  • Validation of the PDPAR as an adolescent diary : effect of accelerometer cut points

    • PURPOSE: To evaluate the validity of the Previous Day Physical Activity Recall (PDPAR) as a physical activity diary in adolescents using two accelerometer intensity classifications.METHODS: One hundred eighth graders (47 boys, 53 girls) used the PDPAR as a daily diary and wore MTI accelerometers for four consecutive days. Measured time spent in moderate (> or = 3 METs) and vigorous (> or = 6 METs) activity was based on two published MTI cut-point limits (that of Freedson et al./Trost et al. and that of Puyau et al.). Spearman rank order correlations and Bland-Altman plots were used to examine agreement between MTI and PDPAR diary estimates of activity.RESULTS: MTI estimates of mean minutes per day of total moderate to vigorous physical activity (MVPA) were 65.2 (+/-43.2) using the Freedson et al./Trost et al. cutoffs and 17.5 (+/-18.5) using those of Puyau et al., while students self-reported 105.1 (+/-80.1) min.d(-1). Significant relationships were observed between the diary and MTI for total MVPA using either the Freedson et al./Trost et al. (r = 0.42) or Puyau et al. (r = 0.41) cutoff as well as raw counts (r = 0.44). Plots showed reasonable agreement between the diary and Freedson et al./Trost et al. MTI estimates of MVPA for daily totals of < or = 60 min, but the Puyau et al. estimates were consistently lower. Diaries overestimated activity as time increased when compared to either MTI cut point, especially on vigorous activity.CONCLUSIONS: Time estimates of MVPA differed by assessment tool, but diary estimates showed adequate association with the MTI. Diaries reflected intensity-specific activity, corresponding most closely with the Freedson et al./Trost et al. classification of moderate, but substantially overestimated vigorous activity regardless of cut-point method. This is likely due to the measurement characteristics of the PDPAR, which classifies activities in 30-min blocks, as well as the nature of common activities in which high levels of intensity are not sustained.

  • More Physically Active and Leaner Adolescents Have Higher Energy Intake

    • Objective To test whether youths who engage in vigorous physical activity are more likely to have lean bodies while ingesting relatively large amounts of energy. For this purpose, we studied the associations of both physical activity and adiposity with energy intake in adolescents.Study design The study subjects were adolescents who participated in 1 of 2 cross-sectional studies, the Healthy Lifestyle in Europe by Nutrition in Adolescence (HELENA) study (n = 1450; mean age, 14.6 years) or the European Youth Heart Study (EYHS; n = 321; mean age, 15.6 years). Physical activity was measured by accelerometry, and energy intake was measured by 24-hour recall. In the HELENA study, body composition was assessed by 2 or more of the following methods: skinfold thickness, bioelectrical impedance analysis, plus dual-energy X-ray absorptiometry or air-displacement plethysmography in a subsample. In the EYHS, body composition was assessed by skinfold thickness.Results Fat mass was inversely associated with energy intake in both studies and using 4 different measurement methods (P <=.006). Overall, fat-free mass was positively associated with energy intake in both studies, yet the results were not consistent across measurement methods in the HELENA study. Vigorous physical activity in the HELENA study (P<.05) and moderate physical activity in the EYHS (P<.01) were positively associated with energy intake. Overall, results remained unchanged after adjustment for potential confounding factors, after mutual adjustment among the main exposures (physical activity and fat mass), and after the elimination of obese subjects, who might tend to under-report energy intake, from the analyses.Conclusion Our data are consistent with the hypothesis that more physically active and leaner adolescents have higher energy intake than less active adolescents with larger amounts of fat mass.

    • PURPOSE: To evaluate the association between upper-limb (UL) clinical tests and UL accelerometry-derived metrics in children with unilateral Cerebral Palsy (CP). METHODS: In this cross-sectional study, twenty children with unilateral CP and Manual Ability Classification System level I-III were included. Outcomes of the Assisting Hand Assessment, Box and Block-Test and accelerometry metrics were collected in the clinical setting and in daily life. UL asymmetry index (i.e., the ratio between the well-functioning UL and the affected UL use) was evaluated in different physical activity levels and relative use of UL was evaluated during daily living. Spearman's correlation was used to determine the association between UL clinical tests and accelerometry metrics in a clinical setting and in daily life.RESULTS: The strongest negative association was between the Assisting Hand Assessment units and accelerometry metrics during the sedentary time in daily life (rs = -0.64). The asymmetries between ULs were highest during the child's sedentary time (asymmetry index: 45.15) compared to when the child was in light (asymmetry index: 23.97) or higher intensity physical activity (asymmetry index: 13.39). The children used both ULs simultaneously for 44% of the time during daily life. CONCLUSION: Accelerometry metrics may provide additional objective information to clinical tests by quantifying the amount of UL movements and the amount of asymmetry between the upper limbs in daily life.

  • The International Physical Activity Questionnaire modified for the elderly : aspects of validity and feasibility

    • OBJECTIVE: To modify the self-administered, short version of the International Physical Activity Questionnaire (IPAQ) for adults to be used in the elderly (aged 65 years and above), and to validate this modified IPAQ for the elderly (IPAQ-E).DESIGN: A direct validity study using accelerometer-measured physical activity (PA) as the criterion measure, and an indirect criterion validity study using high-sensitivity C-reactive protein (hs-CRP) as a biological marker of activity.SETTING: Organisations for retired persons in Sweden.SUBJECTS: The direct validity study consisted of fifty-four participants and the indirect criterion validity study consisted of 359 participants. All participants were retired persons (66-91 years) living independently.RESULTS: All self-reported activity domains (sitting, walking, moderate and vigorous) were positively correlated with the corresponding variable objectively assessed by an accelerometer (rho = 0.277-0.471), but a systematic error was observed. The specificity of IPAQ-E to identify low-active participants was 85 %, and the sensitivity to identify the more active participants was 81 %. A main effect of IPAQ-E category (Low, Moderate or High) was observed for hs-CRP (P = 0.041).CONCLUSIONS: We found this modified version of IPAQ, the IPAQ-E, to be well accepted by our sample of socially active elderly. It provided acceptable estimates of PA, well in line with other questionnaires, even though it had a systematic error. The IPAQ-E was able to identify an expected response of a biomarker (hs-CRP) to PA. We recommend the use of the IPAQ-E to classify participants aged 65 years and above into PA categories, to rank individuals or to identify individuals meeting certain PA criteria.

  • Physiotherapy and physical activity in patients undergoing cardiac or lung cancer surgery

    • Cardiovascular diseases are the leading cause of death worldwide. Cardiac surgery is performed to improve prognosis, relieve symptoms and increase functional capacity in patients with cardiac disease. Postoperative pulmonary complications are common after cardiac surgery and a reduced lung function can persist a long time after surgery. A positive association between level of physical activity and lung function has been proposed in both healthy individuals and people with different disabilities. It is not clear if there is an association between level of physical activity and recovery of lung function after cardiac surgery. Lung cancer is one of the most frequently diagnosed forms of cancer worldwide, and a leading cause of cancer deaths. Surgical resection is the primary approach for curative treatment. Despite the fact that physical activity has many positive effects on health, patients undergoing lung cancer surgery often report a low level of physical activity. Measuring physical activity is not easy, self-reported physical activity remains the most clinically applicable type of measurement, and a simple and valid questionnaire for screening patients would be valuable. Patients undergoing lung cancer surgery are often routinely treated by physiotherapists, but this kind of treatment has not been thoroughly investigated. The purpose of this thesis was to investigate the effect of physiotherapy and physical activity in patients undergoing cardiac or lung cancer surgery. This thesis include one cohort study of physical activity and recovery of lung function in patients undergoing cardiac surgery, one validation study of two self-reported physical activity instruments in patients undergoing lung cancer surgery, and two randomized controlled trials investigating the effect of physiotherapy for patients undergoing lung cancer surgery. In study I, patients who remained active or increased their level of physical activity had better recovery of lung function, compared to patients who remained sedentary or reported a lower level of physical activity postoperatively. In study II, two self-reported physical activity instruments were validated against accelerometer data in patients three and twelve months after lung cancer surgery. Both instruments were found able to identify patients not meeting recommendations on physical activity. In study III, patients treated by physiotherapists were significantly more active during the first three days after lung cancer surgery, compared to an untreated control group. In study IV, no between-group differences three months after surgery were found between patients receiving in-hospital physiotherapy compared to an untreated control group. However, the patients in the treatment group reported an increase of physical activity three months after surgery compared to preoperatively, while the patients in the control group did not.

    • BACKGROUND: This aimed to evaluate the effects of self-monitoring of daily steps with or without counselling support on HbA1c, other cardiometabolic risk factors and objectively measured physical activity (PA) during a 2-year intervention in a population with prediabetes or type 2 diabetes.METHODS: The Sophia Step Study was a three-armed parallel randomised controlled trial. Participants with prediabetes or type 2 diabetes were recruited in a primary care setting. Allocation (1:1:1) was made to a multi-component intervention (self-monitoring of steps with counselling support), a single-component intervention (self-monitoring of steps without counselling support) or standard care. Data were collected for primary outcome HbA1c at baseline and month 6, 12, 18 and 24. Physical activity was assessed as an intermediate outcome by accelerometer (ActiGraph GT1M) for 1 week at baseline and the 6-, 12-, 18- and 24-month follow-up visits. The intervention effects were evaluated by a robust linear mixed model.RESULTS: and HbA1c was 50 (11) mmol/mol, 21% had prediabetes and 40% were female. The dropout rate was 11% at 24 months. Effect size (CI) for the primary outcome (HbA1c) ranged from -1.3 (-4.8 to 2.2) to 1.1 (-2.4 to 4.6) mmol/mol for the multi-component vs control group and from 0.3 (-3.3 to 3.9) to 3.1 (-0.5 to 6.7) mmol/mol for the single-component vs control group. Effect size (CI) for moderate-to-vigorous physical activity ranged from 8.0 (0.4 to 15.7) to 11.1 (3.3 to 19.0) min/day for the multi-component vs control group and from 7.6 (-0.4 to 15.6) to 9.4 (1.4 to 17.4) min/day for the single-component group vs control group.CONCLUSION: This 2-year intervention, including self-monitoring of steps with or without counselling, prevented a decrease in PA but did not provide evidence for improved metabolic control and cardiometabolic risk factors in a population with prediabetes or type 2 diabetes.TRIAL REGISTRATION: ClinicalTrials.gov, NCT02374788 .     • BACKGROUND: Physical activity prevents or delays progression of impaired glucose tolerance in high-risk individuals. Physical activity promotion should serve as a basis in diabetes care. It is necessary to develop and evaluate health-promoting methods that are feasible as well as cost-effective within diabetes care. The aim of Sophia Step Study is to evaluate the impact of a multi-component and a single component physical activity intervention aiming at improving HbA1c (primary outcome) and other metabolic and cardiovascular risk factors, physical activity levels and overall health in patients with pre- and type 2 diabetes.METHODS/DESIGN: Sophia Step Study is a randomized controlled trial and participants are randomly assigned to either a multi-component intervention group (A), a pedometer group (B) or a control group (C). In total, 310 patients will be included and followed for 24 months. Group A participants are offered pedometers and a website to register steps, physical activity on prescription with yearly follow-ups, motivational interviewing (10 occasions) and group consultations (including walks, 12 occasions). Group B participants are offered pedometers and a website to register steps. Group C are offered usual care. The theoretical framework underpinning the interventions is the Health Belief Model, the Stages of Change Model, and the Social Cognitive Theory. Both the multi-component intervention (group A) and the pedometer intervention (group B) are using several techniques for behavior change such as self-monitoring, goal setting, feedback and relapse prevention. Measurements are made at week 0, 8, 12, 16, month 6, 9, 12, 18 and 24, including metabolic and cardiovascular biomarkers (HbA1c as primary health outcome), accelerometry and daily steps. Furthermore, questionnaires were used to evaluate dietary intake, physical activity, perceived ability to perform physical activity, perceived support for being active, quality of life, anxiety, depression, well-being, perceived treatment, perceived stress and diabetes self- efficacy.DISCUSSION: This study will show if a multi-component intervention using pedometers with group- and individual consultations is more effective than a single- component intervention using pedometers alone, in increasing physical activity and improving HbA1c, other metabolic and cardiovascular risk factors, physical activity levels and overall health in patients with pre- and type 2 diabetes.TRIAL REGISTRATION: ClinicalTrials.gov Identifier: NCT02374788 .
