Researching the meaning of support : an empirical study on the meaning of support for family carers who care for a senior person at home

- Nursing care of the elderly is believed to be one of the major future European healthcare challenges. Family carers are and will continue to be providing a considerable and increasing proportion of the care for elderly persons (i.e.>65 years) living at home. Support is a term not seldom seen in documents or literature relating to family carers. Support, it seems, is something that should be provided for family carers for alleviating their burden, stress and worry. However the meaning of Support for family carers is perhaps not as clear as one may be led to believe. The aim of this research project was to illuminate the meaning of support for family carers who care for an elderly person at home. Narrative interviews with 20 family carers and 20 registered nurses in palliative home care were conducted and analysed using a phenomenological hermeneutics inspired by the philosophy of Paul Ricoeur. The results indicate that the meaning of support for family carers was a sense of entrustment, an unwinding in the reassurance of shared responsibility in decisions. It is the respectfulness of others, being seen as a carer with attentiveness and genuine interest. It is sensing at togetherness in caring. For the registered nurses the meaning of being supportive was sensing, to bear witness giving time, daring to stay and listen, to see, hear and recognize. It is a pacing and paving the way for the family. It is to facilitate in uncovering possibilities and being an assurance that carries the family.

- Family carers are a major source of help and assistance to the persons they provide care for. They are also major contributors to the welfare system, balancing the national healthcare expenditure. Increasing attention, in research as well as government policy, is being paid to their role as informal caregivers. Support to family carers seems to be a new “buzz-word” in Swedish government policy. However, supporting family carers may prove to be a more complex endeavour than one initially might be led to believe. Support could here be understood as any services, assistances, education, information, attitudes, lay or professional persons provision for the benefit of the family carer. The aim of this systematic review was to identify modes of, and scientific evidence on, support for family carers of cohabiting elderly persons. The method followed a seven-step model: a focused research question was formulated creating a base for deriving search words and inclusion and exclusion criteria for studies. Systematic database searches identified several studies some of which were retrieved, critically appraised and classified by two independent reviewers. A total of 26 articles were finally included, revealing that family carers fear social isolation and wish to network in groups with peers, either for social or for learning needs purposes. Family carers also desire respite care. However, it is unclear whether they actually benefit from any of the above or how service provision should be attempted.

Data consisted of narrative interviews, which were analyzed in three recurring phases: naive reading, structural analysis and interpreted whole. Twenty nurses from the hospitals in Sweden participated in the study. Two core themes were found: forming a relationship and keeping caregiving at home.

The total of 20 nurses who participated in the study were affiliated with three different hospitals in southern Sweden. Several subthemes and themes were formed from the rich material into two core themes: ‘Forming a relationship’ and ‘Keeping caregiving at home’. In the interpreted whole, dimensions to the meaning of being supportive as illuminated by the nurses narratives are discussed in relation to literature that has potential to open up for a widened and deepened understanding of the phenomenon.

- The aim of this study was to illuminate the meaning of support as narrated by family carers who care for a senior relative at home. This study originates from a context of advanced in-home palliative care in Sweden. Twenty (n=20) family carers, who cared for a senior relative at home, participated. Narrative interviews resulted in a text which was analysed in three recurring phases: naive understanding, structural analyses and comprehensive understanding using a phenomenological hermeneutical method, inspired by the philosophy of Ricoeur. Two themes were formulated from the analysis: “Experiencing a togetherness with others in caring” and “Sensing togetherness with oneself in caring”. The meaning of support was here understood as “Sensing togetherness”. In the comprehensive understanding this meaning was further illuminated by literature on the concepts of friendship, inner strength and manageability through the sense of coherence theory. Suggestions of relevance to practice and further research are given.

Twenty individuals who cared for a senior relative at home participated. Narrative interviews resulted in a text, which the authors analyzed in three recurring phases, naive understanding, structural analysis, and comprehensive understanding, using the phenomenological hermeneutical method. They formulated two themes from the analysis: Experiencing a togetherness with others in caring and Sensingtogetherness with oneself in caring. The meaning of support was understood here as Sensing togetherness. In the comprehensive understanding, the authors further illuminate thismeaning using literature on the concepts of frienship, inner strengt, and manageability through the sense of coherence theory. They provide suggestions for the relevance to practice and further research.
