in molecular clouds and, as such, an important coolant of the dense interstellar medium. However, a number of attempts from both ground and from space have failed to detect O2 emission.Aims: The work described here uses heterodyne spectroscopy from space to search for molecular oxygen in the interstellar medium. Methods: The Odin satellite carries a 1.1 m sub-millimeter dish and a dedicated 119 GHz receiver for the ground state line of O2. Starting in 2002, the star forming molecular cloud core ρ Oph A was observed with Odin for 34 days during several observing runs.Results: We detect a spectral line at v_LSR =+3.5 km s-1 with Δ v_FWHM=1.5 km s-1, parameters which are also common to other species associated with ρ Oph A. This feature is identified as the O2 (NJ = 11 - 1_0) transition at 118 750.343 MHz.Conclusions: The abundance of molecular oxygen, relative to H{2} , is 5 × 10-8 averaged over the Odin beam. This abundance is consistently lower than previously reported upper limits.Based on observations with Odin, a Swedish-led satellite project funded jointly by the Swedish National Space Board (SNSB), the Canadian Space Agency (CSA), the National Technology Agency of Finland (Tekes) and Centre National d'Étude Spatiale (CNES).   • Novel Benchmark Values for Redo Liver Transplantation Does the Outcome Justify the Effort?

    • Objective: To define benchmark cutoffs for redo liver transplantation (redo-LT). Background: In the era of organ shortage, redo-LT is frequently discussed in terms of expected poor outcome and wasteful resources. However, there is a lack of benchmark data to reliably evaluate outcomes after redo-LT. Methods: We collected data on redo-LT between January 2010 and December 2018 from 22 high-volume transplant centers. Benchmark cases were defined as recipients with model of end stage liver disease (MELD) score <= 25, absence of portal vein thrombosis, no mechanical ventilation at the time of surgery, receiving a graft from a donor after brain death. Also, high-urgent priority and early redo-LT including those for primary nonfunction (PNF) or hepatic artery thrombosis were excluded. Benchmark cutoffs were derived from the 75th percentile of the medians of all benchmark centers. Results: Of 1110 redo-LT, 373 (34%) cases qualified as benchmark cases. Among these cases, the rate of postoperative complications until discharge was 76%, and increased up to 87% at 1-year, respectively. One-year overall survival rate was excellent with 90%. Benchmark cutoffs included Comprehensive Complication Index CCI (R) at 1-year of <= 72, and in-hospital and 1-year mortality rates of <= 13% and <= 15%, respectively. In contrast, patients who received a redo-LT for PNF showed worse outcomes with some values dramatically outside the redoLT benchmarks. Conclusion: This study shows that redo-LT achieves good outcome when looking at benchmark scenarios. However, this figure changes in high-risk redo-LT, as for example in PNF. This analysis objectifies for the first-time results and efforts for redo-LT and can serve as a basis for discussion about the use of scarce resources.

    • In the wake of deinstitutionalization, people with psychiatric disabilities are, to a great extent, living in the community. In this thesis everyday life of people with psychiatric disabilities, living in independent housing with community-based social support to manage their daily life, is investigated. Special attention is paid to the characteristics and meaning of community-based social support. Through participating observational studies, interviews and time-geographic diaries, data have been gathered about everyday life of seventeen men and women. It is the interpersonal social conditions that have been the main target of the study, addressed as social networks and social support. Everyday life conditions are dependent on social as well as physical and material circumstances. Although the social conditions are emphasized in the study they are not looked upon as isolated dimensions but as part of everyday life circumstances. The everyday life perspective offers an opportunity to reveal the structures within which everyday life takes place. The time-geographic concepts “community-organized projects” and “individual-organized projects” were used to investigate the structures of everyday life, resulting in four substructures. Four types of everyday life and four patterns of networks connected to the different substructures were identified, showing great variations. The characteristics of community-based social support can be summarized as “the doing”, “the talking” and “the being with” in a reciprocal type of relationship. The meaning of support is shortly described as solution of and relieving problems, social companionship, security, protection and control. Community-based social support show similar importance regardless of the type of everyday life when it comes to solution of problems and different importance when it comes to aspects of support such as social companionship.

  • CONTRIBUTION TO QUANTITATIVE MICROWAVE IMAGING TECHNIQUES FOR BIOMEDICAL APPLICATIONS

    • This dissertation presents a contribution to quantitative microwave imaging for breast tumor detection. The study made in the frame of a joint supervision Ph.D. thesis between University Paris-SUD 11 (France) and Mälardalen University (Sweden), has been conducted through two experimental microwave imaging setups, the existing 2.45 GHz planar camera (France) and the multi-frequency flexible robotic system, (Sweden), under development. In this context a 2D scalar flexible numerical tool based on a Newton-Kantorovich (NK) scheme, has been developed. Quantitative microwave imaging is a three dimensional vectorial nonlinear inverse scattering problem, where the complex permittivity of an object is reconstructed from the measured scattered field, produced by the object. The NK scheme is used in order to deal with the nonlinearity and the ill-posed nature of this problem. A TM polarization and a two dimensional medium configuration have been considered in order to avoid its vectorial aspect. The solution is found iteratively by minimizing the square norm of the error with respect to the scattered field data. Consequently, the convergence of such iterative process requires, at least two conditions. First, an efficient calibration of the experimental system has to be associated to the minimization of model errors. Second, the mean square difference of the scattered field introduced by the presence of the tumor has to be large enough, according to the sensitivity of the imaging system. The existing planar camera associated to a flexible 2D scalar NK code, are considered as an experimental platform for quantitative breast imaging. A preliminary numerical study shows that the multi-view planar system is quite efficient for realistic breast tumor phantoms, according to its characteristics (frequency, planar geometry and water as a coupling medium), as long as realistic noisy data are considered. Furthermore, a multi-incidence planar system, more appropriate in term of antenna-array arrangement, is proposed and its concept is numerically validated. On the other hand, an experimental work which includes a new fluid-mixture for the realization of a narrow band cylindrical breast phantom, a deep investigation in the calibration process and model error minimization, is presented. This conducts to the first quantitative reconstruction of a realistic breast phantom by using multi-view data from the planar camera. Next, both the qualitative and quantitative reconstruction of 3D inclusions into the cylindrical breast phantom, by using data from all the retina, are shown and discussed. Finally, the extended work towards the flexible robotic system is presented.

    • Breast cancer is a global health problem, needing cheep and effective alternative diagnosis methods in order to minimize the mortality. This experimental study is performed in the context of an ongoing collaborative project towards a future planar three-dimensional microwave breast mammography system. Herein the first quantitative image of an inhomogeneous breast tumor phantom, composed by different Triton X-100/water/salt mixtures, is obtained by using the existing planar 2.45 GHz microwave camera.

  • Effectiveness and efficacy of nutritional therapy : A cochrane systematic review

    • BACKGROUND & AIMS: Disease-related malnutrition has deleterious consequences on patients' outcome and healthcare costs. The demonstration of improved outcome by appropriate nutritional management is on occasion difficult. The European Society of Clinical Nutrition and Metabolism (ESPEN) appointed the Nutrition Education Study Group (ESPEN-NESG) to increase recognition of nutritional knowledge and support in health services.METHODS: To obtain the best available evidence on the potential effects of malnutrition on morbidity, mortality and hospital stay; cost of malnutrition; effect of nutritional treatment on outcome parameters and pharmaco-economics of nutritional therapy, a Cochrane systematic review of the literature was performed by the Croatian Cochrane Branch to answer the following key questions: Q1) Is malnutrition an independent predictive factor for readmission within 30 days from hospital discharge? Q2) Does nutritional therapy reduce the risk of readmission within 30 days from hospital discharge? Q3) Is nutritional therapy cost-effective/does it reduce costs in hospitalized patients? and Q4) Is nutritional therapy cost effective/does it reduce costs in outpatients?RESULTS: For Q1 six of 15 identified observational studies indicated that malnutrition was predictive of re-admissions, whereas the remainder did not. For Q2 nine randomized controlled trials and two meta-analyses gave non-conclusive results whether re-admissions could be reduced by nutritional therapy. Economic benefit and cost-effectiveness of nutritional therapy was consistently reported in 16 identified studies for hospitalized patients (Q3), whereas the heterogeneous and limited corresponding data on out-patients (Q4) indicated cost-benefits in some selected sub-groups.CONCLUSIONS: This result of this review supports the use of nutritional therapy to reduce healthcare costs, most evident from large, homogeneous studies. In general, reports are too heterogeneous and overall of limited quality for conclusions on impact of malnutrition and its treatment on readmissions.

    • Adverse early-life conditions have lasting effects on old-age health and mortality (Almond, 2006, Barker, 1992, Barker, 1994, Mazumder et al., 2010 and Roseboom et al., 2006). Some have even considered reductions in early life disease exposure to be a primary driver of historical mortality declines (Finch and Crimmins, 2004). Consequently, understanding the mechanisms through which early and later life are linked is important for the development of strategies for population health and successful ageing. Prior research shows that exposure to disease (Bengtsson and Lindström, 2000, Bengtsson and Lindström, 2003 and Crimmins and Finch, 2006), nutritional deprivation (Barker, 1994), or adverse socioeconomic conditions (Hayward and Gorman, 2004 and Smith, 1997) early in life, possibly as early as in the fetal stage but also in early childhood, are linked to worsened adult health. Evidence from natural or quasi-natural experiments suggests that the association between these and other adverse early-life conditions and later health may be causal. Although numerous pathways have been postulated, including those relating to fetal under-nutrition, dysregulation of the immune function, and compromised socioeconomic attainment (Barker, 1994, Costa, 2000, Gluckman et al., 2008 and Schulz, 2010), the mechanisms remain largely unclear. The aim of this Special Issue is to analyse the pathways through which early-life experiences are related to later-life health, and whether the associations are mediated, moderated, or buffered by intermediate characteristics such as own socioeconomic attainment at young adult ages. We have included 13 papers that cover key early-life exposures and take advantage of two complementary conceptual frameworks for understanding the mechanisms.
