  • Transforaminal steroid injections for the treatment of cervical radiculopathy: a prospective and randomised study.

    • Steroid injections are often employed as an alternative treatment for radicular pain in patients with degenerative spinal disorders. Prospective randomised studies of the lumbar spine reveal contradictory results and non-randomised and most often retrospective studies of the cervical spine indicate pain reduction from steroid injections. No prospective randomised study on transforaminal steroid injections for the treatment of radicular pain in the cervical spine focusing on short-term results has been performed. Forty consecutive patients were employed for the study. The inclusion criteria were one-sided cervical radiculopathy with radicular distribution of arm pain distal to the elbow and corresponding significant degenerative pathology of the cervical spine at one or two levels on the same side as the radicular pain and visualised by MRI. A transforaminal technique was used for all injections. A positive response to a diagnostic selective nerve root block at one or two nerve roots was mandatory for all patients. The patients were randomised for treatment with steroids/local anaesthetics or saline/local anaesthetic. Only the neuroradiologist performing the blocks was aware of the content of the injection; all other persons involved in the study were blinded. Follow up was made 3 weeks after the randomised treatment by a clinical investigation and with a questionnaire focusing on the subjective effects from the injections. At follow up, there were no differences in treatment results in the two patient groups. Statistical analysis of the results confirmed the lack of difference in treatment effect. Further studies have to be performed before excluding steroids in such treatment and for evaluating the influence of local anaesthetics on radiculopathy in transforaminal injections.

  • Perspectives on Sexuality Among Patients with Hypopituitarism: Broadening the Medical Focus on Sexual Function to Include Sexual Wellbeing

    • Hypopituitarism means a partial or complete inability to produce pituitary hormones, including those regulating gonadal function. The condition therefore leads to hormone deficiencies that may affect sexuality in various ways. This study aimed to explore patients' own experiences of hypopituitarism in relation to sexuality, and it is based on interviews with 19 men and women. A qualitative methodology inspired by Gadamer's philosophical hermeneutics was used. Analysis revealed that current healthcare practices, which focus primarily on sexual functioning, fail to address patients' existential concerns about loneliness, loving relationships and selfhood. Another important finding was that women felt that their sexual functioning and wellbeing tended to go unnoticed, while men said that healthcare professionals focused mainly on erectile function rather than problems with sexual wellbeing. Cultural and gender norms were apparent in these healthcare interactions, and proactive efforts to improve awareness is required in order to improve the quality of care provided to these patients. Healthcare professionals need time, space and support for reflection so that they may enhance their knowledge about the complexity of sexuality and discuss the results of research into hormone replacement in men and women. They also need support to develop critical awareness of how gendered norms about sexuality may negatively affect the care provided to persons with hypopituitarism.

    • Having good balance is a facilitating factor in the performance of everyday activities. Good balance is also essential in various sport activities in order to both get results and prevent injury. A common measure of balance is postural sway, which can be measured both antero-posteriorly and medio-laterally. There are several companies marketing wristbands whose intended function is to improve balance, strength and flexibility. Randomized controlled trials have shown that wristbands with holograms have no effect on balance but studies on wristbands with minerals seem to be lacking.

  • Influence of vestibular rehabilitation on neck pain and cervical range of motion among patients with whiplash-associated disorder: A randomized controlled trial.

    • Objective: To describe how vestibular rehabilitation influences pain and range of motion among patients with whiplash-associated disorder and dizziness, and to describe whether pain or range of motion correlated with balance performance or self-perceived dizziness handicap. Subjects: A total of 29 patients, 20 women and 9 men, age range 22-76 years. Methods: Patients with whiplash-associated disorder and dizziness were randomized to either intervention (vestibular rehabilitation) or control. Neck pain intensity, cervical range of motion (CROM), balance and self-perceived dizziness handicap were measured at baseline, 6 weeks and 3 months. Results: There were no differences in neck pain intensity or CROM between the 2 groups either at baseline, 6 weeks or 3 months (p = 0.10-0.89). At baseline, neck pain intensity correlated with CROM (-0.406) and self-perceived dizziness handicap (0.492). CROM correlated with self-perceived dizziness handicap and with 1 balance measure (-0.432). Neck pain intensity did not correlate with balance performance (-0.188-0.049). Conclusion: Neck pain intensity and CROM was not influenced by vestibular rehabilitation. Importantly, the programme did not appear to increase pain or decrease neck motion, as initially thought. Neck pain intensity and CROM correlated with self-perceived dizziness handicap. CROM also correlated with 1 balance measure.

  • Vestibular rehabilitation for persons with stroke and concomitant dizziness : a pilot study

    • Background: Dizziness is common among patients with first time stroke. It affects self-perceived health and is a risk factor for falls. Vestibular rehabilitation (VR) is effective for treating dizziness among various conditions, but the effect of dizziness with origin in the central nervous system is poorly studied.This pilot study of a randomized controlled trial aimed at investigating a vestibular rehabilitation programme among patients with first time stroke and concomitant dizziness. A second aim was to study the feasibility of performing the randomized controlled trial.Methods: The participants were computer generated randomized to either an intervention or a control group. The intervention comprised of four different vestibular rehabilitation exercises, adapted for each patient and usual rehabilitation. The control group received usual rehabilitation without the vestibular rehabilitation exercises. Outcome measures used were The Activities-specific Balance Confidence Scale, the Berg Balance Scale, the Functional Gait Assessment Scale and the EuroQol-5D. Feasibility was studied in terms of recruitment, adherence and retention rates, also as the ability to collect primary and secondary outcomes as well as to find indications of treatment differences.Results: Self-rated health improved for all participants. No other differences between baseline and follow-up were detected neither within nor between groups. Recruitment rate was 23%, adherence to the intervention 90%, retention rate 69% and ability to collect outcome measures 90%. No adverse events occurred.Conclusion: Both the intervention and the control groups improved in self-perceived health. The measures of feasibility were satisfactory in this study, apart from a low recruitment rate.

    • We study the homogenization of a certain linear hyperbolic-parabolic problem exhibiting two rapid spatial scales {ε; ε2}. The homogenization is performed by means of evolution multiscale convergence, a generalization of the concept of two-scale convergence to include any number of scales in both space and time. In particular we apply a compactness result for gradients. The outcome of the homogenization procedure is that we obtain a homogenized problem of hyperbolic-parabolic type together with two elliptic local problems, one for each rapid scale, for the correctors.

The homogenization procedure is performed within the framework of evolution multiscale convergence which is a generalization of two-scale convergence to include several spatial and temporal scales. In order to derive the local problems, one for each spatial scale, the crucial concept of very weak evolution multiscale convergence is utilized since it allows less benign sequences to attain a limit. It turns out that the local problems do not involve the dissipative term g even though the homogenized problem does and, due to the nonlinearity property, an important part of the work is to determine the effective dissipative term. A brief illustration of how to use the main homogenization result is provided by applying it to an example problem exhibiting six spatial and eight temporal scales in such a way that a and g have disparate oscillation patterns.

    • The main contribution of this paper is the homogenization of the linearparabolic equationtu (x, t) − ·axq1, ...,xqn,tr1, ...,trmu (x, t)= f(x, t)exhibiting an arbitrary finite number of both spatial and temporal scales.We briefly recall some fundamentals of multiscale convergence and providea characterization of multiscale limits for gradients in an evolution settingadapted to a quite general class of well-separated scales, which we nameby jointly well-separated scales (see Appendix for the proof). We proceedwith a weaker version of this concept called very weak multiscale convergence.We prove a compactness result with respect to this latter typefor jointly well-separated scales. This is a key result for performing thehomogenization of parabolic problems combining rapid spatial and temporaloscillations such as the problem above. Applying this compactnessresult together with a characterization of multiscale limits of sequences ofgradients we carry out the homogenization procedure, where we togetherwith the homogenized problem obtain n local problems, i.e. one for eachspatial microscale.
