    • Accurate and comprehensive healthcare data coupled with modern analytical tools can play a vital role in enabling care providers to make better-informed decisions, leading to effective and cost-efficient care delivery. This paper describes a novel strategic healthcare analysis and research platform that encapsulates 360-degree pseudo-anonymized data covering clinical, operational capacity and financial data on over 500,000 patients treated since 2009 across all care delivery units in the county of Halland, Sweden. The over-arching goal is to develop a comprehensive healthcare data infrastructure that captures complete care processes at individual, organizational and population levels. These longitudinal linked healthcare data are a valuable tool for research in a broad range of areas including health economy and process development using real world evidence.Key messagesStructured and standardized variables have been linked from different regional healthcare sources into a research information platform including all healthcare visits in the county of Halland in Sweden, from 2009 to date.Since 2015, the regional information platform integrates a cost component to each healthcare visit: thus being able to quantify patient level value, safety and cost efficiency across the continuum of care. © The Author(s) 2020; all rights reserved.     • We have examined the role of microbial communities on the surface of submersed macrophytes and in the underlying sediment for nitrification and denitrification in light and dark in NH(4)(+)-enriched microcosm systems using isotope pairing and dilution techniques. Potamogeton pectinatus L. and intact sediment cores were collected in a shallow reservoir receiving treated municipal wastewater and containing dense submersed vegetation. Chambers containing P. pectinatus shoots, sediment, or both P. pectinatus shoots and sediment were exposed to 6 h of darkness, 6 h of light, and 6 h of darkness. (14)NH(4)(+) and (15)NO(3)(-) were added at ambient concentrations of 15 and 5 mg N liter(-1), respectively. NH(4)(+) was primarily nitrified in the epiphytic microbial communities, and NO; was denitrified in the underlying sediment. In chambers containing macrophytes, there was a net production of O(2) and NO(3)(-) in light and a net consumption in dark, and nitrification was higher in light than in dark. In chambers with only sediment, there was always a net consumption of NO(3)(-), and nitrification was similar in light and dark. The results show that submersed macrophytes can be important for the N metabolism in NH(4)(+)-rich freshwaters (e.g., wastewater treatment systems) by stimulating nitrification through providing surfaces for attached nitrifying bacteria and possibly also through diurnal changes in the water chemistry.

    • 1. The denitrifying capacity of epiphyton was used to evaluate differences in the function of epiphytic microbial communities on submersed macrophytes in nutrient-rich freshwater ecosystems. The denitrifying capacity of epiphyton on Patamogeton perfoliatus shoots of different age and with different epiphytic abundances from a eutrophic lake was investigated in laboratory microcosms in the Light and dark. Additionally, differences between epiphyton on shoots of Potamogeton pectinatus grown under different in Situ nutrient and hydraulic conditions were investigated by examining their denitrifying capacity. 2. Denitrification was registered in well-developed epiphytic layers on both mature and senescent shoots in the dark, with activities 3- to 10-fold higher in the epiphytic communities of senescent shoots. No activity was detected on young shoots with sparse epiphyton or on shoots from which loosely attached epiphyton had been removed. Denitrification never occurred during illumination. 3. Even though the epiphytic abundance was similar in magnitude, the denitrifying capacity of epiphyton adapted to high nutrient loadings was about a hundred times higher than that of epiphyton adapted to lower nutrient levels. Additionally, epiphytic abundance and denitrifying capacity were higher at sites less exposed to wave turbulence or water currents, than at sites with more water turbulence. 4. The results illustrate how the hydraulic and nutrient conditions of the surrounding water affect both the quantity and function of epiphytic microbial communities in nutrient-rich freshwater ecosystems.

  • Nitrogen removal in a wastewater reservoir : The importance of denitrification by epiphytic biofilms on submersed vegetation

    • The aim of this study was to examine the importance of epiphytic denitrifying bacteria on submersed vegetation in removing N from a shallow nutrient-enriched freshwater ecosystem. The investigation was conducted during the summer of 1994 in a surface now reservoir receiving municipal tertiary-treated wastewater. The submersed vegetation in the reservoir was dominated by Potamogeton pectinatus L. and filamentous green algae (FGA). The N loading was 2300 mg N h(-1) m(-2) and the N removal, calculated as the mean difference between influent and effluent N, was 190 mg N h(-1) m(-2) (8%). The majority of influent N consisted of NH4+, but the main part of the N removal was due to the removal of NO3- whereas no net retention of NH4+ was found. Mean total soluble solids and BOD7 retention was 69 and 38%, respectively, Denitrification measurements were conducted in darkness at in situ temperature in microcosms with P. pectinatus, FGA, or infect sediment cores. Epiphytic denitrification ranged between 0.21 to 7.0 mg N h(-1) m(-2) reservoir surface area depending on the abundance of the submersed vegetation (5-140 g DW m(-2)). Sediment denitrification was 4.7 mg N h(-1) m-L reservoir surface area. The mean assimilative N uptake of the submersed vegetation and epiphyton was 3.4 and 1.6 mg N h(-1) m(-2) reservoir surface area, respectively. Measured N removal rates through plant uptake and denitrification could only account for a minor part of the N removal observed by mass balance. However, microcosm denitrification measurements underestimate actual denitrification. Thus, the major part of the N removal was most likely due to denitrification. In conclusion, this study indicates that denitrification in epiphytic microbial communities on submersed vegetation can be of significant importance for the N removal in nutrient-enriched freshwater ecosystems.

  • Health-promoting factors in higher education for a sustainable working life : protocol for a multicenter longitudinal study

    • BACKGROUND: The World Health Organization has highlighted the importance of health promotion for health service providers in order to ensure sustainable working life for individuals involved in providing health services. Such sustainability begins when students are preparing to manage their own future health and welfare in working life. It has been suggested that universities, employees and trainee health professionals should adopt or follow a salutogenic approach that not only complements the providing of information on known health risks but also favors health promotion strategies. This paper describes the study design and data collection methods in a planned study aiming to explore health-promoting factors for a sustainable working life among students in higher education within healthcare and social work. METHODS: This protocol describes a multicenter longitudinal study involving Swedish students on higher education programs in the healthcare and social work sectors. In 2018, the study invited students on seven education programs at six universities to participate. These programs were for qualification as: biomedical laboratory scientists (n = 121); dental hygienists (n = 87); nurses (n = 1411); occupational therapists (n = 111); physiotherapists (n = 48); radiographers (n = 60); and, social workers (n = 443). In total, 2283 students were invited to participate. Participants completed a baseline, a self-reported questionnaire including six validated instruments measuring health-promoting factors and processes. There are to be five follow-up questionnaires. Three while the students are studying, one a year after graduating, and one three years after graduating. Each questionnaire captures different health-promoting dimensions, namely: health-promoting resources (i.e. sense of coherence); occupational balance; emotional intelligence; health and welfare; social interaction; and work and workplace experiences/perceptions. DISCUSSION: This study focuses on the vastly important aspect of promoting a sustainable working life for healthcare and social work employees. In contrast to previous studies in this area, the present study uses different, validated instruments in health promotion, taking a salutogenic approach. It is hoped that, by stimulating the implementation of new strategies, the study's findings will lead to education programs that prepare students better for a sustainable working life in healthcare and social work.

    • This paper summarizes the state-of-the-art with respect to wetlands for nutrient removal and biodiversity enhancement, as expressed at a research workshop in December 2002. In the end of 2002, 260 and 884 wetlands had been constructed in Sweden with subsidies from LIP and the European Union, respectively. Most frequently, the aims were to remove nutrients from water and enhance biodiversity. The question raised is if we know how to design multifunctional wetlands. Should a wetland be deep or shallow, with or without macrophytes to be an efficient nutrient sink? Diverging opinions are presented, but generally it appears that fairly shallow wetlands at least partly covered by emergent macrophytes are favourable. The importance of extreme high flows and hydraulic short-circuiting is highlighted, and Danish and Norwegian approaches to wetlands construction are presented. Also, there is a risk that nutrient retaining wetlands develop a fairly trivial flora and fauna unless special care is taken. Intentional establishment of desirable and less common species, as well as creation of a variation of depth gradients to favour such species are such measures. Others are allowing for water level variations, as well as vegetation management such as grazing and harvesting.

    • Efficient nitrate removal from wetlands depends on denitrification. Macrophytes support denitrification by supplying organic carbon. Organic carbon available to denitrifying bacteria is released from plant litter and from living macrophytes. Macrophytes offer attachment surfaces for epiphytes, also producing organic matter, and for denitrifying bacteria. Emergent macrophytes are generally more productive than submerged macrophytes, but submerged macrophytes have more epiphytes and offer a larger attachment area in the water column for denitrifying bacteria. Emergent and submerged vegetation differ in their seasonal patterns of release of organic carbon. We conclude that a mixture of emergent and submerged macrophytes may be beneficial for nitrogen removal in wetlands with a surface-flow of nitrate-rich water. The influence of vegetation on wetland hydraulics must also be considered. A wetland design with deeper parts favoring submerged macrophytes alternating, along the water flow, with shallower parts covered by emergent macrophytes, may promote denitrification processes and distribution of water flow.

  • Development of a Self-Help Web-Based Intervention Targeting Young Cancer Patients With Sexual Problems and Fertility Distress in Collaboration With Patient Research Partners

    • BACKGROUND: The Internet should be suitable for delivery of interventions targeting young cancer patients. Young people are familiar with the technologies, and this patient group is small and geographically dispersed. Still, only few psycho-educational Web-based interventions are designed for this group. Young cancer patients consider reproductive health, including sexuality, an area of great importance and approximately 50% report sexual problems and fertility-related concerns following cancer treatment. Therefore, we set out to develop a self-help Web-based intervention, Fex-Can, to alleviate such problems. To improve its quality, we decided to involve patients and significant others as research partners. The first 18 months of our collaboration are described in this paper. The intervention will subsequently be tested in a feasibility study followed by a randomized controlled trial.OBJECTIVE: The study aims to describe the development of a Web-based intervention in long-term collaboration with patient research partners (PRPs).METHODS: Ten former cancer patients and two significant others participated in building the Web-based intervention, using a participatory design. The development process is described according to the design step in the holistic framework presented by van Gemert-Pijnen et al and evaluates the PRPs' impact on the content, system, and service quality of the planned intervention.RESULTS: The collaboration between the research group and the PRPs mainly took place in the form of 1-day meetings to develop the key components of the intervention: educational and behavior change content, multimedia (pictures, video vignettes, and audios), interactive online activities (eg, self-monitoring), and partial feedback support (discussion forum, tailored feedback from experts). The PRPs influenced the intervention's content quality in several ways. By repeated feedback on prototypes, the information became more comprehensive, relevant, and understandable. The PRPs gave suggestions concerning the number of exercises and pointed out texts and pictures needing revision (eg, experienced as normative or stereotypical) to increase the persuasiveness of the program. The system quality was improved by PRPs' feedback on design, technical malfunctions, and navigation on the website. Based on feedback about availability of professional support (technical problems and program content), the organization for support was clarified, which increased service quality. The PRPs also influenced the research project on an overall level by suggesting modifications of inclusion criteria for the RCT and by questioning the implementation plan.CONCLUSIONS: With suggestions and continuous feedback from PRPs, it was possible to develop a Web-based intervention with persuasive design, believed to be relevant and attractive for young persons with cancer who have sexual problems or fertility distress.
