  • Genetic associations at 53 loci highlight cell types and biological pathways relevant for kidney function

    • Reduced glomerular filtration rate defines chronic kidney disease and is associated with cardiovascular and all-cause mortality. We conducted a meta-analysis of genome-wide association studies for estimated glomerular filtration rate (eGFR), combining data across 133,413 individuals with replication in up to 42,166 individuals. We identify 24 new and confirm 29 previously identified loci. Of these 53 loci, 19 associate with eGFR among individuals with diabetes. Using bioinformatics, we show that identified genes at eGFR loci are enriched for expression in kidney tissues and in pathways relevant for kidney development and transmembrane transporter activity, kidney structure, and regulation of glucose metabolism. Chromatin state mapping and DNase I hypersensitivity analyses across adult tissues demonstrate preferential mapping of associated variants to regulatory regions in kidney but not extra-renal tissues. These findings suggest that genetic determinants of eGFR are mediated largely through direct effects within the kidney and highlight important cell types and biological pathways.

  • Dementia prevention in memory clinics: recommendations from the European task force for brain health services

    • Observational population studies indicate that prevention of dementia and cognitive decline is being accomplished, possibly as an unintended result of better vascular prevention and healthier lifestyles. Population aging in the coming decades requires deliberate efforts to further decrease its prevalence and societal burden. Increasing evidence sup-ports the efficacy of preventive interventions on persons with intact cognition and high dementia risk. We report recommendations for the deployment of second-generation memory clinics (Brain Health Services) whose mission is evidence-based and ethical dementia prevention in at-risk individuals. The cornerstone interventions consist of (i) assessment of genetic and potentially modifiable risk factors including brain pathology, and risk stratification, (ii) risk communication with ad-hoc protocols, (iii) risk reduction with multi-domain interventions, and (iv) cognitive enhancement with cognitive and physical training. A roadmap is proposed for concept validation and ensuing clinical deployment.

  • Biomechanical characterization of osseointegration during healing: an experimental in vivo study in the rat.

    • This study reports torsion tests and pull-out tests on osseointegrated commercially pure titanium fixtures. The tests were performed in vivo on a total of 26 rats. Three fixtures with a diameter of 2.0 mm were installed bilaterally in the proximal tibia in each animal. The mechanical testing was performed immediately after installation, after 2, 4, 8 and 16 weeks of unloaded healing. The torsional strength started to increase after 4 weeks of unloaded healing and there was a significant increase with time during the initial 16 weeks. The pull-out load increased rapidly during the first 4 weeks; thereafter, a moderate increase occurred during the following 12 weeks. A histological evaluation was performed after 0, 4, 8 and 16 weeks. There were significant (P < 0.01) correlations between torque and percentage of bone in contact with the fixture, and between pull-out load and the bone thickness around the fixture (P < 0.001). Estimations of shear stresses and shear moduli in the bone tissue (pull-out test) and at the interface (torque test) indicated that the increase in bone volume around the implant substantially improved the mechanical capacity.

    • Obemannade farkoster används allt oftare, och i allt fler roller, i dagens kon- flikter. Denna rapport ger en bred överblick över området militära obemannade farkoster, samt rekommendationer för inriktningen av framtida FoU-satsningar inom området.Överblicken över området har fokus på både system, förmågor och verksam- heter som är relevanta för Försvarsmakten. Genom att låta de insatsförmågor som definieras i FMUP (Försvarsmaktens utvecklingsplan) gå som en röd tråd genom rapporten, både när specifika system diskuteras och när möjliga scena- rier där obemannade farkoster kan vara till nytta beskrivs, har vi försökt hålla både bredd och relevans i dokumentet.Rekommendationerna vilar på en genomgång av de inriktningsdokument som producerats i Försvarsmakten, t.ex. Perspektivplanneringen och FMUP, besök vid de enheter som dagligen använder obemannade farkoster, UAV-enheten i Karlsborg och Swedec i Eksjö, samt den områdesöverblick som nämns ovan. Slutsatserna är att den effektivaste kompetensuppbyggnaden och kunskapsöver- föringen fås om man skapar breda tvärvetenskapliga projekt inom respektive systemkategori (UAV, UGV, etc) med nära kontakter till materielförsörjnings- processen och perspektivplaneringen. Dessa kan samla kompetensen inom FHS och FOI, övervaka forskningsfronten genom att bevaka tävlingar, konferenser samt delta i internationella samarbeten, samt överföra det samlade resultaten till Försvarsmakten genom demonstrationer av verkliga eller simulerade delsy- stem och interaktiva simuleringar av hela system. Just systemsimuleringar kan göras särskilt realistiska, eftersom interaktionen med de riktiga obemannade systemen till stor del sker igenom kontrollstationernas datorer. På så sätt ska- pas en känsla för både hot och möjligheter med de nya systemen, vilket gagnar både taktikutveckling och materielprocesser.

  • A foraging cost of migration for a partially migratory cyprinid fish

    • Migration has evolved as a strategy to maximise individual fitness in response to seasonally changing ecological and environmental conditions. However, migration can also incur costs, and quantifying these costs can provide important clues to the ultimate ecological forces that underpin migratory behaviour. A key emerging model to explain migration in many systems posits that migration is driven by seasonal changes to a predation/growth potential (p/g) trade-off that a wide range of animals face. In this study we assess a key assumption of this model for a common cyprinid partial migrant, the roach Rutilus rutilus, which migrates from shallow lakes to streams during winter. By sampling fish from stream and lake habitats in the autumn and spring and measuring their stomach fullness and diet composition, we tested if migrating roach pay a cost of reduced foraging when migrating. Resident fish had fuller stomachs containing more high quality prey items than migrant fish. Hence, we document a feeding cost to migration in roach, which adds additional support for the validity of the p/g model of migration in freshwater systems.

  • The value of post-operative chemotherapy after chemoradiotherapy in patients with high-risk locally advanced rectal cancerdresults from the RAPIDO trial

    • Background: Pre-operative chemoradiotherapy (CRT) rather than radiotherapy (RT) has resulted in fewer locoregional recurrences (LRRs), but no decrease in distant metastasis (DM) rate for patients with locally advanced rectal cancer (LARC). In many countries, patients receive post-operative chemotherapy (pCT) to improve oncological outcomes. We investigated the value of pCT after pre-operative CRT in the RAPIDO trial.Patients and methods: Patients were randomised between experimental (short-course RT, chemotherapy and surgery) and standard-of-care treatment (CRT, surgery and pCT depending on hospital policy). In this substudy, we compared curatively resected patients from the standard-of-care group who received pCT (pCT+ group) with those who did not (pCT- group). Subsequently, patients from the pCT+ group who received at least 75% of the prescribed chemotherapy cycles (pCT >75% group) were compared with patients who did not receive pCT (pCT-/- group). By propensity score stratification (PSS), we adjusted for the following unbalanced confounders: age, clinical extramural vascular invasion, distance to the anal verge, ypT stage, ypN stage, residual tumour, serious adverse event (SAE) and/or readmission within 6 weeks after surgery and SAE related to pre-operative CRT. Cumulative probability of disease-free survival (DFS), DM, LRR and overall survival (OS) was analysed by Cox regression.Results: In total, 396/452 patients had a curative resection. The number of patients in the pCT+, pCT >75%, pCT- and pCT-/- groups was 184, 112, 154 and 149, respectively. The PSS-adjusted analyses for all endpoints demonstrated hazard ratios between approximately 0.7 and 0.8 (pCT+ versus pCT-), and 0.5 and 0.8 (pCT >75% versus pCT-/-). However, all 95% confidence intervals included 1.Conclusions: These data suggest a benefit of pCT after pre-operative CRT for patients with high-risk LARC, with approximately 20%-25% improvement in DFS and OS and 20%-25% risk reductions in DM and LRR. Compliance with pCT additionally reduces or improves all endpoints by 10%-20%. However, differences are not statistically significant.
