  • Central versus Peripheral Postcardiotomy Veno-Arterial Extracorporeal Membrane Oxygenation : Systematic Review and Individual Patient Data Meta-Analysis

    • Background: It is unclear whether peripheral arterial cannulation is superior to central arterial cannulation for postcardiotomy veno-arterial extracorporeal membrane oxygenation (VA-ECMO). Methods: A systematic review was conducted using PubMed, Scopus, and Google Scholar to identify studies on postcardiotomy VA-ECMO for the present individual patient data (IPD) meta-analysis. Analysis was performed according to the intention-to-treat principle. Results: The investigators of 10 studies agreed to participate in the present IPD meta-analysis. Overall, 1269 patients were included in the analysis. Crude rates of in-hospital mortality after central versus peripheral arterial cannulation for VA-ECMO were 70.7% vs. 63.7%, respectively (adjusted OR 1.38, 95% CI 1.08–1.75). Propensity score matching yielded 538 pairs of patients with balanced baseline characteristics and operative variables. Among these matched cohorts, central arterial cannulation VA-ECMO was associated with significantly higher in-hospital mortality compared to peripheral arterial cannulation VA-ECMO (64.5% vs. 70.8%, p = 0.027). These findings were confirmed by aggregate data meta-analysis, which showed that central arterial cannulation was associated with an increased risk of in-hospital mortality compared to peripheral arterial cannulation (OR 1.35, 95% CI 1.04–1.76, I2 21%). Conclusions: Among patients requiring postcardiotomy VA-ECMO, central arterial cannulation was associated with an increased risk of in-hospital mortality compared to peripheral arterial cannulation. This increased risk is of limited magnitude, and further studies are needed to confirm the present findings and to identify the mechanisms underlying the potential beneficial effects of peripheral VA-ECMO.

    • This white paper reports on the discussions of the 2018 Facility for Rare Isotope Beams Theory Alliance (FRIB-TA) topical program ‘From bound states to the continuum: Connecting bound state calculations with scattering and reaction theory’. One of the biggest and most important frontiers in nuclear theory today is to construct better and stronger bridges between bound state calculations and calculations in the continuum, especially scattering and reaction theory, as well as teasing out the influence of the continuum on states near threshold. This is particularly challenging as many-body structure calculations typically use a bound state basis, while reaction calculations more commonly utilize few-body continuum approaches. The many-body bound state and few-body continuum methods use different language and emphasize different properties. To build better foundations for these bridges, we present an overview of several bound state and continuum methods and, where possible, point to current and possible future connections.

    • The paper reviews recent research and development activities in the field of hydropower technology. It covers emerging and advanced technologies to mitigate flow instabilities (active and passive approach) as well as emerging magneto-rheological control techniques. Recent research findings on flow instabilities are also presented, especially concerning fluid-structure interaction and transient operating conditions. As a great number of the existing large-scale hydroelectric facilities were constructed decades ago using technologies that are now considered obsolete, technologies to achieve the digitalisation of hydropower are also analysed. Advances in the electro-mechanical components and generator design are presented; their potential role to adapt hydropower to the current operating conditions is also highlighted. The text explores current efforts to advance hydropower operation, mainly in terms of European projects. It provides a detailed overview of the recent efforts to increase the operational range of hydraulic turbines in order to reach exceptional levels of flexibility, a topic of several recent research projects. Variable speed hydropower generation and its application in pumped storage power plants are presented in detail. Moreover, revolutionary concepts for hydroelectric energy storage are also presented with the analysis focusing on underwater hydro storage and hydropower's hybridisation with fast energy storage systems. Efforts to minimise hydropower's environmental footprint are also presented via the utilisation of small-scale and fish-friendly installations.

  • A revolution without people? Closing the people-policy gap in aquaculture development

    • Failure of the blue revolutionis a global risk. The international problem is that there is a gap in knowledge exchange between the aquaculture industry, policy makers trying to support aquaculture development and people who depend on aquaculture for a job and/or food source. Thus, governments and international organizations promoting aquaculture as the solution to improving food security, nutrition and income are failing to optimise production of natural aquatic resources. We identify a people-policy gap, and suggest that this is an understudied constraint, which needs to be overcome before worldwide food security can be achieved from aquatic environments. We argue that this gap leads to uneven distribution of benefits, a disconnection between benefits and local needs, and detrimental effects on human health and food security, all of which can have negative repercussions on human communities and ecosystems. In order to address this need, we present an analytical framework to guide context specific, policy-relevant assessments of the social, economic and ecological dimensions of aquaculture on a case-by-case basis. The framework is designed to make best use of existing data and scientific tools for decision-making. In conclusion, we argue for: equal consideration of ecological, social and economic issues in aquaculture policy-making; pre-emptive identification of likely social impacts; integration of people-and context-specific social framing conditions into planning and policy review; addressing the social disconnection between global consumption and production; and, encouragement of creative combinations of theories and methods to assess and interpret the social dimensions of aquaculture in multiple contexts.

    • The discovery of the enhancement of Raman scattering by molecules adsorbed on nanostructured metal surfaces is a landmark in the history of spectroscopic and analytical techniques. Significant experimental and theoretical effort has been directed toward understanding the surface-enhanced Raman scattering (SERS) effect and demonstrating its potential in various types of ultrasensitive sensing applications in a wide variety of fields. In the 45 years since its discovery, SERS has blossomed into a rich area of research and technology, but additional efforts are still needed before it can be routinely used analytically and in commercial products. In this Review, prominent authors from around the world joined together to summarize the state of the art in understanding and using SERS and to predict what can be expected in the near future in terms of research, applications, and technological development. This Review is dedicated to SERS pioneer and our coauthor, the late Prof. Richard Van Duyne, whom we lost during the preparation of this article. ©
