  • Bilateral Prophylactic Mastectomy in Swedish Women at High Risk of Breast Cancer: A National Survey.

    • BACKGROUND/OBJECTIVE:: This study attempted a national inventory of all bilateral prophylactic mastectomies performed in Sweden between 1995 and 2005 in high-risk women without a previous breast malignancy. The primary aim was to investigate the breast cancer incidence after surgery. Secondary aims were to describe the preoperative risk assessment, operation techniques, complications, histopathological findings, and regional differences. METHODS:: Geneticists, oncologists and surgeons performing prophylactic breast surgery were asked to identify all women eligible for inclusion in their region. The medical records were reviewed in each region and the data were analyzed centrally. The BOADICEA risk assessment model was used to calculate the number of expected/prevented breast cancers during the follow-up period. RESULTS:: A total of 223 women operated on in 8 hospitals were identified. During a mean follow-up of 6.6 years, no primary breast cancer was observed compared with 12 expected cases. However, 1 woman succumbed 9 years post mastectomy to widespread adenocarcinoma of uncertain origin. Median age at operation was 40 years. A total of 58% were BRCA1/2 mutation carriers. All but 3 women underwent breast reconstruction, 208 with implants and 12 with autologous tissue. Four small, unifocal, invasive cancers and 4 ductal carcinoma in situ were found in the mastectomy specimens. The incidence of nonbreast related complications was low (3%). Implant loss due to infection/necrosis occurred in 21 women (10%) but a majority received a new implant later. In total, 64% of the women underwent at least 1unanticipated secondary operation. CONCLUSIONS:: Bilateral prophylactic mastectomy is safe and efficacious in reducing future breast cancer in asymptomatic women at high risk. Unanticipated reoperations are common. Given the small number of patients centralization seems justified.

    • n the context of green supply chain management, green purchasing has received increased attention over the past decade and the strategic importance of introducing green aspects into purchasing practices has been recognised. Despite this growing importance, little has been written in relation to purchasing green transport and logistics services. Considering the strong environmental impact associated with transport and logistics activities, much remains to be learned concerning buyer’s practices when sourcing more sustainable services from third party logistics companies (3PLs). The aim of this paper is to explore practices of buying green transport and logistics services in three different European countries (Italy, Ireland and Sweden) using a multiple case study research approach. The paper analyses how general environmental company ambitions and environmental purchasing practices are reflected when green transport and logistics services are purchased. The results of the paper indicate that while the case companies show a relatively high concern of green issues at company level, a lower importance is attributed to green issues at the purchasing function level. When green concerns in purchasing transport and logistics services are analysed the level of importance decrease dramatically. It emerges a conflicting attitude among the overall company level and the purchasing of transport and logistics services. This suggests that there is the potential for improvements especially in the area of green collaboration in buyer and supplier relationships.

  • Green Initiatives in the Transport and Logistics Service Industry : An Exploratory Case Study of Logistics Service Providers

    • The impact of business operations on the natural environment is an area in which societies and governments have become more proactive. This specifically affects logistics service providers (LSPs) where the core business has a significant environmental impact. That is why transport and logistics companies are increasingly being asked to respond to the challenges of green issues by implementing more environmentally sustainable strategies. Hence environmental research specific to LSPs has been neglected despite the fact that these companies have assumed a more critical role in the wider supply chain. In order to generate more structured knowledge and understanding of the green initiatives undertaken by LSPs, this paper explores the level of awareness and actual initiatives undertaken, as well as the drivers and barriers these companies encounter in this area today. This has been done through a multiple case study approach investigating a sample of LSPs operating in Sweden, Italy and Ireland. The results of the case study identify the environmental attitude of LSPs and how they are responding to the green pressures on their businesses. Managerial and research implications – in particular, a set a propositions are derived from the study.

    • Considering its strong environmental impact, logistics plays a critical role in green supply chain management. It provides strategic links in the supply chain and is an essential function in the delivery of green products to the consumer. There is a general consensus on the fact that more environmentally sustainable companies may be achieved only if transport and logistics activities also become greener. To achieve this objective, buyer companies need to incorporate green considerations in the purchasing of transport and logistics services. This appears particularly challenging for small and medium sized enterprises (SMEs) because of their traditional lack of managerial, organisational and financial resources that often result in failure to adopt an environmental perspective. In the extant literature, green purchasing has received increased attention over the past decade and the strategic importance of introducing green aspects into purchasing practices has been recognised. However, little has been written in relation to purchasing green transport and logistics services. The aim of this paper is to explore practices in the buying of green transport and logistics services and to derive implications for small buyer companies. The paper analyses how general environmental company ambitions and environmental purchasing practices are reflected when green transport and logistics services are purchased in three different European countries (Italy, Ireland and Sweden) using a multiple case study research approach. The results of the paper indicate that while the case companies show a relatively high concern for green issues at corporate level, a lower importance is attributed to green issues at the purchasing function level. When green concerns in the purchasing of transport and logistics services are analysed the level of importance decreases further. Thus, a conflicting attitude is evident between the overall corporate level and the purchasing of transport and logistics services specifically. This suggests that there is potential for improvement especially in the area of green collaboration in buyer and supplier relationships.

    • Environmental sustainability is an area of increasing importance for third party logistics (3PL) com- panies. As the design and implementation of services requires interaction between buyer and 3PL, the 3PLs are in a critical position to support the efforts towards greening operations of different supply chain participants. However the literature in this field reflects a gap between the perspectives of buy- ers and 3PLs. This chapter attempts to fill this void through an explorative case study analysis on the environmental attitude of 3PLs in order to derive implications for buyers’ behavior. The results indicate that the buyer’s role is critical in different ways in the development of green initiatives among 3PLs. An increased orientation towards longer-term contracts and joint development would likely enhance the level of green initiatives. Indirectly, the buyer has the opportunity to influence its 3PLs through interac- tion with employees on different levels in the company, including top management.

    • BACKGROUND: Bilateral risk-reducing mastectomy (BRRM) is the most effective method to prevent breast cancer in genetically predisposed women and is often performed concomitantly with breast reconstruction. The reconstruction time varies and corrective surgeries are common.METHODS: This study evaluated 185 consecutive cases of BRRM and immediate breast reconstruction with implants with regard to surgical outcome and resource consumption. With an 18-year observation period, it was possible to compare permanent expander implants (PEIs) with permanent fixed volume implants (PIs).RESULTS: A minimum follow-up of 2 years for all participants but one was achieved. Seventy-five percent (n = 138) of the women received PEI and 25% (n = 47) PI. In a multivariate analysis including age, BMI (<25, ≥25), smoking (yes, no), implant type (PEI, PI), incision technique, operation time and specimen weight <350 g, ≥350 g), only BMI ≥25 was associated with an increased risk of an early complication (OR 7.1, 95% CI 2.44-20.4). As expected, there was a significant difference in median reconstruction time between PEI and PI (12.4 vs. 1.0 months, p < 0.001). The cumulative reoperation-free 2-year survival was significantly higher in the PI than in the PEI group (81% vs. 26%, p < 0.001).CONCLUSION: Implant-based reconstruction remains a valid option after BRRM in high-risk women. Whenever possible (low BMI and small breast volume without severe ptosis), permanent fixed volume implants can be safely recommended and are resource saving in comparison with permanent expander implants.

    • Purpose of this paper: Logistics companies need to take account of environmental aspects into their services to strengthen their position on the market and meet the increased demands from the society. Therefore, the purpose of this paper is to present a framework for how to consider sustainability in the development of logistics services. The paper also aims at developing a research agenda for further research needs regarding how logistics companies can include the environmental aspect in their service offerings. Design/methodology/approach: This paper is based on a literature review that has its origin in sustainable service and product development. This is compared with literature about development of offerings for logistics companies. The results from the literature studies are then analyzed in order to identify a research agenda and to present a framework containing aspects of sustainable service offerings of relevance from a logistics company perspective. Findings: The paper suggests how logistics service offerings can be enhanced through sustainable service and product development literature. A framework consisting of aspects to address in the development of sustainable offering for logistics firms is presented. Research limitations/implications: The findings should be considered as conceptual, and provides a basis for further empirical based research Practical implications: The results provide a basis for logistics companies, to build upon in their continuing efforts to develop and enhance more sustainable services to their customers. What is original/value of paper: A large amount of literature can be found concerning sustainable development of offerings. However, literature concerning the development of sustainable services is not that available and there is a large lack regarding the connection to logistics companies.

    • Purpose of this paperAs logistics services increase in both scope and complexity, Logistics Service Providers (LSPs) face increasing challenges in their attempts to improve service performance to customers. One challenge encountered by LSPs comprehends how they should approach environmental issues and also what it is worth for their customers. One way for LSPs to achieve this is to focus on value co-creation between them and their customers. This paper will examine the adequacy of a Service Dominant (S-D) logic perspective in order to better understand value co-creation between LSPs and customers, with a specific focus on environmental aspects as value.Design/methodology/approachWith the starting-point in S-D logic applied to a logistics context, empirical data from case studies with an environmental focus is analysed in order to illustrate LSPs’ situation today.FindingsA range of key elements that can help LSPs to increase value co-creation is proposed. The findings indicate that an S-D logic perspective on environmental co-creation is useful in order to increase the understanding of the value co-creation process between LSPs and their customer.Research limitations/implicationsThe empirical data focuses on environmental aspects of value co-creation within logistics and further research is needed in order to verify the findings on a more general level of value co-creation.Practical implications The results of this paper can inspire LSPs to shift from traditional thinking and working approaches towards a more S-D inspired approach, thereby increasing value co-creation.What is original/value of paperThis paper provides a novel approach when it comes to how to view value co-creation in the relationship between LSPs and their customers. Perspectives and approaches from the S-D logic literature are studied, where LSPs previously are noticed to a very limited extent.
