  • Use of combined hormonal contraceptives among women with systemic lupus erythematosus with and without medical contraindications to oestrogen

    • Objectives. To assess the prevalence of combined hormonal contraceptives (CHCs) in reproductive-age women with SLE with and without possible contraindications and to determine factors associated with their use in the presence of possible contraindications. Methods. This observational cohort study included premenopausal women ages 18-45 years enrolled in the SLICC Registry ≤15 months after SLE onset, with annual assessments spanning 2000-2017. World Health Organization Category 3 or 4 contraindications to CHCs (e.g. hypertension, aPL) were assessed at each study visit. High disease activity (SLEDAI score >12 or use of >0.5 mg/kg/day of prednisone) was considered a relative contraindication. Results. A total of 927 SLE women contributed 6315 visits, of which 3811 (60%) occurred in the presence of one or more possible contraindication to CHCs. Women used CHCs during 512 (8%) visits, of which 281 (55%) took place in the setting of one or more possible contraindication. The most frequently observed contraindications were aPL (52%), hypertension (34%) and migraine with aura (22%). Women with one or more contraindication were slightly less likely to be taking CHCs [7% of visits (95% CI 7, 8)] than women with no contraindications [9% (95% CI 8, 10)]. Conclusion. CHC use was low compared with general population estimates (>35%) and more than half of CHC users had at least one possible contraindication. Many yet unmeasured factors, including patient preferences, may have contributed to these observations. Further work should also aim to clarify outcomes associated with this exposure.

  • Components in evaluation of complex interior truck sounds

    • In order to investigate influences on assessment of interior truck sounds two experiments have been carried out with emphasis on the low frequency content. In the first experiment the participants were presented with recorded sound from the interior of a truck cabin, with different alterations of the low frequency content. In the second experiment the balance between low and high frequency content was altered for the recorded sounds. Three main dimensions important for the assessment of truck sounds were found: an Annoyance factor, a Wakefulness factor and a Quality factor. Additionally it was found that a wellsuited spectral balance could improve positive impressions of truck sound.

    • A typical problem in low-level liquid scintillation (LS) counting is the identification of alpha particles in the presence of a high background of beta and gamma particles. Especially the occurrence of beta-beta and beta-gamma pile-ups may prevent the unambiguous identification of an alpha signal by commonly used analog electronics. In this case, pulse-shape discrimination (PSD) and pile-up rejection (PUR) units show an insufficient performance. This problem was also observed in own earlier experiments on the chemical behaviour of transactinide elements using the liquid-liquid extraction system SISAK in combination with LS counting alpha-particle signals from the decay of the transactinides could not be unambiguously assigned. However, the availability of instruments for the digital recording of LS pulses changes the situation and provides possibilities for new approaches in the treatment of LS pulse shapes. In a SISAK experiment performed at PSI, Villigen, a fast transient recorder, a PC card with oscilloscope characteristics and a sampling rate of 1 giga samples s(-1) (1 ns per point), was used for the first time to record LS signals. It turned out, that the recorded signals were predominantly alpha, beta-beta and beta-gamma pile up, and fission events. This paper describes the subsequent development and use of artificial neural networks (ANN) based on the method of "back-propagation of errors" to automatically distinguish between different pulse shapes. Such networks can "learn" pulse shapes and classify hitherto unknown pulses correctly after a learning period. The results show that ANN in combination with fast digital recording of pulse shapes can be a powerful tool in LS spectrometry even at high background count rates.

  • Preinvasion Assessment of Exotic Bark Beetle-Vectored Fungi to Detect Tree-Killing Pathogens

    • Exotic diseases and pests of trees have caused continental-scale disturbances in forest ecosystems and industries, and their invasions are considered largely unpredictable. We tested the concept of preinvasion assessment of not yet invasive organisms, which enables empirical risk assessment of potential invasion and impact. Our example assesses fungi associated with Old World bark and ambrosia beetles and their potential to impact North American trees. We selected 55 Asian and European scolytine beetle species using host use, economic, and regulatory criteria. We isolated 111 of their most consistent fungal associates and tested their effect on four important southeastern American pine and oak species. Our test dataset found no highly virulent pathogens that should be classified as an imminent threat. Twenty-two fungal species were minor pathogens, which may require context-dependent response for their vectors at North American borders, while most of the tested fungi displayed no significant impact. Our results are significant in three ways; they ease the concerns over multiple overseas fungus vectors suspected of heightened potential risk, they provide a basis for the focus on the prevention of introduction and establishment of species that may be of consequence, and they demonstrate that preinvasion assessment, if scaled up, can support practical risk assessment of exotic pathogens.

    • The purpose of this study was to describe quality-of-life aspects in a group of patients (n = 99) suffering from peripheral vestibular disorder, using three different self-rated instruments, the Vertigo Symptom Scale (VSS), the Vertigo Handicap Questionnaire (VHQ) and the Sickness Impact Profile (SIP). The results showed that the type of dizziness that most influenced the quality-of-life aspects were: frequent short- or long-term dizziness, nausea, and the feeling that the ground was distant or as though the patient were walking on clouds. However, several of the impairments in daily life were neither related to the disease itself nor the demographic data. This verifies the necessity of investigating other factors such as personality and coping capacity. The results of this study also demonstrate the patients' need of psychosocial support. A comprehensive assessment and evaluation is important in order to identify each patient's needs.
