  • Six-week follow-up after HIV-1 exposure: a position statement from the Public Health Agency of Sweden and the Swedish Reference Group for Antiviral Therapy

    • In 2014 the Public Health Agency of Sweden and the Swedish Reference Group for Antiviral Therapy (RAV) conducted a review and analysis of the state of knowledge on the duration of follow-up after exposure to human immunodeficiency virus (HIV). Up until then a follow-up of 12 weeks after exposure had been recommended, but improved tests and new information on early diagnosis motivated a re-evaluation of the national recommendations by experts representing infectious diseases and microbiology, county medical officers, the RAV, the Public Health Agency, and other national authorities. Based on the current state of knowledge the Public Health Agency of Sweden and the RAV recommend, starting in April 2015, a follow-up period of 6 weeks after possible HIV-1 exposure, if HIV testing is performed using laboratory-based combination tests detecting both HIV antibody and antigen. If point-of-care rapid HIV tests are used, a follow-up period of 8 weeks is recommended, because currently available rapid tests have insufficient sensitivity for detection of HIV-1 antigen. A follow-up period of 12 weeks is recommended after a possible exposure for HIV-2, since presently used assays do not include HIV-2 antigens and only limited information is available on the development of HIV antibodies during early HIV-2 infection. If pre- or post-exposure prophylaxis is administered, the follow-up period is recommended to begin after completion of prophylaxis. Even if infection cannot be reliably excluded before the end of the recommended follow-up period, HIV testing should be performed at first contact for persons who seek such testing.

  • Suspicion and treatment of severe sepsis : An overview of the prehospital chain of care

    • BackgroundSepsis is a life-threatening condition where the risk of death has been reported to be even higher than that associated with the major complications of atherosclerosis, i.e. myocardial infarction and stroke. In all three conditions, early treatment could limit organ dysfunction and thereby improve the prognosis.AimTo describe what has been published in the literature a/ with regard to the association between delay until start of treatment and outcome in sepsis with the emphasis on the pre-hospital phase and b/ to present published data and the opportunity to improve various links in the pre-hospital chain of care in sepsis.MethodsA literature search was performed on the PubMed, Embase (Ovid SP) and Cochrane Library databases.ResultsIn overall terms, we found a small number of articles (n=12 of 1,162 unique hits) which addressed the prehospital phase. For each hour of delay until the start of antibiotics, the prognosis appeared to become worse. However, there was no evidence that prehospital treatment improved the prognosis.Studies indicated that about half of the patients with severe sepsis used the emergency medical service (EMS) for transport to hospital. Patients who used the EMS experienced a shorter delay to treatment with antibiotics and the start of early goal-directed therapy (EGDT). Among EMS-transported patients, those in whom the EMS staff already suspected sepsis at the scene had a shorter delay to treatment with antibiotics and the start of EGDT.There are insufficient data on other links in the prehospital chain of care, i.e. patients, bystanders and dispatchers.ConclusionSevere sepsis is a life-threatening condition. Previous studies suggest that, with every hour of delay until the start of antibiotics, the prognosis deteriorates. About half of the patients use the EMS. We need to know more about the present situation with regard to the different links in the prehospital chain of care in sepsis.

    • Inom projektet Biokombi Rya har ett flertal olika forskargrupper samarbetat för att studera system¬effekterna av förgasning av biobränsle ur olika aspekter. Syftet med projektet är att öka kunskapen om biobränsleförgasning i Sverige samt att utreda förutsättningar för att sådana anläggningar ska vara ekonomiskt och miljömässigt intressanta. En referensgrupp har varit kopplad till projektet där förutsättningar, resultat och slutsatser har behandlats.I denna underlagsrapport har slutrapporterna från projektets delprojekt samlats. De beskriver förutsättningar, metodansatser, använda data och resultat utförligt och utgör på så sätt ett viktigt komplement till den mer övergripande beskrivningen i projektets syntesrapport. De delrapporter som ingår har valts för att täcka in samtliga delar av projektet som är av allmänt intresse. Projektresultat som publicerats på annat sätt berörs dock mer kortfattat.Projektet Biokombi Rya har pågått under två år (2005-2006) och drivits av Chalmers EnergiCentrum. Förutom de omfattande analysinsatser som författarna till denna rapport står för, har Avdelningen för kemisk teknologi vid KTH, Siemens Industrial Turbines AB och Göteborg Energi AB bidragit med expertstöd. CIT Industriell Energianalys, med undertecknad som projektledare, har stått för projektledning och koordination.Projektet har finansierats av Energimyndigheten, Göteborg Energis forsknings¬stiftelse samt Göteborg Energi AB.

    • A new language, Feldspar, is presented, enabling high-level and platform-independent description of digital signal processing (DSP) algorithms. Feldspar is a pure functional language embedded in Haskell. It offers a high-level dataflow style of programming, as well as a more mathematical style based on vector indices. The key to generating efficient code from such descriptions is a high-level optimization technique called vector fusion. Feldspar is based on a low-level, functional core language which has a relatively small semantic gap to machine-oriented languages like C. The core language serves as the interface to the back-end code generator, which produces C. For very small examples, the generated code performs comparably to hand-written C code when run on a DSP target. While initial results are promising, to achieve good performance on larger examples, issues related to memory access patterns and array copying will have to be addressed.

    • The potential for greenhouse gas (GHG) reduction in industry through process integration measures depends to a great extent on prevailing technical and economic conditions. A step-wise methodology developed at the author's department based on pinch technology was used to analyse how various parameters influence the cost-optimal configuration for the plant's energy system, and the opportunities for costeffective GHG emissions reduction compared to this solution. The potential for reduction of GHG emissions from a given plant depends primarily on the design of the industrial process and its energy system (internal factors) and on the electricity-to-fuel price ratio and the specific GHG emissions from the national power generation system (external factors).

    • A model for simulating the drug release from a swelling and dissolving polymer tablet is presented and verified to data. The model is based on a mechanistic approach, and it can therefore be employed to study the sensitivity of true physical constants, for instance the drug diffusion coefficient or the drug solubility. The model generates the drug and polymer release profiles and the front positions of the total tablet, the solid core, and of the solid-drug-solubilized-drug interface. The convective contribution to mass transfer is shown to be of great importance. This is most markedly noticed for slowly diffusing drugs. In a simulation with a low value of the drug diffusion coefficient, it is shown that the initial drug release rate is faster than the polymer dissolution rate, followed by a second stage with a slower drug release rate. Furthermore, it is shown that polymer dissolution influences the drug release profile significantly, but not the front position of saturated drug in the gel layer. The model is verified against drug release and polymer dissolution data for the slightly soluble drug Methyl paraben and the soluble drug Saligenin in a poly (ethylene oxide) tablet, resulting in good agreement between model and experiments.     • Software for digital signal processors (DSPs) is traditionally highly hardware-dependent and hence porting it to new processors usually requires significant design effort. In this paper we present Feldspar (Functional Embedded Language for DSP and Parallelism), an embedded, high-level, domain-specific language for DSP algorithm design and the compilation techniques we developed for generating C code from specifications written in Feldspar. While Feldspar allows description of algorithms on specification level, we show that with the right set of abstractions and transformations this high level, functional specification can be transformed into C code that is comparable or better than reference, hand-crafted C language implementations. The Feldspar compiler is highly modular and plugin-based, hence future hardware-specific plugins will enable automatic generation of efficient, hardware-specific code. This approach enables the encapsulation of knowledge of hardware completely in the compiler and thus allows description of algorithms in completely hardware-independent, portable manner.
