  • Medical and licit drug use in an urban/rural study population with refugee background, 7-8 years into resettlement

    • Objective: Research into medical and licit drug use in resettled refugee populations is scarce, despite the fact that mental health status often has been found to be poorer than in general populations. Hence the aim of this study was to estimate the prevalence of self-rated use of medicine and licit drugs among adults who came to Sweden from Bosnia-Herzegovina (1993/94) and who in 2001 were living in either an urban (low employment context) or a rural (high employment context) region (n=4185).Methods: Prevalence was estimated from a cross-sectional questionnaire distributed to a representative sample (n=650) in 2001 (63.5% response rate).Results: The study population estimates of usage of sedatives (26.5%), sleeping tablets (26.2%) and antidepressants (22.3%) did not differ by gender but did so by region, with a higher urban prevalence. The consumption of alcohol (5.1%) and cigarettes (41.0%) did not differ by region but men reported higher alcohol consumption than women.Conclusion: The high consumption of medicine (compared with general populations) raises the question of treatment efficiency and the need for public health attention and evaluation many years after resettlement. Factors to consider for further research with analytic prerequisites concern indications that regional differences may be influenced, not only by urban employment being lower but also by urban/rural differences in prescription rates and/or access to health care; moreover, there might have been a selection to the urban region of older people, with a more vulnerable family situation, and/or poorer mental health. Finally, the overall alcohol (low) and cigarettes (high) consumption in the study population followed prevalence patterns found in Bosnia-Herzegovina rather than in Sweden.

  • Internet-delivered acceptance and commitment therapy (iACT) for chronic pain : feasibility and preliminary effects in clinical and self-referred patients

    • Background: Acceptance and commitment therapy (ACT) is an evidence-based treatment to improve functioning and quality of life (QoL) for chronic pain patients, but outreach of this treatment is unsatisfactory. Internet-delivery has been shown to increase treatment access but there is limited evidence regarding feasibility and effectiveness of web-based ACT for chronic pain. The aim of the study was to evaluate and iterate a novel internet-delivered ACT program, iACT, in a clinical and a self-referred sample of chronic pain patients. The intervention was developed in close collaboration with patients. To enhance learning, content was organized in short episodes to promote daily engagement in treatment. In both the clinical and self-referred samples, three critical domains were evaluated: (I) feasibility (acceptability, practicality and usage); (II) preliminary efficacy on pain interference, psychological inflexibility, value orientation, QoL, pain intensity, anxiety, insomnia and depressive symptoms; and (III) potential treatment mechanisms.Methods: This was an open pilot study with two samples: 15 patients from a tertiary pain clinic and 24 self-referred chronic pain participants, recruited from October 2015 until January 2017. Data were collected via an online platform in free text and self-report measures, as well as through individual oral feedback. Group differences were analyzed with Chi square-, Mann-Whitney U- or t-test. Preliminary efficacy and treatment mechanism data were collected via self-report and analyzed with multilevel linear modeling for repeated measures.Results: Feasibility: patient feedback guided modifications to refine the intervention and indicated that iACT was acceptable in both samples. User insights provided input for both immediate and future actions to improve feasibility. Comprehensiveness, workability and treatment credibility were adequate in both samples. Psychologists spent on average 13.5 minutes per week per clinical patient, and 8 minutes per self-referred patient (P=0.004). Recruitment rate was 24 times faster in the self-referred sample (24 patients in 1 month, compared to 15 patients in 15 months, P<0.001) and the median distance to the clinic was 40 km in the clinical sample, and 426 km in the self-referred sample (P<0.001). Preliminary effects: post-assessments were completed by 26 participants (67%). Significant effects of time were seen from pre- to post-treatment across all outcome variables. Within group effect sizes (Cohen’s d) at post-treatment ranged from small to large: pain interference (d=0.64, P<0.001), psychological inflexibility (d=1.43, P<0.001), value progress (d=0.72, P<0.001), value obstruction (d=0.42, P<0.001), physical QoL (d=0.41, P=0.005), mental QoL (d=0.67, P=0.005), insomnia (d=0.31, P<0.001), depressive symptoms (d=0.47, P<0.001), pain intensity (d=0.78, P=0.001) and anxiety (d=0.46, P<0.001). Improvements were sustained at 1-year follow-up. Psychological inflexibility and value progress were found to be potential treatment mechanisms.Conclusions: The results from the present study suggests that iACT was feasible in both the clinical and the self-referred sample. Together with the positive preliminary results on all outcomes, the findings from this feasibility study pave the way for a subsequent large randomized efficacy trial.

  • School, Learning and Mental Health : A systematic review

    • Rapporten presenterar resultaten från en systematisk översikt av forskning om skola, lärande och barns psykiska hälsa. Kungliga Vetenskapsakademiens Hälsoutskottet har givit uppdraget att genomföra en sådan översikt till en arbetsgrupp som har arbetat med uppdraget från hösten 2008 till mars 2009. Det första syftet med översikten är att genomföra en kartläggning av forskning inom det breda fält som behandlar frågor om skola, lärande och barns och ungdomars psykiska hälsa. Det andra syftet är att genomföra en narrativ syntes av forskning som undersökt orsaksförhållanden mellan psykisk hälsa å ena sidan och skolresultat och lärande å den andra sidan. Det tredje syftet är att redovisa resultat från forskning som har studerat svenska barns och ungdomars erfarenheter och upplevelser av skola och undervisningssituationer. För att uppnå de första två syftena genomfördes systematiska litteratursökningar i bibliografiska databaser av artiklar publicerade i vetenskapliga internationella tidskrifter inom olika discipliner. Det tredje syftet undersöktes med litteratursökningar av kvalitativa svenska studier i bibliografiska databaser. Slutsatser På grundval dels av kartläggningen av forskning om skola, lärande och psykisk hälsa, dels av de två fördjupade översikterna kan följande slutsatser dras: • Omfattningen av forskning som undersöker relationerna mellan olika aspekter av skola och psykisk hälsa är begränsad och i synnerhet gäller detta forskning som undersöker organisationsfaktorer och undervisnings-faktorer, aktiviteter, läroplaners utformning, resurser, specialpedagogiskt stöd, och olika former av betyg och bedömning. • Tidiga svårigheter i skolan och i synnerhet läs- och skrivsvårigheter kan orsaka internaliserande och externaliserande psykiska problem. • Svårigheter i skola och psykiska problem tenderar att vara stabila över tid. • Skolrelaterade hälsoproblem tenderar att minska när eleverna börjar på gymnasiet och får tillgång till nya områden av aktiviteter, roller och valmöjligheter. • Att genomföra stora ansträngningar utan att detta leder till resultat är relaterat till utveckling av depression. Problem i skolan med skolresultat och prestationer orsakar inter-naliserande symptom för flickor under tonåren. • Det finns samband mellan olika typer av psykiska problem och de är också relaterade till ett brett spektrum av somatiska och psykosomatiska symptom. • Internaliserande och externaliserande psykiska problem har negativa effekter på skolprestationer genom mekanismer som är delvis ålders- och genusspecifika. • Kompetenser och prestationer i skolan är relaterade till psykisk hälsa. • Goda resultat i skolan har en positiv effekt på självuppfattning. • En god självuppfattning bidrar inte direkt till bättre resultat, men andra faktorer som är relaterade till självuppfattning (motivation och upplevd inre/yttre kontroll) påverkar lärande och resultat • Relationer med klasskamrater och lärare bidrar till processer som kopplar skolmisslyckande till psykisk ohälsa. Relationer med kamrater och lärare kan också skydda mot utvecklingen av psykiska problem. • Jämförelser med klasskamrater påverkar självuppfattningen, med effekter som varierar beroende på gruppsammansättning och typ av skola.

  • The Effectiveness of Payments for Environmental Services

    • We adopt a theory-based approach to synthesize research on the effectiveness of payments for environmental services in achieving environmental objectives and socio-economic co-benefits in varying contexts. Our theory of change builds on established conceptual models of impact pathways and highlights the role of (1) contextual dimensions (e.g., political, institutional, and socio-economic conditions, spatial heterogeneity in environmental service values and provision costs, and interactions with pre-existing policies), and (2) scheme design (e.g., payment type and level, contract length, targeting, and differentiation of payments) in determining environmental and socio-economic outcomes. To shed light on the overall effectiveness of payment schemes, and its determinants, we review counterfactual-based empirical evaluations, comparative analyses of case-studies, and meta-analyses. Our review suggests that program effectiveness often lags behind the expectations of early theorists. However, we also find that theory has advanced sufficiently to identify common reasons for why payment schemes fail or succeed. Moreover, payment schemes are often rolled out along with other policy instruments in so-called policy mixes. Advances in theory and evaluation research are needed to improve our understanding of how such policy mixes interact with the targeted social-ecological systems. © 2017

    • Non-governmental organizations (NGO) and government donor agencies (GDA) are often caught in a dilemma; an NGO between responsiveness to its target group(s), expectations of individual donors and demands of its GDA; GDA between its policy to respect NGO's integrity, its wish to keep NGOs accountable for received fund and its operation within the bounds of its general policies. This dilemma is mirrored in the NGO-GDA negotiation for funds. Based on negotiation theory and using three explanatory approaches, 18 years of negotiations between an NGO, Vi Skogen (ViS) and its GDA, the Swedish International Development Cooperation Agency (Sida), are analyzed in order to demonstrate how organizational structures, power relations and the context influence the outcome of the negotiations. All three approaches help to explain how ViS managed, mainly in the interest of its individual donors, to resist changes demanded by Sida and also to explain how the agendas of ViS and Sida finally converged.

  • Perceived benefits from agroforestry landscapes across North-Eastern Europe : What matters and for whom?

    • Agroforestry landscapes are crucial to human wellbeing; however, they are in sharp decline across Europe. Improved understanding of the complexity of agroforestry landscapes within different biophysical, social cultural, economic and governance contexts is essential for designing effective policy and management interventions that are more tightly aligned with societal expectations and aspirations. This paper identifies and compares values that people attribute to agroforestry landscapes across North-Eastern Europe, using case studies in Sweden, Latvia, Belarus, and the Russian Federation. We apply the multiple-value approach developed for the conceptual framework of the Intergovernmental Platform on Biodiversity and Ecosystem Services to an assessment of agroforestry landscapes. Using data from a total of 1634 face-to-face structured interviews, we (i) analyse and explore the preferences of diverse groups of respondents for agroforestry landscapes; (ii) identify a broad range of nature's contributions to people (NCP) that were attributed to agroforestry landscapes by respondents; and, (iii) analyse values of agroforestry landscapes across different contexts in NorthEastern Europe. We found that a highly heterogenous group of people - broadly irrespective of age, education, gender, place of residence, as well as political, economic, or social-cultural context - perceive agroforestry landscapes to be important to their quality of life. Respondents attributed multiple NCP to agroforestry landscapes, and nonmaterial NCP are the most frequently assigned in all four countries. An absolute majority of respondents across all case studies considered relational values of agroforestry landscapes to be important for their quality of life with identity as the most often associated with agroforestry landscapes. We discuss how relational values might be incorporated in policies and practices related to agroforestry landscapes in North-Eastern Europe.

  • Evaluering av forskningsprogrammene PRAKSISFOU og UTDANNING2020 : Granskning av Norska Forskningsrådets Strategiska Program för Utbildningsforskning

    • In May 2012 The Research Council of Norway (RCN) created a Nordic research panel for evaluating three research programs in Educational Research, together embracing 690 million NOK in resources; PRAKSISFOU: Praxis directed Research and Development for early childhood education, comprehensive education and teacher education. (Final evaluation) UTDANNING2020: Norwegian Educational Research towards 2020. (Half-way evaluation) PRAKUT: Praxis directed Educational Research. (Initial evaluation related to experiencesfrom PRAKSISFOU and considering benefits in cooperation with UTDANNING2020) The research panel took a formative stance in the evaluation starting from program theories – that is constellations of goals, instruments used and expected results and outcomes. Themes for evaluation were; scientific qualities, relevance qualities, social and intellectual organisation – including crossdisciplinarity, internationalization, and forms of knowledge. The panel analysed policy documents and background materials, reports and publications. To this was added interviews, conversations and surveys to actors and participants (in sum almost one hundred persons). The following general conclusions were put forwards: Given the large amount of resources PRAKSISFOU and UTDANNING2020 it is reasonable that the programs will contribute to strengthening research of relevance for policy-making and for the teaching professions. Furthermore, the programs were having an impact in increased research competences in teacher education and in the number of disciplines engaging in educational research. However, to these general conclusions were added a set of comments: First, the panel found significant differences between PRAKSISFOU and UTDANNING2020 over the themes, partly consistent to their respective program theories. For instance, PRAKSISFOU was of a rather low scientific quality, measured by articles in international scientific journals, while UTDANNING2020 was more promising in this respect. Second, within both programs there were large differences over the themes presented above and the activities in the programs were partly overlapping. Third, productivity in terms of international high-quality publication and co-publication was not very high so far. Forthly, the instruments used for realization of the programs should be improved according to the panel, for instance more valid indicators in relation to the goals, or more precise ways in engaging in researcher-user cooperation. Fifthly, there was a tendency to manage the programs in a too undifferentiated way, neglecting that ”one size does not fit all” in different kinds of educational research and development. In addition, directives by The Ministry of Education and Research were sometimes too detailed to be productive for research. The support from the RCN and the program boards were highly appreciated by the participants. Considering recommendations it was concluded by the panel: UTDANNING2020 should clarify the current state of knowledge in the program and develop actions necessary to further strengthen scientific quality, knowledge organisation and internationalisation. Similar actions are necessary for PRAKUT, based on the experiences from PRAKSISFOU. This should be done in cooperation with The Norwegian Knowledge Centre for Education. Such a knowledge clarification and organisation is necessary in order to recognize and evaluate the actual significance of knowledge production and research relevance in the programs. Finally, PRAKUT and UTDANNING2020 should cooperate and eventually merge, for reasons in terms of knowledge organisation as well as in resource efficiency. However, it is necessary to differentiate between strivings for new knowledge on one side and strivings for innovation on the other side. These are different kinds of tasks, and should consequently be using different kinds of critieria in order to identify progress in education and research. Such a differentiation would in turn improve communication considering education, research and innovation.
