  • Measures against preschool noise and its adverse effects on the personnel : an intervention study

    • The aim of the study was to analyze the exposure effects of different types of noise measures carried out at preschools. The project was carried out as an intervention study. The investigation included 89 employees at 17 preschools in the northern part of Sweden. Individual noise recordings and recordings in dining rooms and play halls were made at two departments in each preschool. The adverse effects on the employees were analyzed with validated questionnaires and saliva cortisol samples. Evaluations were made before and 1 year after the first measurement. Between the two measurements, measures were taken to improve the sound environments at the preschools. The effects of the measures varied a lot, with respect to both the sound environments and health. Regarding acoustical measures, significant changes were seen for some of the variables analyzed. For most of the tested effects, the changes, however, were very small and non-significant. The effects of organizational measures on the objective and subjective noise values were in overall less pronounced. Acoustical measures improved the subjectively rated sound environment more than organizational measures. This may be due to the high work effort needed to implement organizational measures. Even though the sound level was not lower, the personnel experienced improvements of the sound environment.

  • Relapse Prevention Therapy for Problem Gaming or Internet Gaming Disorder in Swedish Child and Youth Psychiatric Clinics : Protocol for a Randomized Controlled Trial

    • Background: Although gaming is a common arena where children socialize, an increasing number of children are exhibiting signs of problem gaming or internet gaming disorder. An important factor to the development of problem gaming is parent-child relationships. A cognitive behavioral therapy–based form of treatment, labeled relapse prevention, has been developed as a treatment for child and adolescent problem gaming or internet gaming disorder. However, no study has evaluated the effect of this treatment among Swedish children and youth nor the role of the parent-child relationships in this treatment.Objective: This study aims (1) to evaluate a relapse prevention treatment for patients showing signs of problem gaming or internet gaming disorder recruited from child and youth psychiatric clinics and (2) to test whether the quality of parent-child relationships plays a role in the effect of relapse prevention treatment and vice versa—whether the relapse prevention treatment has a spillover effect on the quality of parent-child relationships. Moreover, we explore the carer’s attitudes about parent-child relationships and child gaming, as well as experiences of the treatment among the children, their carers, and the clinicians who carried out the treatment.Methods: This study is a 2-arm, parallel-group, early-stage randomized controlled trial with embedded qualitative components. Children aged 12-18 years who meet the criteria for problem gaming or internet gaming disorder will be randomized in a 1:1 ratio to either intervention (relapse prevention treatment) or control (treatment as usual), with a total of 160 (80 + 80) participants. The primary outcomes are measures of gaming and gambling behavior before and after intervention, and the secondary outcomes include child ratings of parent-child communication and family functioning. The study is supplemented with a qualitative component with semistructured interviews to capture participants’ and clinicians’ experiences of the relapse prevention, as well as attitudes about parent-child relationships and parenting needs in carers whose children completed the treatment.Results: The trial started in January 2022 and is expected to end in December 2023. The first results are expected in March 2023.Conclusions: This study will be the first randomized controlled trial evaluating relapse prevention as a treatment for child and adolescent problem gaming and internet gaming disorder in Sweden. Since problem behaviors in children interact with the family context, investigating parent-child relationships adjacent to the treatment of child problem gaming and internet gaming disorder is an important strength of the study. Further, different parties, ie, children, carers, and clinicians, will be directly or indirectly involved in the evaluation of the treatment, providing more knowledge of the treatment and its effect. Limitations include comorbidity in children with problem gaming and internet gaming disorder and challenges with the recruitment of participants.

  • Undergraduate Nursing Students' Experiences of Learning Caring Using a Variety of Learning Didactics

    • This study examines undergraduate nursing students' experiences of participating in a Caring Behavior Course using various learning didactics. Twenty-five students participated in one of five focus group interviews with data analyzed according to qualitative content analysis. The main theme to emerge, an insightful and sudden awakening that caring is not only theoretical words, was further explained with three themes and nine subthemes. The Caring Behavior Course demonstrates effective learning didactics to develop awareness of values that influence caring behaviors and can contribute to patient well-being, particularly relevant for the care challenges in the time of COVID-19 and beyond.

    • Aim:To describe undergraduate nursing students' attitudes to learning during clinical practice in different semesters when using the conceptual learning model, Model for Improvements in Learning Outcomes (MILO) grounded in a caritative caring perspective.Background: With the intention to support interlinking between theory and praxis and offer understanding and structure to facilitate learning, MILO, theoretically grounded in hermeneutics and a caritative caring perspective based on ethical values, was implemented. MILO consists of four contextual concepts (peer learning, co-clinical teachers, student-centred and student-active supervision) and four intrapersonal concepts (nursing, a reflective approach, a critical approach, quality and safety).Methods: A descriptive comparative quantitative study design was applied at a Swedish university, 3 hospitals and 13 municipalities in one county. Cross-sectional data collected via a questionnaire developed to assess attitudes to learning related to MILO's contextual and intrapersonal concepts and their applications were used.Results: 209 students in semester 3, 4 and 6 participated in 6 different clinical practice courses. In comparison, intrapersonal concepts, that is, the student's own characteristics and abilities were viewed to be of greater value for learning than contextual, that is, organisational-related concepts in all semesters. Understanding the needs of others and reflective learning were rated to be of major importance. Students in semester 3 valued the use of the applications the highest. To be supervised in pairs was rated the lowest in semester 6. Some of the concepts and their applications were to great extent not applied.Conclusions: In all semesters, fundamentals in caritative caring and characteristics and abilities related to the individual student were rated to be of greater importance for learning than environmental support. Providing students opportunities to develop independency seems essential. Use of a learning model such as MILO is dependent on a bearing of a caritative caring culture and a shared understanding between all involved in student learning during clinical practice.

  • Bridging the Gap between Caring and Nursing Practice : Undergraduate Nursing Students Experiences of Learning Caring Behaviors with Standaradized Patients in Simulation

    • Motivation & AimAlthough school of nursing curricula often refer to caring as the central core in nursing practice, students often have a gap in understanding and knowing how to intertwine caring in all aspect of nursing practice. Thus, nursing curricula often tend to focus more on developing psychomotor skills and knowledge, referred to as doing, with less concentration on how one interacts with patients. Even though there has been extensive research on simulation in nursing education there are limited studies focusing on the intentional and visible incorporation of compassionate and competent caring behavior in simulations. Therefore, the aim was to describe undergraduate nursing students’ experiences of practicing caring behaviors with a standardized patient. MethodA sample of forty-eight fourth semester undergraduate nursing students enrolled in a five-week caring behavior course at a Swedish university, were at the first and last week individually video-recorded during two caring behavior simulations with a standardized patient. After observing each of their video-recordings, students completed individual written reflection on their own verbal and nonverbal caring behavior. The written reflections were analyzed using qualitative content analysis. ConclusionSimulated encounters with standardized patients that visibly intertwine caring with nursing practice facilitates nursing students’ learning of compassionate and competent caring behaviorsThe learning experience made the students aware of the impact of practicing caring in nursing practice, recognizing that being with is not the same as doing for the patient, and thus, how challenging it is to be mindfully present in patient encountersVideo-recorded caring behavior simulations provide a feasible educational learning didactic, facilitating students’ learning to apply caring behaviors

  • The Caring Behavior Coding Scheme : Development of an Observational Behavioral Scheme

    • Motivation & AimIn today's complex healthcare organizations there is an increasing recognition of the need to enhance care quality and patient safety. Simultaneously, patient boards receive an increasing number of complaints from patients and significant others regarding healthcare providers’ non-caring behaviors. Defining and measuring both verbal and nonverbal caring and non-caring behavior in healthcare providers’ care delivery is vital to address such complaints. However, no studies were found that incorporated a comprehensive theory of caring to code encounters between healthcare providers and patients. Therefore, the aim was to develop and test a caring behavior coding scheme based on Swanson’s Theory of Caring.MethodAn instrument development process was used for behavioral coding including observational data from thirty-eight video recordings collected in an undergraduate nursing course at a Swedish University. The observational data depicts a six minutes simulated interaction between an undergraduate nursing student and a standardized patient focusing caring in nursing practice. ResultThe Caring Behavior Coding Scheme (the CBCS) entails eight nonverbal and seventeen verbal behavioral codes, categorized as caring and non-caring behaviors in accordance with Swanson’s Theory of Caring. Content validity was examined by Dr. Swanson, face validity was conducted by two clinical-nurses. A timed-event sequential continuous coding was performed in INTERACT®, the coder achieved excellent agreement with the developed gold standard (k = 0.87), and excellent mean inter-rater reliability (k = 0.82). The CBCS adequately captured caring behaviors as all domains in Swanson’s Theory of Caring were rated in the interaction.

    • This study investigated the effect of confirmatory and disconfirmatory feedback on the accuracy (realism) in witnesses’ confidence judgements of their event memory. After watching a short video of a kidnapping the participants answered 44 two-alternative questions on the videotape. Two weeks later the participants were handed their old question-answering sheet with the original questions and their own answers. In addition feedback was provided in writing for each answer. The participants were asked to give confidence ratings with respect to the correctness of each answer. In the Control condition the participants received no feedback. Overall, the witnesses’ confidence ratings showed overconfidence in all conditions. Furthermore, confirmatory feedback caused higher overconfidence, compared both with disconfirmatory feedback and no feedback. The results show that the realism in the eyewitnesses’ confidence judgements of their event memory is indeed sensitive to co-witness feedback, especially to confirming feedback.

    • This study investigated the effect of two types of feedback (confirmatory and disconfirmatory) on the accuracy in witness’ confidence judgements of their event memory. Overall the witnesses evidenced overconfidence both when they received feedback and in the control condition (no feedback). The results showed that confirmatory feedback caused higher overconfidence, compared both with when receiving disconfirmatory and no feedback. The results suggest that the impact of feedback on the accuracy of confidence judgments show the same pattern of results for event memory as for line-up identification tasks. Finally, when witnesses rated the total number of questions that they had answered correctly they gave fairly correct estimates compared with their actual number of correct answers.
