  • Organisation of preventive child health services : Key to socio-economic equity in vaccine uptake?

    • BACKGROUND: Measles has made a comeback in Western Europe, with more cases being reported each year. One factor behind this development is low vaccination coverage in socially disadvantaged segments of the population in many countries. This study investigates whether socioeconomic patterns of uptake of the measles, mumps and rubella (MMR) vaccine in the Nordic countries differ by national organisation of preventive health services for children. METHODS: MMR vaccine uptake before the age of two years was analysed in register data from Denmark, Finland, Iceland and Sweden, linked to family indicators of socio-economic status (SES) from national registers. RESULTS: Denmark, a country where child vaccinations are administered by general practitioners, presented the lowest overall coverage of MMR at 83%. It also had the greatest difference between subpopulations of low and high SES at 14 percentage points. Finland, Iceland and Sweden, countries where preschool children are vaccinated in 'well-baby' clinics, had a higher overall coverage at 91-94%, with a more equal distribution between SES groups at 1-4 percentage points. CONCLUSIONS: This study suggests that the organisation of preventive health care in special units, 'well-baby' clinics, facilitates vaccine uptake among children with low SES in a Nordic welfare context.

    • Konkurrens är en viktig och vitt spridd styrform i dagens samhälle. Tillhandahållandet av allt från dagligvaror och bilar till sjukvård, skola och tv-program styrs i allt högre grad genom att utförare får konkurrera med varandra. Konkurrens har blivit en så vanlig styrform att den ofta ses som ett ”naturligt tillstånd”, som om det inte finns några alternativ.Huvudbudskapet i denna bok är att konkurrens inte är ett naturligt tillstånd. Konkurrens behöver oftast beslutas och organiseras fram, vilket innebär att saker och ting ofta behöver ändras för att det ska kunna bli konkurrens. Författarna beskriver hur konkurrens faktiskt fungerar, varför konkurrens införs, hur konkurrens skiljer sig från andra mer välkända styrformer och vilka resultat man kan vänta sig vid införandet av konkurrens som styrform. Till exempel förklaras varför konkurrens ofta leder till mer av samma i stället för ökad variation, och hur professioner som läkare och lärare påverkas av att konkurrera.Exemplen i boken kommer framför allt från offentlig sektor, men principerna fungerar lika väl för andra sektorer. Författarna menar att detta är värdefull kunskap för alla som är chefer, medarbetare eller kunder i organisationer som har konkurrensutsatts – vilket inbegriper de flesta i vårt samhälle.

    • In this article, Bojanic, Jonsson, Neergaard and Sauer present a synthetic overview of the five country cases included in the special issue that analyse the emergence of cultures of rejection since 2015. In general, they discuss the conceptual framework of ‘Cultures of Rejection’, elaborated throughout the issue as a more encompassing approach that is sensitive to the values, norms and affects that underlie different or similar patterns of exclusion and rejection in different contexts. These cultures are located in the everyday lives of people. The article, therefore, first identifies contexts, objects of rejection­—often migrants and racialized Others, but also ‘the political’ or state institutions—narratives and components of cultures of rejection that we label reflexivity, affect, nostalgia and moralistic judgement. The contrasting reading of the five cases shows that people struggle for agency under precarious and insecure conditions, and fight against imagined enemies. As Bojanić, Jonsson, Neergaard and Sauer conclude, cultures of rejection mirror ongoing processes of neoliberal dispossession, authoritarization and depolitization that culminate in a wish for agency and resovereignization. Second, and based on this overview, trends in cultures of rejection are detected against different national contexts as well as against common trends of social and economic transformations and crises, such as, for instance, the COVID-19 pandemic. This results, finally, in a discussion of ways of challenging the cultures of rejection towards more democratic and solidaristic societies. One starting point might be the ‘re-embedding’ of the economy in society, that is, a more equal distribution of resources and future perspectives.

    • Metal casting is an industrially important manufacturing process offering a superior combination of design flexibility, productivity and cost-effectiveness, but has limitations due to filling related defects. Several semisolid casting processes are available capable of casting at a range of solid fractions to overcome this. The current communication aims to review the filling front behaviour and give a new perspective to the gate design in semisolid processing compared to conventional high-pressure die-casting. It is shown that solid fraction and gate widths are critical to avoid instability and spraying.

    • Semisolid casting can provide excellent castings, but the nature of the pore-forming mechanisms has not been properly clarified. In the current communication, it was suggested that hydrogen precipitated during slurry making might have a decisive role in the formation of both gas and shrinkage porosity. Intensive stirring at the end of the slurry making process may act as a degassing step. Without the intense shearing, structures of primary slurry particles form around the hydrogen pores, strongly affecting pore formation and feeding during the intensification stage.

    • Grænsen mellem Europa og Afrika er både flydende og fast. Det er en reali-tet som erkendes af de mange tusinde bådflygtninge, som hvert år drukner i deres forsøg på at krydse den hårdt håndhævede usynlige linje, som skæ-rer sig gennem Middelhavet. For selvom tragedien er alt for håndgribelig, er den også i en vis forstand et skue, som på en gang tjener til at forsikre europæerne om, at grænsen er fast og bevogtet, og til at bekræfte dem i deres medlidenhed med de flygtende andre. Når vi ser billederne fra Mid-delhavet forsikres vi om, at vi er i sikkerhed (om end truet), men vi får også mulighed for at vise vores humanitære side ved at forlange en mere human hjemsendelse af staklerne til Afrika på den anden side.Bidragene skal måske derfor ses som fragmenter i en større sammen-hæng, hvor Skandinavien og Europa konfronteres med et mindre smig-rende billede af det forgangne, og hvor europæere såvel som afrikanere får mulighed for at afprøve nye relationer og identiteter. Men fremfor alt tilhører de en sammenhæng, hvor flere af de traditioner, som længe har præget europæisk humaniora og samfundsvidenskab, støder mod sine be-grænsninger, tvinges til at gentænke sine begreber og opfinde nye kategori-er til en verden, hvor relationen er konstitutiv for enhver identitet, og hvor ethvert sted er en mødeplads for verdens trafik.   • A State of the Art Paper on Improving Salt Extraction from Lake Katwe Raw Materials In Uganda

    • The characteristics of Katwe salt lake are briefly discussed. The lake is the largest of the eight saline lakes in the Katwe-Kikorongo volcanic field and is a major source of salt production in Uganda. Today, salt production at the lake is carried out using traditional and artisanal mining methods. Attempts to mechanize the production of domestic and commercial grade salt at the lake were unsuccessful due to the use of a wrong technology. In this paper, the most common available technologies for salt extraction from brine are described. These are divided into four broad categories, namely thermal, membrane, chemical and hybrid processes. A review of the state of the art, previous research and developments in these technologies is presented. A detailed analysis of the processes used was done based on studies reported in the literature. From the analysis, it was observed that thermal salt production processes, especially distillation and solar evaporation have the highest share in installed capacities worldwide. Membrane technologies such as Electro-dialysis, Reverse Osmosis and chemical technologies have not found wide application in the commercial salt industry. Electro-dialysis and Reverse Osmosis have been used mainly as pre-concentration processes for subsequent thermal processes. Prospects for application of hybrid systems for salt production through integration of thermal desalting processes should be investigated for better performance efficiencies and recoveries at the salt lake.

    • Lake Katwe brines and evaporites were investigated to determine their chemical, mineralogical and morphological composition. 30 brine samples and 3 solid salt samples (evaporites) were collected from different locations of the lake deposit. Several analytical techniques were used to determine the chemical composition of the samples including Inductively Coupled Plasma-Atomic Emission Spectrometry (ICP-AES), Inductively Coupled Plasma-Sector Field Mass Spectrometry (ICP-SFMS), ion chromatography, and potentiometric titration. The mineralogical composition and morphology of the evaporites was determined using X-ray diffraction (XRD) and scanning electron microscopy (SEM), respectively. Physical parameters of the lake brines such as density, electrical conductivity, pH, and salinity were also studied. The results show that the lake brines are highly alkaline and rich in Na+, Cl-, CO32-, SO42-, and HCO3- with lesser amounts of K+, Mg2+, Ca2+, Br-, and F- ions. The brines show an intermediate transition between Na-Cl and Na-HCO3 water types. Among the trace metals, the lake brines were found to be enriched in B, I, Sr, Fe, Mo, Ba, and Mn. The solid salts are composed of halite mixed with other salts such as hanksite, burkeite and trona. It was also observed that the composition of the salts varies considerably even within the same grades.
