  • C4b-Binding Protein Deposition is Induced in Diseased Aortic Heart Valves, Coinciding with C3d

    • Background and aim of the study: It has been found recently that activated complement is more widespread in diseased aortic valves compared to the endogenous complement inhibitors C1-inhibitor and clusterin. Previously, another endogenous inhibitor of complement, C4b-binding protein (C4BP) has been described in atherosclerotic diseased coronary arteries. The study aim was to analyze C4BP levels in diseased aortic valves. Methods: Aortic valve tissue was derived from surgical procedures and classified as 'degenerative', 'atherosclerotic' or 'atherosclerotic with bacterial infection'. Valves were stained with specific antibodies against C4BP, C3d and caspase-3. Areas of positivity were then quantified using computer assisted morphometry. Results: In atherosclerotic valves, the areas of C4BP and C3d positivity (38.8 +/- 0.4% versus 32.7 +/- 1.0%, respectively) were significantly higher compared to the degenerative and control groups. In atherosclerotic valves with bacterial infection, the area of positivity for C4BP was even further increased compared to atherosclerotic valves (65.1 +/- 1.2%; 70.1 +/- 1.9% for C3d). The areas of C4BP and C3d positivity were not significantly different in all groups. Caspase-3 was only present in <10% of endothelial cells in the atherosclerotic valves without bacterial infection and in neutrophilic granulocytes in atherosclerotic valves, with and without bacterial infection. Conclusion: It has been shown for the first time that C4BP is deposited in the diseased aortic valve, coinciding with C3d. The area of C4BP positivity was more extensive compared to the areas of other endogenous complement inhibitors (C1-inhibitor and clusterin).

    • The instrument called Families Importance in Nursing Care-Nurses Attitudes (FINC-NA) is used to measure nurses attitudes toward involving families in their nursing care. The aim of this study is to evaluate the FINC-NA scale in a population of Dutch nurses and add new psychometric information to existing knowledge about this instrument. Using a cross-sectional design, 1,211 nurses received an online application in 2015. Psychometric properties were based on polychoric correlations and the Generalized Partial Credit Model. A total of 597 (49%) nurses responded to the online application. Results confirmed a four-subscale structure. All response categories were utilized, although some ceiling effects occurred. Most items increase monotonically, and the majority of items discriminate well between different latent trait scores of nurses with some items providing more information than others. This study reports the psychometric properties of the Dutch language FINC-NA instrument. New insights into the construct and content of items enable the possibility of a more generic instrument that could be valid across several cultures.

  • Validation and psychometric evaluation of the Dutch person-centred care of older people with cognitive impairment in acute care (POPAC) scale

    • Background: Person-centred care is the preferred model for caring for people with dementia. Knowledge of the level of person-centred care is essential for improving the quality of care for patients with dementia. The person-centred care of older people with cognitive impairment in acute care (POPAC) scale is a tool to determine the level of person-centred care. This study aimed to translate and validate the Dutch POPAC scale and evaluate its psychometric properties to enable international comparison of data and outcomes.Methods: After double-blinded forward and backward translations, a total of 159 nurses recruited from six hospitals (n=114) and via social media (n=45) completed the POPAC scale. By performing confirmatory factor analysis, construct validity was tested. Cronbach’s alpha scale was utilized to establish internal consistency.Results: The confirmatory factor analysis showed that the comparative fit index (0.89) was slightly lower than 0.9. The root mean square error of approximation (0.075, p=0.012, CI 0.057–0.092) and the standardized root mean square residual (0.063) were acceptable, with values less than 0.08. The findings revealed a three-dimensional structure. The factor loadings (0.69–0.77) indicated the items to be strongly associated with their respective factors. The results also indicated that deleting Item 5 improved the Cronbach’s alpha of the instrument as well as of the subscale ‘using cognitive assessments and care interventions’. Instead of deleting this item, we suggest rephrasing it into a positively worded item.Conclusions: Our findings suggest that the Dutch POPAC scale is sufficiently valid and reliable and can be utilized for assessing person-centred care in acute care hospitals. The study enables nurses to interpret and compare person-centred care levels in wards and hospital levels nationally and internationally. The results form an important basis for improving the quality of care and nurse-sensitive outcomes, such as preventing complications and hospital stay length.

  • Arterial Blood Pressure Induces Transient C4b-Binding Protein in Human Saphenous Vein Grafts

    • Background: Complement is an important mediator in arterial blood pressure-induced vein graft failure. Previously, we noted activation of cell protective mechanisms in human saphenous veins too. Here we have analyzed whether C4b-binding protein (C4bp), an endogenous complement inhibitor, is present in the vein wall. Methods: Human saphenous vein segments obtained from patients undergoing coronary artery bypass grafting (n = 55) were perfused in vitro at arterial blood pressure with either autologous blood for 1, 2, 4, or 6 hr or with autologous blood supplemented with reactive oxygen species scavenger N-acetylcysteine. The segments were subsequently analyzed quantitatively for presence of C4bp and complement activation product C3d using immunohistochemistry. Results: Perfusion induced deposition of C3d and C4bp within the media of the vessel wall, which increased reproducibly and significantly over a period of 4 hr up to 3.8% for C3d and 81% for C4bp of the total vessel area. Remarkably after 6 hr of perfusion, the C3d-positive area decreased significantly to 1.3% and the C4bp-positive area to 19% of the total area of the vein. The areas positive for both C4bp and C3d were increased in the presence of N-acetylcysteine. Conclusions: Exposure to arterial blood pressure leads to a transient presence of C4bp in the vein wall. This may be part of a cell-protective mechanism to counteract arterial blood pressure-induced cellular stress and inflammation in grafted veins.

    • Standardised data formats and data models are essential for data integration and interoperability, which in turn adds value to data by allowing its reuse in multiple contexts. For this reason, in recent years extensive efforts have been focused on standards development. When representing the built environment, 3D city models and Building Information Models are particularly relevant, and their integration is now required to underpin use cases that cover the full life-cycle of a built asset, including design and planning as well as operations and management, and to support legal applications such as cadastral systems. For those kinds of data, CityGML by the Open Geospatial Consortium and Industry Foundation Classes by buildingSMART are the most popular reference standards. However, many users report, often through informal channels, the difficulties of working with these formats. This paper summarizes the outcomes of the GeoBIM Benchmark 2019, a scientific initiative funded by ISPRS and EuroSDR to collect insights into the most relevant issues encountered in the management of CityGML and IFC within existing software. Alongside data management (import, visualisation, analysis, export) problems, issues of particular consequence in terms of integration relate to georeferencing IFC files and the conversions among the two kinds of formats and models. Thus, the benchmark was designed to explore these tasks in available software. Following analysis of the benchmark results, a key outcome is the impossibility to find clear patterns in the behaviour of tools, which consequently means there is no consistency in the implementation of standards. Although the results could seem disappointing, the criticality in managing these standards as they are was described and this awareness can be the starting point for further research or further standards development. Finally, this project was useful to gather a wide community around this topic, and the discussion about the GeoBIM-related issues was definitely pushed.
