  • The effects on depression of Internet-administered behavioural activation and physical exercise with treatment rationale and relapse prevention : study protocol for a randomised controlled trial

    • Background: Despite their potential as low-threshold, low-cost and high-flexibility treatments of depression, behavioural activation and physical exercise have not yet been directly compared. This study will examine the effects of these interventions, administered via the Internet. The added effect of providing a treatment rationale will also be studied, as well as a relapse prevention program featuring cognitive behavioural therapy components.Methods/Design: This randomised controlled trial will include 500 participants meeting the diagnostic criteria for major depression, recruited in multiple cycles and randomised to either a waiting list control group with delayed treatment, or one of the four treatment groups: (1) physical exercise without a clear treatment rationale; (2) physical exercise with treatment rationale; (3) behavioural activation with treatment rationale; or (4) behavioural activation without a clear treatment rationale. Post treatment, half of the participants will be offered a relapse prevention program. Primary outcome measure will be the Patient Health Questionnaire 9-item. Secondary measures include diagnostic criteria for depression, as well as self-reported anxiety, physical activity and quality of life. Measurements - done via telephone and the Internet - will be collected pre-treatment, weekly during treatment period, immediately post treatment and then monthly during a 24-month follow-up period.Discussion: The results of this study will constitute an important contribution to the body of knowledge of the respective interventions. Limitations are discussed.

  • Prevalence of burnout in competitive adolescent athletes

    • This study examined the factorial validity of the Eades Burnout Inventory (EABI) and the prevalence of burnout in adolescent elite athletes and whether burnout is more common in individual sports than in team sports. The EABI was distributed to 980 athletes (402 females and 578 males) in 29 different sports. Confirmatory-factor analyses revealed an acceptable factorial validity for a theoretically supported four-factor model of the EABI. Between 1% and 9% of the athletes displayed elevated burnout scores on these four subscales. The hypothesis of higher prevalence of burnout in individual sports was, however, not supported. Furthermore, no correlation between training load and burnout scores was found. These findings suggest that factors other than training load must be considered when athletes at risk for burnout are investigated.

    • The aim of this study was to gain a better understanding of the process of burning out in endurance athletes. The experiences of three elite cross-country skiers who left Their sport due to burnout were explored. Semi-structured interviews were conducted and inductively analyzed. The Athlete Burnout Questionnaire and training logs were used to validate the interviews and to enrich the analysis. The burnout process was found to evolve with different severity and time perspectives in the three cases. Athletic identity and achievement strivings to validate self-esteem were found to be important driving forces in the burnout process. Also, chronic lack of mental and physical recovery as well as early skiing success leading to high expectations comprised common themes in the burnout process.

  • Intra-individual variability in state anxiety and self-confidence in elite golfers

    • Precompetition anxiety levels are assumed to moderate athletic performance. Unfortunately, cross-sectional and nomothetic research designs have often shown non-significant findings; intra-individual variability may be a contributing factor. The extent of variability in precompetition anxiety and self-confidence responses as related to golf performance and trait measures were therefore examined using an idiographic approach. Individual patterns of variability were found for Cognitive and Somatic Anxiety and Self-Confidence scores yielded prior to the games played. Variability in Somatic Anxiety was significantly related to variability in golf performance. Players low in anxiety variability scored significantly higher on Private Self-Consciousness. The findings suggest the influence of anxiety and self-confidence on performance may be better understood when trait characteristics of the individual are also considered.

  • Who seeks ICBT for depression and how do they get there? : effects of recruitment source on patient demographics and clinical characteristics

    • Studies on internet-administered cognitive behavior therapy (ICBT) frequently use several different sources of recruitment, yet no study has investigated whether different recruitment sources produce different clinical and demographic profiles among participants. Using data from a large sample (n = 982) seeking ICBT for depression, we compared these characteristics on the basis of self-reported recruitment source. Recruitment sources that imply more active treatment-seeking behaviors (Google searches, viewing postings on mental health websites) presented more severe depression and anxiety than those recruited through more passive sources of information (newspaper advertisements, referrals by friends and family). In addition, a number of demographic differences between groups were found. These findings have important implications for ICBT research projects and clinical programs who employ open recruitment procedures and multi-modal recruitment strategies, and who wish to recruit representative samples or target specific subgroups. Replications in other countries will however be required to establish cross-cultural patterns.

    • The Competitive State Anxiety Inventory-2 (CSAI-2) is one of the most frequently used instruments when assessing competitive state anxiety in sport psychology research. However, doubts have been expressed about the factorial validity of both the English and the Greek versions of the scale. Hence, a revised version of the inventory (CSAI-2R) has recently been suggested to be more psychometrically sound (Cox et al., 2003). In the present study, the aim was to evaluate the psychometric properties of the Swedish version of the CSAI-2 using confirmatory factor analyses. A total of 969 athletes (571 men and 398 women) competing in 26 different sports completed the Swedish version of the CSAI-2. Three different factor structures were evaluated: the original three-factor model (with cognitive anxiety, somatic anxiety and self-confidence), a two-factor model in which self-confidence was excluded, and a three-factor model containing 17 items (CSAI-2R). The results revealed that only the 17-item model displayed an acceptable fit to the data. Although some doubts remain about the amount of variance that can be attributed to error variance in the subscales, the results suggest that it is better to use the CSAI-2R rather than the original CSAI-2.

    • Depression is an increasing public health concern with rising prevalence. Nevertheless, far from everyone seeks help or receives adequate treatment. Although psychotherapy and antidepressants still constitute the bulk of treatments offered, recent research suggests that physical activity (PA) can be a powerful adjunct therapy while sedentary behavior (SB) is a definite risk factor for developing depression. The aim of the present study was to investigate the relationship between PA, SB and depressive symptoms in a population (n = 962) of applicants for an online treatment study. This study hypothesised that there will be; (1) a positive relationship between SB and depressive symptoms, and (2) a negative relationship between PA and depressive symptoms. In addition we investigated whether the combination of a sedentary lifestyle and physical inactivity increased the risk for depressive symptoms. Finally, we also examined whether gender, age, marital status, educational level, or medication affected the relationship between PA, SB, and depressive symptoms. The results showed a positive correlation between SB and depression. There was, however, no statistically significant support for a negative relation between PA and depressive symptoms. Even though no conclusions about causality can be drawn, our results suggest that high SB, being a woman, being young, not being in a stable relationship, and current or previous medication are risk factors for depression. To be able to determine the causal direction, that is, whether high SB increases the risk for depressive symptoms, or if depressive symptoms increase the likelihood of high SB, further research is needed.

  • Behavioral activation versus physical activity via the internet : A randomized controlled trial

    • Background: A major problem today is that only about fifty percent of those affected by depressionseeks help. One way to reach more sufferers would be by offering easily accessible internet based treatments. The purpose of this study was to compare/evaluate four therapist supported internet administered treatments.Method/results: Two hundred eighty six participants were included. The treatment period lasted twelve weeks, consisting of the following treatments: 1) physical activity without treatment rational, 2) physical activity with treatment rational, 3) behavioral activation without treatment rational and 4) behavioral activation with treatment rational. All groups (including a control-group) showed a significant decrease in depressive symptoms. When the treatment groups were pooled and compared to the control group, there were significant differences from pretest to posttest (Hedges gav treatment =1.01, control group =0.47). This held true also when each of the four treatment groups was compared to the control group, with one exception: Physical activity without treatment rationale.Limitations: The differences between how many modules the participants completed could indicate that there are other factors than the treatments that caused the symptom reduction, however, the dose-response analysis did not detect any significant differences on account of modules completed.Conclusions: The results support the positive effects of internet administered treatments for depression, and highlights the importance of psychoeducation, which tends to affect both the treatment outcome and the probability of remaining in treatment. These aspects need to be considered when developing and conducting new treatments for depression, since they would increase the likelihood of positive treatment outcomes.

  • Internet-delivered therapist-guided physical activity for mild to moderate depression : a randomized controlled trial

    • Objective. The main hypothesis, and the objective of the study, was to test if the participants allocated to the treatment group would show a larger reduction in depressive symptoms than those in the control group.Methods. This study was a randomized nine week trial of an Internet-administered treatment based on guided physical exercise for Major Depressive Disorder (MDD). A total of 48 participants with mild to moderate depression, diagnosed using the Structured Clinical Interview for DSM-IV Axis I Disorders, were randomized either to a treatment intervention or to a waiting-list control group. The main outcome measure for depression was the Beck Depression Inventory-II (BDI-II), and physical activity level was measured using the International Physical Activity Questionnaire (IPAQ). The treatment program consisted of nine text modules, and included therapist guidance on a weekly basis.Results. The results showed significant reductions of depressive symptoms in the treatment group compared to the control group, with a moderate between-group effect size (Cohen's d = 0.67; 95% confidence interval: 0.09-1.25). No difference was found between the groups with regards to increase of physical activity level. For the treatment group, the reduction in depressive symptoms persisted at six months follow-up.Conclusions. Physical activity as a treatment for depression can be delivered in the form of guided Internet-based self-help.
