  • Technological and informational frames : explaining age-related variation in the use of patient accessible electronic health records as technology and information

    • Purpose Data from a national patient survey (N = 1,155) of the Swedish PAEHR "Journalen" users were analysed, and an extended version of the theory of technological frames was developed to explain the variation in the technological and informational framing of information technologies found in the data.Design/methodology/approach Patient Accessible Electronic Health Records (PAEHRs) are implemented globally to address challenges with an ageing population. However, firstly, little is known about age-related variation in PAEHR use, and secondly, user perceptions of the PAEHR technology and the health record information and how the technology and information-related perceptions are linked to each other. The purpose of this study is to investigate these two under-studied aspects of PAEHRs and propose a framework based on the theory of technological frames to support studying the second aspect, i.e. the interplay of information and technology-related perceptions.Findings The results suggest that younger respondents were more likely to be interested in PAEHR contents for general interest. However, they did not value online access to the information as high as older ones. Older respondents were instead inclined to use medical records information to understand their health condition, prepare for visits, become involved in their own healthcare and think that technology has a much potential. Moreover, the oldest respondents were more likely to consider the information in PAEHRs useful and aimed for them but to experience the technology as inherently difficult to use.Research limitations/implications The sample excludes non-users and is not a representative sample of the population of Sweden. However, although the data contain an unknown bias, there are no specific reasons to believe that it would differently affect the survey’s age groups.Practical implications Age should be taken into account as a key factor that influences perceptions of the usefulness of PAEHRs. It is also crucial to consider separately patients’ views of PAEHRs as a technology and of the information contained in the EHR when developing and evaluating existing and future systems and information provision for patients.Social implications This study contributes to bridging the gap between information behaviour and systems design research by showing how the theory of technological frames complemented with parallel informational frames to provide a potentially powerful framework for elucidating distinct conceptualisations of (information) technologies and the information they mediate. The empirical findings show how information and information technology needs relating to PAEHRs vary according to age. In contrast to the assumptions in much of the earlier work, they need to be addressed separately.Originality/value Few earlier studies focus on (1) age-related variation in PAEHR use and (2) user perceptions of the PAEHR technology and the health record information and how the technology and information-related perceptions are linked to each other.

  • Användbara IT-stöd : En utvärdering av ett forskningsprojekt vid CSN, Centrala studiestödsnämnden

    • Utvecklingsrådet för den statliga sektorn har tagit ett initiativ för att stödja myndigheternas arbete med att förbättra arbetsmiljön och sänka sjukfrånvaron. Därför startades programmet Satsa friskt. Programmet ger stöd i form av ekonomiskt bidrag och experthjälp till olika projekt inom statliga myndigheter. Ett av Satsa Friskts insatsområden är Människa IT . Inom detta område har det sedan 2004 bedrivits ett antal olika projekt vid flera statliga verk. Vi har från MDI (avdelningen för människa-datorinteraktion, institutionen för informationsteknologi) vid Uppsala universitet varit mer omfattande inblandad i tre olika sådana projekt: vid CSN, Migrationsverket och SMHI. Syftet och innehållet har varierat en del mellan de olika projekten, men ett huvudsakligt fokus har varit hur man kan se till att de framtida IT-stödda arbetena inom myndigheterna blir effektivare och arbetsmiljön bättre. Genom att bättre beakta användbarhets- och arbetsmiljöaspekter vid kravställande, utveckling och införande av IT-stöd kan man stödja en positiv utveckling av verksamheten som sådan liksom av arbetsinnehåll och arbetsmiljö för den enskilde individen. Resultatet kan då bli effektivare verksamhet, bättre service till kunder och ett hälsosammare och hållbarare arbete för de anställda. Denna rapport beskriver det arbete som under åren 2005-2007 genomförts i samverkan mellan CSN, Centrala studiestödsnämnden, och MDI, Uppsala universitet. Förutom en kort beskrivning av innehållet i det arbete som gjorts inom projektet redovisar denna rapport en utvärdering av resultatet, de lärdomar som vi gjort från projektet, de råd vi kan ge till andra som vill ta del av dessa lärdomar samt en förteckning av de rapporter av olika slag som producerats.

  • Making Visible and Modeling the Underrepresented : Teachers' Reflections on Their Role Modeling in Higher Education

    • This work contributes to a better understanding of computing teachers' perceptions of themselves as role models. Role models are described as important to address under-representation, yet there is little in-depth research on how role modeling works and what university teachers in computing can model to broaden participation in the discipline. We will analyze teachers' reflections on how they may, or want to, be perceived by their students, particularly in terms of professional competencies, emotions and attitudes towards well-being. We will use and further develop an already existing framework on role modeling in computing, and we will relate our findings to existing research on computing and science identities. Modeling aspects outside the computing norm can help provide students with a wider notion of what it means to be a computer scientist. Besides developing the theoretical understanding of computing teachers as role models , our work can support various ways of developing computing teachers' competences and departments' teaching culture. The results are one way to contribute to student diversity and equitable access, and more broadly increase the relevance of computing education for sustainability.

    • A key component of the tertiary education system is the negotiation of common expectations in terms of pedagogy and the manner in which learning is scaffolded in the learning context. This paper addresses this interplay of perspectives by drawing on two elements of our previous work, a longitudinal study of student identity development [1] and a study contrasting project course students' experiences with teachers' expectations [2]. The paper develops a model of student interaction with teachers and the higher education system, which contributes to a better understanding of the consequences of recent changes and trends in higher education, e.g. demands for activating students, increased level of detail in course specifications, and examination of "non-core subject content". This is an immensely complex area and we approach this challenge with a focus on the issue of students rejecting learning opportunities. Through this lens we will identify and illustrate some essential aspects of how to adapt educational settings to better accommodate how students behave and view the goal of their education.

    • Globalisation of both the workplace and higher education is a fact for many Universities and Industries. For the universities the challenge lies both in identifying the needs and developing pedagogies suitable for preparing their students to become a workforce that can contribute in a global economy. Thus, a central issue is how to provide an appropriate level of background knowledge and skills which contribute to the success of the individual in the workplace, and to the success of their employers. Helping students develop the skills required for success in global software development teams provides a number of unique challenges and opportunities for the designers of engineering degree programmes. A central issue is that students bring with them local cultural norms, different native languages, and rhetorical traditions, which place an additional burden to the already difficult task of working effectively in a student engineering team. The contribution of this paper is the presentation of two learning activities; 1) structured role-play and 2) hypothetical archetypes, used within a method we call the "Archetype Learning Method" (ALM). The activities are geared towards enhancing the students' awareness of the underlying complexities inherent in participating in global software engineering and through that provide scaffolding to the development of teamwork skills. Our method provides students with strategies for engaging more productively in teamwork in a global context, hence contributing to the systematic development of one of the critical professional skills identified in the CHAOS report.

    • Work environment and occupational health problems of different nature are constantly increasing in computer supported work. Most efforts to improve the work environment are focused primarily on physical aspects, and to some extent on psychosocial aspects. Mental workload and cognitive problems are of a more complex nature, more difficult to measure and provide efficient solutions to, and are more seldom studied or solved. Solutions to work environment problems are usually applied to already existing work situations through improved equipment and work place design, health programmes, education, reorganizations, etc. The problems are seldom prevented by means of applying relevant methods early in the systems development process, before the artefacts have been designed and implemented. This paper, and the following papers of this special issue, will focus on the need to integrate different interdisciplinary methods at different phases in the development process of computerized support systems, with the ultimate goal to prevent work environment problems and decrease the health risks to the users.

    • In the modern fast changing world no formal education is able to provide learners with a complete set of knowledge, skills and competences that they would need to successfully compete on tomorrow's job market. Therefore, the role of universities is increasingly shifting towards provision of an environment where students have a chance to acquire lifelong learning skills. This paper presents underlying ideas of, and practical experiences with, an innovative pedagogy that addresses the lifelong learning skills acquisition along with additional benefits for science and technology students. The proposed approach, called self-flipped classroom (SFC), is built on a synergy of two pedagogies: learning through making and flipped classroom. To unveil the construct of the SFC, we discuss each of its components individually presenting appropriate theoretical grounding. We also report on our experiences from self-flipped classroom implementations in two countries, UK and Sweden, and in three different educational settings. From our work with the SFC concept we have identified four different roles the students can assume in a SFC scenario: creators, collaborators, communicators, and learners. We present our observations regarding the identified roles that have been found in the studied settings. We also outline some implications for teaching using the SFC concept and future research directions in this space.

    • Background: Roles in the doctor-patient relationship are changing and patient participation in health care is increasingly emphasized. Electronic health (eHealth) services such as patient accessible electronic health records (PAEHRs) have been implemented to support patient participation. Little is known about practical use of PAEHR and its effect on roles of doctors and patients. Objective: This qualitative study aimed to investigate how physicians view the idea of patient participation, in particular in relation to the PAEHR system. Hereby, the paper aims to contribute to a deeper understanding of physicians' constructions of PAEHR, roles in the doctor-patient relationship, and levels and limits of involvement. Methods: A total of 12 semistructured interviews were conducted with physicians in different fields. Interviews were transcribed, translated, and a theoretically informed thematic analysis was performed. Results: Two important aspects were identified that are related to the doctor-patient relationship: roles and involvement. The physicians viewed their role as being the ones to take on the responsibility, determining treatment options, and to be someone who should be trusted. In relation to the patient's role, lack of skills (technical or regarding medical jargon), motives to read, and patients' characteristics were aspects identified in the interviews. Patients were often referred to as static entities disregarding their potential to develop skills and knowledge over time. Involvement captures aspects that support or hinder patients to take an active role in their care. Conclusions: Literature of at least two decades suggests an overall agreement that the paternalistic approach in health care is inappropriate, and a collaborative process with patients should be adopted. Although the physicians in this study stated that they, in principle, were in favor of patient participation, the analysis found little support in their descriptions of their daily practice that participation is actualized. As seen from the results, paternalistic practices are still present, even if professionals might not be aware of this. This can create a conflict between patients who strive to become more informed and their questions being interpreted as signs of critique and mistrust toward the physician. We thus believe that the full potential of PAEHRs is not reached yet and argue that the concept of patient empowerment is problematic as it triggers an interpretation of "power" in health care as a zero-sum, which is not helpful for the maintenance of the relationship between the actors. Patient involvement is often discussed merely in relation to decision making; however, this study emphasizes the need to include also sensemaking and learning activities. This would provide an alternative understanding of patients asking questions, not in terms of "monitoring the doctor" but to make sense of the situation.

  • Age-related differences in seeking clarification to understand medical record information

    • Introduction Patient accessible electronic health records can be used to inform and empower patients. However, their use may require complementary information seeking since they can be difficult to interpret. So far, relatively little is known of the information seeking that takes place in connection to health record use, and especially the way it varies in different age groups. A better understanding of patients' preferences of where and how to find explanatory information provides valuable input for the development of health information provision and counselling services. Method. The analysis is based on the results of a national survey of Swedish individuals (N=1,411) who had used a national patient accessible electronic health record system (Journalen). Analysis. The data were analysed in SPSS 24.0 using Kruskal-Wallis tests for detecting groupwise differences and Jonckheere-Terpstra tests for discovering age-related trends in the data. Findings. Older patients were more likely to use a telephone and younger patients to use socia l contacts to ask for clarification. Generally, older adults born between 1946-1960 appear as passive information seekers. Conclusion. Age gro ups differ in their preferences on how to seek clarification, which underlines the importance of a better understanding of individual differences in delivering not only technically but also intellectually accessible health information. Calling by telephone could be a habit of present older generations whereas, to a degree, searching information online could be a comparable habit of current younger generations.
