    • The chemical composition of marabou (Dichrostachys cinerea) wood and its treatment with acetic acid were investigated. Two different treatment approaches, direct acetosolv and combined acid prehydrolysis/acetosolv, were evaluated. The effects of acetic acid concentration (50%, 70% and 90%) and temperature (normal boiling temperature and 121°C) on yield of solids, solubilization of lignin and hemicelluloses and recovery of cellulose were evaluated for both treatments. High solubilization of marabou components was observed in the direct acetosolv treatment at 121°C, especially at the highest acetic acid concentration, where around 84.8% of lignin and 78% of hemicelluloses were removed. When the material was subjected to acid prehydrolysis prior to acetosolv treatment, lignin solubilization was improved, especially at low acetic acid concentrations. Above 80% of the solubilized lignin was recovered from the liquors in the direct acetosolv treatment, but the recovery was lower in the combined treatment. Cellulose was well preserved in all the treatment schemes.

  • Fractionation of the main components of barley spent grains from a microbrewery

    • The chemical composition of barley spent grains generated in a microbrewery and their fractionation by acid hydrolysis and delignification were investigated. The material contained high amount of carbohydrates (60%), while its lignin content was lower than that reported for other sorts of barley spent grains. Different dilute-acid hydrolysis methods were evaluated for separating the main components of the spent grains, without affecting the sugars generated by starch hydrolysis. The utilization of a two-step dilute-acid hydrolysis approach allowed to hydrolyse starch in a first step, at 100 ºC, and hemicelluloses in a second step, at 121 ºC. Acetosolv and alkaline delignification were used for solubilising the lignin fraction. A higher lignin solubilisation (95% of the lignin contained in the raw material) was achieved after alkaline delignification, whereas only 34% of the initial lignin was removed by direct acetosolv. When the acetosolv treatment was combined with acid hydrolysis, lignin solubilisation increased to 74%. Lignin was precipitated from the liquors at recovery rates from 40 to 93%, as depending on the hydrolysis/delignification method used.

    • Jatropha curcas L. is a tropical plant with considerable potential for producing biodiesel and other products of high economic and social interest. During the biodiesel production process from J. curcas different residues, such as shells and husks are generated. In this work, the physical characterization of J. curcas fruits was performed, and the chemical composition of a mixture of shells and husks was determined. The physical characterization revealed that shells and husks account, respectively, for 25.0 and 27.8% of the fruit weight. The compositional analyses of the material showed a rather high content of glucans (32.8% w/w) and xylans (16.4% w/w), which indicates the potential of J. curcas shells and husks for production of ethanol, xylitol and other glucose- and xylose-derived products. Acid hydrolysis was applied to a mixture of shells and husks under different sulphuric acid concentrations (from 0.5 to 4.5%), temperatures (170 – 220ºC) and time (10 – 20 min), and the hydrolytic conversion of xylan was evaluated. A zone of experimental conditions giving maximal xylan conversion was identified at around 4% H2SO4, 180ºC and reaction time below 10 min.

  • Process mining for healthcare : Characteristics and challenges

    • Process mining techniques can be used to analyse business processes using the data logged during their execution. These techniques are leveraged in a wide range of domains, including healthcare, where it focuses mainly on the analysis of diagnostic, treatment, and organisational processes. Despite the huge amount of data generated in hospitals by staff and machinery involved in healthcare processes, there is no evidence of a systematic uptake of process mining beyond targeted case studies in a research context. When developing and using process mining in healthcare, distinguishing characteristics of healthcare processes such as their variability and patient-centred focus require targeted attention. Against this background, the Process-Oriented Data Science in Healthcare Alliance has been established to propagate the research and application of techniques targeting the data-driven improvement of healthcare processes. This paper, an initiative of the alliance, presents the distinguishing characteristics of the healthcare domain that need to be considered to successfully use process mining, as well as open challenges that need to be addressed by the community in the future.

  • Low adherence to legislation regarding Do-Not-Attempt-Cardiopulmonary-Resuscitation orders in a Swedish University Hospital

    • Background The ethical principles of resuscitation have been incorporated into Swedish legislation so that a decision to not attempt cardiopulmonary resuscitation (DNACPR) entails (1) consultation with patient or relatives if consultation with patient was not possible and documentation of their attitudes; (2) consultation with other licensed caregivers; (3) documentation of the grounds for the DNACPR. Our aim was to evaluate adherence to this legislation, explore the grounds for the decision and the attitudes of patients and relatives towards DNACPR orders. Methods We included DNACPR forms issued after admission through the emergency department at Karolinska University Hospital between 1st January and 31st October, 2015. Quantitative analysis evaluated adherence to legislation and qualitative analysis of a random sample of 20% evaluated the grounds for the decision and the attitudes. Results The cohort consisted of 3583 DNACPR forms. In 40% of these it was impossible to consult the patient, and relatives were consulted in 46% of these cases. For competent patients, consultation occurred in 28% and the most common attitude was to wish to refrain from resuscitation. Relatives were consulted in 26% and they mainly agreed with the decision. Grounds for the DNAR decision was most commonly severe chronic comorbidity, malignancy or multimorbidity with or without an acute condition. All requirements of the legislation were fulfilled in 10% of the cases. Conclusion In 90% of the cases physicians failed to fulfil all requirements in the Swedish legislation regarding DNAR orders. The decision was mostly based on chronic, severe comorbidity or multimorbidity.

  • Association between use of pre-hospital ECG and 30-day mortality: A large cohort study of patients experiencing chest pain

    • Background: In the assessment of patients with chest pain, there is support for the use of pre-hospital ECG in the literature and in the care guidelines. Using propensity score methods, we aim to examine whether the mere acquisition of a pre-hospital ECG among patients with chest pain affects the outcome (30-day mortality). Methods: The association between pre-hospital ECG and 30-day mortality was studied in the overall cohort (n=13151), as well as in the one-to-one matched cohort with 2524 patients not examined with pre-hospital ECG and 2524 patients examined with pre-hospital ECG. Results: In the overall cohort, 21% (n=2809) did not undergo an ECG tracing in the pre-hospital setting. Among those who had pain during transport, 14% (n=1159) did not undergo a pre-hospital ECG while 32% (n=1135) of those who did not have pain underwent an ECG tracing. In the overall cohort, the OR for 30-day mortality in patients who had a pre-hospital ECG, as compared with those who did not, was 0.63 (95% CI 0.05-0.79; p < 0.001). In the matched cohort, the OR was 0.65 (95% CI 0.49-0.85; p < 0.001). Using the propensity score, in the overall cohort, the corresponding HR was 0.65 (95% CI 0.58-0.74). Conclusion: Using propensity score methods, we provide real-world data demonstrating that the adjusted risk of death was considerably lower among the cases in whoma pre-hospital ECG was used. The PH-ECG is underused among patients with chest discomfort and the mere acquisition of a pre-hospital ECG may reduce mortality. (C) 2017 Elsevier B.V. All rights reserved.
