    • Obsessive-Compulsive Disorder (OCD) is a leading cause of disability world-wide (World Health Organization, 2008). Treatment of OCD is a specialized field whose aim is recovery from illness for as many patients as possible. The evidence-based psychotherapeutic treatment for OCD is specialized cognitive behavior therapy (CBT, NICE, 2005, Koran and Simpson, 2013). However, these treatments are not accessible to many sufferers around the world. Currently available guidelines for care are deemed to be essential but insufficient because of highly variable clinician knowledge and competencies specific to OCD. The phase two mandate of the 14 nation International OCD Accreditation Task Force (ATF) created by the Canadian Institute for Obsessive Compulsive Disorders is development of knowledge and competency standards for specialized treatments for OCD through the lifespan deemed by experts to be foundational to transformative change in this field. This paper presents knowledge and competency standards for specialized CBT for adult OCD developed to inform, advance, and offer a model for clinical practice and training for OCD. During upcoming ATF phases three and four criteria and processes for training in specialized treatments for OCD through the lifespan for certification (individuals) and accreditation (sites) will be developed based on the ATF standards

  • Negative effects in psychotherapy : Current understanding and future research directions

    • Introduction: Psychotherapy can help alleviate mental distress and improve quality of life, but little is known about the potential negative effects of psychotherapy and how to determine their frequency and characteristics. Meanwhile, many researchers and clinicians are unfamiliar to the idea of patients faring worse or experiencing adverse and unwanted events during treatment. The present report thus intends to provide an overview of the current understanding and future research directions of negative effects in psychotherapy. Method: An anonymous survey was distributed to a selection of highly renowned researchers, using an analytical framework known as Strengths, Weaknesses, Opportunities, and Threats (SWOT). Results: The researchers recognize an increased awareness of negative effects in psychotherapy in recent years, but also describe many issues related to their definition, assessment, and reporting, e.g., the lack of a clear and coherent terminology. Both qualitative methods and naturalistic designs are regarded as important to find out what negative effects exist and how to prevent them, although a number of methodological obstacles associated with such methods are identified and need to be resolved. Discussion: Negative effects of psychotherapy are multifaceted, warranting careful considerations in order to be monitored and reported in both routine care and research settings.

  • Mind the gap : Improving the dissemination of CBT

    • Empirically supported psychological treatments have been developed for a range of psychiatric disorders but there is evidence that patients are not receiving them in routine clinical care. Furthermore, even when patients do receive these treatments there is evidence that they are often not well delivered. The aim of this paper is to identify the barriers to the dissemination of evidence-based psychological treatments and then propose ways of overcoming them, hence potentially bridging the gap between research findings and clinical practice.

  • Internet-Based Cognitive Behavior Therapy for Loneliness : A Pilot Randomized Controlled Trial

    • Loneliness has been described as a common source of discomfort based on a subjective discrepancy between the actual and desired social situation. For some people this feeling may become a sustained state that is associated with a wide range of psychiatric and psychosocial problems. While there are few existing treatment protocols, interventions based on cognitive behavioral therapy (CBT) have shown positive effects. The current study investigated the efficacy of an 8-week internet-based treatment containing CBT components aimed at reducing feelings of loneliness. Seventy-three participants were recruited from the general public and randomly allocated to treatment or a wait-list control condition. Participants were assessed with standardized self-report measures of loneliness, depression, social anxiety, worry, and quality of life at pretreatment and posttreatment. Robust linear regression analysis of all randomized participants showed significant treatment effects on the primary outcome measure of loneliness (between group Cohen’s d = 0.77), and on secondary outcomes measuring quality of life and social anxiety relative to control at postassessment. The results suggest the potential utility of internet-based CBT in alleviating loneliness but more research on the long-term effects and the mechanisms underlying the effects is needed.
