    • Mosquito-borne alphaviruses have the potential to cause large outbreaks throughout the world. Here we investigated the causative agent of an unexpected Sindbis virus (SINV) outbreak during August-September, 2013, in a previously nonendemic region of Sweden. Mosquitoes were collected using carbon dioxide-baited CDC traps at locations close to human cases. The mosquitoes were initially screened as large pools by SINV-specific quantitative RT-PCR, and the SINV-positive mosquitoes were species determined by single-nucleotide polymorphism (SNP) analysis, followed by sequencing the barcoding region of the cytochrome oxidase I gene. The proportion of the collected mosquitoes was determined by a metabarcoding strategy. By using novel strategies for PCR screening and genetic typing, a new SINV strain, Lovanger, was isolated from a pool of 1600 mosquitoes composed of Culex, Culiseta, and Aedes mosquitoes as determined by metabarcoding. The SINV-positive mosquito Culiseta morsitans was identified by SNP analysis and sequencing. After whole-genome sequencing and phylogenetic analysis, the SINV Lovanger isolate was shown to be most closely similar to recent Finnish SINV isolates. In conclusion, within a few weeks, we were able to detect and isolate a novel SINV strain and identify the mosquito vector during a sudden SINV outbreak.

  • Outcome of Radical Surgical Treatment of Abdominal Aortic Graft and Endograft Infections Comparing Extra-anatomic Bypass with In Situ Reconstruction : A Nationwide Multicentre Study

    • Objective: Abdominal aortic graft and endograft infection (AGI) is primarily treated by resection of the infected graft and restoration of distal perfusion through extra-anatomic bypass (EAB) or in situ reconstruction/repair (ISR). The aim of this study was to compare these surgical strategies in a nationwide multicentre retrospective cohort study.Methods: The Swedish Vascular Registry (Swedvasc) was used to identify surgically treated abdominal AGIs in Sweden between January 1995 and May 2017. The primary aim was to compare short and long term survival, as well as complications for EAB and ISR.Results: Some 126 radically surgically treated AGI patients were identified – 102 graft infections and 24 endograft infections – treated by EAB: 71 and ISR: 55 (23 neo-aorto-iliac systems, NAISs). No differences in early 30 day (EAB 81.7% vs. ISR 76.4%, p =.46), or long term five year survival (48.2% vs. 49.9%, p =.87) were identified. There was no survival difference comparing NAIS to other ISR strategies. The frequency of recurrent graft infection during follow up was similar: EAB 20.3% vs. ISR 17.0% (p =.56). Survival and re-infection rates of the new conduit did not differ between NAIS and other ISR strategies. Age ≥ 75 years (odds ratio [OR] 4.0, confidence interval [CI] 1.1 – 14.8), coronary artery disease (OR 4.2, CI 1.2 – 15.1) and post-operative circulatory complications (OR 5.2, CI 1.2 – 22.5) were associated with early death. Prolonged antimicrobial therapy (> 3 months) was associated with reduced long term mortality (HR 0.3, CI 0.1 – 0.9).Conclusion: In this nationwide multicentre study comparing outcomes of radically treated AGI, no differences in survival or re-infection rate could be identified comparing EAB and ISR.

  • Semi-Conservative Treatment Versus Radical Surgery in Abdominal Aortic Graft and Endograft Infections

    • Objective: Abdominal aortic graft and endograft infections (AGIs) are rare complications following aortic surgery. Radical surgery (RS) with resection of the infected graft and reconstruction with extra-anatomical bypass or in situ reconstruction is the preferred therapy. For patients unfit for RS, a semi-conservative (SC), graft preserving strategy is possible. This paper aimed to compare survival and infection outcomes between RS and SC treatment for AGI in a nationwide cohort.Methods: Patients with abdominal AGI related surgery in Sweden between January 1995 and May 2017 were identified. The Management of Aortic Graft Infection Collaboration (MAGIC) criteria were used for the definition of AGI. Multivariable regression was performed to identify factors associated with mortality.Results: One hundred and sixty-nine patients with surgically treated abdominal AGI were identified, comprising 43 SC (14 endografts; 53% with a graft enteric fistula [GEF] in total) and 126 RS (26 endografts; 50% with a GEF in total). The SC cohort was older and had a higher frequency of cardiac comorbidities. There was a non-significant trend towards lower Kaplan -Meier estimated five year survival for SC vs. RS (30.2% vs. 48.4%; p = .066). A non-significant trend was identified towards worse Kaplan -Meier estimated five year survival for SC patients with a GEF vs. without a GEF (21.7% vs. 40.1%; p = .097). There were significantly more recurrent graft infections comparing SC with RS (45.4% vs. 19.3%; p < .001). In a Cox regression model adjusting for confounders, there was no difference in five year survival comparing SC vs. RS (HR 1.0, 95% CI 0.6 -1.5).Conclusion: In this national AGI cohort, there was no mortality difference comparing SC and RS for AGI when adjusting for comorbidities. Presence of GEF probably negatively impacts survival outcomes of SC patients. Rates of recurrent infection remain high for SC treated patients.

  • Överprövningar av offentliga upphandlingar : En intervjustudie om skillnader mellan LOU och LUF

    • Syftet med denna studie är att analysera möjliga förklaringar till att upphandlingarsom görs enligt Lagen (2007:1092) om offentlig upphandling inom områdenavatten, energi, transporter och posttjänster (LUF) tycks föranleda en mindre andelöverprövningar än upphandlingar som görs enligt Lagen (2007:1091) om offentligupphandling (LOU). Den senare lagstiftningen är tillämplig på offentliga upphandlingarav tjänster, varor och byggentreprenader som inte faller under de områdensom anges i LUF (se ovan).Tillgänglig statistik visar att överprövning är mer vanligt förekommande blandupphandlingar som regleras av LOU. Detta förhållande kvarstår med hänsyn tagentill att det är mer vanligt att upphandla enligt LOU än LUF. För att belysa möjligaförklaringar till detta har 28 intervjuer genomförts med beställare/upphandlare frånstatliga och kommunala organ samt med företrädare för leverantörer från olikabranscher.Utgångspunkterna för intervjuerna har varit frågor relaterade till upphandlingssituationen(t.ex. organisation och kompetens), juridiska faktorer (t.ex. förhandlingsmöjligheter)samt leverantörernas syn på upphandling (t.ex. överprövningsom affärsstrategi).Intervjuerna ger vid handen att en avgörande anledning till att upphandlingarenligt LUF överprövas i mindre utsträckning än upphandlingar enligt LOU är detmer omfattande förhandlingsutrymme som till skillnad från LOU finns i LUF. Frånupphandlarsidan menar de intervjuade att förhandling förebygger och ger möjlighetatt hantera missförstånd och felaktigheter under upphandlingens gång. Dettaanses minska risken för att en leverantör upplever sig felbehandlad eller upphandlingensom illa genomförd. En annan intressant iakttagelse är att de intervjuadeupphandlarna beskriver att de branscher som upphandlas enligt LOU i högreutsträckning präglas av en ”överprövningskultur”. Av de intervjuade nämner fleraatt LUF-upphandlingar ofta är mer komplexa och att myndigheter som upphandlarunder LUF därför håller sig med upphandlare med hög marknadsspecifikkompetens. En sådan lösning är något som skulle vara mer resurskrävande förkommunala organ, då de ofta förfogar över en i högre grad diversifieradupphandlingsportfölj.

  • An absolute risk model to identify individuals at elevated risk for pancreatic cancer in the general population.

    • PURPOSE: We developed an absolute risk model to identify individuals in the general population at elevated risk of pancreatic cancer.PATIENTS AND METHODS: Using data on 3,349 cases and 3,654 controls from the PanScan Consortium, we developed a relative risk model for men and women of European ancestry based on non-genetic and genetic risk factors for pancreatic cancer. We estimated absolute risks based on these relative risks and population incidence rates.RESULTS: Our risk model included current smoking (multivariable adjusted odds ratio (OR) and 95% confidence interval: 2.20 [1.84-2.62]), heavy alcohol use (>3 drinks/day) (OR: 1.45 [1.19-1.76]), obesity (body mass index >30 kg/m(2)) (OR: 1.26 [1.09-1.45]), diabetes >3 years (nested case-control OR: 1.57 [1.13-2.18], case-control OR: 1.80 [1.40-2.32]), family history of pancreatic cancer (OR: 1.60 [1.20-2.12]), non-O ABO genotype (AO vs. OO genotype) (OR: 1.23 [1.10-1.37]) to (BB vs. OO genotype) (OR 1.58 [0.97-2.59]), rs3790844(chr1q32.1) (OR: 1.29 [1.19-1.40]), rs401681(5p15.33) (OR: 1.18 [1.10-1.26]) and rs9543325(13q22.1) (OR: 1.27 [1.18-1.36]). The areas under the ROC curve for risk models including only non-genetic factors, only genetic factors, and both non-genetic and genetic factors were 58%, 57% and 61%, respectively. We estimate that fewer than 3/1,000 U.S. non-Hispanic whites have more than a 5% predicted lifetime absolute risk.CONCLUSION: Although absolute risk modeling using established risk factors may help to identify a group of individuals at higher than average risk of pancreatic cancer, the immediate clinical utility of our model is limited. However, a risk model can increase awareness of the various risk factors for pancreatic cancer, including modifiable behaviors.

    • Amyloid-β peptide (Aβ), a key substance in Alzheimers disease (AD), is characterized by its abnormal folding into neurotoxic aggregates. Since Aβ comprises an extracellular and transmembrane domain, some of its neurotoxic actions might be exerted via interactions with neuronal membranes. Wideline and magic angle spinning 14N and 31P NMR have been used in combination with differential scanning calorimetry and circular dichroism spectroscopy to investigate the association between Aβ1–40 peptide and membranes with different electrostatic surface potentials. Calorimetric measurements showed that all membrane systems were in the liquid crystalline state at 308 K. Binding of Aβ1–40 at a 30 1 lipid/peptide ratio to membranes composed of neutral dimyristoyl-phosphatidylcholine (DMPC) and negatively charged dimyristoylphosphatidylglycerol (DMPG) at a 4 : 1 molar ratio is mainly driven electrostatically, reflected in characteristic changes of the isotropic 31P chemical shift values for both lipids. In addition, the average orientation of the choline headgroup of DMPC, with its electric P––N+(CH3)3 dipole, changed directly in response to the reduced negative membrane surface potential. The deviation in tilt angle of the PN vector relative to the membrane surface is manifested in the observed 14N NMR quadrupole splitting and can therefore be described semiquantitatively. Adding Aβ1–40 to membranes with nominal neutral surface charge, but composed of a ternary mixture of DMPC with DMPG and the cationic amphiphile didodecyldimethyl–ammonium bromide (DDAB) at a 3 : 1 : 1 molar ratio revealed surprisingly electrostatic interactions visible in the NMR spectra. Since Aβ1–40 does not bind to neutral DMPC bilayers a model is proposed, in which on a molecular level the charged residues of Aβ1–40 peptide can interact independently with lipid headgroups of various charges in these microscopically heterogeneous systems.

  • The Human Biting Culex pipiens Bioform molestus Detected in Several Areas in Southern Sweden

    • Background: The mosquito species Culex pipiens is a known vector of several pathogens and occurs in two distinct bioforms, pipiens and molestus. The bioform molestus thrives in urban environments where there are below-ground habitats; it can mate in confined spaces and feed on mammals as well as birds. In contrast, the bioform pipiens is found above ground, is thought to require more space for mating, and mainly feeds on birds. The pipiens bioform is present in large parts of Sweden but the molestus bioform has previously only been found in major cities.Materials and Methods: People experiencing mosquito nuisance in southern Sweden submitted mosquito samples as part of a citizen science project, and these samples were analyzed to determine the geographical distribution of the molestus bioform of Cx. pipiens. Mosquito specimens were identified to the species level by DNA barcoding of the cytochrome C oxidase subunit I (COI) gene, and the bioforms were determined through the CQ11 microsatellite marker.Results:Culex pipiens f molestus was observed to be spread across large parts of Gothenburg as well as in the suburbs. This bioform was found both in urban and rural areas at several sites across southern Sweden. In one site, hybrids between the two bioforms were found.Conclusions: The detection of Cx. pipiens f molestus in several rural areas was surprising, indicating that it may be more widely spread than urban areas alone, where it has been previously reported.

  • Identification of candidate cerebrospinal fluid biomarkers in parkinsonism using quantitative proteomics

    • Introduction: Neurodegenerative parkinsonian syndromes have significant clinical and pathological overlap, making early diagnosis difficult. Cerebrospinal fluid (CSF) biomarkers may aid the differentiation of these disorders, but other than a-synuclein and neurofilament light chain protein, which have limited diagnostic power, specific protein biomarkers remain elusive. Objectives: To study disease mechanisms and identify possible CSF diagnostic biomarkers through discovery proteomics, which discriminate parkinsonian syndromes from healthy controls. Methods: CSF was collected consecutively from 134 participants; Parkinson's disease (n = 26), atypical parkinsonian syndromes (n = 78, including progressive supranuclear palsy (n = 36), multiple system atrophy (n = 28), corticobasal syndrome (n = 14)), and elderly healthy controls (n = 30). Participants were divided into a discovery and a validation set for analysis. The samples were subjected to tryptic digestion, followed by liquid chromatography-mass spectrometry analysis for identification and relative quantification by isobaric labelling. Candidate protein biomarkers were identified based on the relative abundances of the identified tryptic peptides. Their predictive performance was evaluated by analysis of the validation set. Results: 79 tryptic peptides, derived from 26 proteins were found to differ significantly between atypical parkinsonism patients and controls. They included acute phase/inflammatory markers and neuronal/synaptic markers, which were respectively increased or decreased in atypical parkinsonism, while their levels in PD subjects were intermediate between controls and atypical parkinsonism. Conclusion: Using an unbiased proteomic approach, proteins were identified that were able to differentiate atypical parkinsonian syndrome patients from healthy controls. Our study indicates that markers that may reflect neuronal function and/or plasticity, such as the amyloid precursor protein, and inflammatory markers may hold future promise as candidate biomarkers in parkinsonism.   • Ideologins apparatur : reproduktionsperspektiv på kyrka och skola i 1600- och 1700-talens Sverige

    • The present dissertation examines various aspects of church organization in Sweden during the 17th and 18th centuries. In Sweden, as in many other European countries, the Christian religion and the organization of the Church comprised the dominant ideological power structure. Through the process of state-building and the Protestant Reformation, these structures were charged with new energy. In the present dissertation, the fact that nations were dependent on and exploited ideological power structures in order to control their respective populations and survive the international struggle of the state system has grown to become a central theme.The fundamental questions of this dissertation concerning how and why social institutions were reproduced, and the theoretical context of these questions, has been based in the fact that both church and state are social institutions whose construction must include structures for a constant recreation of their forms and functions. The contribution of the present author has been to construct and apply a general model for understanding the organization and reproduction of institutional structures from a materialistic and structural basis. From a discussion of Louis Althusser's theory of the oppressive state apparatus and the ideological state apparatus, I have developed fundamental concepts with the purpose of constructing a theoretical model of the reproduction of an ideological state apparatus. I have dubbed this model the "competitive system", designating a system of material and symbolic structures and processes which, through competition between individuals, produces and maintains the functionaries of said state apparatus.In one subsidiary study, the involvement of the repressive state apparatus in the creation of new church dioceses (so-called "superintendant dioceses") in Sweden during the first half of the 17th century is studied. This study showed that the creation of superintendant dioceses was a government-initiated project whose actual motive was the ambition of the repressive state to conquer and secure borders and central regions in the realm.In another subsidiary study, some of the structures which reproduced the church and the educational system in Sweden during the 17th and 18th centuries are examined. This study indicated how the system of promotion constructed after the Reformation resulted in an overproduction of clerical labour during the 17th and 18th centuries. At the same time, through the agency of the crown the educational system expanded enormously during the 17th century and, by providing the opportunity for teachers to move into careers in the church, competition in the church apparatus became even keener.In a final subsidiary study, the reproduction of diocesans (bishops and superintendents) in Sweden during the period 1580-1800 is analyzed. At the beginning of the period under investigation, careers in the normal competitive system of the church comprised the dominant form of qualification. The court and royal chaplain ministries available there were of great significance for recruitment. By the latter half of the 17th century, the role of the educational system in the reproduction of diocesans increased. In accordance with the Swedish central government´s increasing need to formalize its central competitive system, the educational system was expanded and reinforced, which had reprecussions on the competitive system of the ideological state apparatus of the church and its reproduction of diocesans.Keywords: ideological state apparatus, repressive state apparatus, reproduction, church organization, educational system, competitive system, clergy, superintendent, bishop, superintendant dioceses, state building, 17th century, 18th century, Sweden.
