  • Prevention of Cardiovascular Disease and Cancer Mortality by Achieving Healthy Dietary Goals for the Swedish Population : A Macro-Simulation Modelling Study

    • The objective is to estimate the number of deaths attributable to cardiovascular diseases and diet-related cancers that could be prevented or delayed in Sweden if adults adhere to the official dietary recommendations. We used an age-group and sex-specific epidemiological macro-simulation model to estimate preventable deaths due to the discrepancies between actual intake and recommended intake of changes in food components. Data included in the model are a baseline scenario (actual dietary intake), a counterfactual scenario (recommended intake) and age- and sex-specific mortality for cardiovascular and diet-related cancer diseases together compared with the total population risk of a specific year. Monte Carlo analyses with 5000 iterations was performed to produce the 95% uncertainty intervals (UI). The model predicts that 6405 (95% UI: 5086-7086) deaths could be prevented or delayed if the Swedish population could adhere to official dietary recommendations in a year. More deaths would be saved for men than women. The recommendations for fruits and vegetables could have saved 47% of the deaths, followed by fiber intake (32%). For men, fruits and vegetables could have saved more compared to other dietary components, while for women dietary fiber was the prominent factor. Public health policies should consider ensuring healthy eating practices for the Swedish population.

    • Hur är det att vara barn och växa upp i en religiös minoritetsgrupp som Hare Krishna, Scientologi-kyrkan, Jehovas Vittnen, Knutby Filadelfia, Plymouth-bröderna, Enighetskyrkan eller Guds Barn/Familjen? Boken bygger framför allt på ett 70-tal intervjuer med såväl vuxna som barn som vuxit upp i dessa grupper, samt på intervjuer med ett 20-tal föräldrar, men också på fältobservationer och textstudier av gruppernas material kring barn och barnuppfostran. Livsvillkoren för barnen kan vara mycket olika, bland annat för att de olika religiösa grupperna har olika ideologier och syn på barnuppfostran, men också på individuella omständigheter inom olika familjer, fas i gruppens utveckling samt samhällets gensvar och reaktioner på gruppen ifråga. Boken diskuterar teman som socialisation, identitet, avhopp och skolgång, men tar också upp farhågor kring dessa grupper som auktoritär uppfostran, aga, isolering från samhället, separationer mellan föräldrar och barn, och bristande omsorg vad gäller mat och hälsa.   • Obesity is associated with coronary artery stenosis independently of metabolic risk factors : the population-based SCAPIS study

    • Background and aims: Previous studies reported divergent results on whether metabolically healthy obesity is associated with increased coronary artery calcium and carotid plaques. We investigated this in a cross-sectional fashion in a large, well-defined, middle-aged population using coronary CT angiography (CCTA) and carotid ultrasound. Methods: In the SCAPIS study (50–65 years, 51% female), CCTA and carotid artery ultrasound were performed in 23,674 individuals without clinical atherosclerotic disease. These subjects were divided into six groups according to BMI (normal weight, overweight, obese) and the presence of metabolic syndrome (MetS) according to the NCEP consensus criteria. Results: The severity of coronary artery stenosis was increased in individuals with obesity without MetS compared to normal-weight individuals without MetS (OR 1.47, 95%CI 1.34–1.62; p < 0.0001), even after adjusting for non-HDL-cholesterol and several lifestyle factors. Such difference was not observed for the presence of carotid artery plaques (OR 0.94, 95%CI 0.87–1.02; p = 0.11). Obese or overweight individuals without any MetS criteria (except the waist criterion) showed significantly more pronounced stenosis in the coronary arteries as compared to the normal-weight individuals, while one criterion was needed to show increased plaque prevalence in the carotid arteries. High blood pressure was the most important single criterion for increased atherosclerosis in this respect. Conclusions: Individuals with obesity without MetS showed increased severity of coronary artery stenosis, but no increased occurrence of carotid artery plaques compared to normal-weight individuals without MetS, further emphasizing that obesity is not a benign condition even in the absence of MetS.

  • Pregnancy-associated plasma protein A and mortality in hemodialysis

    • BACKGROUND: Elevated pregnancy-associated plasma protein A (PAPP-A) levels are associated with increased risk of death in ischemic heart disease as well as in hemodialysis patients. Previous research indicates that the prognostic value of PAPP-A may be stronger in patients with concomitant diabetes mellitus or signs of inflammation. We studied the association between PAPP-A and outcomes in prevalent hemodialysis patients and hypothesized that diabetes mellitus and inflammation status act as effect modifiers.MATERIALS AND METHODS: Circulating PAPP-A levels were quantified using ELISA. Cox proportional hazards- and quantile regression models were used for associations between PAPP-A and mortality. PAPP-A levels were log-transformed for Normality.RESULTS: During 60 months follow-up, 37 (40%) of the 92 participants died. Higher PAPP-A was associated with increased risk of mortality in unadjusted analysis (HR per SD = 1.4, 95% CI = 1 - 1.9, p = 0.03) and when adjusted for confounders and cardiovascular risk factors (HR = 1.8, 95% CI = 1.18-2.73, p = 0.006). An interaction between PAPP-A levels and diabetes mellitus on mortality was found (HR for the multiplicative interaction term = 2.74 95% CI = 1.02-7.37, p = 0.05). In a quantile regression adjusted for age and sex, one SD increase of PAPP-A was associated with 22 months shorter estimated time until 25% of the patients died (95% CI -35 to -9.1 months).CONCLUSIONS: Increased PAPP-A levels are associated with higher all-cause mortality in prevalent hemodialysis patients with concomitant diabetes mellitus.

  • Are empathy and compassion bad for the professional social worker?

    • Recent studies have shown that social workers and other professional helpers who work with traumatized individuals run a risk of developing compassion fatigue or secondary traumatic stress. Some researchers have hypothesized that helpers do this as a result of feeling too much empathy or too much compassion for their clients, thereby implying that empathy and compassion may be bad for the professional social worker. This paper investigates these hypotheses. Based on a review of current research about empathy and compassion it is argued that these states are not the causes of compassion fatigue. Hence, it is argued that empathy and compassion are not bad for the professional social worker in the sense that too much of one or the other will lead to compassion fatigue.

  • Butler's stone and ultimate psychological hedonism

    • This paper discusses psychological hedonism with special reference to the writings of Bishop Butler, and Elliott Sober and David Sloan Wilson. Contrary to philosophical orthodoxy, Sober and Wilson have argued that Butler failed to refute psychological hedonism. In this paper it is argued: (1) that there is a difference between reductive and ultimate psychological hedonism; (2) that Butler failed to refute ultimate psychological hedonism, but that he succeeded in refuting reductive psychological hedonism; and, finally and more importantly, (3) that Butler's criticism of reductive hedonism can be used as a stepping-stone in another argument showing the implausibility of ultimate psychological hedonism as well.

    • What is the proper place of compassion within social work? Should social workers feel compassion for their clients, or is this something that should be avoided? Opinions differ. While some have claimed that compassion should be avoided in favor of a more detached and professional approach, others argue that compassion is an important, and unavoidable part, of a proper working-alliance. While some claim that compassion can be beneficial for the client, recent studies have suggested that too much compassion can lead to so-called compassion fatigue and be detrimental for the professional. If we want to assess these claims, we must first consider what compassion is. That is the purpose of this study. The aim is to gain a better understanding of what compassion is, with a special eye to the question as to whether compassion is a resource or threat within professional social work. Methods include conceptual analysis and phenomenological descriptions of compassion and related phenomena. The results can be summarized in two main claims. The first is that compassion is a particular kind of suffering directed towards the suffering of someone. What is special about compassion is that it is a suffering for another person’s sake: When you feel compassion (and only compassion) for another person, then you suffer over her suffering for her sake and not your own sake. The second claim is that compassion in and of itself is not harmful to the person feeling it. Although compassion consists in suffering, since it is a suffering for another person’s sake, a person feeling compassion is not, and does not take herself to be, the victim of something bad. It is an essential feature of compassion that if you feel compassion (and only compassion) for another person, then you take this person to be the victim of a harm but you do not take yourself to be so. In conclusion it is argued that these results point towards a more positive view of compassion and its place within social work.

  • Empathy and emotions : on the notion of empathy as emotional sharing

    • The topic of this study is a notion of empathy that is common in philosophy and in the behavioral sciences. It is here referred to as ‘the notion of empathy as emotional sharing’, and it is characterized in terms of three ideas. If a person, S, has empathy with respect to an emotion of another person, O, then (i) S experiences an emotion that is similar to an emotion that O is currently having, (ii) S’s emotion is caused, in a particular way, by the state of O or by S’s entertaining an idea of the state or situation of O, and (iii) S experiences this emotion in a way that does not entail that S is in the corresponding emotional state. The aim of the study is to clarify this notion of empathy by clarifying these three ideas and by tracing the history of their development in philosophy.The study consists of two parts. Part one contains a short and selective account of the history in Western philosophy of the notion of empathy as emotional sharing. In chapter 2 Spinoza’s theory of imitation of affects and Hume’s theory of sympathy are presented. It is argued that these theories only exemplify the second idea characteristic of the notion of empathy as emotional sharing. Chapter 3 contains presentations of Adam Smith’s theory of sympathy, and Schopenhauer’s theory of compassion. These theories are shown to exemplify the second and the third idea. In chapter 4 there are presentations of Edith Stein’s description of Einfühlung, and Max Scheler’s account of empathy and fellow-feeling. It is shown that these accounts contain explicit specifications of the third idea, and it is argued that they also exemplify the second idea.In part two, the three ideas are further clarified and the notion of empathy as emotional sharing is defined. Chapter 5 contains a discussion of the main contemporary philosophical analyses of empathy. Three different views are distinguished: one that construes empathetic emotions as emotional states, one that construes them as imagined emotions, and one that construes them as off-line emotions. The first two views are criticized and rejected. The third is accepted and further developed in chapter 6, which contains a general analysis of the emotions. A distinction is made between two ways of experiencing an emotion, and it is argued that it is possible to have the affective experience characteristic of a particular kind of emotional state without being in that kind of state. In chapter 7, a definition of ‘empathy’ is proposed. This definition contains specifications of the three ideas characteristic of the notion of empathy as emotional sharing, and it shows both how the empathizer’s emotion resembles the emotion of the empathee, and how this emotion is caused and experienced.
