    • Hypertension and proteinuria are commonly observed side-effects for anti-angiogenic drugs targeting the VEGF pathway. In most cases, hypertension can be controlled by prescription of anti-hypertensive (AH) therapy, while proteinuria often requires dose reductions or dose delays. We aimed to construct a pharmacokinetic-pharmacodynamic (PK-PD) model for hypertension and proteinuria following treatment with the experimental VEGF-inhibitor E7080, which would allow optimization of treatment, by assessing the influence of anti-hypertensive medication and dose reduction or dose delays in treating and avoiding toxicity. Data was collected from a phase I study of E7080 (n = 67), an inhibitor of multiple tyrosine kinases, among which VEGF. Blood pressure and urinalysis data were recorded weekly. Modeling was performed in NONMEM, and direct and indirect response PK-PD models were evaluated. A previously developed PK model was used. An indirect response PK-PD model described the increase in BP best, while the probability of developing proteinuria toxicity in response to exposure to E7080, was best described by a Markov transition model. This model may guide clinical interventions and provide treatment recommendations for E7080, and may serve as a template model for other drugs in this class.

    • Recently Minor and co-workers described patients with sound- and pressure-induced vertigo due to dehiscence of the superior semicircular canal. Identifying patients with this 'new' vestibular entity is important, not only because the symptoms are sometimes very incapacitating, but also because they can be treated. We present symptoms and findings in eight such patients, all of whom reported pressure-induced vertigo that increased during periods of upper respiratory infections. Pulse-synchronous tinnitus and gaze instability during head movements were also common complaints. All patients lateralized Weber's test to the symptomatic ear. In some of the patients the audiogram also revealed a small conductive hearing loss. However, the stapedius reflexes were always normal. A vertical/torsional eye movement related to the superior semicircular canal was seen in most of the patients in response to pressure changes and/or sound stimulation. One patient also had superior canal-related positioning nystagmus. Testing vestibular evoked myogenic potentials revealed in all patients a vestibular hypersensitivity to sounds. In the coronal high-resolution 1-mm section CT scans the dehiscence was visible on 1 to 4 sections. Moreover, the skull base was rather thin in this area and cortical bone separating the middle ear and the antrum from the middle cranial fossa was absent in many of the patients. Two of the patients have undergone plugging of the superior semicircular canal using a transmastoid approach and both patients were relieved of the pressure-induced symptoms.

    • A typical problem in low-level liquid scintillation (LS) counting is the identification of alpha particles in the presence of a high background of beta and gamma particles. Especially the occurrence of beta-beta and beta-gamma pile-ups may prevent the unambiguous identification of an alpha signal by commonly used analog electronics. In this case, pulse-shape discrimination (PSD) and pile-up rejection (PUR) units show an insufficient performance. This problem was also observed in own earlier experiments on the chemical behaviour of transactinide elements using the liquid-liquid extraction system SISAK in combination with LS counting alpha-particle signals from the decay of the transactinides could not be unambiguously assigned. However, the availability of instruments for the digital recording of LS pulses changes the situation and provides possibilities for new approaches in the treatment of LS pulse shapes. In a SISAK experiment performed at PSI, Villigen, a fast transient recorder, a PC card with oscilloscope characteristics and a sampling rate of 1 giga samples s(-1) (1 ns per point), was used for the first time to record LS signals. It turned out, that the recorded signals were predominantly alpha, beta-beta and beta-gamma pile up, and fission events. This paper describes the subsequent development and use of artificial neural networks (ANN) based on the method of "back-propagation of errors" to automatically distinguish between different pulse shapes. Such networks can "learn" pulse shapes and classify hitherto unknown pulses correctly after a learning period. The results show that ANN in combination with fast digital recording of pulse shapes can be a powerful tool in LS spectrometry even at high background count rates.

  • Living with dizziness : an explorative study

    • A qualitative study was conducted for the purpose of exploring how patients with dizziness manage their daily living. Ten patients with history of dizziness were recruited from the out-patient clinic, Department of Audiology, in a hospital in Stockholm. Collection and analysis of data were inspired by the method of grounded theory. Four themes were developed that described the experience-of living with dizziness: vulnerable reactions, affirmation and non-affirmation, finding ways to carry on daily living, and expressing the need for health care support. These themes seem to conceptualize these patients' difficulties and needs in relationship to living with dizziness and, given the small sample, have implications for nursing practice and provide a framework for a broader study.
