Thinking beyond the Cure : a constructive design research investigation into the patient experience of radiotherapy

- This constructive design research dissertation aims to understand how design can be used as part of a composite research approach to generate knowledge about how complex phenomena are composed through their interactions and relationships with various actors, both human and non-human. It has done this by investigating a single phenomenon, the patient experience of radiotherapy.Through the purposeful selection and application of methods, theories, and existing research from design, nursing, and STS, this thesis utilizes a mixed-method approach comprised of qualitative, quantitative methods, and design experimentation, across multiple research sites and patient populations, in three research projects – PERT, DUMBO, and POIS – to generate rich and layered knowledge of the patient experience. Experience prototypes are used to challenge, through intervention or provocation, the relationships between the various radiotherapy actors identified through the empirical methods. Together, the research generated in PERT, DUMBO, and POIS construct a map of the networked, interdependent actors which shape the patient’s emotional experience of radiotherapy: the staff, technology, information, environment, and institutions. It also calls attention to the problematic relationship between radiotherapy patients and the technologies used to treat them, which can lead to anxiety, worry, and fear.This thesis offers contributions related to both improving patient experience and designing for complex social issues. First, this research suggests that individuals, other than primary users, need to be acknowledged in the design of medical technologies. It proposes calling attention to patients by naming them as interactors in their relationships with the aforementioned technologies, removing them from the role of implicated actor. Second, this thesis problematizes treating the actors within a network as independent entities, which medical research and user-centered design often does, and calls for a new type of design practice which attends to these networked relationships. Third, this thesis suggests two ways in which design research practice should be shifted methodologically if it wants to engage with and design for complex social issues like patient experience; widening the researcher’s perspective on the issue through the use of a composite methodology, and having the researcher maintain this scope by remaining closely connected to their research context. The implications of this work concern how design research, design education, and design practice might shift their approaches to fully acknowledge and attend to the complexity of systems like healthcare.

- In this paper we explore how human-centered design can expand the solution space surrounding patient experience in healthcare, looking specifically at patient emotional wellbeing. The findings presented here are taken from our ethnographic research of cancer patients undergoing radiotherapy treatment; we investigated these individuals' emotional experiences during their time in the treatment facility. Building upon previous findings within the field of nursing concerned with the prevalence of anxiety in cancer patients and the importance of person-centered care, we implement a human-centered design research approach to investigate the situational triggers of patient anxiety within the radiotherapy treatment experience. Through 'quick ethnography', we recognize that the fixation technology used within radiotherapy is a key trigger for anxiety in patients. Application of theory from the field of science and technology studies to our analysis of this technologically-mediated anxiety suggests that the fixation device confines the patient to a passive, disempowered role within its interactions due to it being embedded with the socially scripted 'sick role'. Summarizing our insights, we find that human-centered design is capable of looking holistically at patient experience to discover new solutions spaces for mediating and preventing situational anxiety, and turning person-centered care within Radiotherapy into a pre-emptive practice instead of a responsive one.

Boundaries, believers and bodies : a cultural analysis of a multidisciplinary research community.

- The aim of this study is to analyze the construction of research culture and collaboration within the research studio Tools for Creativity, one node in the larger Interactive Institute. This studio is an arena that in today’s society is associated with boundary crossing, dynamics and variability: An environment with high-tech equipment, a staff equipped with diverse skills, and a flexible approach with the ambition of developing innovative tools based on ICT to strengthen human creativity. The present thesis is divided into four main parts. In Part One, culture as analytical framework is presented. This is followed by a presentation of field work, data and field site, the studio Tools for Creativity, and its employees. This includes a discussion of the methods of participant observations and deep inter-views. In the theoretical framework overview, perspectives used in this thesis is presented, including the so-called “new research landscape” debate, a background to this thesis. The introduction concludes with a chapter whith a reflexivity discussion including the making of the research self during field work and in the written text. In Part Two, entitled “Technology”, the informants’ definition of technology in relation ICT and the prototypes produced is ana-lyzed. The concepts “enlightenment optimism” and “romantic uneasiness” are presented as theoretical entrances to the chapter. This is the background for an analysis of the future- and speed-oriented discourse that characterizes the informants’ perception of technology. The aim of using technology to support human creativity, challenge presence and facilitate multi-cultural communication is further discussed. This is juxtaposed with another aspect of technol-ogy, namely the informant’s critique of technology’s impact on mankind, humanity and society. Part Three, “Re-search”, deals with interpretations and negotiations of the concept of research and the researcher conducted by the informants at Tools for Creativity. First, the concept of “boundary object” is presented followed by its use order to analyze the construction of research and the researcher in a multi-disciplinary arena like the studio. An important part of the making of the researcher is the trading of skills in the attempt of legitimizing the individuals’ efforts at conducting research. Here, focus is the negotiation of research as an activity between individuals representing the sciences and the arts, as well as those with formal education and autodidacts. Attempts to manage a broader research concept are placed in relation to academic quality demands. In this analysis, the point of departure is Pierre Bourdieu’s concepts of symbolic capital. Here, gender is included as symbolic capital. Part Four is called “Reflections” and contains a discussion of the study and reflections concerning field work. This is followed by a summation of what happened to Tools for Creativity and Interactive Institute after finished field work.

The Logical Structure of the Moral Concepts : An Essay in Propositional Deontic Logic

- In this thesis, the main focus is on deontic logic as a tool for formal representation of moral reasoning in natural language. The simple standard system of deontic logic (SDL), i.e. the minimal Kripkean modal logic extended with the deontic axiom, stating that necessity (interpreted as obligation) implies possibility (interpreted as permission), has often been considered inadequate for this aim, due to different problems, e.g. the so-called deontic paradoxes. A general survey of deontic logic and the problems with SDL is made in chapter 1. In chapter 2, a system denoted Classical Deontic-Modal logic (CDM1) is defined. In this system, there is a primary obligation operator indexed to sets of possible worlds, and a secondary requirement operator, defined in terms of strictly necessary conditions for fulfilling an obligation. This secondary operator has most of the properties of the necessity operator in SDL. In chapters 3 and 4, it is argued that CDM1 is able to handle the SDL problems presented in chapter 1 in an adequate way, and the treatment of these problems in CDM1 is also compared with their treatment in some other well-known deontic systems. In chapter 5, it is argued that even though the problems related to quantification in modal contexts are relevant to deontic logic, these issues are not specific to deontic logic. In chapter 6, the relations between some controversial features of moral reasoning, such as moral dilemmas and “non-standard” deontic categories like supererogation, and deontic logic are discussed. It is shown how CDM1 can be modified in order to accommodate these features.
