    • In general, measles virus (MV) immunoglobulin G (IgG) antibody titres decline over time. However, we found that serum and CSF MV antibody titres increased with age (slope=0.038, p<0.001 and slope=0.022, p=0.008), respectively, and disease duration (slope=0.031, p=0.002 and slope=0.032, p=0.005), respectively, in patients with multiple sclerosis (MS) or clinically isolated syndrome (CIS). The age dependency of serum antibody levels differed between patients and controls (slope=0.038 versus -0.004, p<0.001). The increasing MV antibody litres over time in MS/CIS patients support a general nonspecific stimulation of B cells and plasma cells that is not confined only to the CNS/CSF compartment.

    • High occurrence of measles, rubella and varicella zoster antibodies has been used as a biomarker for MS (the MRZ test). We analyzed measles antibody titres with respect to measles infection/measles vaccination status in 166 patients with MS or clinically isolated syndrome. Fifty blood donors served as controls. Measles vaccination yielded CSF measles antibodies in fewer patients (62%) than measles infection did (87%, p=0.001) and yielded lower measles titres in both serum and CSF (p<0.001). Controls had lower CSF measles titres than patients with measles vaccination alone (p<0.001). Childhood vaccinations probably reduce the sensitivity of the MRZ diagnostic test for MS.

    • Tumour hypoxia is associated with increased metastatic potential and resistance to radiotherapy and chemotherapy. Ileal carcinoids are usually metastatic at the time of diagnosis and respond poorly to chemotherapy. The aim of this study was to investigate the extent of hypoxia in ileal carcinoids and the response of tumour cells to induced hypoxia. VEGF, CA-IX, HIF-1alpha and HIF-2alpha were studied by immunohistochemistry in biopsies from 24 patients with ileal carcinoids. All hypoxic markers were shown to be highly expressed in localized areas of the tumours irrespective of tumour location or stage. However, HIF-2alpha expression was significantly higher in distant metastases compared to primary tumours in the same patient. Global gene expression profiling of GOT1 carcinoid cells revealed a marked response to hypoxia. Expression of genes related to epithelial-to-mesenchymal transition (EMT) and development was altered including increased expression of the chemokine receptor CXCR4, an important regulator of invasive growth and metastasis formation. High expression of CXCR4 was confirmed by immunohistochemistry in tumour biopsies. Stimulation of GOT1 cells by the CXCR4 ligand CXCL12 (SDF-1) activated the MAPK p42/44 signalling pathway and increased tumour cell migration. We conclude that ileal carcinoids contain hypoxic areas expressing HIF-1alpha, and HIF-2alpha and CXCR4. Signalling through the CXCL12-CXCR4 axis may contribute to the metastatic potential of ileal carcinoids. Targeting of HIFs and/or the CXCR4 signalling pathway may offer new therapeutic strategies for this carcinoid tumour disease.

  • miRNA profiling of small intestinal neuroendocrine tumors defines novel molecular subtypes and identifies miR-375 as a biomarker of patient survival

The aim of this study was to define the miRNA profile of small intestinal neuroendocrine tumors and to search for novel molecular subgroups and prognostic biomarkers. miRNA profiling was conducted on 42 tumors from 37 patients who underwent surgery for small intestinal neuroendocrine tumors. Unsupervised hierarchical clustering analysis of miRNA profiles identified two groups of tumor metastases, denoted cluster M1 and M2. The smaller cluster M1 was associated with shorter overall survival and contained tumors with higher grade (WHO grade G2/3) and multiple chromosomal gains including gain of chromosome 14. Tumors of cluster M1 had elevated expression of miR-1246 and miR-663a, and reduced levels of miR-488-3p. Pathway analysis predicted Wnt signaling to be the most significantly altered signaling pathway between clusters M1 and M2. Analysis of miRNA expression in relation to tumor proliferation rate showed significant alterations including downregulation of miR-137 and miR-204-5p in tumors with Ki67 index above 3%. Similarly, tumor progression was associated with significant alterations in miRNA expression, e.g. higher expression of miR-95 and miR-210, and lower expression of miR-378a-3p in metastases. Pathway analysis predicted Wnt signaling to be altered during tumor progression, which was supported by decreased nuclear translocation of β-catenin in metastases. Survival analysis revealed that downregulation of miR-375 was associated with shorter overall survival. We performed in situ hybridization on biopsies from an independent cohort of small intestinal neuroendocrine tumors using tissue microarrays. Expression of miR-375 was found in 578/635 (91%) biopsies and survival analysis confirmed that there was a correlation between downregulation of miR-375 in tumor metastases and shorter patient survival. We conclude that miRNA profiling defines novel molecular subgroups of metastatic small intestinal neuroendocrine tumors and identifies miRNAs associated with tumor proliferation rate and progression. miR-375 is highly expressed in small intestinal neuroendocrine tumors and may be used as a prognostic biomarker.

  • Field test of a floating thermal pile in sensitive clay

    • The response of floating pile foundations in deep deposits of soft clay is governed by the settlements within the clay deposit surrounding the piles. A long-duration thermal response test (TRT) has been performed to assess the impact of heating and cooling on the geotechnical performance of a vertically loaded slender tubular steel pile in a deep deposit of sensitive clay. The results from the instrumented test site indicate that negligible excess pore water pressures, up to 3 kPa during heating and down to -1·1 kPa during cooling, were generated in the soil adjacent to the pile for a 50 W/m heat flux. The heating rate was sufficiently low to facilitate the drainage of pore water. The influence radius for the excess pore water pressures, however, extended beyond the soil volume affected by temperature change. The absence of thermally triggered creep settlements could be explained by the overlap between the remoulded zone resulting from pile installation, the extent of the heating, and the low creep susceptibility of remoulded sensitive clay for moderate temperature changes. Furthermore, no difference in the ultimate bearing capacity was found between the thermal pile and the reference pile, which was only loaded with a serviceability load during the test series. The current test data, therefore, indicate that driven floating piles in a deep deposit of soft clay also function as heat exchanger piles with minimal detrimental effects, for similar operational conditions as used in the TRT. It should be noted, however, that other pile types that do not extensively remould the clay adjacent to the pile shaft may show more significant influence of the thermal changes.

  • Floating Thermal Piles in Soft Sensitive Clays

    • Thermal piles are building foundation piles which, in addition to transferring the load of the building to the ground, are also used as heat exchangers to provide thermal exchange with the ground. This article shortly presents field testing of thermal piles installed in soft sensitive clay in Gothenburg. The aim of the field tests is to study the thermo-mechanical response of the heating and cooling cycles on the pile/clay system, and to gather representative data for further analysis. The soil properties have been determined from traditional sampling, complemented with a new block sampling method. Instrumentation for measuring pore pressures, temperaturesand settlements has been installed at different levels in the soil and at different distances from the test piles. A state-of-the-art thermal response test rig has been developed to perform response testing in both heating and cooling modes. The first heating cycle started in December 2016.
